Involvement of electrophysiological localization of the subthalamic nucleus in deep brain stimulation for Parkinson’s disease
We studied the involvement of the electrophysiological localization of the subthalamic nucleus (NST) using a multi-unit recording technique by means of semi-microelectrode in a set of thirty Parkinson’s patients who benefited from a bilateral stimulation of the NST and who were operated on under local or general anesthesia. The multi-unit recording
technique by means of semi-microelectrodes appeared efficient,capable of improving the localization of the NST and leading to improvement in clinical results. We believe that the use of our technique will allow for time savings while providing good results, and that the choice of the angle of the trajectory will allow for improved localization
of the NST and thus improved clinical results
Cloud Computing Awareness among Practitioners in Yemeni Universities: An Exploratory Study
الحوسبة السحابية هي نموذج جديد لتكنولوجيا المعلومات اُعتمد في العديد من مؤسسات التعليم العالي للحصول على ميزة تنافسية. تهدف هذه الدراسة إلى استكشاف الوعي بتكنولوجيا الحوسبة السحابية في مؤسسات التعليم العالي في اليمن. تم اجراء البحث على عينة من الأكاديميين والإداريين، وطُلب من المشاركين إظهار مستوى وعيهم بهذه التكنولوجيا الناشئة، ومدى استخدامهم لهذه التكنولوجيا، وإبداء آرائهم حول المزايا والتحديات والعوائق التي تحول دون استخدام هذه التكنولوجيا. أظهرت النتائج مستوى عال من الوعي بالحوسبة السحابية بين المستجوبين. إلى جانب ذلك، فإن الجامعات على استعداد لتبني هذه التكنولوجيا عندما تتمكن من التغلب على معظم التحديات التي أهمها التكلفة، سرعة الإنترنت، الخصوصية، وقلة المعرفة بكيفية تطبيق هذه التكنولوجيا.Cloud computing is a new IT model adopted by many higher education institutions to gain competitive advantage. This study aims to explore the awareness of cloud computing technology among higher education institutions in Yemen. Using academic and administrative staff as the sample, the participants were asked to show their level of awareness of this emerging technology, the extent they utilize the technology, and to give their opinions about the advantages, challenges, and barriers of using this technology. The findings show high level awareness of cloud computing importance for higher education institutions. Besides, the universities are ready to adopt this technology when they can overcome the most challenges which are cost, Internet speed, privacy, and lack of knowledge on how to apply this technology
Diffusion weighted MRI in evaluation of transplanted kidney: Preliminary clinical experience
Purpose: To evaluate the diagnostic performance ofDiffusion Weighted (DW) magnetic resonance(MR) imaging in evaluation of transplantedkidneys.Patients and methods: One hundred twelve patientswith transplanted kidney from live kidney donorswere evaluated with coronal T2w and DW MRI ofthe kidney. There was 86 males and 26 females andthe mean age was 26.911.5ys (range 10-55).Apparent diffusion coefficient (ADC) wascalculated and the kidneys studied for any areasdiffusion restriction. Our patients classified into 2groups: Group 1 included 81 patients with stablekidney function and normal serum creatinine andthe second group included 31 patients with alteredkidney function, it includes 18 patients with chronicnephropathies and 13 patients with acute cellularrejection.Results: The mean ADC values for group 1 was2.70.26 x 10-3 mm2 /sec (range 1.93-3.6). In casesof chronic nephropathies, the mean ADC valueswas 2.30.22mm2/sec (range 2.05-2.77) while incases of acute cellular rejection it was1.80.2mm2/sec (range 1.7-2.2). When we use the2.4mm2/sec as a low cutoff ADC value fordiagnosis of normal kidney function, the sensitivity,specificity and overall accuracy for DWI MRI was80%, 96% and 93.5% respectively.When we used the ADC value of 2mm2/sec as acutoff value between acute cellular rejection andchronic nephropathies, the sensitivity, specificityand overall accuracy of DWI MRI was 90%, 98%and 90% respectively.Conclusion: In this relatively large study includingstable and abnormal function in transplantedkidneys, we can conclude that DW MRI isrelatively a new technique that allows diagnosis oftransplanted kidney with normal and alteredfunction
Aplastic anemia secondary to adjuvant Osimertinib therapy: a case report and a review of literature
Aplastic anemia is a rare hematological disorder characterized by suppressed hematopoiesis and pancytopenia. Although several drugs have been associated with aplastic anemia, its occurrence in response to Osimertinib, a third-generation epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI), is extremely rare. We present a case report of a 63-year-old patient with locally advanced non-small cell lung cancer (NSCLC) who developed aplastic anemia following adjuvant treatment with Osimertinib. Extensive investigations ruled out infectious etiology, and the absence of bone marrow involvement or other identifiable causes suggested a drug-induced etiology, specifically Osimertinib. This case report emphasizes the importance of recognizing this adverse event and considering it as a potential complication of Osimertinib therapy. Vigilant monitoring and prompt management are essential for optimizing patient outcomes. Further studies are needed to better understand the risk factors, underlying mechanisms, and management strategies for Osimertinib-induced aplastic anemia in the adjuvant settings
