    Food adulteration consists of changing the original structure of a food, and so, by its definition, it is a process not admitted by law. Adulterations can not only have commercial consequences, but also hygienic and nutritional ones, and in some cases, they can cause a serious danger to public health. Therefore, it is of great interest to understand and identify the modifications that alter the original chemical composition of a food item (nutrition label). Among the food processing sectors, the pasta food chain is a fascinating production process, which finds its roots in a mixture of a few, simple ingredients; in particular, in its basic formulation to produce pasta, exclusively durum wheat mixed with water is used, while soft wheat and therefore the flour obtained from it are destined for bakeries and the confectionery industry. In this work, a procedure was developed that allows the detection of the possible presence of soft wheat in durum wheat-based products, both in the flour and after the pasta-making process. It is to point out that this adulteration is only commercial fraud and there are no consequences for people’s health. In detail, the method was based on the isolation of the lipid fraction of flours and pastas and using the gas chromatographic analysis of the sterol ester fraction, which were not altered during the pasta-making phase, because they have very high boiling points. Based on the evaluation of some specific ratios between sterol esters, it was possible to trace the percentage of soft wheat present in a mixture with durum wheat, both in flour products and pastas

    Supercritical fluid extraction of pyrethrins from pyrethrum flowers (Chrysanthemum cinerariifolium) compared to traditional maceration and cyclic pressurization extraction

    In this work, a comparison between three extraction processes, including traditional maceration in n-hexane and ethyl alcohol, supercritical fluid extraction (SFE) and cyclically pressurized extraction known as rapid solid-liquid dynamic extraction (RSLDE), has been carried out for the extraction of pyrethrins, predominantly nonpolar natural compounds with insecticidal properties found in pyrethrum, an extract of certain species of chrysanthemums. Pyrethrins are often used in household insecticides and for the control of parasites on pets or livestock. Maceration is the cheapest method, but the values of the percent of extracted material compared to the weight of the dried flowers are lower. The extraction with supercritical CO2 is less efficient than the Naviglio extractor, but it has the advantage of not requiring the use of solvents. Consequently, all three techniques are valid for the extraction of compounds from pyrethrum flowers. The results obtained show that by suitably varying the parameters of the three extraction procedures, it is possible to obtain pyrethrin extracts for use in the production of low toxicity insecticides for warm-blooded animals and low pollution. In particular, extraction with supercritical CO2 presents an additional advantage; although oleoresin is a natural product, its extraction involves the use of solvents, while SFE is a highly efficient extraction process due to the use of CO2 in the supercritical phase without the use of solvents. On the other hand, the Naviglio extractor is an inexpensive technique and requires a minimum energy loss when compared to the extraction with supercritical fluids. Thus, the latter is more convenient on an industrial level but is not universally applicable. Nevertheless, the supercritical extraction can be used as a medium detergent of the oleoresin produced to avoid residues of the solvents left during the extraction process. Therefore, for demonstration purposes, for the SFE CO2 extraction process, numerical simulations have been performed, allowing for helpful results to optimize the process for further tests

    The term “officinal” derives from the Latin and includes all medicinal, aromatic and perfume plant species, which have long been a subject of interest for multiple purposes: health, food, pharmacological, cosmetic and so on. In this work, a study on six different species of medicinal plants, particularly characterized by digestive, choleretic and diuretic properties, was carried out: rosemary (Rosmarinus officinalis), sage (Salvia officinalis), laurel (Laurus nobilis), gentian (Gentiana lutea), dandelion (Taraxacum officinale) and rhubarb (Rheum palmatum). The roots and aerial parts of plants were separately extracted with two different techniques—maceration and rapid solid–liquid dynamic extraction (RSLDE)—and the quali/quantitative analysis of active ingredients have been determined by applying dry residue, Folin–Ciocalteu and DPPH assays. Data obtained have provided useful answers regarding the efficiency of the extraction carried out on a mixture or on single plants, allowing us to evaluate the best choice according to the cases and the final uses

    Comment on " Eshelby twist and correlation effects in diffraction from nanocrystals "

