    Polystyrene-supported 1-methylimidazolium tetrachloro ferrate: Synthesis, characterization, and application as an efficient and reusable heterogeneous catalyst for one-pot synthesis of 4h-chromene derivatives in aqueous media

    Polystyrene-supported 1-methylimidazolium tetrachloroferrate (PS[mim][FeCl4]) was prepared and characterized by Fourier transform infrared spectroscopy (FT-IR), scanning electron microscopy (SEM) and energy dispersive X-ray analysis (EDX). The catalytic activity of the heterogeneous catalyst was evaluated through one-pot synthesis of 2-amino-4H-chromene-3-carbonitriles via the three-component condensation reaction of aromatic aldehydes, malononitrile and dimedone or 2-naphthol in water.     State of Art and Perspectives in Catalytic Ozonation for Removal of Organic Pollutants in Water: Influence of Process and Operational Parameters

    The number of organic pollutants detected in water and wastewater is continuously increasing thus causing additional concerns about their impact on public and environmental health. Therefore, catalytic processes have gained interest as they can produce radicals able to degrade recalcitrant micropollutants. Specifically, catalytic ozonation has received considerable attention due to its ability to achieve advanced treatment performances at reduced ozone doses. This study surveys and summarizes the application of catalytic ozonation in water and wastewater treatment, paying attention to both homogeneous and heterogeneous catalysts. This review integrates bibliometric analysis using VOS viewer with systematic paper reviews, to obtain detailed summary tables where process and operational parameters relevant to catalytic ozonation are reported. New insights emerging from heterogeneous and homogenous catalytic ozonation applied to water and wastewater treatment for the removal of organic pollutants in water have emerged and are discussed in this paper. Finally, the activities of a variety of heterogeneous catalysts have been assessed using their chemicalphysical parameters such as point of zero charge (PZC), pKa, and pH, which can determine the effect of the catalysts (positive or negative) on catalytic ozonation processes

    Successful Suicide in a Child: Depression-Related or Paroxetine-Induced?

    Background: Children and adolescents are prone to develop psychiatric problems after stressful life events. These problems need appropriate treatment because of negative consequences like disease-related suicide. Antidepressants (especially "Serotonin Selective Reuptake Inhibitors") are common treatments for psychiatric problems of children and adolescents; but different side effects, including drug-induced suicide, have been reported.Case Report: In this article, we describe a nine-year-old girl who developed depression after parental divorce and was prescribed paroxetine. During pharmacotherapy, she had suicidal thoughts and several unsuccessful attempts which have been neglected and finally last attempt was successful. This report is an evidence for physicians to prescribe antidepressants cautiously with reasonable indication. As still there is no certain contraindication of using antidepressants in pediatric patients, importance of follow-ups and screening of suicide in pediatric patients during treatment with antidepressants seems essential.Conclusion: This study discloses the magnitude of explaining the side effects of antidepressants to caregivers of children with psychiatric problems

    A new method for critical path method with fuzzy processing time

    Critical path method plays an important role on managing medium to large-scale problems. It is often difficult to determine the critical path for different reasons such as the existing uncertainties in processing tasks. One alternative to handle the uncertainty associated with the processing time is to use fuzzy techniques. We present a new method to calculate the critical path method when the processing times follow trapezoidal fuzzy numbers. The proposed model of this paper does not use any defuzzification technique to find the final processing time. The implementation of the proposed model is compared with other techniques using a well-known example from the literature

    Response of armour steel plates to localised air blast load : a dimensional analysis

    We report on the results of dimensional analyses on the dynamic plastic response of square armour steel plates due to detonation of proximal cylindrical charges and ensued air blast loading. By assuming a generic function for the blast load, which is multiplicative comprising its spatial and temporal parts, a set of 14 dimensionless parameters, representative of the load and plate deformation, were identified and recast in the form of dimensionless functions of stand-off to charge diameter ratio. Parametric studies were performed using commercial code ABAQUS’s module of Finite Element hydrocode using MMALE and MMAE techniques, and combined with regression analyses to quantify the dimensional parameters and the expressions for dimensionless functions. A few numerical studies with various FE mesh types were also performed to validate the transient deflections against the small-scale experiments. For pulse loading due to proximal charges of small orders of stand-off/charge diameter ratio, the magnitude of the transverse deflection increased abruptly with incremental decrease in stand-off, in contradistinction to the plate deformations at higher stand-offs where variations in displacement are smooth. This confirmed the existence of a stand-off at which a transition in behaviour takes place. For stand-off values less than charge diameter, a dimensionless energy absorbing effectiveness factor was considered to investigate the prediction of rupture in the plate corresponding to different charge masses. This factor is measured as a baseline parameter to predict, using solely numerical means, the blast loads which ensue rupture on full-scale prototypes

