    Disabled Beggars in Addis Ababa

    This study brings together qualitative and quantitative data to better understand the lives of people with disabilities who beg in Ethiopia. It sets out to provide an initial understanding of the lives of disabled beggars with particular emphasis on determining social and economic factors, and sequences of events or patterns of behaviour that are common to people with disabilities who now work as beggars. Chapter 3 provides an overview of the demographic characteristics of the survey respondents, including their education and vocational training levels and work history. Additional attention was directed to identifying possible areas of intervention that might sever the links between disability and poverty. The study yielded an intriguing set of results that identify: the complex set of issues with which disabled beggars grapple; a series of points where targeted intervention by governments, UN agencies, NGOs and disabled people's organizations could help break the on-going cycle of disability and poverty; and choices that lead some men and women with disabilities to beg. The study contains recommendations for policy, programming and areas for further research

    Poverty and Disability: A critical review of the literature in low and middle-income countries

    While it is widely assumed that disability, poverty and health are closely linked, this is the first critical review on the subject that explicitly asks: ‘What is the current evidence base for the link between poverty, disability and health in low- and middle-income countries? The methods used have been adapted from the EPPI Centre (EPPI-Centre, 2007) and the work of Greenhalgh (Greenhalgh, Robert, Macfarlane, Bate, Kyriakidou, & Peacock, 2005) A total of 964 papers were identified and, of these, 293 were selected for further review based on appropriateness of fit. An initial review of the 293 papers, paying particular attention to those papers that presented an evidence base, found only 27 papers (9.2% of total papers reviewed) met the established inclusion criteria for a critical review. Widening these inclusion criteria did not produce significantly more evidence based papers for review. Thus, the most significant finding from this study is the current lack of strong evidence on the links between disability, poverty and health in LMICs upon which to build global policy and programming. Within the group of papers available for review, we identify a small but growing evidence base that indicates that there are substantial links between disability, poverty and health; however emerging research indicates that these links are more complex and nuanced than is currently assumed. We conclude with a call for more attention to building an evidence base on the interactions between disability, poverty and health. The absence of a robust evidence base that explicitly links these issues, in conjunction with the lack of appropriate benchmarks and indicators to measure disability rights commitments (including poverty reduction), will otherwise result in a “democratic deficit”

    Engineering Silicon Nanocrystals: Theoretical study of the effect of Codoping with Boron and Phosphorus

    We show that the optical and electronic properties of nanocrystalline silicon can be efficiently tuned using impurity doping. In particular, we give evidence, by means of ab-initio calculations, that by properly controlling the doping with either one or two atomic species, a significant modification of both the absorption and the emission of light can be achieved. We have considered impurities, either boron or phosphorous (doping) or both (codoping), located at different substitutional sites of silicon nanocrystals with size ranging from 1.1 nm to 1.8 nm in diameter. We have found that the codoped nanocrystals have the lowest impurity formation energies when the two impurities occupy nearest neighbor sites near the surface. In addition, such systems present band-edge states localized on the impurities giving rise to a red-shift of the absorption thresholds with respect to that of undoped nanocrystals. Our detailed theoretical analysis shows that the creation of an electron-hole pair due to light absorption determines a geometry distortion that in turn results in a Stokes shift between adsorption and emission spectra. In order to give a deeper insight in this effect, in one case we have calculated the absorption and emission spectra going beyond the single-particle approach showing the important role played by many-body effects. The entire set of results we have collected in this work give a strong indication that with the doping it is possible to tune the optical properties of silicon nanocrystals.Comment: 14 pages 19 figure

    Silicon nanofluidic membrane for electrostatic control of drugs and analytes elution

