    O apoio clínico aos serviços hospitalares representa uma parcela importante da actividade dos serviços de Medicina Interna. Esta actividade não está quantificada e é muitas vezes subvalorizada. Os autores, com a finalidade de caracterizarem a carga de trabalho desenvolvida pela Medicina Interna na área da consulta intra-hospitalar, realizaram um trabalho prospectivo com a duração de 12 meses, quantificando e caracterizando os pedidos de observação recebidos. Foram acolhidos 556 pedidos de consulta. Mais de metade das chamadas foram feitas a partir das 16 horas e 18% requisitadas com carácter de urgência. Atenderam-se no próprio dia 96,9% dos pedidos de observação. A maioria dos pedidos de consulta foram provenientes das áreas de Ortopedia e Traumatologia (33%), Cirurgia Geral (18%) e Unidade de Cuidados Intermédios Cirúrgica (17%). Os principais motivos de chamada foram intercorrências dos foros cardiovascular, infeccioso ou neurológico. Nas chamadas recebidas no horário normal de trabalho não houve contacto com o Médico assistente do doente em 54% dos casos. Esta vertente da actividade assistencial representa uma importante carga de trabalho adicional, com crescente relevância na qualidade dos cuidados médicos prestados. Os nossos resultados realçam a necessidade de formalizar um serviço de consulta intrahospitalar em cada serviço de Medicina e de incluir esta actividade em futuros planos de formação

    This paper was purpose to explore the relationship among the four dimensions of technology readiness index (optimism, innovativeness, discomfort, insecurity) and intention to use computer tablet that moderated by age. The data were analyzed using multiple linear regression method with no moderating variables (t test) and multiple linear regression method with moderating variables (test of interaction). The result of analysis found that four dimensions of technology readiness index only optimism and innovativeness dimension has a significant influence, while discomfort and insecurity has not influence. The result of multiple regression analysis with moderation variable not found as a moderating variable

    Implementasi Metode Kriptografi Advanced Encryption Standard (AES) untuk Proteksi Pesan Audio

    This study is to solve the problem Cryptographic techniques used to protect and maintain the confidentiality of audio messages by changing audio messages into audio messages that cannot be played by doing the encryption process and can be played back after the audio message is decrypted. To protect audio messages, it is used in the process of encrypting and decrypting audio messages using the Advanced Encryption Standard (AES) method. The Advanced Encryption Standard (AES) uses symmetric key block passwords with varying key sizes of 128 bits, 192 bits, and 256 bits. The Advanced Encryption Standard (AES) method used is 128 bit key size and there are 10 rounds in the process of encryption and decryption. Audio messages that have been encrypted using the Advanced Encryption Standard (AES) method cannot be played by parties who do not know the key to encrypt and decrypt the audio message. Audio messages are protected and protected from irresponsible parties who want to damage or just listen to the audio message

    Inflationary Universe in Deformed Phase Space Scenario

    We consider a noncommutative (NC) inflationary model with a homogeneous scalar field minimally coupled to gravity. The particular NC inflationary setting produces entirely new consequences. We first analyze the free field case and subsequently examine the situation where the scalar field is subjected to a polynomial and exponential potentials. We propose to use a canonical deformation between momenta, in a spatially flat FLRW universe, and while the Friedmann equation remains unaffected the Friedmann acceleration equation (and thus the Klein-Gordon equation) is modified by an extra term linear in the NC parameter. This concrete noncommutativity on the momenta allows interesting dynamics that other NC models seem not to allow. Let us be more precise. This extra term behaves as the sole explicit pressure that under the right circumstances implies a period of accelerated expansion of the universe. We find that in the absence of the scalar field potential, and in contrast with the commutative case, in which the scale factor always decelerates, we obtain an inflationary phase for small negative values of the NC parameter. Subsequently, the period of accelerated expansion is smoothly replaced by an appropriate deceleration phase providing an interesting model regarding the graceful exit problem in inflationary models. Moreover, in the case of the free scalar field, we show that not only the horizon problem is solved but also there is some resemblance between the evolution equation of the scale factor associated to our model and that for the R2R^2 (Starobinsky) inflationary model. Therefore, our herein NC model not only can be taken as an appropriate scenario to get a successful kinetic inflation, but also is a convenient setting to obtain inflationary universe possessing the graceful exit when scalar field potentials are present.Comment: 15 pages, 9 figures, Accepted to publish in Annals of Physic

    Rock bearing resistance of bored piles socketed into rock

    In view of the large movement required to mobilise the base resistance of bored piles and difficulty in base cleaning, the end bearing resistance often ignored in current design practice that will result in excessive rock socket length. Many attempts have been made to correlate the end bearing resistance with the uniaxial compressive strength of intact rock and the RQD but it is uncertain how applicable they are to rock type in Malaysia. This paper attempts to review the applicability of the formulas from previous studies to rock type in Malaysia. A program of field tests for 13 bored piles with diameter varying from 1000 mm to 1500 mm constructed in granite was conducted to measure the axial response of bored piles, tested using static load test and high strain load dynamic test to verify its integrity and performance. The results were evaluated and compared to the predicted rock bearing resistance. Based on the result obtained, the method by AASHTO gives the best prediction of rock bearing resistance for granite in Malaysia. However the relationship between compressive strength and rock discontinuities with the rock bearing resistance showed scattered results

