    Corticosteroids for the prevention of bronchopulmonary dysplasia in preterm infants: a network meta-analysis

    Objective: To determine the comparative efficacy and safety of corticosteroids in the prevention of bronchopulmonary dysplasia (BPD) in preterm infants.  Study design: We systematically searched PubMed, EMBASE and the Cochrane Library. Two reviewers independently selected randomised controlled trials (RCTs) of postnatal corticosteroids in preterm infants. A Bayesian network meta-analysis and subgroup analyses were performed.  Results: We included 47 RCTs with 6747 participants. The use of dexamethasone at either high dose or low dose decreased the risk of BPD (OR 0.29, 95% credible interval (CrI) 0.14 to 0.52; OR 0.58, 95% CrI 0.39 to 0.76, respectively). High-dose dexamethasone was more effective than hydrocortisone, beclomethasone and low-dose dexamethasone. Early and long-term dexamethasone at either high dose or low dose decreased the risk of BPD (OR 0.11, 95% CrI 0.02 to 0.4; OR 0.37, 95% CrI 0.16 to 0.67, respectively). There were no statistically significant differences in the risk of cerebral palsy (CP) between different corticosteroids. However, high-dose and long-term dexamethasone ranked lower than placebo and other regimens in terms of CP. Subgroup analyses indicated budesonide was associated with a decreased risk of BPD in extremely preterm and extremely low birthweight infants (OR 0.60, 95% CrI 0.36 to 0.93).  Conclusions: Dexamethasone can reduce the risk of BPD in preterm infants. Of the different dexamethasone regimens, aggressive initiation seems beneficial, while a combination of high-dose and long-term use should be avoided because of the possible adverse neurodevelopmental outcome. Dexamethasone and inhaled corticosteroids need to be further evaluated in large-scale RCTs with long-term follow-ups

    Preliminary study of the antioxidant properties of flowers and roots of Pyrostegia venusta (Ker Gawl) Miers

    <p>Abstract</p> <p>Background</p> <p>Free radical stress leads to tissue injury and can eventually to arthritis, atherosclerosis, diabetes mellitus, neurodegenerative diseases and carcinogenesis. Several studies are ongoing worldwide to find natural antioxidants of plant origin. We assessed the <it>in-vitro </it>antioxidant activities and screened the phytochemical constituents of methanolic extracts of <it>Pyrostegia venusta </it>(Ker Gawl) <it>Miers</it>.</p> <p>Methods</p> <p>We evaluated the antioxidant potential and phytochemical constituents of <it>P. venusta </it>using 1,1-Diphenyl-2-picrylhydrazyl (DPPH), 2, 2'-azinobis-3-ethylbenzothiazoline-6-sulfonic acid (ABTS) and ferric reducing antioxidant power (FRAP) assays. Gas chromatography-mass spectroscopy (GC-MS) studies were also undertaken to assess the phytochemical composition of the flower extracts.</p> <p>Results</p> <p>Phytochemical analyses revealed the presence of terpenoids, alkaloids, tannins, steroids, and saponins. The reducing ability of both extracts was in the range (in Όm Fe(II)/g) of 112.49-3046.98 compared with butylated hydroxytoluene (BHT; 63.56 ± 2.62), catechin (972.02 ± 0.72 Όm) and quercetin 3208.27 ± 31.29. A significant inhibitory effect of extracts of flowers (IC<sub>50 </sub>= 0.018 ± 0.69 mg/ml) and roots (IC<sub>50 </sub>= 0.026 ± 0.94 mg/ml) on ABTS free radicals was detected. The antioxidant activity of the extracts of flowers (95%) and roots (94%) on DPPH radicals was comparable with that of ascorbic acid (98.9%) and BHT (97.6%). GC-MS study revealed the presence of myoinositol, hexadecanoic acid, linoleic acid, palmitic acid and oleic acid in the flower extracts.</p> <p>Conclusion</p> <p>These data suggest that <it>P. venusta </it>is a natural source of antioxidants. The extracts of flowers and roots of <it>P. venusta </it>contain significant amounts of phytochemicals with antioxidative properties and could serve as inhibitors or scavengers of free radicals. <it>P. venusta </it>could be exploited as a potential source for plant-based pharmaceutical products. These results could form a sound basis for further investigation in the potential discovery of new natural bioactive compounds.</p

