    Evaluation of the Effectiveness of Bevacizumab Treatment in Patients with Metastatic Colorectal Cancer with PET-CT

    Purpose: To evaluate the response to therapy with PET-CT in metastatic colorectal cancer patients treated with Bevacizumab and chemotherapy.Methods: Twenty-two patients with metastatic colorectal cancer that were treated with bevacizumab and 6 cycles of chemotherapy were evaluated by whole-body PET-CT scan before and after the treatment in accordance with the European Organization for Research and Treatment of Cancer criteria.Results: While 31.8% of patients responded to treatment (complete response + partial response), 68.2% did not respond to treatment (stable disease + progressive disease). The mean hepatic, extra hepatic, abdomen, lung and bone metastases SUVmax values were higher after treatment in comparison to the pre-treatment values. There was an increase in SUVmax values in those who did not respond to the treatment, while a decrease was observed in those who responded to the treatment. Survival was significantly increased in all patients that responded to the treatment. The difference in terms of gender, histological subtype, histological grade, primary tumor location, presence of metastases in regional lymph nodes and liver at the time of diagnosis or the response to the treatment was not statistically significant.Conclusions: In this study we detected metabolic response before anatomical response with PET-CT in one third of metastatic colorectal cancer patients treated with Bevacizumab and chemotherapy. This finding suggests that PET-CT may be used as a measure to follow therapy response and predict the prognosis in metastatic colorectal cancer patients

    Evaluating solid waste landfill site selection using multi-criteria decision analysis and geographic information systems in the city of Elazig, Turkey

    WOS: 000498891200008Solid waste landfill site selection is a relatively complex process and depends on several criteria and official regulations. In this study, the suitability of solid waste landfill site selection for the city of Elazig, Turkey, was determined by Multi-Criteria Decision Analysis (MCDA) and Geographic Information Systems (GIS). For this purpose, nine different criteria, namely distance from residential areas, distance from roads, distance from rivers, distance from spring wells, geology, slope, aspect, land capability classification and land use, were taken into consideration and their degrees of relative importance were defined via MCDA and the schematic maps for each criterion were prepared with GIS applications. The final landfill site suitability map for the study area was obtained by the overlay analysis tool of ArcGIS 9.3. The study results reveal that the landfill suitability index values for the selected site range between 2.64 and 6.10. The major part of the landfill site has relatively low index values implying that the selected site is suitable for solid waste landfill

    14th Turkish National Software Engineering Symposium (UYMS) -- OCT 07-09, 2020 -- ELECTR NETWORKIn the internet advertising industry, web and mobile applications that display ads need to choose high-paying ads to increase their revenue. Ad mediators create various decision mechanisms to select ads that will generate higher revenues in order to increase the revenue of advertising applications. One type of these decision mechanisms is to select and deliver the ad with the highest eCPM (Effective Cost Per Mille) value from ads that can be placed in an ad slot. The eCPM value varies depending on different external factors for different applications. It is not possible for domain experts to make successful predictions by analyzing different sets of external factors for many applications and to keep these predictions constantly updated. Therefore, eCPM values were automatically predicted separately for each application on different ad slots and different countries using time series analysis and machine learning algorithms. SARIMA, MLP, CNN and LSTM algorithms are used to make predictions. The LSTM algorithm has generally yielded better results in eCPM estimation.     The aim of this paper is to demonstrate the unusual MR features of thoracic syringomyelia following TB meningitis and to discuss the neurosurgical aspect of the treatment of this rare entity. Four years after a TB meningitis episode, a 30 year-old female patient developed a progressive spastic paraparesis. MR studies revealed multiloculated syrinxes throughout the thoracic cord. She had a syringo-subarachnoid shunt with a silastic "T" tube inserted. On the first postoperative day, she showed a dramatic neurological improvement, but unfortunately her paraparesis progressed to the preoperative level within a month despite diminished size of the syrinxes on the control MRI examination. Two and a half years after the operation the patient complained of having a burning type of central pain, and further deterioration in neurological function. Thoracic spinal MRI examination demonstrated enlarged syringomyelic cavities. At the second operation syringo-peritoneal shunt insertion was performed via right T10-11 hemilaminectomy using a "T" tube. At present, 4 months after the second operation, the patient's neurological examination demonstrated decreased spasticity, and improved strength in the legs compared to the preoperative level. MRI is the first choice of investigation in detecting TB related myelopathy as it provides a greater detail of pathological changes within and around the spinal cord such as syrinx formation and arachnoiditis. The MR findings are also helpful in deciding the management and predicting the outcome. Presence of multifocal loculations and arachnoid adhesions is the likely cause of treatment failures and poor prognosis

    Unexpected synchronous diagnosis of papillary thyroid carcinoma in a patient with lower lip squamous cell carcinoma

    The presentation of differentiated thyroid carcinoma occurring as a second primary associated with head and neck squamous cell carcinoma is quite unusual. Here, we present an unexpected synchronous diagnosis of two independent head and neck malignancies. A 78-year-old male with a rapidly growing, firm, and ulcerated lesion on the lower lip was referred to our clinic. Positron emission tomography-computed tomography scan performed for systemic screening demonstrated a dense hypermetabolic activity located in the right thyroid lobe, suggesting a second malignancy. Fine-needle aspiration cytology of the right thyroid gland performed to differentiate the presence of a metastatic lesion, or primary thyroid cancer was compatible with malign cytology related to papillary thyroid carcinoma. Multiple malignancies may be diagnosed synchronously in the head and neck region and these patients with multiple malignant lesions should be evaluated carefully before being accepted as metastatic
