    A pilot randomised controlled trial of an internet-based cognitive behavioural therapy self-management programme (MS Invigor8) for multiple sclerosis fatigue

    The majority of people affected by Multiple Sclerosis (PaMS) experience severe and disabling fatigue. MS Fatigue is poorly understood and most existing treatments have limited effectiveness. However, a recent randomised controlled trial (RCT) showed that cognitive-behaviour therapy with a clinical psychologist was effective in reducing MS fatigue severity and impact. The current study developed an Internet-based version of this intervention to make it available to a wider group of PaMS and conducted preliminary investigations of its efficacy, feasibility and cost-effectiveness in a pilot RCT. The ‘MS Invigor8’ website was developed using agile design and substantial input from PaMS. The programme includes eight online tailored and interactive sessions along with homework tasks, intended to be accessed weekly. In the pilot trial, 40 patients were randomised to MS Invigor8 (n=23) or standard care (n=17). The MS Invigor8 group accessed sessions over 8-10 weeks and received up to three 30-50 minute telephone support sessions. Participants completed online questionnaires assessing fatigue, mood and quality of life at baseline and 10 weeks follow-up. Large between group treatment effects were found for the primary outcomes of fatigue severity (d=1.19) and impact (d =1.22). The MS Invigor8 group also reported significantly greater improvements in anxiety and depression. Analysis suggested that the intervention may be cost-effective. Qualitative feedback suggested that participants considered this treatment approach acceptable and helpful. Technical website problems negatively affected some users’ experiences and need to be resolved. Given the promising results a larger RCT with longer term follow-up is warranted. <br/

    Making history: intentional capture of future memories

    Lifelogging' technology makes it possible to amass digital data about every aspect of our everyday lives. Instead of focusing on such technical possibilities, here we investigate the way people compose long-term mnemonic representations of their lives. We asked 10 families to create a time capsule, a collection of objects used to trigger remembering in the distant future. Our results show that contrary to the lifelogging view, people are less interested in exhaustively digitally recording their past than in reconstructing it from carefully selected cues that are often physical objects. Time capsules were highly expressive and personal, many objects were made explicitly for inclusion, however with little object annotation. We use these findings to propose principles for designing technology that supports the active reconstruction of our future past

    The proper motion of the Arches cluster with Keck Laser-Guide Star Adaptive Optics

    We present the first measurement of the proper motion of the young, compact Arches cluster near the Galactic center from near-infrared adaptive optics (AO) data taken with the recently commissioned laser-guide star (LGS) at the Keck 10-m telescope. The excellent astrometric accuracy achieved with LGS-AO provides the basis for a detailed comparison with VLT/NAOS-CONICA data taken 4.3 years earlier. Over the 4.3 year baseline, a spatial displacement of the Arches cluster with respect to the field population is measured to be 24.0 +/- 2.2 mas, corresponding to a proper motion of 5.6 +/- 0.5 mas/yr or 212 +/- 29 km/s at a distance of 8 kpc. In combination with the known line-of-sight velocity of the cluster, we derive a 3D space motion of 232 +/- 30 km/s of the Arches relative to the field. The large proper motion of the Arches cannot be explained with any of the closed orbital families observed in gas clouds in the bar potential of the inner Galaxy, but would be consistent with the Arches being on a transitional trajectory from x1 to x2 orbits. We investigate a cloud-cloud collision as the possible origin for the Arches cluster. The integration of the cluster orbit in the potential of the inner Galaxy suggests that the cluster passes within 10 pc of the supermassive black hole only if its true GC distance is very close to its projected distance. A contribution of young stars from the Arches cluster to the young stellar population in the inner few parsecs of the GC thus appears increasingly unlikely. The measurement of the 3D velocity and orbital analysis provides the first observational evidence that Arches-like clusters do not spiral into the GC.     Annular electroconvection with shear

    We report experiments on convection driven by a radial electrical force in suspended annular smectic A liquid crystal films. In the absence of an externally imposed azimuthal shear, a stationary one-dimensional (1D) pattern consisting of symmetric vortex pairs is formed via a supercritical transition at the onset of convection. Shearing reduces the symmetries of the base state and produces a traveling 1D pattern whose basic periodic unit is a pair of asymmetric vortices. For a sufficiently large shear, the primary bifurcation changes from supercritical to subcritical. We describe measurements of the resulting hysteresis as a function of the shear at radius ratio η0.8\eta \sim 0.8. This simple pattern forming system has an unusual combination of symmetries and control parameters and should be amenable to quantitative theoretical analysis.Comment: 12 preprint pages, 3 figures in 2 parts each.     The quantum inequalities do not forbid spacetime shortcuts

