    Perceptions and Attitudes of Student Teachers and Their Cognitive-Metacognitive Awareness in Mathematics in Colleges of Education in Zambia

    The purpose of the study was to establish perceptions and attitudes of student teachers and their cognitive-metacognitive awareness in mathematics in colleges of education in Zambia. Although there has been abundant research into perceptions, attitudes and cognitive-metacognitive awareness in teacher education, relatively little research has focused on student teachers’ perceptions and attitudes and their cognitive-metacognitive awareness in Zambia. Perception is a cognitive process through which individuals view, interpret, and understand their surroundings and environment and includes impressions formed about objects, events, and people. Attitude towards mathematics denotes interest or feeling towards studying mathematics. Metacognition is intertwined with cognition and affect. Metacognition is “thinking about thinking” or cognition about cognition. The key findings indicated that student teachers had moderately high metacognitive awareness levels in both colleges. According to the results of the analysis, there was not a significant difference among the scores of metacognitive awareness of student teachers 14(F=0.522; 蟻=0.491>0.05)">  according to means. We accept the null hypothesis that the means in the two colleges of education do not vary since . Results indicated that student teachers in both colleges of education had higher levels of their perceptions and attitudes towards mathematics. Further, results from the Focus Group Discussion (FGDs) indicated that student teachers perception of their performance is attributed to lecturers’ methods of teaching and lecturers’ attitudes towards them. Results from the focus group with all the years of study indicated that lecturers teach them procedures of solving problem without student teachers’ participation. The results showed that lecturers teach the ‘how’ approach of solving mathematical problems without the ‘when’ and ‘why’ processes of solving problems.  Therefore, this study recommends that teacher training programmes should include activities through the development and support of metacognitive awareness and affective factors that will be helpful in terms of professional and personal development for mathematics teacher trainees.     Influence of gendered roles on legume utilization and improved child dietary intake in Malawi

    The relationship between gender roles, legume production, utilization and child feeding practices in rural smallholder households of Dedza and Ntcheu districts in Malawi was investigated and analyzed. A cross-sectional research study was conducted with legume farming households with children aged 6-23 months who were part of the Africa RISING ‘mother trial’ or ‘baby trials’ for two farming seasons (2014/2015 to 2015/2016). Africa RISING project encourages smallholder farmers to grow legumes namely, groundnut (Arachis hypogaea), cowpea (Vigna unguiculata), pigeon pea (Cajanus cajan) and soya bean (Glycin max) as one way of improving the wellbeing of smallholder farmers. The key objective of the research was to assess the influence of gender roles, legume production, utilization and infant and young child dietary intake. In total, 291 households with children 6-23 months were targeted. Eight focus group discussions (FDGs) for in-depth qualitative data were conducted. Two 24-hour dietary recalls and food frequency questionnaires were used to collect data on infant and young child feeding (IYCF) practices. Data collected were extracted, arranged, recorded and analyzed by using SPSS version 21. About 18% of children aged 6-23 months received a minimum dietary diversity (the consumption of four or more food groups from the seven food groups), 2.5% received a minimum acceptable diet (indicator measures both the minimum feeding frequency and minimum dietary diversity, as appropriate for various age groups) and 37.5% of children received a minimum meal frequency (frequency of receiving solid, semi-solid, or soft foods at the minimum numbers of two and three times for children aged 6–8 months, and 9–23 months, respectively). Control on use of income by women had a positive and significant association with minimum dietary diversity (P<0.05), minimum meal frequency (P<0.05) and minimum acceptable diets (P<0.05) among children of both sexes. Children aged 6-23 months from households where women were actively involved in partial processing and budgeting of legumes met minimum dietary diversity than children from nonparticipating households. Women farmers were more knowledgeable about legumes; played an important role in seed selection, storage and processing; however, the findings signal an opportunity to increase women’s income by involving them in market information. Increasing legume production at household level does not mean increasing the nutritional status among children (6-23 months).     Impact of Xpert MTB/RIF rollout on management of tuberculosis in a South African community