    Individualized long-term management of chronic pathologies remains an elusive goal despite recent progress in drug formulation and implantable devices. The lack of advanced systems for therapeutic administration that can be controlled and tailored based on patient needs precludes optimal management of pathologies, such as diabetes, hypertension, rheumatoid arthritis. Several triggered systems for drug delivery have been demonstrated. However, they mostly rely on continuous external stimuli, which hinder their application for long-term treatments. In this work, we investigated a silicon nanofluidic technology that incorporates a gate electrode and examined its ability to achieve reproducible control of drug release. Silicon carbide (SiC) was used to coat the membrane surface, including nanochannels, ensuring biocompatibility and chemical inertness for long-term stability for in vivo deployment. With the application of a small voltage (≀ 3 V DC) to the buried polysilicon electrode, we showed in vitro repeatable modulation of membrane permeability of two model analytes—methotrexate and quantum dots. Methotrexate is a first-line therapeutic approach for rheumatoid arthritis; quantum dots represent multi-functional nanoparticles with broad applicability from bio-labeling to targeted drug delivery. Importantly, SiC coating demonstrated optimal properties as a gate dielectric, which rendered our membrane relevant for multiple applications beyond drug delivery, such as lab on a chip and micro total analysis systems (”TAS)

    Mental illness, poverty and stigma in India: a case-control study

    Objective: To assess the effect of experienced stigma on depth of multidimensional poverty of persons with severe mentalillness(PSMI) in Delhi, India, controlling for gender, age and caste. Design: Matching case(hospital)–control(population) study. Setting:University Hospital (cases) and National Capital Region (controls),India. Participants: A case–control study was conducted from November 2011 to June 2012. 647 cases diagnosed with schizophreniaor affective disorders were recruited and 647 individuals of same age,sex and location of residence were matched as controls at a ratio of 1:2:1. Individuals who refused consent or provided incomplete interview were excluded. Main outcome measures: Higher risk of poverty due to stigma among PSMI. Results :38.5% of PSMI compared with 22.2%of controls were found poor on six dimensions or more. The difference in multidimensional poverty index was 69% between groups with employment and income of the main contributors. Multidimensional poverty was strongly associated with stigma(OR 2.60,95%CI 1.27 to 5.31) ,scheduled castes/scheduled tribes/other backward castes(2.39,1.39 to 4.08),mental illness (2.07,1.25 to 3.41)and female gender(1.87,1.36 to 2.58).A significant interaction between stigma,mental illness and gender or caste indicates female PSMI or PSMI from ‘lower castes’ were more likely to be poor due to stigma than male controls(p\u3c0.001)or controls from other castes(p\u3c0.001). Conclusions: Public stigma and multidimensional poverty linked to SMI are pervasive and intertwined.In particular for low caste and women,it is a strong predictor of poverty.Exclusion from employment linked to negative attitudes and lack of income are the highest contributors to multidimensional poverty,increasing the burden for the family. Mental health professionals need to be aware of and address these issues

    Disabled women’s attendance at community women’s groups in rural Nepal

    There is strong evidence that participatory approaches to health and participatory women’s groups hold great potential to improve the health of women and children in resource poor settings. It is important to consider if interventions are reaching the most marginalized, and therefore we examined disabled women’s participation in women’s groups and other community groups in rural Nepal. People with disabilities constitute 15% of the world’s population and face high levels of poverty, stigma, social marginalization and unequal access to health resources, and therefore their access to women’s groups is particularly important. We used a mixed methods approach to describe attendance in groups among disabled and non-disabled women, considering different types and severities of disability. We found no significant differences in the percentage of women that had ever attended at least one of our women’s groups, between non-disabled and disabled women. This was true for women with all severities and types of disability, except physically disabled women who were slightly less likely to have attended. Barriers such as poverty, lack of family support, lack of self-confidence and attendance in many groups prevented women from attending groups. Our findings are particularly significant because disabled people’s participation in broader community groups, not focused on disability, has been little studied. We conclude that women’s groups are an important way to reach disabled women in resource poor communities. We recommend that disabled persons organizations help to increase awareness of disability issues among organizations running community groups to further increase their effectiveness in reaching disabled women

