    The Chandra X-Ray Observatory's Radiation Environment and the AP-8/AE-8 Model

    The Chandra X-ray Observatory (CXO) was launched on July 23, 1999 and reached its final orbit on August 7, 1999. The CXO is in a highly elliptical orbit, approximately 140,000 km x 10,000 km, and has a period of approximately 63.5 hours (~ 2.65 days). It transits the Earth's Van Allen belts once per orbit during which no science observations can be performed due to the high radiation environment. The Chandra X-ray Observatory Center (CXC) currently uses the National Space Science Data Center's ``near Earth'' AP-8/AE-8 radiation belt model to predict the start and end times of passage through the radiation belts. However, our scheduling software uses only a simple dipole model of the Earth's magnetic field. The resulting B, L magnetic coordinates, do not always give sufficiently accurate predictions of the start and end times of transit of the Van Allen belts. We show this by comparing to the data from Chandra's on-board radiation monitor, the EPHIN (Electron, Proton, Helium Instrument particle detector) instrument. We present evidence that demonstrates this mis-timing of the outer electron radiation belt as well as data that also demonstrate the significant variablity of one radiation belt transit to the next as experienced by the CXO. We also present an explanation for why the dipole implementation of the AP-8/AE-8 model is not ideally suited for the CXO. Lastly, we provide a brief discussion of our on-going efforts to identify a model that accounts for radiation belt variability, geometry, and one that can be used for observation scheduling purposes.Comment: 12 pgs, 6 figs, for SPIE 4012 (Paper 76

    Spontaneous bleeding in a patient with malignant lymphoma: A case of acquired hemophilia

    Background: Acquired hemophilia is a rare condition which can be associated with lymphoproliferative disease. Case Report: Eleven yea rs after the diagnosis of immunocytoma had been made, a 72-year-old man developed a high-titer factor VIII inhibitor. At this time, the lymphoma was without significant progress and there was no paraprotein in the serum. Partial thromboplastin time (PTT) was 83 a, factor-VIII clotting activity was <1%, and inhibitor level was 50.4 Bethesda units. The patient presented with spontaneous hematomas in the skin and musculature of the extremities. Following combination chemotherapy with cyclophosphamide, vincristine and prednisolone (COP), there was a prompt disappearance of the inhibitor and normalization of coagulation; however, the patient developed serious infectious complications. When the inhibitor recurred he was treated with low-dose cyclophosphamide and prednisolone. This time there was a more delayed response, but the inhibitor disappeared again completely. Two months after cessation of therapy, there was again relapse. Conclusion: Causal relationship between lymphoma and acquired hemophilia remains speculative. At least in some cases of factor VIII inhibitors associated with malignant disease, immunosuppressive therapy may be sufficient to suppress the inhibitor

    Fluctuation relations and rare realizations of transport observables

    Fluctuation relations establish rigorous identities for the nonequilibrium averages of observables. Starting from a general transport master equation with time-dependent rates, we employ the stochastic path integral approach to study statistical fluctuations around such averages. We show how under nonequilibrium conditions, rare realizations of transport observables are crucial and imply massive fluctuations that may completely mask such identities. Quantitative estimates for these fluctuations are provided.     Phonon-phonon interactions and phonon damping in carbon nanotubes

    We formulate and study the effective low-energy quantum theory of interacting long-wavelength acoustic phonons in carbon nanotubes within the framework of continuum elasticity theory. A general and analytical derivation of all three- and four-phonon processes is provided, and the relevant coupling constants are determined in terms of few elastic coefficients. Due to the low dimensionality and the parabolic dispersion, the finite-temperature density of noninteracting flexural phonons diverges, and a nonperturbative approach to their interactions is necessary. Within a mean-field description, we find that a dynamical gap opens. In practice, this gap is thermally smeared, but still has important consequences. Using our theory, we compute the decay rates of acoustic phonons due to phonon-phonon and electron-phonon interactions, implying upper bounds for their quality factor.Comment: 15 pages, 2 figures, published versio

Mit Hilfe einer mikrocontrollergesteuerten Einheit werden gespeicherte technische Solldruckkurven einstellbarer Amplitude und Frequenz sowie physiologische Druckverläufe erzeugt und die Erfassung der Meßdaten vorgenommen. Über eine serielle Schnittstelle können darüber hinaus beliebige Solldruckkurven von und Meßdaten zu einem Personalcomputer übertragen und dort ausgewertet werden. Für möglichst realitätsnahe Messungen wurden zusätzlich ein Spülsystem sowie eine Flußsimulation implementiert

