    Survey of Intestinal Parasites Including Associated Risk Factors Among Food Vendors and Slaughterhouse Workers in Metro Manila, Philippines

    Infections by intestinal parasites are considered as one of the major health concerns in developing countries afflicting different groups of people including food handlers and food vendors and are linked to poor personal hygiene and sanitation. This raises public health issues as food vendors and handlers may potentially become agents for the fecal-oral transmission of intestinal parasitic infections to consumers. This study focused on determining the prevalence of intestinal parasites among slaughter house workers and food vendors and examined their personal and food hygiene practices. A small-scale survey was conducted and selected a total of 91 slaughter house workers and food vendors from different areas in Metro Manila. Microscopic examination ofthe fecal samples collected was done following standard procedures by the World Health Organization (WHO) thru direct smear, formalin-ethyl acetate sedimentation and staining methods. Participants were also interviewed on their food and personal hygiene practices using a questionnaire. The overall prevalence of parasitic infection was 90% with helminthic predominating protozoan infections. Eight (8) different intestinal parasites were identified: Entamoeba histolytica/Entamoeba dispar (15.6%), Balantidium coli (8.4%), Giardia lamblia (4.2%), Ascaris lumbricoides (30%), Trichuris trichiura (14.9%), Ancyclostoma duodenale/ Necator americanus (2.3%). Taenia spp. (2.4%), and Enterobius vermicularis (2.9%). Other amoeba-like protozoans (19.2%) were also observed suggestive of exposure to fecal materials. Based on the results obtained, there is high levels of parasitic infections among slaughter house workers and food vendors. Raising awareness on proper food handling, improved personal hygiene and sanitation is needed to prevent further transmission of parasites to the public.     Socio-demographic determinants of Toxoplasma gondii seroprevalence in migrant workers of Peninsular Malaysia

    Background The number of migrants working in Malaysia has increased sharply since the 1970’s and there is concern that infectious diseases endemic in other (e.g. neighbouring) countries may be inadvertently imported. Compulsory medical screening prior to entering the workforce does not include parasitic infections such as toxoplasmosis. Therefore, this study aimed to evaluate the seroprevalence of T. gondii infection among migrant workers in Peninsular Malaysia by means of serosurveys conducted on a voluntary basis among low-skilled and semi-skilled workers from five working sectors, namely, manufacturing, food service, agriculture and plantation, construction and domestic work. Methods A total of 484 migrant workers originating from rural locations in neighbouring countries, namely, Indonesia (n = 247, 51.0%), Nepal (n = 99, 20.5%), Bangladesh (n = 72, 14.9%), India (n = 52, 10.7%) and Myanmar (n = 14, 2.9%) were included in this study. Results The overall seroprevalence of T. gondii was 57.4% (n = 278; 95% CI: 52.7–61.8%) with 52.9% (n = 256; 95% CI: 48.4–57.2%) seropositive for anti-Toxoplasma IgG only, 0.8% (n = 4; 95% CI: 0.2–1.7%) seropositive for anti-Toxoplasma IgM only and 3.7% (n = 18; 95% CI: 2.1–5.4%) seropositive with both IgG and IgM antibodies. All positive samples with both IgG and IgM antibodies showed high avidity (> 40%), suggesting latent infection. Age (being older than 45 years), Nepalese nationality, manufacturing occupation, and being a newcomer in Malaysia (excepting domestic work) were positively and statistically significantly associated with seroprevalence (P < 0.05). Conclusions The results of this study suggest that better promotion of knowledge about parasite transmission is required for both migrant workers and permanent residents in Malaysia. Efforts should be made to encourage improved personal hygiene before consumption of food and fluids, thorough cooking of meat and better disposal of feline excreta from domestic pets

