    Cannabinoid-Mediated Inhibition of Recurrent Excitatory Circuitry in the Dentate Gyrus in a Mouse Model of Temporal Lobe Epilepsy

    Temporal lobe epilepsy (TLE) is a neurological condition associated with neuron loss, axon sprouting, and hippocampal sclerosis, which results in modified synaptic circuitry. Cannabinoids appear to be anti-convulsive in patients and animal models of TLE, but the mechanisms of this effect are not known. A pilocarpine-induced status epilepticus mouse model of TLE was used to study the effect of cannabinoid agonists on recurrent excitatory circuits of the dentate gyrus using electrophysiological recordings in hippocampal slices isolated from control mice and mice with TLE. Cannabinoid agonists WIN 55,212-2, anandamide (AEA), or 2-arachydonoylglycerol (2-AG) reduced the frequency of spontaneous and tetrodotoxin-resistant excitatory postsynaptic currents (EPSCs) in mice with TLE, but not in controls. WIN 55,212-2 also reduced the frequency of EPSCs evoked by glutamate-photolysis activation of other granule cells in epileptic mice. Secondary population discharges evoked after antidromic electrical stimulation of mossy fibers in the hilus were also attenuated by cannabinoid agonists. Agonist effects were blocked by the cannabinoid type 1 receptor (CB1R) antagonist AM251. No change in glutamate release was observed in slices from mice that did not undergo status epilepticus. Western blot analysis suggested an up-regulation of CB1R in the dentate gyrus of animals with TLE. These findings indicate that activation of CB1R present on nerve terminals can suppress recurrent excitation in the dentate gyrus of mice with TLE. This suggests a mechanism for the anti-convulsive role of cannabinoids aimed at modulating receptors on synaptic terminals expressed de novo after epileptogenesis

    Cage fish farmers across the coastal districts of Kerala suffered massive losses in the floods of August, 2018. The flooding and crop loss was also a setback to efforts of ICAR-CMFRI to popularize cage fish farming. The flood waters which completely washed many cages along with the cultured fishes meant loss was in terms of cage structure and nets, harvest ready fishes and juvenile fishes stocked for new cropping in cages.     Ethnic differences in guideline-indicated statin initiation for people with type 2 diabetes in UK primary care, 2006-2019: A cohort study.

    BACKGROUND: Type 2 diabetes is 2-3 times more prevalent in people of South Asian and African/African Caribbean ethnicity than people of European ethnicity living in the UK. The former 2 groups also experience excess atherosclerotic cardiovascular disease (ASCVD) complications of diabetes. We aimed to study ethnic differences in statin initiation, a cornerstone of ASCVD primary prevention, for people with type 2 diabetes. METHODS AND FINDINGS: Observational cohort study of UK primary care records, from 1 January 2006 to 30 June 2019. Data were studied from 27,511 (88%) people of European ethnicity, 2,386 (8%) people of South Asian ethnicity, and 1,142 (4%) people of African/African Caribbean ethnicity with incident type 2 diabetes, no previous ASCVD, and statin use indicated by guidelines. Statin initiation rates were contrasted by ethnicity, and the number of ASCVD events that could be prevented by equalising prescribing rates across ethnic groups was estimated. Median time to statin initiation was 79, 109, and 84 days for people of European, South Asian, and African/African Caribbean ethnicity, respectively. People of African/African Caribbean ethnicity were a third less likely to receive guideline-indicated statins than European people (n/N [%]: 605/1,142 [53%] and 18,803/27,511 [68%], respectively; age- and gender-adjusted HR 0.67 [95% CI 0.60 to 0.76], p < 0.001). The HR attenuated marginally in a model adjusting for total cholesterol/high-density lipoprotein cholesterol ratio (0.77 [95% CI 0.69 to 0.85], p < 0.001), with no further diminution when deprivation, ASCVD risk factors, comorbidity, polypharmacy, and healthcare usage were accounted for (fully adjusted HR 0.76 [95% CI 0.68, 0.85], p < 0.001). People of South Asian ethnicity were 10% less likely to receive a statin than European people (1,489/2,386 [62%] and 18,803/27,511 [68%], respectively; fully adjusted HR 0.91 [95% CI 0.85 to 0.98], p = 0.008, adjusting for all covariates). We estimated that up to 12,600 ASCVD events could be prevented over the lifetimes of people currently affected by type 2 diabetes in the UK by equalising statin prescribing across ethnic groups. Limitations included incompleteness of recording of routinely collected data. CONCLUSIONS: In this study we observed that people of African/African Caribbean ethnicity with type 2 diabetes were substantially less likely, and people of South Asian ethnicity marginally less likely, to receive guideline-indicated statins than people of European ethnicity, even after accounting for sociodemographics, healthcare usage, ASCVD risk factors, and comorbidity. Underuse of statins in people of African/African Caribbean or South Asian ethnicity with type 2 diabetes is a missed opportunity to prevent cardiovascular events

