    Influencia da refundição de liga de paladio-prata na desadaptação cervical e interna de coroas totais metalicas

Este trabalho propõe avaliar os desajustes cervical e interno de coroas totais metálicas fundidas com liga de Pd-Ag (Pors-on 4, Degussa S/A), sob influência da refundição e de diferentes configurações marginais (ombro reto, ombro biselado em 20° e chanfro reto em 45°), com a fonte de calor de fundição gás-oxigênio. Foram confeccionados 30 troquéis de alumínio, sendo 10 para cada tipo de término cervical, sobre os quais foram encerados coroas com auxílio de uma matriz cilíndrica metálica bipartida, com o propósito de padronizar a espessura das paredes. Os padrões de cera foram incluídos em revestimento aglutinado por fosfato (Deguvest) e a fundição realizada após aquecimento em forno (EDG). Posteriormente, as coroas metálicas foram assentadas nos respectivos troquéis, sob carga estática de 9 kgf, e a leitura do desajuste cervical realizada com um microscópio metrológico (Carl Zeiss), provido de um relógio comparador digital (Mitutoyo, série 543 IDF), com precisão de 0,001 mm. Em seguida, as amostras foram incluídas em resina de poliéster e após a polimerização, seccionadas ao meio no sentido longitudinal, para as leituras da adaptação interna, efetuadas da mesma maneira como descrito para a adaptação cervical. Os resultados foram submetidos à análise de variância e ao teste de Tukey (5%) e demonstraram que a liga virgem apresentou melhor adaptação, tanto cervical quanto interna, quando comparada com a liga refundida; o tipo de término não exerceu influência nos desajustes, quando a coroa foi fundida com liga virgem. Para a liga refundida, o término em ombro reto demonstrou valores de adaptação estatisticamente superiores ao ombro biselado em 20°, sendo que o chanfro em 45° não diferiu de ambosAbstract: The metal recast process is traditionally used by dental laboratories to produce prosthesis and restorations. The aim of this study was to evaluate the cervical and internal fits of total metal crowns, which were casted and recasted with palladium-silver alloy (Pors-on 4, Degussa S/A), with three different marginal configuration: straight shoulder, 20-degree bevel shoulder and 45-degree chamfered. The oxigen-gas heat source was used to melt the alloy. Thirty aluminium dies were made, 10 for each margin configuration. The dies were waxed using a cylindrical metal matrix to standardize the thickness of the walls. The wax patterns were invested in a phosphate investment (Deguvest) and cast after heated in an oven (EDG), according to the manufacturer's instruction. Afterwards, the specimens were seated on the dies, with a static load of 9 kgf. The marginal discrepancy was measured with a metric microscope (Carl Zeiss), using a digital micrometer (Mitutoyo, 543 IDF series), with accuracy of 0,001 mm. The samples were embedding in polyester resin and longitudinally cut for internal discrepancy measurements, measured by the same way made in the marginal discrepancy. The results were submitted to analysis of variance and Tuckey's test (5%) and indicated that the new metal presented better adaptation than the recast one for both marginal and internal discrepancy measurements. The margin designs didn't have any influence when new metal were used.     Interproximal space recovery using an orthodontic elastic separator before prosthetic restoration: a case report

    The presence of a dental contact point is essential to maintain normal occlusion and to preserve the health of periodontal structures. Occasionally dental caries could compromise the interproximal contact point, leading to migration of the adjacent teeth with consequent retention of food, gingival inflammation, pocket formation, bone loss and dental mobility. In order to achieve an adequate restorative treatment, reestablishment of interproximal space is necessary. This report describes the interproximal space recovery between the maxillary first and second molars of a 45-year-old female patient before tooth reconstruction. In this case, an orthodontic elastic separator was indicated as an alternative to fixed orthodontic appliances in order to recover the necessary space. The technique was proven efficient, effective, economical, easy to perform and, above all, less invasive than other methods. The interproximal space was regained and a ceramic onlay was cemented for restoring the maxillary second molar. Periodical clinical and radiographic follow-up should be maintained to ensure long-term success.A presença dos pontos de contato dentais é essencial para a manutenção da oclusão habitual e da saúde das estruturas periodontais. Ocasionalmente as cáries dentárias podem comprometer o ponto de contato interproximal, induzindo a migração de dentes adjacentes com conseqüente retenção de alimento, inflamação gengival, formação de bolsa periodontal, perda óssea e mobilidade dental. A fim de realizar um tratamento restaurador adequado, o restabelecimento do espaço interproximal é necessário. O presente relato descreve a recuperação do espaço interproximal entre molares de uma paciente de 45 anos de idade antes da reconstrução dentária. Neste caso, um elástico ortodôntico separador foi indicado como alternativa aos dispositivos ortodônticos fixos no restabelecimento do espaço necessário. A técnica utilizada provou ser eficiente, efetiva e econômica, e principalmente menos invasiva e fácil de ser realizada. Contudo para que o sucesso clínico se estabeleça ao longo do tempo, faz-se necessário um controle clínico e radiográfico

