    Ultrasound-Assisted PSA Catalyzed One-Pot Green Synthesis of Pyrazolyl Pyrrole Derivatives

    A fast, efficient and environmentally benign synthesis of pyrazolyl pyrrole derivatives has been developed by the one-pot multicomponent reaction of an aldehyde, nitroalkane, amine, and enolizable reactant with active C-H group, using Phosphosulfonic acid (PSA) as reusable solid acid catalyst under mild, solvent-free, ultra-sonication conditions. In comparison to the existing conventional methods, this green and efficient protocol provides remarkable advantages such as good to excellent yields, lower reaction time, less cost, easy work-up and solvent-free. © 2020 Author(s).The authors are thankful to Prof. C.     Intracranial-extracranial meningioma mimicking an aggressive skull bone tumor

    Intracranial meningiomas with extracranial extension are rare lesions with only few reported cases in literature. In present article we report a case of 62 year male patient presented with progressive swelling over right side frontal region of 2 year duration. In our case the tumor was in close proximity to right fronto-parietal suture and there was a possibility that suture line could have paved the way for its extracranial spread and also there was evidence of transdiploic extension on histopatholgical examination

    Green Synthesis of 2-amino-3-cyano-4H-chromen-4-ylphosphonates

    A facile and highly efficient and green synthetic protocol is developed for the bioactive 2-amino-3-cyano-4H-chromen-4-ylphosphonates by the one-pot reaction of various salicylaldehydes, malononitrile, and diethyl phosphite using sulfamic acid as an efficient, reusable and heterogeneous solid acid catalyst. All the synthesized compounds were characterized by their 1H-NMR, 13C-NMR, 31P-NMR, and Mass spectral studies. © 2020 Author(s).The authors are thankful to Prof. C.     Recurrent extraventricular anaplastic ependymoma with scalp metastasis

    Extraneural metastasis from anaplastic ependymoma is uncommon. In a study from Memorial Sloan Kettering Cancer Center where the authors reviewed 81 ependymomas cases (between 1956 and 1989) there were only five (6.2%) cases had extraneural metastases. We present a case of anaplastic ependymoma with scalp metastasis and discuss the possible mechanism of spread. In majority most of the cases of metastatic extracranial ependymoma patients have underlying progressive intracranial disease. Although these patients receive standard treatment for the primary tumor (Gross total resection and radiotherapy) and the management options for recurrences includes re-excision, focal re-irradiation, stereotactic radiosurgery, or craniospinal radiotherapy for metastatic disease the long term outcome is not favorable

    Cladophialophora bantiana brain abscess masquerading cerebral tuberculoma in an immunocompetent host

    Phaeohyphomycosis is a term that collectively describes fungal infections caused by moulds and yeasts that have brown-pigmented cell walls (due to the presence of melanin). We report a case of 45 year female who had multiple coalescing lesions in the right basal ganglionic and thalamic region. Based on the imaging and investigation findings a diagnosis of cerebral tuberculoma was suspected. Histopathology of the excised specimen showed brown colored fungal hyphae surrounded by neutrophilic infiltrate. A diagnosis of phaeohyphomycosis caused by Cladophialophora bantiana was made and accordingly antifungal treatment was started. Brain abscess caused by Cladophialophora bantiana in an immunocompetent host is relatively uncommon and usually associated with overall high mortality. The best outcomes have been reported in patients who receive both surgical excision of the abscess followed by systemic antifungal therapy. In view of its rarity of these lesions pre-operative diagnosis is difficult particularly in an immunocompetent host and absence of other risk factors

    Digitally strengthened, midwife-led intervention to reach the unreached mothers across ten conflict-prone provinces of Afghanistan during humanitarian crisis

    Background: Coronavirus disease 2019 (COVID-19) pandemic had significant negative impact on sexual and reproductive health (SRH) with devastating impact on pregnant women in resource constrain humanitarian settings. This paper provides detailed account of a community midwife-led intervention in ten humanitarian settings of Afghanistan using world health organization (WHO) emergency disaster risk management (EDRM) framework.Objectives: The project is aimed at increasing access to Integrated Package of Essential SRH Services and Minimal Initial Service Package (MISP) with a specific focus on prevention of Postpartum Haemorrhage (PPH) and screening and management of preeclampsia and eclampsia.Methods: The project was implemented through 150 Community outreach midwives (COMs). Each midwife served 300 households; mentored by gynaecologists and supervisors. Midwives were trained through a digitally enabled, simulation based training and equipped with a set of off-the shelf devices and kits.Results: During COVID-19 pandemic and in absence of health care services during crisis, this intervention has played as a lifesaving intervention for the community in Afghanistan. Variable digital literacy, sociocultural barriers, reluctance in adapting to digital platforms, security and uncertainties were some of the challenges faced. Adaptation of outreach methods integrated high impactful digital technologies has been the most appropriate strategy "to reach the unreached".Conclusion: Through this model, national and global stakeholders were engaged even during the crisis in Afghanistan. It also provided vital inputs for the donors, governments, civil society organizations and other stakeholders for sustaining and advancing the delivery of quality SRH services in humanitarian settings