    Background. The Xpert MTB/RIF test shortens the time to microbiological confirmation of pulmonary tuberculosis (TB) under research conditions.Objective. To evaluate the field impact of Xpert MTB/RIF rollout on TB diagnostic yield and time to treatment in a South African (SA) community.Methods. We compared TB investigation outcomes for 6-month calendar periods before and after Xpert MTB/RIF rollout in a semi-rural area of SA. The proportion of adult patients who tested positive by sputum smear microscopy, liquid culture or Xpert MTB/RIF and the proportion of positive sputum smear, liquid culture or Xpert MTB/RIF tests were compared. Secondary outcomes included time to laboratory diagnosis and treatment initiation. Data were collected from the National Health Laboratory Service database and from the Western Cape Provincial Department of Health TB register.Results. Regional rollout of Xpert MTB/RIF testing occurred in 2013. Of the 15 629 patients investigated in the post-rollout period, 7.9% tested positive on GeneXpert, compared with 6.4% of the 10 741 investigated in the pre-rollout period who tested positive by sputum smear microscopy (p<0.001). Median laboratory processing time was <1 day for Xpert MTB/RIF (interquartile range (IQR) 0 - 1) compared with 1 day (IQR 0 - 16) for sputum smear microscopy (p=0.001). The median time to TB treatment initiation was 4 days (IQR 2 - 8) after rollout compared with 5 days (IQR 2 - 14) before (p=0.001).Conclusions. Patients investigated for suspected pulmonary TB were more likely to be diagnosed after rollout of Xpert MTB/RIF testing, although the benefit to diagnostic yield was modest, and Xpert MTB/RIF testing was associated with a marginal improvement in time to treatment initiation

    HIV clinical stage progression of patients at 241 outpatient clinics in Democratic Republic of Congo: Disparities by gender, TB status and rurality

    Background: HIV clinical care programs are increasingly cognizant of the importance of customizing services according to patients’ clinical stage progression (WHO\u27s four-tiered staging) and other risk assessments. Understanding factors associated with Persons Living with HIV (PLHIV) patients’ progression through the treatment cascade and clinical stages is essential for programs to provide patient-centered, evidence-based services. Methods and materials: To analyze patient characteristics associated with disease progression stages for PLHIV on antiretroviral therapy (ART), this quantitative study used data, from January 2014–June 2019, from 49,460 PLHIV on ART from 241 HIV/AIDS outpatient clinics in 23 health zones in Haut-Katanga and Kinshasa provinces, Democratic Republic of Congo. To assess bivariate and multivariate associations, we performed Chi-square and multinomial logistic regression. Results: Among PLHIV receiving ART, 4.4% were at stage 4, and 30.7% at stage 3. Those at the less severe stages 2 and 1 constituted 22.9% and 41.9%. After controlling for covariates, patients with no TB were significantly more likely than those with TB (p\u3c = .05) to be at stage 1, rather than 3 or 4 (adjusted odds ratio or AOR, 5.73; confidence interval or CI, 4.98–6.59). Other characteristics significantly associated with higher odds of being at stage 1 included being female (AOR, 1.35; CI, 1.29–1.42), and shorter duration on ART (vs. \u3e 40.37 months); for ART duration less than 3.23 months the AOR was 2.47, for 3.23–14.52 months duration the AOR was 2.60, and for 14.53–40.37 months duration the AOR was 1.77 (quartile cut points used). Compared to patients in urban health zones, those in rural (AOR, 0.32) and semi-rural health zones (AOR, 0.79) were less likely to be at stage 1. Conclusion: Significant and substantial variation in HIV clinical progression stage by geographic location and demographic characteristics existed, indicative of the need for targeted efforts to improve the effectiveness of HIV care. Patients with TB coinfection compared to those without coinfection had a much greater risk of being at stage 3 or 4, implying a need for customized approaches and clinical regimens for this high-risk population

    Incremental impact upon malaria transmission of supplementing pyrethroid-impregnated long-lasting insecticidal nets with indoor residual spraying using pyrethroids or the organophosphate, pirimiphos methyl

    Background Long-lasting, insecticidal nets (LLINs) and indoor residual spraying (IRS) are the most widely accepted and applied malaria vector control methods. However, evidence that incremental impact is achieved when they are combined remains limited and inconsistent. Methods Fourteen population clusters of approximately 1000 residents each in Zambia’s Luangwa and Nyimba districts, which had high pre-existing usage rates (81.7 %) of pyrethroid-impregnated LLINs were quasi-randomly assigned to receive IRS with either of two pyrethroids, namely deltamethrin [Wetable granules (WG)] and lambdacyhalothrin [capsule suspension (CS)], with an emulsifiable concentrate (EC) or CS formulation of the organophosphate pirimiphos methyl (PM), or with no supplementary vector control measure. Diagnostic positivity of patients tested for malaria by community health workers in these clusters was surveyed longitudinally over pre- and post-treatment periods spanning 29 months, over which the treatments were allocated and re-allocated in advance of three sequential rainy seasons. Results Supplementation of LLINs with PM CS offered the greatest initial level of protection against malaria in the first 3 months of application (incremental protective efficacy (IPE) [95 % confidence interval (CI)] = 0.63 [CI 0.57, 0.69], P < 0.001), followed by lambdacyhalothrin (IPE [95 % CI] = 0.31 [0.10, 0.47], P = 0.006) and PM EC (IPE, 0.23 [CI 0.15, 0.31], P < 0.001) and then by deltamethrin (IPE [95 % CI] = 0.19 [−0.01, 0.35], P = 0.064). Neither pyrethroid formulation provided protection beyond 3 months after spraying, but the protection provided by both PM formulations persisted undiminished for longer periods: 6 months for CS and 12 months for EC. The CS formulation of PM provided greater protection than the combined pyrethroid IRS formulations throughout its effective life IPE [95 % CI] = 0.79 [0.75, 0.83] over 6 months. The EC formulation of PM provided incremental protection for the first 3 months (IPE [95 % CI] = 0.23 [0.15, 0.31]) that was approximately equivalent to the two pyrethroid formulations (lambdacyhalothrin, IPE [95 % CI] = 0.31 [0.10, 0.47] and deltamethrin, IPE [95 % CI] = 0.19 [−0.01, 0.35]) but the additional protection provided by the former, apparently lasted an entire year. Conclusion Where universal coverage targets for LLIN utilization has been achieved, supplementing LLINs with IRS using pyrethroids may reduce malaria transmission below levels achieved by LLIN use alone, even in settings where pyrethroid resistance occurs in the vector population. However, far greater reduction of transmission can be achieved under such conditions by supplementing LLINs with IRS using non-pyrethroid insecticide classes, such as organophosphates, so this is a viable approach to mitigating and managing pyrethroid resistance

