Relationship between maternal blood ceruloplasmin level, catalase and myeloperoxidase activity and neural tube defects
Objectives: The exact pathogenesis of neural tube defects (NTDs) is poorly understood. We aimed at evaluating maternal anti-oxidant capacity (ceruloplasmin level, myeloperoxidase and catalase activity) in pregnancies complicated by NTDs.
Material and methods: Fifty-four mothers with NTD-affected pregnancies and 61 healthy mothers, matched for gestational age, were recruited. Maternal venous blood samples were obtained after detailed fetal ultrasound examination to measure myeloperoxidase, catalase activity and ceruloplasmin levels. The clinical characteristics of all participants were collected.
Results: Maternal blood catalase activity was significantly lower in the study group (117.1 ± 64.8 kU/L) as compared to controls (152.2 ± 110.6 kU/L) (p = 0.044). Maternal blood ceruloplasmin levels were also significantly lower in the study group (180.5 ± 37.7 U/L) as compared to controls (197.9 ± 35.9 U/L) (p = 0.012). Myeloperoxidase activity was similar in both groups (112.6 ± 22.2 U/L vs. 113.6 ± 38.1 U/L) (p = 0.869).
Conclusions: In the present study, maternal blood ceruloplasmin level and catalase activity were found to be lower in NTD-affected pregnancies as compared to healthy controls. Thus, it seems safe to conclude that impaired antioxidant capacity may play a role in the development of NTDs during pregnancy, in addition to the genetic, environmental and metabolic factors

AbstractThiol groups are important anti-oxidants and essential molecules protecting organism against the harmful effects of reactive oxygen species (ROS). The aim of our study is to evaluate thiol–disulphide homeostasis with a novel recent automated method in patients with localized prostate cancer (PC) before and six months after radical prostatectomy (RP). 18 patients with PC and 17 healthy control subjects were enrolled into the study. Blood samples were collected from the controls subjects and patients before and six months after RP. Thiol–disulphide homeostasis was determined using a recently developed novel method. Prostate-specific antigen (PSA), albumin, total protein, total thiol, native thiol, disulphide and total antioxidant status (TAS) were measured and compared between the groups. Antioxidant effect of Abelmoschus Esculentus against acetaminophen-induced nephrotoxicity: an experimental study
Acetaminophen(APAP) intoxication is an important cause of nephrotoxicity and hepatotoxicity. N-acetylcysteine(NAC) is used in the treatment, but it has some serious side effects. Abelmoschus esculentus(AE) has various benefits as well as antioxidant effects. This study aims to investigate the effect of AE in APAP-induced acute nephrotoxicity. Forty male Wistar rats were divided into five equal groups: Control, AE, APAP, APAP+AE, and APAP+AE+NAC. Significant changes were observed in serum levels of malondialdehyde (MDA), superoxide dismutase (SOD), Neutrophil Gelatinase-Associated Lipocalin(NGAL) and Kidney Injury Molecule-1(KIM-1) after induction with APAP. NGAL and KIM-1 in the AE group remained low compared to those receiving APAP (p=0.022 and p0.001, respectively). When the APAP group was compared with the AE and AE+NAC groups, it was found that even the administration of AE alone significantly decreased NGAL and KIM-1(p=0.036 vs.p=0.029 and p0.001 vs. p0.001, respectively), these results were attributed to the effects of AE on reducing MDA and increasing SOD. Histopathological studies also confirmed these results. These results demonstrated that AE had protective and therapeutic effects on APAP-induced nephrotoxicity. Can The Thiol/Disulfide Imbalance Be A Predictor Of Colchicine Resistance In Familial Mediterranean Fever?
Familial Mediterranean fever (FMF) is a chronic autoinflammatory condition characterized by fever attacks and recurrent polyserositis. Subclinical inflammation that persists during attack-free periods can result in oxidative stress (OS) damage. Thiol groups bind to reactive oxygen radicals and protect cells and tissues from OS damage. The aim of this study was to investigate the relationship between thiol-disulfide balance and colchicine resistance in FMF patients during an attack or attack-free period. A newly developed spectrophotometric method was used to measure native thiol (NT) and disulfide (DS) levels in FMF patients and an age-sex matched group of healthy controls. NT and DS levels were compared in FMF patients 1) with vs. without colchicine resistance; and 2) during an attack (FMF-AP) vs. attack-free period (FMF-AFP). A total of 118 FMF patients and 60 healthy controls were studied. NT (P < 0.001) and total thiol (TT) (P < 0.001) levels in FMF patients were significantly lower compared to healthy controls. NT (P = 0.030) and TT (P = 0.010) levels of FMF-AP patients were significantly lower than that of FMF-AFP patients. FMF-AP patients had significantly higher DS levels than FMF-AFP patients (P = 0.039). Compared to FMF patients without colchicine resistance, elevated levels of DS (P = 0.019) but not NT (P = 0.620) and TT (P = 0.718) were found in those with colchicine resistance. The evaluation of maternal systemic thiol/disulphide homeostasis for the short-term prediction of preterm birth in women with threatened preterm labour: a pilot study
The aim of this study was to investigate maternal systemic thiol/disulphide homeostasis (TDH) for the short-term prediction of preterm birth in women with threatened preterm labour (TPL). This prospective study included 75 pregnant women whose pregnancies were complicated by TPL. Thirty-seven of them delivered within 7 days and 38 of them delivered beyond 7 days. Maternal serum samples were collected at the day of diagnosis and the TDH was measured. The maternal disulphide level was significantly higher in pregnant women who delivered within 7 days (25.0 ± 9.8 μmol/L vs 19.4 ± 9.8 μmol/L, p: .015). The threshold value of 22.1 μmol/L for maternal disulphide level predicted delivery within 7 days with 62.2% sensitivity and 60.5% specificity (area under curve 0.651, confidence interval 0.53–0.78). The likelihood ratios for short cervix (≤25 mm) and maternal disulphide level (≥22 μmol/L) to predict delivery within 7 days was found to be 8.7 and 7.3, respectively. The