    Parent-clinician communication intervention during end-of-life decision making for children with incurable cancer.

    Background: In this single-site study, we evaluated the feasibility of a parent-clinician communication intervention designed to: identify parents\u27 rationale for the phase I, do-not-resuscitate (DNR), or terminal care decision made on behalf of their child with incurable cancer; identify their definition of being a good parent to their ill child; and provide this information to the child\u27s clinicians in time to be of use in the family\u27s care. Methods: Sixty-two parents of 58 children and 126 clinicians participated. Within 72 hours after the treatment decision, parents responded to 6 open-ended interview questions and completed a 10-item questionnaire about the end-of-life communication with their child\u27s clinicians. They completed the questionnaire again two to three weeks later and responded to three open-ended questions to assess the benefit:risk ratio of their study participation three months after the intervention. Clinicians received the interview data within hours of the parent interview and evaluated the usefulness of the information three weeks later. Results: All preestablished intervention feasibility criteria were met; 77.3% of families consented; and in 100% of interventions, information was successfully provided individually to 3 to 11 clinicians per child before the child died. No harm was reported by parents as a result of participating; satisfaction and other benefits were reported. Clinicians reported moderate to strong satisfaction with the intervention. Conclusion: The communication intervention was feasible within hours of decision making, was acceptable and beneficial without harm to participating parents, and was acceptable and useful to clinicians in their care of families

    The Global Health Nursing Imperative: Using Competency-Based Analysis to Strengthen Accountability for Population Focused Practice, Education, and Research

    BackgroundThe Quad Council Competencies for Public Health Nurses are used to guide community and public health nursing course development in baccalaureate nursing programs. As clinical practice has expanded to global settings, the appropriateness of the 2011 Quad Council Public Health Nursing (PHN) Competencies to guide global clinical practice and evaluation was questioned.ObjectiveTo describe a comparison and analysis of three sets of competencies: PHN competencies, competencies for global health nurses, and interprofessional competencies for health professionals.MethodA literature review identified the most current guidelines and competencies for global health practice. Two seminal articles, “Global Health Competencies for Nurses in the Americas” and “Identifying Interprofessional Global Health Competencies for 21st-Century Health Professionals,” were reviewed and compared with the 2011 Quad Council Public Health Nursing Competencies. A six-member multi-site team conducted a qualitative thematic approach to competency analysis. A four column crosswalk spreadsheet grid was used for comparison of the three sets of competencies. Column four was created to identify possible exemplars for clinical practice.FindingsGaps exist in the PHN competencies for specific global and interprofessional competencies.RecommendationsEnhanced and consistent emphasis on population/global health, and interprofessional content throughout nursing curricula is necessary to prepare providers for practice in global settings. Incorporation of global and interprofessional competencies should be considered in the revision of competencies for PHN practice to enhance productive contributions to community health outcomes. Consideration of proper placement of content gaps within basic and advanced nursing education as well as leveling for community/public health nursing practice needs to be addressed by nursing education and practice. In the interim, a special course or elective may be appropriate, especially for schools having clinical nursing practicums in international settings. Clinical evaluation in low-resource settings needs to be enhanced and aligned with competencies

    Torus and AGN properties of nearby Seyfert galaxies: Results from fitting IR spectral energy distributions and spectroscopy

    We used the CLUMPY torus models and a Bayesian approach to fit the infrared spectral energy distributions (SEDs) and ground-based high-angular resolution mid-infrared spectroscopy of 13 nearby Seyfert galaxies. This allowed us to put tight constraints on torus model parameters such as the viewing angle, the radial thickness of the torus Y, the angular size of the cloud distribution sigma_torus, and the average number of clouds along radial equatorial rays N_0. The viewing angle is not the only parameter controlling the classification of a galaxy into a type 1 or a type 2. In principle type 2s could be viewed at any viewing angle as long as there is one cloud along the line of sight. A more relevant quantity for clumpy media is the probability for an AGN photon to escape unabsorbed. In our sample, type 1s have relatively high escape probabilities, while in type 2s, as expected, tend to be low. Our fits also confirmed that the tori of Seyfert galaxies are compact with torus model radii in the range 1-6pc. The scaling of the models to the data also provided the AGN bolometric luminosities, which were found to be in good agreement with estimates from the literature. When we combined our sample of Seyfert galaxies with a sample of PG quasars from the literature to span a range of L_bol(AGN)~10^{43}-10^{47}erg/s, we found plausible evidence of the receding torus. That is, there is a tendency for the torus geometrical covering factor to be lower at high AGN luminosities than at low AGN luminosities. This is because at low AGN luminosities the tori appear to have wider angular sizes and more clouds along radial equatorial rays. We cannot, however rule out the possibility that this is due to contamination by extended dust structures not associated with the dusty torus at low AGN luminosities, since most of these in our sample are hosted in highly inclined galaxies.     Polygenic prediction of educational attainment within and between families from genome-wide association analyses in 3 million individuals

    We conduct a genome-wide association study (GWAS) of educational attainment (EA) in a sample of ~3 million individuals and identify 3,952 approximately uncorrelated genome-wide-significant single-nucleotide polymorphisms (SNPs). A genome-wide polygenic predictor, or polygenic index (PGI), explains 12–16% of EA variance and contributes to risk prediction for ten diseases. Direct effects (i.e., controlling for parental PGIs) explain roughly half the PGI’s magnitude of association with EA and other phenotypes. The correlation between mate-pair PGIs is far too large to be consistent with phenotypic assortment alone, implying additional assortment on PGI-associated factors. In an additional GWAS of dominance deviations from the additive model, we identify no genome-wide-significant SNPs, and a separate X-chromosome additive GWAS identifies 57