    Estudo comparativo do tipo de substrato dental utilizado em testes de resistência de união ao cisalhamento

    The purpose of this study was to compare shear bond strength values obtained in human enamel and dentin with the values obtained in bovine teeth using two adhesive systems with different actions. Forty human tooth half-crowns and forty bovine tooth crowns were flattened to a minimum plain area of 5 mm in diameter. The samples were divided in four groups of 20 specimens each: 1) human enamel; 2) bovine enamel; 3) human dentin; 4) bovine dentin. The samples of each group were divided in 2 subgroups of 10 samples each, according to the adhesive system used: 1) Scotchbond Multi-Purpose (SBMP); and 2) Clearfil Liner Bond 2V (CLB2V) applied according to the manufacturer's recommendations. Afterwards, restorations of Z100 composite with cylindrical shape (4 mm diameter x 5 mm height) were made using a metallic mold to submit the samples to shear bond testing on an Instron universal testing machine, at a crosshead speed of 0.5 mm/min. The data were submitted to ANOVA and Tukey's test (5%). In enamel, there was no statistical difference between bovine and human teeth for SBMP (7.36 MPa, human; 8.24 MPa, bovine), nor for CLB2V (10.01 MPa, human; 7.95, bovine). In dentin, SBMP showed a statistically lower mean on human dentin (7.01 MPa) than on bovine dentin (11.74 MPa). For CLB2V, there was no statistical difference between human (7.43 MPa) and bovine (9.27 MPa) substrates.O propósito deste estudo foi comparar os valores de resistência de união sobre esmalte e dentina humanos com os valores obtidos em dentes bovinos, utilizando dois sistemas de união com princípios de atuação distintos. Para isso, desgastaram-se 20 meias coroas dentais humanas e 40 coroas bovinas, até obter uma área plana de pelo menos 5 mm de diâmetro. As 80 amostras foram separadas em 4 grupos de 20 amostras cada, sendo: 1) dente humano em esmalte, 2) dente bovino em esmalte, 3) dente humano em dentina, 4) dente bovino em dentina. As amostras de cada grupo foram divididas em 2 subgrupos de 10 amostras cada, de acordo com o sistema de união utilizado: 1) Scotchbond Multi-Uso (SBMU); e 2) Clearfil Liner Bond 2V (CLB2V). Em seguida, confeccionou-se um cilindro do compósito Z100 (4 mm de diâmetro x 5 mm de altura) utilizando-se uma matriz bipartida, para submeter os corpos-de-prova ao ensaio de cisalhamento numa máquina de ensaio Instron com velocidade de 0,5 mm/min. Os dados foram submetidos à análise de variância e as médias, ao teste de Tukey (5%). Em esmalte, não se verificou diferença estatística entre os dentes humanos e bovinos para os materiais SBMU (7,36 MPa, humano; e 8,24 MPa, bovino) e CLB2V (10,01 MPa, humano; e 7,95 MPa, bovino). Verificou-se que o SBMU apresentou média estatisticamente inferior em dentina humana (7,01 MPa), quando comparado à dentina bovina (11,74 MPa). Para o material CLB2V, não houve diferença estatística entre os substratos humano (7,43 MPa) e bovino (9,27 MPa)

    Rubbing time and bonding performance of one-step adhesives to primary enamel and dentin