    Chiral interface states in p-n graphene junctions

    We present a theoretical analysis of unidirectional interface states which form near p−n junctions in a graphene monolayer subject to a homogeneous magnetic field. The semiclassical limit of these states corresponds to trajectories propagating along the p−n interface by a combined skipping-snaking motion. Studying the two-dimensional Dirac equation with a magnetic field and an electrostatic potential step, we provide and discuss the exact and essentially analytical solution of the quantum-mechanical eigenproblem for both a straight and a circularly shaped junction. The spectrum consists of localized Landau-like and unidirectional snaking-skipping interface states, where we always find at least one chiral interface state. For a straight junction and at energies near the Dirac point, when increasing the potential step height, the group velocity of this state interpolates in an oscillatory manner between the classical drift velocity in a crossed electromagnetic field and the semiclassical value expected for a purely snaking motion. Away from the Dirac point, chiral interface states instead resemble the conventional skipping (edge-type) motion found also in the corresponding Schrödinger case. We also investigate the circular geometry, where chiral interface states are predicted to induce sizable equilibrium ring currents

    Acupuncture in Seasonal Allergic Rhinitis (ACUSAR) - Design and Protocol of a Randomised Controlled Multi-Centre Trial

    Background: We report on the study design and protocol of a randomised controlled trial (Acupuncture in Seasonal Allergic Rhinitis, ACUSAR) that investigates the efficacy of acupuncture in the treatment of seasonal allergic rhinitis (SAR). Objective: To investigate whether acupuncture is non-inferior or superior to (a) penetrating sham acupuncture and (b) rescue medication in the treatment of SAR. Design: 3-armed, randomised controlled multi-centre trial with a total follow-up time of 16 weeks in the 1st year and 8 weeks in the 2nd year. Setting: 41 physicians in 37 out-patient units in Germany specialised in acupuncture treatment. Patients: 400 seasonal allergic rhinitis patients with clinical symptoms and test-positive (skin-prick test and/or specific IgE) to both birch and grass pollen. Interventions: Patients will be randomised in a 2:1:1 ratio to one of three groups: (a) semi-standardised acupuncture plus rescue medication (cetirizine); (b) penetrating sham acupuncture at non-acupuncture points plus rescue medication; or (c) rescue medication alone for 8 weeks (standard treatment group). Acupuncture and sham acupuncture will consist of 12 treatments per patient over 8 weeks. Main Outcome Measures: Average means of the Rhinitis Quality of Life Questionnaire (RQLQ) overall score and the Rescue Medication Score (RMS) between weeks 6 and 8 in the first year, adjusted for baseline values. Outlook: The results of this trial available in 2011 will have a major impact on the decision of whether acupuncture should be considered as a therapeutic option in the treatment of SAR

    Potential drug-drug interactions in patients with indication for prophylactic implantation of a cardioverter defibrillator: a cross-sectional analysis

    Background: Due to demographic transition, multimorbidity and high numbers of medicinal products, polypharmacy rates will presumably further increase. This could lead to higher risks of potentially inappropriate medications with potential drug-drug interactions (PDDI). PDDI has already been investigated by several studies, but not for patients with indication for prophylactic implantation of a cardioverter defibrillator (ICD). Thus, the objective of this analysis was to examine the frequency of PDDI in that specific group of patients and compare patients with or without PDDI regarding potential underlying factors. Methods: Cross-sectional data analyses were performed using data of the prospective EU-CERT-ICD study that primarily aimed to assess ICD effectiveness in Europe. Self-reported baseline medication data of patients from Germany and Switzerland were used. Patients who reported to take at least two drugs simultaneously for at least 80 days were defined as population at risk. By means of a publicly available interaction checker, we analyzed the medication data regarding occurrence and characteristics of PDDI categorized as minor, moderate, and major PDDI. The analyses were done using descriptive methods and chi square testing. Results: The total population (n = 524) and the population at risk (n = 383) were rather similar with an average age of 64 years and about 80% male. PDDIs were found for 296 patients (in 57% of total population vs. 77% of population at risk). The moderate PDDI category was most frequently with 268 affected patients. Comparing patients with and without any PDDI, the proportion of patients with place of residence in Germany varied distinctly (93% vs. 78%). The frequency of any PDDI for the total population was twice as high in Germany as in Switzerland (p value < 0.001). Conclusions: PDDIs were frequently observed in this selected patient population and differed markedly between German and Swiss patients. The results should lead to higher awareness of polypharmacy and PDDIs. Adequate cooperation between health care providers should be promoted and new technologies such as drug interaction information systems or digital patient files used.