    Socioeconomic status and other factors associated with HIV status among OVC in Democratic Republic of Congo (DRC)

    Background: Orphans and vulnerable children (OVC) are a high-risk group for HIV infection, particularly in Sub-Saharan Africa. Purpose: This study aims to portray the socioeconomic profile of OVC and examine the association of household and parent/guardian characteristics with the HIV status of OVC. Methods: For this quantitative retrospective study, we obtained data from ICAP/DRC for a total of 1,624 OVC from households enrolled for social, financial, and clinical services between January 2017 and April 2020 in two provinces of the Democratic Republic of Congo, Haut-Katanga and Kinshasa. We computed descriptive statistics for OVC and their parents\u27 or guardians\u27 characteristics. We used the chi-square test to determine bivariate associations of the predictor variables with the dichotomous dependent variable, HIV positivity status. To analyze the association between these independent variables and the dichotomous dependent variable HIV status after controlling for other covariates, we performed firth\u27s logistic regression. Results: Of the OVC included in this study, 18% were orphans, and 10.9% were HIV+. The chi-square analysis showed that among parents/guardians that were HIV+, a significantly lower proportion of OVC (11.7%) were HIV+ rather than HIV- (26.3%). In contrast, for parents/guardians with HIV- status, 9.0% of OVC were HIV-negative, and 11.7% of OVC were OVC+. The firth\u27s logistic regression also showed the adjusted odds of HIV+ status were significantly lower for OVC with parents/guardians having HIV+ status themselves (AOR, 0.335; 95% CI, 0.171–0.656) compared with HIV-negative parents/guardians. The adjusted odds of HIV+ status were significantly lower for OVC with a monthly household income of \u3c 30(AOR,0.421;9530 (AOR, 0.421; 95% CI, 0.202–0.877) compared with OVC with a monthly household income \u3e 30. Conclusions: Our results suggest that, with the exception of a few household and parent/guardian characteristics, the risk of HIV+ status is prevalent across all groups of OVC within this study, which is consistent with the existing body of evidence showing that OVC are in general vulnerable to HIV infection. With a notable proportion of children who are single or double orphans in DRC, HIV+ OVC constitute a high-risk group that merits customized HIV services. The findings of this study provide data-driven scientific evidence to guide such customization of HIV services

    Investigating the suitability and cost-benefit of copper tailings as partial replacement of sand in concrete in Zambia: An exploratory study

    Purpose: This study investigated the suitability and cost-benefit of using copper tailings as partial replacement of sand in concrete production. The study was motivated by the accumulation and non- utilisation of Copper Tailings in dams among them Tailing Dam 25 also known as TD 25 in Kitwe city of the Copperbelt province in Zambia that take up approximately 111 hectares of unutilised land. Design/methodology/approach: Laboratory experimental approach of concrete production based on water/cement ratios of 0.3 and 0.5 was used because this was an exploratory study designed to establish the primary performance of .concrete. 30 concrete cubes were cast based on the two water-cement ratios. 0% to 30% partial sand replacement with copper tailings was used in both mixes with the 0% copper tailings replacement being the control mix and reference point. Other concrete tests included workability, density, compressive strength and element composition analysis. Findings: Results revealed that copper tailings from TD 25 were suitable for partial replacement of sand in concrete. 30% of sand replacement with copper tailings was established as the maximum replacement amount in order to produce optimum compressive strength values from both mixes. The drier mix of 0.3 water-cement ratio produced higher compressive strength results of 23 MPa at 28 days of concrete curing with 2.34% as optimum concrete cost reduction. Originality/Value: The study provides guidance on optimum concrete grade produced and cost reduction details of copper tailing-based concrete to support for local authorities in suitable land wand waste management using real data
