Tracing a certain slant of light--forming a personal philosophy and voice in music composition
"This thesis serves to examine philosophical and technical influences and their effect on the author's personal style of music composition. Specific composers, performers, and the influence of their respective practices in my thinking are considered. Selections of compositions from the author's catalogue are inspected, revealing such techniques in maturation. Megascopic Quantum Phenomena. A Critical Study of Physical Interpretations
A megascopic revalidation is offered providing responses and resolutions of
current inconsistencies and existing contradictions in present-day quantum
theory. As the core of this study we present an independent proof of the
Goldstone theorem for a quantum field formulation of molecules and solids.
Along with phonons two types of new quasiparticles appear: rotons and
translons. In full analogy with Lorentz covariance, combining space and time
coordinates, a new covariance is necessary, binding together the internal and
external degrees of freedom, without explicitly separating the centre-of-mass,
which normally applies in both classical and quantum formulations. The
generally accepted view regarding the lack of a simple correspondence between
the Goldstone modes and broken symmetries, has significant consequences: an
ambiguous BCS theory as well as a subsequent Higgs mechanism. The application
of the archetype of the classical spontaneous symmetry breaking, i.e. the
Mexican hat, as compared to standard quantum relations, i.e. the Jahn-Teller
effect, superconductivity or the Higgs mechanism, becomes a disparity. In
short, symmetry broken states have a microscopic causal origin, but transitions
between them have a teleological component. The different treatments of the
problem of the centre of gravity in quantum mechanics and in field theories
imply a second type of Bohr complementarity on the many-body level opening the
door for megascopic representations of all basic microscopic quantum axioms
with further readings for teleonomic megascopic quantum phenomena, which have
no microscopic rationale: isomeric transitions, Jahn-Teller effect, chemical
reactions, Einstein-de Haas effect, superconductivity-superfluidity, and

HIV infection of non-dividing cells: a divisive problem
Understanding how lentiviruses can infect terminally differentiated, non-dividing cells has proven a very complex and controversial problem. It is, however, a problem worth investigating, for it is central to HIV-1 transmission and AIDS pathogenesis. Here I shall attempt to summarise what is our current understanding for HIV-1 infection of non-dividing cells. In some cases I shall also attempt to make sense of controversies in the field and advance one or two modest proposals
International Consensus Statement on Rhinology and Allergy: Rhinosinusitis
Background: The 5 years since the publication of the first International Consensus Statement on Allergy and Rhinology: Rhinosinusitis (ICARâRS) has witnessed foundational progress in our understanding and treatment of rhinologic disease. These advances are reflected within the more than 40 new topics covered within the ICARâRSâ2021 as well as updates to the original 140 topics. This executive summary consolidates the evidenceâbased findings of the document. Methods: ICARâRS presents over 180 topics in the forms of evidenceâbased reviews with recommendations (EBRRs), evidenceâbased reviews, and literature reviews. The highest grade structured recommendations of the EBRR sections are summarized in this executive summary. Results: ICARâRSâ2021 covers 22 topics regarding the medical management of RS, which are grade A/B and are presented in the executive summary. Additionally, 4 topics regarding the surgical management of RS are grade A/B and are presented in the executive summary. Finally, a comprehensive evidenceâbased management algorithm is provided. Conclusion: This ICARâRSâ2021 executive summary provides a compilation of the evidenceâbased recommendations for medical and surgical treatment of the most common forms of RS
Hemophilia B: Molecular pathogenesis and mutation analysis
Hemophilia B is an X-chromosome-linked inherited bleeding disorder primarily affecting males, but those carrier females with reduced factor IX activity (FIX:C) levels may also experience some bleeding. Genetic analysis has been undertaken for hemophilia B since the mid-1980s, through linkage analysis to track inheritance of an affected allele, and to enable determination of the familial mutation. Mutation analysis using PCR and Sanger sequencing along with dosage analysis for detection of large deletions/duplications enables mutation detection in > 97% of patients with hemophilia B. The risk of the development of inhibitory antibodies, which are reported in ~ 2% of patients with hemophilia B, can be predicted, especially in patients with large deletions, and these individuals are also at risk of anaphylaxis, and nephrotic syndrome if they receive immune tolerance induction. Inhibitors also occur in patients with nonsense mutations, occasionally in patients with small insertions/deletions or splice mutations, and rarely in patients with missense mutations (p.Gln237Lys and p.Gln241His). Hemophilia B results from several different mechanisms, and those associated with hemophilia B Leyden, ribosome readthrough of nonsense mutations and apparently âsilentâ changes that do not alter amino acid coding are explored. Large databases of genetic variants in healthy individuals and patients with a range of disorders, including hemophilia B, are yielding useful information on sequence variant frequency to help establish possible variant pathogenicity, and a growing range of algorithms are available to help predict pathogenicity for previously unreported variants
Reuse of a previously transplanted kidney: Does success come with a price?
Longer wait times for deceased donor kidney transplant have prompted newer initiatives to expedite the process. Reuse of a previously transplanted kidney might be appropriate in certain circumstances. However, one must also consider the unique issues that may arise after such transplants. We describe our experience in one such case where the donor kidney had lesions of focal and segmental glomerulosclerosis and signs of alloreactivity (positive C4d staining) prior to transplantation and the recipient developed ganciclovir-resistant cytomegalovirus (CMV) infection, which was perhaps transmitted from the donor. Despite the challenges, the allograft function remained stable 5 years after reuse