    RETINAL CHANGES IN PREGNANCY INDUCED-HYPERTENSION

    Objective: The aim is to determine the prevalence of retinal changes in Pregnancy Induced Hypertension (PIH) and its association with age, parity, proteinuria, blood pressureand severity of PIH. Methods: This is a tertiary hospital based Cross-Sectional study including all patients, clinically diagnosed with Pregnancy Induced Hypertension. General demographic details along with age, gravida, gestation period, proteinuria, blood pressure and severity of PIH were noted. Ophthalmic examination was performed and detailed fundus examination was done. Results: A total of 52 patients were included in this study. Their mean age was 24.9 y; 32 out of 52 patients were primigravida (61%) and 20 patients were multi gravida (39%). Out of 52 patients 34 (65.38%) had Gestational Hypertension, 16 (30.76%) had preeclampsia, and 2 (3.8%) had eclampsia. 3 (5.76%) out of 29 patients with BP&lt;160/100 mmHg and 10 (19.23%) out of 23 patients with BP&gt;160/100 mmHg had developed Hypertensive Retinopathy. Hypertensive Retinopathy was seen in 13 (25%). Proteinuria was seen in 10 (19.23%) patients ranging between+to+++on the dipstick. Conclusion: Statistically significant correlation between retinal changes and proteinuria, blood pressure and severity of Disease.The most important requisite in a case of Pregnancy Induced Hypertension is Fundoscopic examination of retina. The retinal vessels during PIH form a gateway to visualize changes in the body and placental vessels, and may play a key role in early detection and treatment of PIH for protection of the mother as well as the fetus

    EFFECT OF MYOPIA ON PRIMARY OPEN ANGLE GLAUCOMA

    Objective: It is a retrospective study to evaluate the effect of myopia on primary open-angle glaucoma by classifying the eyes into NMG (non-myopic glaucoma), HMG (highly myopic glaucoma) And MMG (mild to moderate myopic glaucoma). Methods: The study was performed on 120 patients with primary open-angle glaucoma who were medically treated. Any participant who had surgery done was excluded from the study. The relation between glaucoma and different myopia and progression were assessed on the basis of age, gender, risk factors. Results: Out of the 120 cases assessed, 46 [38 percent] were female and 74 [62 percent] were males. On the basis of age there were 9 cases between 31-40 y, 30 cases between the ages 41-50, 40 cases between 51-60, 32 cases between 61-70 and 9 cases between 71-80, on the basis of myopia 59 [49 percent] were NMG 47 [39 percent] were MMG and 14 [12 percent] were HMG. On the basis of risk factors,12 of them had Diabetes Mellitus, 7 had Hypertension, 7 had a history of steroid use, 3 had a history of migraines and 6 of them had a family history of glaucoma. In the observed one year period 73 percent if the cases were not progressive while 27 percent were progressive. In this study, it has been observed that the males are more commonly affected and the age group with the most cases was the 51 to 60 age group. The majority of the cases showed no risk factors though Diabetes Mellitus is the most common. The progression of the disease is seen more frequently in cases associated with Diabetes Mellitus and Hypertension. In MMG 12 out of the 47 cases were progressive and in NMG 15 out of the 59 cases were progressive. Conclusion: Though high myopia is important in the pathogenesis of glaucoma there was no evidence that high myopia increases the progression of the disease of the 14 cases, only 5 were progressive

