    UTILIZATION OF METADATA FOR QUALITY ASSURANCE IN THE INTERNATIONAL SHARING AND REUSE OF OPEN EDUCATIONAL RESOURCES

    In order to sustain the development and utilization of learning content under the limited financial and human resources, the sharing and reuse of open educational resources (OERs) have been promoted in various school levels and disciplines. In many countries, central organization(s) collect both learning content and its metadata, and provide them to learners and teachers with distinctive value‐added services from their portals. Collaborations among the national portals have also emerged beyond borders.Global Learning Object Brokered Exchange (GLOBE) is an international consortium of the hub organizations, which manage a federated repository and/or a meta‐referatory in each country and region. GLOBE was established in September 2004 by five founding members, and now twelve organizations in the world participate in. By adopting IEEE LOM version 1.0 for the metadata standards, SQI for the query language, and OAI‐PMH for harvesting, GLOBE realized a global search and delivery infrastructure for lifelong learning (LLL). At present, while the repositories all over the world store huge number of learning content and metadata, the variance of the quality becomes much bigger. In addition, because the effectiveness of learning content is relative and dependent on contextual factors, both optimizations specialized in learner characteristics and localizations to each language/culture are indispensable. The users need some supports on the quality and pedagogical guidance when they find their right content. GLOBE, based on multilingualism, multiculturalism and pluralism, collaborates to build up standardized services on the quality assurance of the content utilizing the “Educational” items of LOM. In AAOU framework also, the organizations can promote “openness”, and share the quality learning content and educational information by using a standardized content search and delivery infrastructure

    Extracellular Water May Mask Actual Muscle Atrophy During Aging

    Background.\nSkeletal muscle tissue holds a large volume of water partitioned into extracellular water (ECW) and intracellular water (ICW) fractions. As the ECW may not be related to muscle strength directly, we hypothesized that excluding ECW from muscle volume would strengthen the correlation with muscle strength.Methods.\nA total of 119 healthy men aged 20–88 years old participated in this study. Knee isometric extension strength, vertical jump, and standing from a chair were measured as indices of muscle strength and power in the lower extremities. The regional lean volume (LV), total water (TW), ICW, and ECW in the lower leg were estimated by anthropometry (skinfold and circumference measurements) and segmental multifrequency bioelectrical impedance spectroscopy (S-BIS). Then, we calculated the ECW/TW and ICW/TW ratios.Results.\nAlthough ICW and the LV index decreased significantly with age (p < .001), no significant changes in ECW were observed (p = .134). Consequently, the ECW/TW ratio increased significantly (p < .001) with age (young adult, 27.0 ± 2.9%; elderly, 34.3 ± 4.9%; advanced elderly, 37.2 ± 7.0%). Adjusting for this by including the ICW/TW ratio in our models significantly improved the correlation between the LV index and strength-related measurements and correlated with strength-related measurements independently of the LV index (p < .001).Conclusions.\nThe ECW/TW ratio increases in the lower leg with age. The results suggest that the expansion of ECW relative to ICW and the LV masked actual muscle cell atrophy with aging

    Significant association between high serum CCL5 levels and better disease‐free survival of patients with early breast cancer

    Analysis of anticancer immunity aids in assessing the prognosis of patients with breast cancer. From 250 operated breast cancers, we focused on serum levels of C-C motif chemokine ligand 5 (CCL5), which is involved in cancer immune reactions. Serum levels of CCL5 were measured using a cytometric bead-based immunoassay kit and CCL5 expression in cancer cells was determined using immunohistochemical staining. In addition, mRNA in cancer and stromal cells was analyzed by microdissection and comparison with the public dataset. Disease-free survival (DFS) of patients with high CCL5 levels (cut-off, 13.87 ng/mL; n = 192) was significantly better than those with low CCL5 levels (n = 58; hazard ratio, 0.20; 95% confidence interval, 0.10- 0.39; P < .0001). An improved overall survival was observed in patients with high CCL5 levels compared to those with low CCL5 levels (P = .024). On the contrary, high immunohistochemical expression of CCL5 in cancer cells was significantly associated with decreased DFS. As serum CCL5 levels did not correlate with CCL5 expression in cancer cells and the relative expression of mRNA CCL5 was elevated in stromal cells in relation to cancer cells, serum CCL5 might be derived not from cancer cells, but from stromal cells. Expression of CCL5 in serum, but not in cancer cells, might contribute to improved patient prognosis mediating through not only immune reaction, but through other mechanisms. Determination of circulating CCL5 levels could be useful for predicting patient prognosis

    Predictors of sedation period for critical illness patients focusing on early rehabilitation on the bed