    A class of spacetimes (comprising the Alcubierre bubble, Krasnikov tube, and a certain type of wormholes) is considered that admits `superluminal travel' in a strictly defined sense. Such spacetimes (they are called `shortcuts' in this paper) were suspected to be impossible because calculations based on `quantum inequalities' suggest that their existence would involve Planck-scale energy densities and hence unphysically large values of the `total amount of negative energy' E_tot. I argue that the spacetimes of this type may not be unphysical at all. By explicit examples I prove that: 1) the relevant quantum inequality does not (always) imply large energy densities; 2) large densities may not lead to large values of E_tot; 3) large E_tot, being physically meaningless in some relevant situations, does not necessarily exclude shortcuts.Comment: Minor corrections and addition

    A genome-wide association study suggests an association of Chr8p21.3 (GFRA2) with diabetic neuropathic pain

    BACKGROUND: Neuropathic pain, caused by a lesion or a disease affecting the somatosensory system, is one of the most common complications in diabetic patients. The purpose of this study is to identify genetic factors contributing to this type of pain in a general diabetic population. METHOD: We accessed the Genetics of Diabetes Audit and Research Tayside (GoDARTS) datasets that contain prescription information and monofilament test results for 9439 diabetic patients, among which 6927 diabetic individuals were genotyped by Affymetrix SNP6.0 or Illumina OmniExpress chips. Cases of neuropathic pain were defined as diabetic patients with a prescription history of at least one of five drugs specifically indicated for the treatment of neuropathic pain and in whom monofilament test result was positive for sensory neuropathy in at least one foot. Controls were individuals who did not have a record of receiving any opioid analgesics. Imputation of non‐genotyped SNPs was performed by IMPUTE2, with reference files from 1000 Genomes Phase I datasets. RESULTS: After data cleaning and relevant exclusions, imputed genotypes of 572 diabetic neuropathic pain cases and 2491 diabetic controls were used in the Fisher's exact test. We identified a cluster in the Chr8p21.3, next to GFRA2 with a lowest p‐value of 1.77 × 10(−7) at rs17428041. The narrow‐sense heritability of this phenotype was 11.00%. CONCLUSION: This genome‐wide association study on diabetic neuropathic pain suggests new evidence for the involvement of variants near GFRA2 with the disorder, which needs to be verified in an independent cohort and at the molecular level

    Measuring Strategic Uncertainty in Coordination Games

In a laboratory experiment we measure subjects' certainty equivalents for three coordination games and one lottery. Attitudes towards strategic uncertainty in coordination games are related to risk aversion, experience seeking, gender and age. From the distribution of certainty equivalents among participating students we estimate probabilities for successful coordination in a wide range of coordination games. For many games success of coordination is predictable with a reasonable error rate. The best response of a risk neutral player is close to the global-game solution. Comparing choices in coordination games with revealed risk aversion, we estimate subjective probabilities for successful coordination. In games with a low coordination requirement, most subjects underestimate the probability of success. In games with a high coordination requirement, most subjects overestimate this probability. Data indicate that subjects have probabilistic beliefs about success or failure of coordination rather than beliefs about individual behavior of other players

    The Interaction of Genetic Predisposition and Socioeconomic Position With Type 2 Diabetes Mellitus:Cross-Sectional and Longitudinal Analyses From the Lifelines Cohort and Biobank Study

    OBJECTIVE: A strong genetic predisposition for type 2 diabetes mellitus (T2DM) may aggravate the negative effects of low socioeconomic position (SEP) in the etiology of the disorder. This study aimed to examine cross-sectional and longitudinal associations and interactions of a genetic risk score (GRS) and SEP with T2DM, and to investigate whether clinical and behavioral risk factors can explain these associations and interactions. METHODS: We used data from 13,027 genotyped participants from the Lifelines study. The GRS was based on single-nucleotide polymorphisms (SNPs) genome-wide associated with T2DM and was categorized into tertiles. SEP was measured as educational level. T2DM was based on biological markers, recorded medication use, and self-reports. Cross-sectional and longitudinal associations, and interactions, between the GRS and SEP on T2DM were examined. RESULTS: The combination of a high GRS and low SEP had the strongest association with T2DM in cross-sectional (OR: 3.84; 95% CI: 2.28, 6.46) and longitudinal analyses (HR: 2.71; 1.39, 5.27), compared to a low GRS and high SEP. Interaction between a high GRS and a low SEP was observed in cross-sectional (relative excess risk due to interaction: 1.85; 0.65, 3.05) but not in longitudinal analyses. Clinical and behavioral risk factors mostly explained the observed associations and interactions. CONCLUSIONS: A high GRS combined with a low SEP provides the highest risk for T2DM. These factors also exacerbated each other's impact cross-sectionally but not longitudinally. Preventive measures should target individual and contextual factors of this high-risk group to reduce the risk of T2DM