    The aim of this comment is to show that anisotropic effects and image fields should not be omitted as they are in the publication of A. Leonardi, S. Ryu, N. M. Pugno, and P. Scardi (LRPS) [J. Appl. Phys. 117, 164304 (2015)] on Pd cylindrical nanowires containing an axial screw dislocation. Indeed, according to our previous study [Phys. Rev. B 88, 224101 (2013)], the axial displacement field along the nanowire exhibits both a radial and an azimuthal dependence with a twofold symmetry due the orientation. As a consequence, the deviatoric strain term used by LRPS is not suitable to analyze the anisotropic strain fields that should be observed in their atomistic simulations. In this comment, we first illustrate the importance of anisotropy in Pd nanowire by calculating the azimuthal dependence of the deviatoric strain term. Then the expression of the anisotropic elastic field is recalled in term of strain tensor components to show that image fields should be also considered. The other aspect of this comment concerns the supposedly loss of correlation along the nanorod caused by the twist. It is claimed for instance by LRPS that : "As an effect of the dislocation strain and twist, if the cylinder is long enough, upper/lower regions tend to lose correlation, as if the rod were made of different sub-domains.". This assertion appears to us misleading since for any twist the position of all the atoms in the nanorod is perfectly defined and therefore prevents any loss of correlation. To clarify this point, it should be specified that this apparent loss of correlation can not be ascribed to the twisted state of the nanowire but is rather due to a limitation of the X-ray powder diffraction. Considering for instance coherent X-ray diffraction, we show an example of high twist where the simulated diffractogram presents a clear signature of the perfect correlation

    We investigated the presence of human T-lymphotropic virus type 2 (HTLV-2) DNA in the peripheral blood mononuclear cell subsets obtained from 18 patients coinfected with human immunodeficiency virus type 1 and HTLV-2, 6 of whom also had predominantly sensory polyneuropathy (PSP). HTLV-2 DNA and RNA were found in CD8- and CD19-positive cells, and, for patients with PSP, in CD14-positive cells as well. Furthermore, the patients with PSP had higher proviral loads than those without PSP

    : The major histocompatibility complex-class I chain related proteins A and B (MICA/B) is upregulated because of cellular stress and MICA/B shedding by cancer cells causes escape from NKG2D recognition favoring the emergence of cancers. Cholangiocarcinoma (CCA) is a relatively rare, though increasingly prevalent, primary liver cancer characterized by a late clinical presentation and a dismal prognosis. We explored the NKG2D-MICA/B axis in NK cells from 41 patients with intrahepatic cholangiocarcinoma (iCCA). The MICA/B-specific 7C6 mAb was used for ex vivo antibody-dependent cytotoxicity (ADCC) experiments using circulating, non tumor liver- and tumor-infiltrating NK cells against the HuCCT-1 cell line and patient-derived primary iCCA cells as targets. MICA/B were more expressed in iCCA than in non-tumoral tissue, MICA transcription being higher in moderately-differentiated compared with poorly-differentiated cancer. Serum MICA was elevated in iCCA patients in line with higher expression of ADAM10 and ADAM17 that are responsible for proteolytic release of MICA/B from tumor. Addition of 7C6 significantly boosted peripheral, liver- and tumor-infiltrating-NK cell degranulation and IFNÎł production toward MICA/B-expressing established cell lines and autologous iCCA patient target cells. Our data show that anti-MICA/B drives NK cell anti-tumor activity, and provide preclinical evidence in support of 7C6 as a potential immunotherapeutic tool for iCCA

    Ceramide present in cholangiocarcinoma-derived extracellular vesicle induces a pro-inflammatory state in monocytes

    Abstract Cholangiocarcinoma (CCA) is a rare cancer characterized by a global increasing incidence. Extracellular vesicles (EV) contribute to many of the hallmarks of cancer through transfer of their cargo molecules. The sphingolipid (SPL) profile of intrahepatic CCA (iCCA)-derived EVs was characterized by liquid chromatography-tandem mass spectrometry analysis. The effect of iCCA-derived EVs as mediators of inflammation was assessed on monocytes by flow cytometry. iCCA-derived EVs showed downregulation of all SPL species. Of note, poorly-differentiated iCCA-derived EVs showed a higher ceramide and dihydroceramide content compared with moderately-differentiated iCCA-derived EVs. Of note, higher dihydroceramide content was associated with vascular invasion. Cancer-derived EVs induced the release of pro-inflammatory cytokines in monocytes. Inhibition of synthesis of ceramide with Myriocin, a specific inhibitor of the serine palmitoyl transferase, reduced the pro-inflammatory activity of iCCA-derived EVs, demonstrating a role for ceramide as mediator of inflammation in iCCA. In conclusion, iCCA-derived EVs may promote iCCA progression by exporting the excess of pro-apoptotic and pro-inflammatory ceramides

    Emergency Department and Out-of-Hospital Emergency System (112\u2014AREU 118) integrated response to Coronavirus Disease 2019 in a Northern Italy centre