    Objectives: This study investigated whether increasing the concentration of acidic monomers in one-step adhesives would allow reducing their application time without interfering with the bonding ability to primary enamel and dentin. Material and methods: Experimental one-step self-etch adhesives were formulated with 5 wt% (AD5), 20 wt% (AD20), or 35 wt% (AD35) acidic monomer. The adhesives were applied using rubbing motion for 5, 10, or 20 s. Bond strengths to primary enamel and dentin were tested under shear stress.     CONDIÇÕES E PRÁTICAS DE SAÚDE BUCAL EM PACIENTES COM DEFICIÊNCIA VISUAL- REVISÃO DE LITERATURA

    The literature shows that many visually impaired people do not receive adequate guidance on hygiene techniques and this is a worrying fact, since these patients often need special help to learn how to use a toothbrush and floss. The aim of this study was to evaluate the oral health status and practices of visually impaired patients by means of a literature review. A bibliographic review of the literature was carried out in the PubMed Central, Virtual Health Library (VHL), Scopus and Google Scholar databases, using descriptors specific to health education strategies for visually impaired patients, as well as the oral conditions of these patients, with no publication date limit. All articles published in English, Spanish or Portuguese were selected. Out of a total of 80 articles, 5 were eliminated for repetition, 10 for their title and abstract. Of the remaining 65, 15 were excluded because they did not address the data against the inclusion criteria. Thus, 50 articles were selected to form part of this review. Based on the review presented, we can conclude that many visually impaired people do not receive adequate guidance on hygiene techniques and the difficulties in learning and maintaining oral hygiene for these individuals may be associated mainly with a lack of adequate stimulation.La bibliografía muestra que muchos discapacitados visuales no reciben una orientación adecuada sobre técnicas de higiene, lo cual es preocupante, ya que estos pacientes suelen necesitar ayuda especial para aprender a utilizar el cepillo de dientes y el hilo dental. El objetivo de este estudio era, por tanto, evaluar el estado y las prácticas de salud bucodental de los pacientes con discapacidad visual mediante una revisión bibliográfica. Se realizó una revisión bibliográfica en las bases de datos PubMed Central, Virtual Health Library (BVS), Scopus y Google Scholar, utilizando descriptores específicos sobre estrategias de educación sanitaria para pacientes con discapacidad visual, así como sobre las condiciones bucodentales de estos pacientes, sin límite de fecha de publicación. Se seleccionaron todos los artículos publicados en inglés, español o portugués. De un total de 80 artículos, 5 fueron eliminados por repetición, 10 por su título y resumen. De los 65 restantes, 15 fueron excluidos porque no abordaban los datos según los criterios de inclusión. Así, 50 artículos fueron seleccionados para formar parte de esta revisión. A partir de la revisión presentada, podemos concluir que muchas personas con deficiencia visual no reciben orientación adecuada sobre técnicas de higiene y las dificultades en el aprendizaje y mantenimiento de la higiene bucal de estos individuos pueden estar asociadas principalmente a la falta de estimulación adecuada.A literatura demonstra que muitos deficientes visuais não recebem orientações adequadas sobre técnicas de higienização e este é um fato preocupante, visto que frequentemente estes pacientes necessitam de ajuda especial para aprender a utilizar a escova e o fio dental. Sendo assim, o objetivo desta pesquisa é avaliar por meio de uma revisão bibliográfica, a condição e prática de saúde bucal nos pacientes com deficiência visual. Foi realizada revisão bibliográfica nas bases de dados PubMed Central, Biblioteca Virtual de Saúde (BVS), Scopus e Google acadêmico, através de descritores específicos à estratégias educacionais em saúde para pacientes com deficiência visual, bem como as condições bucais desses pacientes, sem limite de data de publicação. Foram selecionados todos os artigos publicados em idioma inglês, espanhol ou português. Em um total de 80 artigos, 5 foram eliminados por repetição, 10 pelo título e resumo. Dos 65 restantes, 15 foram excluídos por não abordarem os dados frente aos critérios de inclusão. Assim, foram selecionados 50 artigos para fazer parte desta revisão.     Anterior aesthetic rehabilitation associating periodontal surgery with ceramic laminates: case report