    Delayed Onset Brain Hypoxia and Subsequent Seizures: a Rare Fatal Consequence of Undiagnosed Foreign Body Aspiration

    Background: Foreign body aspiration is a common medical emergency among children that can lead to lethal complications especially in neglected or misdiagnosed cases.Case Report: This article is a report of a 15-month-old child who aspirated food materials and after chocking crisis, she presented with unspecific respiratory symptoms and treated for respiratory tract infections. After five days, sudden airway obstruction led to hypoxic brain damage and seizure. Due to unavailability of bronchoscopic facilities, extraction of foreign body was postponed and persistent hypoxia led to irreversible brain damage, seizures and finally death. This report reveals the key role of physicians in prompt diagnosis and the importance of early extraction of aspirated foreign bodies, even in asymptomatic cases, to prevent later complications and related mortality and morbidity.Conclusion: This report contains warning hints for professionals in different fields of medicine that deal with pediatric patients

    Transfusion audit of blood products using the World Health Organization Basic Information Sheet in Qazvin, Islamic Republic of Iran

    We assessed the practicality of using the transfusion Basic Information Sheet (BIS) for data collection, to determine the overall adequacy of physician documentation of blood product transfusion, and to make an audit of the appropriateness of blood product transfusion. The transfusion process and clinical indications for transfusions administered to adult hospitalized patients in 3 tertiary care teaching hospitals in Qazvin were prospectively reviewed. Adequate documentation was achieved in 62.6 of all transfusion episodes, range 41?73, depending on the medical specialty; 15.7 of red blood cells and whole blood requests, 40.8 of platelet requests and 34.1 of fresh frozen plasma requests were inappropriate. BIS-based information along with data collection can be used to provide feedback regarding the effectiveness of and compliance with local and national transfusion guidelines

    Total Reconstruction of Lower Lip and Chin Following Firework Injury Using Composite Bilateral Radial Forearm-Fascia Lata Flaps: A Case Report

    BACKGROUND: Primary explosion injuries with fireworks can lead to devastating and geometrically complex facial traumas that present a challenge to the reconstructive surgeon. Our patient, a woman in her early thirties, was hit directly in her chin by a large artillery shell firework. This caused complete soft tissue loss of the lower lip and chin beyond the oral commissures, complicated further by a comminuted mandible fracture. METHODS: After external fixation, our patient underwent a 2-stage reconstruction with a novel composite flap arrangement. Soft tissue coverage and lip reconstruction were performed with opposing bilateral radial forearm free flaps. The outer flap constituted the soft tissue of the new chin and outer lower lip, whereas the inner flap composed the intraoral lining. In the second stage, portions of the inner upper lip mucosa and superior orbicularis oris muscle were flipped down as a bipedicle, axial pattern bucket-handle type flap to the lower lip to reconstruct the vermilion. A graft of fascia lata was attached to the modioli of the orbicularis oris and interpositioned beneath the vermilion flap and the radial forearms to restore static and some dynamic sphincter control. One month later, the mandibular fractures underwent open reduction and internal fixation. RESULTS: Two months after soft tissue reconstruction with no complications, our patient had satisfactory aesthetic outcomes, oral competence, and speech. CONCLUSIONS: This case has shown that use of bilateral, fascia lata-reinforced radial forearm flaps may be an effective choice for soft tissue reconstruction and oral competence restoration in cases of severe facial explosion trauma

    Helicobacter pylori infection is a significant factor risk for hyperhomocysteinemia in the patients with coronary artery disease

    This work aimed to determine whether seropositivity to Helicobacter pylori infection was an independent risk factor for hyperhomocysteinemia patients with cardiovascular disease. The H. pylori IgG, IgA and homocystein levels in 96 patients with cardiovascular disease and 64 participants free of cardiovascular disease as control subjects were determined by ELISA assay. The results showed that seropositivity to H. pylori IgG and IgA levels of coronary artery disease (CAD)patients was significantly higher than the controls and CAD patients with H. pylori IgG and IgA negative antibodies. A significant correlation was found between the seropositivity to H. pylori IgG and homocysteine levels of CAD patients in comparison with the controls and CAD patients with seronegativity to H. pylori IgG and IgA (r = 0.233, P = 0.019). The involvement of H. pylori infection in atherosclerosis process was based on the chronic inflammation, which might facilitate the CAD-related pathologies. The effect of the presence of H. pylori infection on homocysteine levels elevation in the CAD patients (as a risk factor independent of other traditional factors) was remarkable