    Investigations of Olive Oil Industry By-Products Extracts with Potential Skin Benefits in Topical Formulations

However, they can be recovered and reused, since their components have antioxidant, anti-inflammatory, and photoprotector properties. In this work, oil-in-water creams containing three different olive oil industry by-products extracts were produced without the use of organic solvents. First, the extracts were thoroughly characterized in vitro for cytotoxicity, inhibition of skin enzymes, and antioxidant and photoprotection capacities. Safety studies were then performed, including ocular and skin irritation tests, ecotoxicity evaluation, and in vivo Human Repeat Insult Patch Test. The results obtained in this initial characterization supported the incorporation of the extracts in the cream formulations. After preparation, the creams were characterized for their organoleptic, physicochemical, droplet size and rheological properties, and microbial contamination. The results showed that all formulations were semi-solid creams, with stable pH, compatible with the skin, without microbial contamination, and with the expected droplet size range. The rheological analysis showed shear-thinning behavior with yield stress, with the viscosity decreasing with increasing shear rate. The oscillatory results suggest that the creams have a strong network structure, being easily rubbed into the skin. Finally, compatibility, acceptability and antioxidant efficacy were evaluated in vivo, in human volunteers. No adverse reactions were observed after application of the formulations on skin and the cream with the highest concentrations of phenolic compounds showed the highest antioxidant efficiency. In conclusion, the results suggest that olive oil industry by-products extracts have valuable properties that favor their re-use in the cosmetic industry.     Dilaton Quantum Cosmology with a Schrodinger-like equation

    A quantum cosmological model with radiation and a dilaton scalar field is analysed. The Wheeler-deWitt equation in the mini-superspace induces a Schr\"odinger equation, which can be solved. An explicit wavepacket is constructed for a particular choice of the ordering factor. A consistent solution is possible only when the scalar field is a phantom field. Moreover, although the wavepacket is time dependent, a Bohmian analysis allows to extract a bouncing behaviour for the scale factor.Comment: 14 pages, 3 figures in eps format.     Endovascular therapy versus no endovascular therapy in patients receiving best medical management for acute isolated occlusion of the posterior cerebral artery : A systematic review and meta-analysis

    Background and purpose Endovascular therapy (EVT) is increasingly reported for treatment of isolated posterior cerebral artery (PCA) occlusions although its clinical benefit remains uncertain. This study-level meta-analysis investigated the functional outcomes and safety of EVT and best medical management (BMM) compared to BMM alone for treatment of PCA occlusion stroke. Methods We conducted a literature search in PubMed, Web of Science and Embase for studies in patients with isolated PCA occlusion stroke treated with EVT + BMM or BMM including intravenous thrombolysis. There were no randomized trials and all studies were retrospective. The primary outcome was modified Rankin Scale score of 0-2 at 3 months, while safety outcomes included mortality rate and incidence of symptomatic intracranial hemorrhage (sICH). Results Twelve studies with a total of 679 patients were included in the meta-analysis: 338 patients with EVT + BMM and 341 patients receiving BMM alone. Good functional outcome at 3 months was achieved in 58.0% (95% confidence interval [CI] 43.83-70.95) of patients receiving EVT + BMM and 48.1% (95% CI 40.35-55.92) of patients who received BMM alone, with respective mortality rates of 12.6% (95% CI 7.30-20.93) and 12.3% (95% CI 8.64-17.33). sICH occurred in 4.2% (95% CI 2.47-7.03) of patients treated with EVT + BMM and 3.2% (95% CI 1.75-5.92) of patients treated with BMM alone. Comparative analyses were performed on studies that included both treatments and these demonstrated no significant differences. Conclusions Our results demonstrate that EVT represents a safe treatment for patients with isolated PCA occlusion stroke. There were no differences in clinical or safety outcomes between treatments, supporting randomization of future patients into distal vessel occlusion trials.Peer reviewe

    Covalent targeting of remote cysteine residues to develop CDK12 and CDK13 inhibitors

    Cyclin-dependent kinases 12 and 13 (CDK12 and CDK13) play critical roles in the regulation of gene transcription. However, the absence of CDK12 and CDK13 inhibitors has hindered the ability to investigate the consequences of their inhibition in healthy cells and cancer cells. Here we describe the rational design of a first-in-class CDK12 and CDK13 covalent inhibitor, THZ531. Co-crystallization of THZ531 with CDK12–cyclin K indicates that THZ531 irreversibly targets a cysteine located outside the kinase domain. THZ531 causes a loss of gene expression with concurrent loss of elongating and hyperphosphorylated RNA polymerase II. In particular, THZ531 substantially decreases the expression of DNA damage response genes and key super-enhancer-associated transcription factor genes. Coincident with transcriptional perturbation, THZ531 dramatically induced apoptotic cell death. Small molecules capable of specifically targeting CDK12 and CDK13 may thus help identify cancer subtypes that are particularly dependent on their kinase activities.United States.