    Objetivo: Este trabalho teve como objetivo relatar um caso de reabilitação estética através de laminados cerâmicos e cirurgia periodontal como forma de tratamento para a agenesia do elemento dental 22 e sorriso gengival associado à retração labial. Relato do Caso: Paciente, gênero feminino procurou a Clínica Multidisciplinar da Unipar (Sede/Umuarama) relatando insatisfação com a harmonia estética do sorriso, apresentando coroas curtas e agenesia do elemento dental 22. Com base na anatomia do contorno gengival, foi indicada cirurgia de gengivectomia (bisel interno com osteotomia) para a região ântero-superior. Através do enceramento diagnóstico projetou-se o aumento de coroa clínica, obtendo-se assim uma guia, que foi utilizado para a realização do ensaio diagnóstico intrabucal, mock-up, e auxílio na cirurgia periodontal. Após 60 dias a paciente foi encaminhada para a realização dos procedimentos restauradores com a confecção de laminados cerâmicos. Conclusão: Pôde-se concluir através dos resultados clínicos obtidos, que a atuação multidisciplinar devolveu a harmonia estética do sorriso e melhora na qualidade de vida da paciente.Objective: The aim of this study was to report a case of aesthetic rehabilitation through laminates and periodontal surgery as a form of treatment for the dental element agenesis 22 and gummy smile associated with lip retraction. Case Report: Patient, female gender, came to the Clinic Multidisciplinary of Unipar (head office/Umuarama) reporting dissatisfaction with the smile aesthetic harmony, with short crown and dental element agenesis. Based on gingival contour anatomy, a gingivectomy surgery (inner bevel with osteotomy) was indicated for the anterossuperior region. The increase in clinical crown was projected by the termination of diagnosis, thereby a guide, that it was used for the realization of the intra-oral diagnostic test mock-up, and support at the periodontal surgery. After 60 days the patient referred for the performance of restorative procedures with the making of laminates. Conclusion: It was concluded through clinical results obtained, that the multidisciplinary action gave her the smile aesthetic harmony back and improvement in quality of life

    Characterization Of Morphology And Composition Of Inorganic Fillers In Dental Alginates.

    Energy dispersive X-ray spectroscopy microanalysis (EDX), scanning electron microscopy (SEM), and Archimedes' Principle were used to determine the characteristics of inorganic filler particles in five dental alginates, including Cavex ColorChange (C), Hydrogum 5 (H5), Hydrogum (H), Orthoprint (O), and Jeltrate Plus (JP). The different alginate powders (0.5 mg) were fixed on plastic stubs (n = 5) and sputter coated with carbon for EDX analysis, then coated with gold, and observed using SEM. Volume fractions were determined by weighing a sample of each material in water before and after calcining at 450(°)C for 3 h. The alginate materials were mainly composed of silicon (Si) by weight (C-81.59%, H-79.89%, O-78.87%, H5-77.95%, JP-66.88%, wt). The filler fractions in volume (vt) were as follows: H5-84.85%, JP-74.76%, H-70.03%, O-68.31%, and C-56.10%.     Effect of different surface penetrating sealants on the roughness of a nanofiller composite resin

    This study evaluated the effectiveness of different sealants applied to a nanofiller composite resin. Forty specimens of Filtek Z-350 were obtained after inserting the material in a 6x3 mm stainless steel mold followed by light activation for 20 s. The groups were divided (n=10) according to the surface treatment applied: Control group (no surface treatment), Fortify, Fortify Plus and Biscover LV. The specimens were subjected to simulated toothbrushing using a 200 g load and 250 strokes/min to simulate 1 week, 1, 3 and 6 months and 1 and 3 years in the mouth, considering 10,000 cycles equivalent to 1 year of toothbrushing. Oral-B soft-bristle-tip toothbrush heads and Colgate Total dentifrice at a 1:2 water-dilution were used. After each simulated time, surface roughness was assessed in random triplicate readings. The data were submitted to two-way ANOVA and Tukey's test at a 95% confidence level. The specimens were observed under scanning electron microscopy (SEM) after each toothbrushing cycle. The control group was not significantly different (p&gt;0.05) from the other groups, except for Fortify Plus (p<0.05), which was rougher. No significant differences (p&gt;0.05) were observed at the 1-month assessment between the experimental and control groups. Fortify and Fortify Plus presented a rougher surface over time, differing from the baseline (p<0.05). Biscover LV did not differ (p&gt;0.05) from the baseline at any time. None of the experimental groups showed a significantly better performance (p&gt;0.05) than the control group at any time. SEM confirmed the differences found during the roughness testing.