    There are various interventions of rehabilitation on the bed, but these are time-consuming and cannot be performed for all patients. The purpose of this study was to identify the patients who require early mobilization based on the level of sedation. We retrospectively evaluated the data of patients who underwent physical therapy, ICU admission of > 48 h, and were discharged alive. Sedation was defined as using sedative drugs and a Richmond Agitation–Sedation Scale score of < − 2. Multiple regression analysis was performed using sedation period as the objective variable, and receiver operating characteristic (ROC) curve and Spearman’s rank correlation coefficient were performed. Of 462 patients admitted to the ICU, the data of 138 patients were analyzed. The Sequential Organ Failure Assessment (SOFA) score and non-surgery and emergency surgery cases were extracted as significant factors. The ROC curve with a positive sedation period of more than 3 days revealed the SOFA cutoff score was 10. A significant positive correlation was found between sedation period and the initial day on early mobilization. High SOFA scores, non-surgery and emergency surgery cases may be indicators of early mobilization on the bed in the ICU

    Locality and diel cycling of viral production revealed by a 24 h time course cross-omics analysis in a coastal region of Japan

    International audienceViruses infecting microorganisms are ubiquitous and abundant in the ocean. However, it is unclear when and where the numerous viral particles we observe in the sea are produced and whether they are active. To address these questions, we performed time-series analyses of viral metagenomes and microbial metatranscriptomes collected over a period of 24 h at a Japanese coastal site. Through mapping the metatranscriptomic reads on three sets of viral genomes ((i) 878 contigs of Osaka Bay viromes (OBV), (ii) 1766 environmental viral genomes from marine viromes, and (iii) 2429 reference viral genomes), we revealed that all the local OBV contigs were transcribed in the host fraction. This indicates that the majority of viral populations detected in viromes are active, and suggests that virions are rapidly diluted as a result of diffusion, currents, and mixing. Our data further revealed a peak of cyanophage gene expression in the afternoon/dusk followed by an increase of genomes from their virions at night and less-coherent infectious patterns for viruses putatively infecting various groups of heterotrophs. This suggests that cyanophages drive the diel release of cyanobacteria-derived organic matter into the environment and viruses of heterotrophic bacteria might have adapted to the population-specific life cycles of hosts

    Despite some slight differences in symptomatology, differential diagnosis of methamphetamine-induced psychosis (MAP) versus schizophrenia can be challenging because both disorders present a large overlap in their clinical symptoms. However, a recent study has shown that near-infrared spectroscopy (NIRS) performed during a cognitive task can be a powerful tool to differentiate between these two disorders. Here, we evaluated verbal fluency task performance during NIRS in 15 patients diagnosed with MAP and 19 with schizophrenia matched for age and sex. We used prefrontal probes and a 24-channel NIRS machine to measure the relative concentrations of oxyhaemoglobin every 0.1 s during the task. For each patient, the neurocognitive function and clinical psychopathology were evaluated using the Positive and Negative Symptom Scale (PANSS), and the Brief Assessment of Cognition in Schizophrenia (BACS). Oxyhaemoglobin changes in the prefrontal cortex were significantly higher in the MAP group compared to those in the schizophrenia group, particularly in the right dorsolateral prefrontal cortex. In contrast, we found no significant difference in PANSS and BACS scores. Our findings suggest that NIRS measurement could be applied to differentiate patients with MAP from those with schizophrenia, even in cases where clinical symptoms are similar.博士(医学)・甲第645号・平成28年3月15日This work is licensed under a Creative Commons Attribution 4.0 International License.     Vagus Nerve Stimulation with Mild Stimulation Intensity Exerts Anti-Inflammatory and Neuroprotective Effects in Parkinson's Disease Model Rats

    Background: The major surgical treatment for Parkinson's disease (PD) is deep brain stimulation (DBS), but a less invasive treatment is desired. Vagus nerve stimulation (VNS) is a relatively safe treatment without cerebral invasiveness. In this study, we developed a wireless controllable electrical stimulator to examine the efficacy of VNS on PD model rats. Methods: Adult female Sprague-Dawley rats underwent placement of a cuff-type electrode and stimulator on the vagus nerve. Following which, 6-hydroxydopamine (6-OHDA) was administered into the left striatum to prepare a PD model. VNS was started immediately after 6-OHDA administration and continued for 14 days. We evaluated the therapeutic effects of VNS with behavioral and immunohistochemical outcome assays under different stimulation intensity (0.1, 0.25, 0.5 and 1 mA). Results: VNS with 0.25-0.5 mA intensity remarkably improved behavioral impairment, preserved dopamine neurons, reduced inflammatory glial cells, and increased noradrenergic neurons. On the other hand, VNS with 0.1 mA and 1 mA intensity did not display significant therapeutic efficacy. Conclusions: VNS with 0.25-0.5 mA intensity has anti-inflammatory and neuroprotective effects on PD model rats induced by 6-OHDA administration.