    Since December 2019, the world has been facing the life-threatening disease, named Coronavirus disease-19 (COVID-19), recognized as a pandemic by the World Health Organization. The response of the Emergency Medicine network, integrating \u201cout-of-hospital\u201d and \u201chospital\u201d activation, is crucial whenever the health system has to face a medical emergency, being caused by natural or human-derived disasters as well as by a rapidly spreading epidemic outbreak. We here report the Pavia Emergency Medicine network response to the COVID-19 outbreak. The \u201cout-of-hospital\u201d response was analysed in terms of calls, rescues and missions, whereas the \u201chospital\u201d response was detailed as number of admitted patients and subsequent hospitalisation or discharge. The data in the first 5 weeks of the Covid-19 outbreak (February 21\u2013March 26, 2020) were compared with a reference time window referring to the previous 5 weeks (January 17\u2013February 20, 2020) and with the corresponding historical average data from the previous 5 years (February 21\u2013March 26). Since February 21, 2020, a sudden and sustained increase in the calls to the AREU 112 system was noted (+ 440%). After 5 weeks, the number of calls and missions was still higher as compared to both the reference pre-Covid-19 period (+ 48% and + 10%, respectively) and the historical control (+ 53% and + 22%, respectively). Owing to the overflow from the neighbouring hospitals, which rapidly became overwhelmed and had to temporarily close patient access, the population served by the Pavia system more than doubled (from 547.251 to 1.135.977 inhabitants, + 108%). To minimize the possibility of intra-hospital spreading of the infection, a separate \u201cEmergency Department\u2014Infective Disease\u201d was created, which evaluated 1241 patients with suspected infection (38% of total ED admissions). Out of these 1241 patients, 58.0% (n = 720) were admitted in general wards (n = 629) or intensive care unit (n = 91). To allow this massive number of admissions, the hospital reshaped many general ward Units, which became Covid-19 Units (up to 270 beds) and increased the intensive care unit beds from 32 to 60. In the setting of a long-standing continuing emergency like the present Covid-19 outbreak, the integration, interaction and team work of the \u201cout-of-hospital\u201d and \u201cin-hospital\u201d systems have a pivotal role. The present study reports how the rapid and coordinated reorganization of both might help in facing such a disaster. AREU-112 and the Emergency Department should be ready to finely tune their usual cooperation to respond to a sudden and overwhelming increase in the healthcare needs brought about by a pandemia like the current one. This lesson should shape and reinforce the future

    Tocilizumab for treatment of severe covid-19 patients: Preliminary results from smatteo covid19 registry (smacore)

    Objective: This study aimed to assess the role of Tocilizumab therapy (TCZ) in terms of ICU admission and mortality rate of critically ill patients with severe COVID-19 pneumonia. Design: Patients with COVID-19 pneumonia were prospectively enrolled in SMAtteo COvid19 REgistry (SMACORE). A retrospective analysis of patients treated with TCZ matched using propensity score to patients treated with Standard Of Care (SOC) was conducted. Setting: The study was conducted at IRCCS Policlinico San Matteo Hospital, Pavia, Italy, from March 14, 2020 to March 27, 2020. Participants: Patients with a confirmed diagnosis of COVID-19 hospitalized in our institution at the time of TCZ availability. Interventions: TCZ was administered to 21 patients. The first administration was 8 mg/kg (up to a maximum 800 mg per dose) of Tocilizumab intravenously, repeated after 12 h if no side effects were reported after the first dose. Main Outcomes and Measures: ICU admission and 7-day mortality rate. Secondary outcomes included clinical and laboratory data. Results: There were 112 patients evaluated (82 were male and 30 were female, with a median age of 63.55 years). Using propensity scores, the 21 patients who received TCZ were matched to 21 patients who received SOC (a combination of hydroxychloroquine, azithromycin and prophylactic dose of low weight heparin). No adverse event was detected following TCZ administration. This study found that treatment with TCZ did not significantly affect ICU admission (OR 0.11; 95% CI between 0.00 and 3.38; p = 0.22) or 7-day mortality rate (OR 0.78; 95% CI between 0.06 and 9.34; p = 0.84) when compared with SOC. Analysis of laboratory measures showed significant interactions between time and treatment regarding C-Reactive Protein (CRP), alanine aminotransferase (ALT), platelets and international normalized ratio (INR) levels. Variation in lymphocytes count was observed over time, irrespective of treatment. Conclusions: TCZ administration did not reduce ICU admission or mortality rate in a cohort of 21 patients. Additional data are needed to understand the effect(s) of TCZ in treating patients diagnosed with COVID-19