    Postprandial glucose and HbA1c are associated with severity of obstructive sleep apnoea in non-diabetic obese subjects

    Introduction: Obstructive sleep apnoea (OSA) is an underdiagnosed condition frequently associated with glycaemic control impairment in patients with type 2 diabetes. Aim: To assess the relationship between glycometabolic parameters and OSA in obese non-diabetic subjects. Methods: Ninety consecutive subjects (mean age 44.9 ± 12 years, mean BMI 42.1 ± 9 kg/m2) underwent polysomnography and a 2-h oral glucose tolerance test (OGTT). Results: OSA was identified in 75% of subjects, with a higher prevalence of males compared to the group of subjects without OSA (62% vs 32%, p = 0.02). Patients with OSA had comparable BMI (42.8 kg/m2 vs 39.4 kg/m2), a higher average HbA1c (5.8% vs 5.4%, p < 0.001), plasma glucose at 120 min during OGTT (2 h-PG; 123 mg/dl vs 97 mg/dl, p = 0.009) and diastolic blood pressure (81.1 mmHg vs 76.2 mmHg, p = 0.046) than obese subjects without OSA. HbA1c and 2 h-PG were found to be correlated with the apnoea-hypopnoea index (AHI; r = 0.35 and r = 0.42, respectively) and with percent of sleep time with oxyhaemoglobin saturation < 90% (ST90; r = 0.44 and r = 0.39, respectively). Further, in a linear regression model, ST90 and AHI were found to be the main determinants of 2 h-PG (ÎČ = 0.81, p < 0.01 and ÎČ = 0.75, p = 0.02, respectively) after controlling for age, sex, waist circumference, physical activity, and C-reactive protein. Similarly, ST90 and AHI persisted as independent determinants of HbA1c (ÎČ = 0.01, p = 0.01 and ÎČ = 0.01, p = 0.01, respectively). Conclusion: Beyond the traditional clinical parameters, the presence of a normal-high value of 2 h-PG and HbA1c should raise suspicion of the presence of OSA in obese subjects

    Adiabatic description of nonspherical quantum dot models

    Within the effective mass approximation an adiabatic description of spheroidal and dumbbell quantum dot models in the regime of strong dimensional quantization is presented using the expansion of the wave function in appropriate sets of single-parameter basis functions. The comparison is given and the peculiarities are considered for spectral and optical characteristics of the models with axially symmetric confining potentials depending on their geometric size making use of the total sets of exact and adiabatic quantum numbers in appropriate analytic approximations

    DYNAMICS OF TIDALLY CAPTURED PLANETS IN THE GALACTIC CENTER

    Recent observations suggest ongoing planet formation in the innermost parsec of the Galactic center. The supermassive black hole (SMBH) might strip planets or planetary embryos from their parent star, bringing them close enough to be tidally disrupted. Photoevaporation by the ultraviolet field of young stars, combined with ongoing tidal disruption, could enhance the near-infrared luminosity of such starless planets, making their detection possible even with current facilities. In this paper, we investigate the chance of planet tidal captures by means of high-accuracy N-body simulations exploiting Mikkola's algorithmic regularization. We consider both planets lying in the clockwise (CW) disk and planets initially bound to the S-stars. We show that tidally captured planets remain on orbits close to those of their parent star. Moreover, the semimajor axis of the planetary orbit can be predicted by simple analytic assumptions in the case of prograde orbits. We find that starless planets that were initially bound to CW disk stars have mild eccentricities and tend to remain in the CW disk. However, we speculate that angular momentum diffusion and scattering by other young stars in the CW disk might bring starless planets into orbits with low angular momentum. In contrast, planets initially bound to S-stars are captured by the SMBH on highly eccentric orbits, matching the orbital properties of the clouds G1 and G2. Our predictions apply not only to planets but also to low-mass stars initially bound to the S-stars and tidally captured by the SMBH