    ParaSitic iNFEctiON: a rEcUrriNg PHENOmENON iN maLaYSia

    Abstract. A total of 255 patients including 179 (70.2%) of non-HIV and 76 (29.8%) HIV-infected patients were recruited in this descriptive study. The subjects was significantly found to be male Chinese (157; 61.6% vs 74; 47.1%) followed by female Malays (98; 38.4% vs 35; 35.7%) (p&lt;0.05). The majority of subjects (124; 48.6%) were in the age group of 21-39 years, however, no statistical difference was found between the various age groups (p&gt;0.05). Overall seroprevalence of latent Toxoplasma infection was 82/183 (44.8%) being; 3 (3.7%) positive for IgM, 74 (90.2%) for IgG, and 5 (6.1%) for IgG and IgM antibodies. The prevalence was more relatively found in the Chinese (28; 15.3%) and Malays (27; 14.8%) than others (p&lt;0.05). While, 23/76 (30.3%) of HIV-positive patients were shown Toxoplasma seropositivity. The majority of these subjects (138/181; 76.2%) were significantly asymptomatic (p = 0.000), while the others were clinically evident cases of toxoplasmosis. Of this, 37 patients were included in differential diagnosis relating to ocular diseases and only 4 patients were confirmed as having ocular toxoplasmosis. Toxoplasmic encephalitis (TE) was based on presumptive diagnosis, particularly found in 5 patients with AIDS. Seventeen patients were clinically diagnosed as having malaria being; 8 for P. vivax, 4 for P. falciparum, 3 for P. malariae, and 2 for mixed infections. All cases resolved satisfactorily after treatment with antimalarial drugs. Other important emerging parasitic diseases were also detected in these patients including amebiasis (2), blastocystosis (1), cryptosporidiosis (1), filariasis (1), and giardiasis (2) during the time of this study

    Retrospective study of Dengue Fever (DF) and Dengue Hemorrhagic Fever (DHF) patients at University Malaya Medical Center, Kuala Lumpur, Malaysia in the year 2005

    The aim of this retrospective study was to determine the number of dengue fever (DF) and dengue hemorrhagic fever (DHF) cases admitted to University Malaya Medical Center (UMMC) in the year 2005 together with their clinical presentations and epidemiology. The data for 2005 was collected from the medical records department of UMMC. A total of 1,279 cases were admitted in 2005 with DF (81, n=1,040) and DHF (19, n=239). January had the greatest number of cases of DF and DHF (22, n=281) and April had the least (4, n=49). The greatest number of DF cases (38, n=392) were seen in the 20-29 year age group, while the greatest number of DHF cases (32, n=76) were in the 10-19 year age group. In regard to race, the greatest number of cases were seen in Malays (48, DF; 49, DHF). Males were more commonly infected than females. The majority of patients infected were students (30, n=385). All patients with DF and DHF presented with fever. The symptoms reported included nausea and vomiting, joint pain, gum bleeding and dehydration. The mean value hemoglobin, white blood cell count and platelet count were 14.4 g/dl, 4 x103/dl, 75 x106/dl for DF and 15.1 g/dl, 4 x103dl,52x106/dl for DHF. The majority of patients were treated with hydration therapy. There were three deaths reported, caused by dengue shock syndrome (DSS)

    Parasitic helminth infections of dogs, wolves, foxes, and golden jackals in Mazandaran Province, North of Iran

    Background and Aim: There is a large amount of information on intestinal parasites in stray dogs and golden jackals (Canis aureus) in Mazandaran Province, Iran. However, there is little information about foxes and wolves, which have a potential role in the spread of dangerous parasitic diseases, such as echinococcosis and toxocariasis. The aim of the present study was to identify the genus or species of parasitic worms in stool samples obtained from carnivores in Mazandaran Province, Iran, from August 2017 to April 2018. Materials and Methods: A total of 274 fecal samples were collected from carnivores, including dog, fox, wolf, and C. aureus in three areas of Mazandaran Province, Iran. All specimens were examined by centrifugal fecal flotation using a solution of Sheather's sugar to detect helminths eggs. Then, all samples were assessed using a light microscope. Data analysis was performed by SPSS version 18 (Chicago, IL, USA). Results: In this study, seven genera of helminths were observed, including Ancylostoma, Uncinaria, Toxocara, Dipylidium, Toxascaris, Taenia, and Spirocerca. The prevalence of helminth infections was 97.7 (127 out of 130), 56.7 (51 out of 90), 51.4 (18 out of 35), and 52.6 (10 out of 19), among dogs, C. aureus, foxes, and wolves, respectively. The highest prevalence of Ancylostoma and Toxocara infections occurred in the eastern and central areas of the province (42.1 and 35.7, respectively). Conclusion: Based on the results of this study, the infection with intestinal zoonotic helminths in carnivores was an important public health factor in Mazandaran.     Seroepidemiology of human Toxoplasma gondii infection in China