likelihood ratio of combining two tests to predict delivery within 7 days was found to be 11.4. The maternal TDH, which is an indicator of oxidative stress status in maternal compartment, is disturbed in TPL cases who delivered within 7 days. Elevated maternal disulphide level along with cervical length screening predicts a short latency period in pregnancies with TPL. IMPACT STATEMENT What is already known on this subject? Spontaneous preterm delivery is one of the major complication of pregnancy and the common cause of neonatal morbidity and mortality. Threatened preterm labour (TPL) is also a frequent complaint in obstetric emergency care units in all around the world. Triaging women with TPL is mandatory for planning further management therapies, since the most of them will eventually deliver at term. Only the measurement of cervical length in symptomatic women has moderate accuracy in predicting preterm delivery. Short cervix is described as an independent predictor of preterm delivery in women with TPL, its predictive accuracy as a single measurement is relatively limited. On this account, several potential markers like foetal fibronectin in the cervicovaginal fluid, salivary oestriol, prolactin in vaginal discharge, maternal serum calponin and interleukin-6 in the amniotic fluid were examined to predict preterm delivery in previous studies. However, none of them represented an excessive predictive accuracy like high sensitivity, PPV or NPV. What do the results of this study add? We report a method which has higher diagnostic and predictive performance to identifying TPL women with high risk of preterm delivery. According to the current literature, there are accumulated data about the correlation between oxidative stress (OS) and preterm delivery regardless of the amniotic membrane status. However, it is still debated whether OS is a trigger or a consequence of preterm delivery. Our study provides evidence for the first time that maternal serum thiol/disulphide homeostasis, which is an indicator of OS in maternal compartment, is disturbed in TPL cases who delivered within 7 days. The high disulphide level in maternal serum, along with cervical length measurement (short cervix) accurately predicts a short latency period in TPL cases. What are the implications of these findings for clinical practice and/or further research? This novel test combination (maternal serum disulphide level and cervical length measurement) could be used clinically to triage pregnant women presenting with TPL, avoiding overtreatment, unnecessary hospitalisations and increased medical costs. The future research would be addressed on reducing maternal OS by using new antioxidant treatment strategies to improve perinatal and long-term childhood outcomes
Association of Low Fecal Elastase-1 and Non-Ulcer Dyspepsia
Non-ulcer dyspepsia (NUD) is a term used to define a set of symptoms that are believed to originate from the gastroduodenal region, and no underlying organic, systemic, or metabolic reason can be found. The majority of patients suffer from chronic symptoms although half of the patients report improvement in symptoms with time. The potential role exocrine pancreatic insufficiency in NUD patients has not been clarified yet. We aimed to identify exocrine pancreas function with pancreatic fecal elastase-1 in patients diagnosed with non-ulcer dyspepsia and no typical exocrine pancreatic insufficiency (EPI) symptoms. Thirty-five patients referred to gastroenterology clinics with NUD and 35 people with no dyspeptic symptoms as a control group were included in this prospective study. Non-ulcer dyspepsia patients were classified as group 1 and control subjects classified as group 2. Upper gastrointestinal endoscopies were performed in both groups. Assessment of exocrine pancreatic function was performed by measuring fecal elastase-1 concentration with a commercial ELISA kit using polyclonal antibodies (BioServ Diagnostics) in NUD patients compared to control subjects. Mean fecal elastase-1 levels were significantly lower in group 1 patients compared with group 2 (367.47 ± 43.27; 502.48 ± 50.94 respectively; p = 0.04). The percentage of the patients with EPI was significantly higher in group 1 (p = 0.02). Patients with NUD should be re-evaluated if they do not show satisfactory improvement with treatment. Exocrine pancreatic insufficiency was significantly higher in patients with NUD in our study. Evaluation for the presence of EPI can be a cost effective approach in management of refractory patients during the process of ruling out organic reasons
Abelmoschus esculentus (L.) Moench seed extract alleviates acute acetaminophen induced liver damage in rats
852-860Acetaminophen (N-acetyl-p-aminophenol, APAP), commonly called the ‘paracetamol’, is one of the most regularly utilized medicines, particularly in children. When administered at the recommended doses, it is a safe medication. However, oxidative stress and inflammation caused by exposure to toxic doses lead to centrilobular hepatic necrosis. N-acetylcysteine (NAC) is utilized in the therapy, however it has potential adverse effects. On the other hand, it is known that the seeds of the common vegetable Okra, Abelmoschus esculentus (AE), a herbal product, possess antioxidant and anti-inflammatory qualities. In the present study, we explored whether AE can be used as an alternative to standard NAC therapy without any adverse effect in the treatment of acute APAP induced liver injury. Forty male Wistar rats were placed into five groups: Control, AE, APAP, APAP+AE and APAP+AE+NAC groups. Antioxidants such as native thiol and total thiol were found rised in the APAP group by adding AE (p =0.043 and p =0.028, respectively). Anti-inflammatory indicator IL-10 was also found increased, while marker ALT, which is a sign of hepatotoxicity, got decreased (P=0.005 and P <0.001, respectively). Histologically, AE has been shown to improve worsened congestion (P =0.003), cytoplasmic vacuolization (P =0.01), sinusoidal dilatation (P =0.001), Kupffer cell proliferation (P <0.001) and inflammation (P <0.001). These results suggest that the okra seeds may be a potential therapeutic agent for paracetamol induced hepatotoxicity and it can be attributed to the antioxidant and anti-inflammatory properties