    A prospective study of the use of antibiotics in the Emergency Department of a Chinese University Hospital

    Background: Antibiotics are one of the most widely misused group of medicines. The aim of this study was to investigate the use of antibiotics in one of the paediatric emergency departments in China. Methods: We performed a prospective, cross-sectional study of antibiotic use in the paediatric emergency room of West China Second University Hospital. A total of 500 consecutive patients from March 25 to April 3 2013 were included. Clinical details of the patients were also collected in order to analyse antibiotic use. Key findings: The median age of patients was 2 years 2 months. The five most common conditions seen in the emergency department were wheezy bronchitis, upper respiratory tract infections, tonsillitis, pneumonia and diarrhoea. A total of 311 children (62%) received antibiotics. The antibiotics prescribed were predominantly cephalosporins and penicillins. More than one antibiotic was used in 51 patients. In total, 75% of the antibiotics prescribed were cephalosporins. More than three-quarters of the young children with wheezy bronchitis received antibiotics. Antibiotic use for children with an upper respiratory tract infections or tonsillitis was greater than the 20% maximum recommended by the European Surveillance of Antimicrobial Consumption. Conclusions: The majority of children attending the emergency department received antibiotics. For many of the conditions, the use of antibiotics was inappropriate

    Semi-Supervised Deep Learning for Lunar Crater Detection Using CE-2 DOM

    Lunar craters are very important for estimating the geological age of the Moon, studying the evolution of the Moon, and for landing site selection. Due to a lack of labeled samples, processing times due to high-resolution imagery, the small number of suitable detection models, and the influence of solar illumination, Crater Detection Algorithms (CDAs) based on Digital Orthophoto Maps (DOMs) have not yet been well-developed. In this paper, a large number of training data are labeled manually in the Highland and Maria regions, using the Chang’E-2 (CE-2) DOM; however, the labeled data cannot cover all kinds of crater types. To solve the problem of small crater detection, a new crater detection model (Crater R-CNN) is proposed, which can effectively extract the spatial and semantic information of craters from DOM data. As incomplete labeled samples are not conducive for model training, the Two-Teachers Self-training with Noise (TTSN) method is used to train the Crater R-CNN model, thus constructing a new model—called Crater R-CNN with TTSN—which can achieve state-of-the-art performance. To evaluate the accuracy of the model, three other detection models (Mask R-CNN, no-Mask R-CNN, and Crater R-CNN) based on semi-supervised deep learning were used to detect craters in the Highland and Maria regions. The results indicate that Crater R-CNN with TTSN achieved the highest precision (of 91.4% and 88.5%, respectively) in the Highland and Maria regions, even obtaining the highest recall and F1 score. Compared with Mask R-CNN, no-Mask R-CNN, and Crater R-CNN, Crater R-CNN with TTSN had strong robustness and better generalization ability for crater detection within 1 km in different terrains, making it possible to detect small craters with high accuracy when using DOM data

    Abstract Progress intreating cerebral ischemia with an extract of Ginkgo biloba L. (Ginkgoaceae), commonly known as EGb 761, has been made in the past. However, neuroprotective interaction of the two main active fractions, flavonoids and terpenoids, still remains vague. In the current study, we investigated the neuroprotective effects of Ginkgo flavonoids, terpenoids, and their combinations in ratios of 16:1, 4:1, and 1:1 on brain damage in mice injured by ischemia-reperfusion. Pretreatment with total flavonoids, total terpenoids, their three combinations, or EGb 761 for 7 days each at two doses of 15 and 30 mg=kg might elevate latency and decrease error numbers to different extents in the passive avoidance test, but only the action of the 4:1 combination besides EGb 761 was significant. In addition, the gasping time in decapitated mice was prolonged markedly by the 4:1 and 1:1 combinations at 15 and 30 mg=kg and by total flavonoids other than total terpenoids at 30 mg=kg. Neurochemical assays further showed that flavonoids significantly decreased malondialdehyde, superoxide anion contents, and xanthine oxidase activity, meanwhile terpenoids markedly suppressed myeloperoxidase activity besides its slight antioxidant effect. These findings suggest that flavonoids and terpenoids contribute synergistically to the neuroprotection of EGb 761 mainly via antioxidant activity and anti-inflammatory activity, respectively, and the 4:1 combination, namely the natural ratio in EGb 761, shows the strongest effects among those of the three combinations