    <p>Abstract</p> <p>Background</p> <p>Toxoplasmosis is an important zoonotic parasitic disease worldwide. In immune competent individuals, <it>Toxoplasma gondii </it>preferentially infects tissues of central nervous systems, which might be an adding factor of certain psychiatric disorders. Congenital transmission of <it>T. gondii </it>during pregnancy has been regarded as a risk factor for the health of newborn infants. While in immune-compromised individuals, the parasite can cause life-threatening infections. This study aims to investigate the prevalence of <it>T. gondii </it>infection among clinically healthy <b>i</b>ndividuals and patients with psychiatric disorders in China and to identify the potential risk factors related to the vulnerability of infection in the population.</p> <p>Methods</p> <p>Serum samples from 2634 healthy individuals and 547 patients with certain psychiatric disorders in Changchun and Daqing in the northeast, and in Shanghai in the south of China were examined respectively for the levels of anti-<it>T. gondii </it>IgG by indirect ELISA and a direct agglutination assay. Prevalence of <it>T. gondii </it>infection in the Chinese population in respect of gender, age, residence and health status was systematically analyzed.</p> <p>Results</p> <p>The overall anti-<it>T. gondii </it>IgG prevalence in the study population was 12.3%. In the clinically healthy population 12.5% was sero-positive and in the group with psychiatric disorders 11.3% of these patients were positive with anti-<it>T. gondii </it>IgG. A significant difference (P = 0.004) was found between male and female in the healthy population, the seroprevalence was 10.5% in men versus 14.3% in women. Furthermore, the difference of <it>T. gondii </it>infection rate between male and female in the 20-19 year's group was more obvious, with 6.4% in male population and 14.6% in female population.</p> <p>Conclusion</p> <p>A significant higher prevalence of <it>T. gondii </it>infection was observed in female in the clinically healthy population. No correlation was found between <it>T. gondii </it>infection and psychiatric disorders in this study. Results suggest that women are more exposed to <it>T. gondii </it>infection than men in China. The data argue for deeper investigations for the potential risk factors that threat the female populations.</p

    Detection of Naegleria Species in Environmental Samples from Peninsular Malaysia

    In Malaysia, researchers and medical practitioners are unfamiliar with Naegleria infections. Thus little is known about the existence of pathogenic Naegleria fowleri, and the resultant primary amoebic meningoencephalitis (PAM) is seldom included in the differential diagnosis of central nervous system infections. This study was conducted to detect the presence of Naegleria species in various environmental samples.A total of 41 Naegleria-like isolates were isolated from water and dust samples. All these isolates were subjected to PCR using two primer sets designed from the ITS1-ITS2 regions. The N. fowleri species-specific primer set failed to produce the expected amplicon. The Naegleria genus-specific primers produced amplicons of 408 bp (35), 450 bp (2), 457 bp (2) or 381 bp (2) from all 41 isolates isolated from aquatic (33) and dust (8) samples. Analysis of the sequences from 10 representative isolates revealed that amplicons with fragments 408, 450 and 457 bp showed homology with non-pathogenic Naegleria species, and 381 bp showed homology with Vahlkampfia species. These results concurred with the morphological observation that all 39 isolates which exhibited flagella were Naegleria, while 2 isolates (AC7, JN034055 and AC8, JN034056) that did not exhibit flagella were Vahlkampfia species.To date, pathogenic species of N. fowleri have not been isolated from Malaysia. All 39 isolates that produced amplicons (408, 450 and 457 bp) from the genus-specific primers were identified as being similar to nonpathogenic Naegleria. Amplicon 408 bp from 5 representative isolates showed 100% and 99.7% identity to Naegleria philippinensis isolate RJTM (AM167890) and is thus believed to be the most common species in our environment. Amplicons 450 bp and 457 bp were respectively believed to be from 2 new species of Naegleria, since representative isolates showed lower homology and had a longer base pair length when compared to the reference species in the Genbank, Naegleria schusteri (AJ566626) and Naegleria laresi (AJ566630), respectively