    In this study, variability of two oceanic cyclogenesis metrics, tropical cyclone heat potential (TCHP) and effective oceanic layer for cyclogenesis (EOLC) in the Bay of Bengal (BoB) during NARGIS cyclone is investigated. EOLC represents the geopotential thickness of near surface stratified layer forms because of the spread of low salinity waters due to fresh water influx from rivers and precipitation. Climatological fields of TCHP and EOLC reveal that NARGIS translated towards the region of higher EOLC as seen in the observations. A maximum daily sea surface cooling of 2 °C is observed along the right and rear side of NARGIS track

    All-optical spiking neurosynaptic networks with self-learning capabilities

Yet, unlike real neural tissue, traditional computing architectures physically separate the core computing functions of memory and processing, making fast, efficient and low-energy computing difficult to achieve. To overcome such limitations, an attractive alternative is to design hardware that mimics neurons and synapses. Such hardware, when connected in networks or neuromorphic systems, processes information in a way more analogous to brains. Here we present an all-optical version of such a neurosynaptic system, capable of supervised and unsupervised learning. We exploit wavelength division multiplexing techniques to implement a scalable circuit architecture for photonic neural networks, successfully demonstrating pattern recognition directly in the optical domain.     ASSESSMENT OF THE INCIDENCE AND PREVALENCE OF DRY EYES IN DIABETIC INDIVIDUALS AND IT’S EFFECT ON VISUAL ACUITY

    Objective: To find out the incidence and prevalence of Dry Eyes in Diabetic patients and find out its effect on visual acuity. Methods: A prospective study was conducted in Saveetha Medical College and Hospital. Schirmer’s test was done for 100 diabetic patients taking part in the research. Those with dry eyes were tested for their visual acuity and were asked to fill a questionnaire based on associated ocular symptoms. The collected data were analysed using Microsoft Excel. Results: There is about an incidence of 82% of dry eyes in diabetic patients. It’s more prevalent among elderly women. There seems to be a significant effect of dry eys on visual acuity as 31% of those tested with dry eyes have a moderate visual impairment. Conclusion: People with diabetes are at a higher risk of developing dry eye. So diabetic patients should undergo regular ocular examination and must be screened for dry eyes. Early diagnosis and treatment of dry eyes is the best choice to prevent ocular complications in the future

    COMPARISON OF POSTOPERATIVE CORNEAL ASTIGMATISM IN PHACOEMULSIFICATION AND MANUAL SMALL INCISION CATARACT SURGERY

    Objective: Globally a significant proportion of treatable blindness is caused by cataract, especially in India and southeast Asia. Treatment of cataract is surgical correction with intraocular lens implantation. The main drawback of surgical correction is induction of postoperative astigmatism in patients. The aim of this study was to compare the degree of astigmatism in manual small incision cataract surgery and phacoemulsification 3 mo postoperatively Methods: The study was a retrospective case study on postoperative corneal astigmatism after cataract surgery. It was conducted in a tertiary care hospital in Thandalam, Tamil Nadu. A total of 100 patients were selected and divided into two groups, group A (=50) underwent phacoemulsification and group B (=50) underwent manual small incision cataract surgery. Preoperative astigmatic status of the patients was noted from patient records. Both groups were evaluated 3 mo postoperatively using automated keratometry. The data collected was analyzed using Microsoft Excel Independent T test, p&lt;0.05 was considered statistically significant. Power of the study was 80% with an alpha error of 5%. Results: Mean postoperative astigmatism at 3 mo was 0.91±0.255D and 0.34±0.110D due to manual small incision cataract surgery and phacoemulsification, respectively. Conclusion: Postoperative astigmatism was greater in manual small incision cataract surgery than phacoemulsification. Improvement in preoperative astigmatism was seen in patients who underwent superotemporal incision phacoemulsificatio