    Dietary fibres modulate the composition and activity of butyrate-producing bacteria in the large intestine of suckling piglets

    Dietary fibres have been shown to affect early-life microbiota colonization in the large intestine of suckling piglets, however, much less is known as to whether they also modulate the composition and activity of butyrate-producing bacteria. Here, we investigated the effect of dietary fibres on the abundance, composition, and activity of butyrate-producing bacteria in suckling piglets. Piglets were fed a control diet or creep feeds containing alfalfa, wheat bran, or pure cellulose, respectively, from postnatal day 7 to 22. Large intestinal digesta and mucosa samples were collected for quantitative analysis of bacterial group-specific 16S ribosomal RNA- and butyrate production-related genes, and digesta samples for quantification of short-chain fatty acids. The alfalfa diet increased (P 0.05), compared to other groups. In the distal colonic mucosa, the cellulose diet increased (P < 0.05) the abundance of Clostridium cluster XIVa and copies of the butyryl-CoA:acetate CoA-transferase gene compared to the alfalfa diet. These results indicated that dietary fibres modulate the abundance and activity of butyrate-producing bacteria in the large intestine of suckling piglets, and that a moderate supplementation of alfalfa and cellulose may benefit early-life gut health through the delivery of butyrate to the mucosa

    The chang’E-1 topographic atlas of the Moon

    This atlas is based on the lunar global Digital Elevation Models (DEM) of Chang'E-1 (CE-1), and presents CCD stereo image data with digital photogrammetry. The spatial resolution of the DEM in this atlas is 500m, with horizontal accuracy of 192m and vertical accuracy of 120m. Color-shaded relief maps with contour lines are used to show the lunar topographical characteristics. The topographical data gathered by CE-1 can provide fundamental information for the study of lunar topographical, morphological and geological structures, as well as for lunar evolution research

    5G MIMO Conformal Microstrip Antenna Design

    With the development of wireless communication technology, 5G will develop into a new generation of wireless mobile communication systems. MIMO (multiple-input multiple-output) technology is expected to be one of the key technologies in the field of 5G wireless communications. In this paper, 4 pairs of microstrip MIMO conformal antennas of 35 GHz have been designed. Eight-element microstrip Taylor antenna array with series-feeding not only achieves the deviation of the main lobe of the pattern but also increases the bandwidth of the antenna array and reduces sidelobe. MIMO antennas have been fabricated and measured. Measurement results match the simulation results well. The return loss of the antenna at 35 GHz is better than 20 dB, the first sidelobe level is −16 dB, and the angle between the main lobe and the plane of array is 60°

    Effects of dietary fibre source on microbiota composition in the large intestine of suckling piglets

    This study aimed to investigate the effects of dietary fibre sources on the gut microbiota in suckling piglets, and to test the hypothesis that a moderate increase of dietary fibre may affect the gut microbiota during the suckling period. Suckling piglets were fed different fibre-containing diets or a control diet from postnatal day 7 to 22. Digesta samples from cecum, proximal colon and distal colon were used for Pig Intestinal Tract Chip analysis. The data showed that the effects of fibre-containing diet on the gut microbiota differed in the fibre source and gut location. The alfalfa diet increased Clostridium cluster XIVb and Sporobacter termitidis in the cecum compared to the pure cellulose diet. Compared to the control diet, the alfalfa diet also increased Coprococcus eutactus in the distal colon, while the pure cellulose diet decreased Eubacterium pyruvativorans in the cecum. The pure cellulose diet increased Prevotella ruminicola compared to the wheat bran diet. Interestingly, the alfalfa group had the lowest abundance of the potential pathogen Streptococcus suis in the cecum and distal colon. These results indicated that a moderate increase in dietary fibres affected the microbial composition in suckling piglets, and that the alfalfa inclusion produced some beneficial effects on the microbial communities.</p