Velocity-slip boundary conditions and shape factor effects on MHD hybrid nanofluid flow via converging/diverging channels

No data was used for the research described in the article.The most important objective of this research-work is to investigate the impacts of velocity-slip boundary conditions and shape factor of solid nanoparticles on the hydrodynamic behavior of the nonlinear problem of MHD Jeffery–Hamel hybrid nanofluid flow where the mixture H2O C2H6O2 (50% 50%) was utilized as a base fluid. Using appropriate velocity transformations, the basic partial differential equations arising from mathematical modeling are transformed into non-linear ordinary differential equations. Afterwards, the determined nonlinear equation was numerically solved utilizing Runge-Kutta-Fehlberg 4th–5th order approach featuring shooting technique and analytically with the help of Duan–Rach Approach (DRA). The impact of active factors like Reynolds number, channel half-angle, Hartman number, base fluids nature, hybrid nanoparticles, velocity-slip boundary conditions, shape and Geometry of solid nanoparticles on hybrid nanofluid velocity and skin friction coefficient are visualized and investigated. The minimal local skin friction is found to be obtainable with the nanoparticles of Platelet form and second-order slip model where a reduction of 70% is gained compared to the local skin friction coefficient with spherical nanoparticles when the Hartmann number is higher. Results obtained also reveal that a higher reduction of 69% in local skin friction coefficient intensity is observed for both hybrid phase (Al2O3 Cu) and mixture base fluid (H2O C2H6O2) with second-order slip boundary condition model when Knudsen number Kn = 0,08. A comparison was made between the results obtained from this investigation in particular cases and the results obtained via the HAM-based Mathematica package for validation. Also, the obtained analytical DRA data are compared with numerical RKF45 data and the ones represented in the literature. Impact of safety-related dose reductions or discontinuations on sustained virologic response in HCV-infected patients: Results from the GUARD-C Cohort
BACKGROUND:
Despite the introduction of direct-acting antiviral agents for chronic hepatitis C virus (HCV) infection, peginterferon alfa/ribavirin remains relevant in many resource-constrained settings. The non-randomized GUARD-C cohort investigated baseline predictors of safety-related dose reductions or discontinuations (sr-RD) and their impact on sustained virologic response (SVR) in patients receiving peginterferon alfa/ribavirin in routine practice.
METHODS:
A total of 3181 HCV-mono-infected treatment-naive patients were assigned to 24 or 48 weeks of peginterferon alfa/ribavirin by their physician. Patients were categorized by time-to-first sr-RD (Week 4/12). Detailed analyses of the impact of sr-RD on SVR24 (HCV RNA <50 IU/mL) were conducted in 951 Caucasian, noncirrhotic genotype (G)1 patients assigned to peginterferon alfa-2a/ribavirin for 48 weeks. The probability of SVR24 was identified by a baseline scoring system (range: 0-9 points) on which scores of 5 to 9 and <5 represent high and low probability of SVR24, respectively.
RESULTS:
SVR24 rates were 46.1% (754/1634), 77.1% (279/362), 68.0% (514/756), and 51.3% (203/396), respectively, in G1, 2, 3, and 4 patients. Overall, 16.9% and 21.8% patients experienced 651 sr-RD for peginterferon alfa and ribavirin, respectively. Among Caucasian noncirrhotic G1 patients: female sex, lower body mass index, pre-existing cardiovascular/pulmonary disease, and low hematological indices were prognostic factors of sr-RD; SVR24 was lower in patients with 651 vs. no sr-RD by Week 4 (37.9% vs. 54.4%; P = 0.0046) and Week 12 (41.7% vs. 55.3%; P = 0.0016); sr-RD by Week 4/12 significantly reduced SVR24 in patients with scores <5 but not 655.
CONCLUSIONS:
In conclusion, sr-RD to peginterferon alfa-2a/ribavirin significantly impacts on SVR24 rates in treatment-naive G1 noncirrhotic Caucasian patients. Baseline characteristics can help select patients with a high probability of SVR24 and a low probability of sr-RD with peginterferon alfa-2a/ribavirin
Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey
Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training. Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults
Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We
estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from
1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories.
Methods We used data from 3663 population-based studies with 222 million participants that measured height and
weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate
trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children
and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the
individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference)
and obesity (BMI >2 SD above the median).
Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in
11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed
changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and
140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of
underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and
countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior
probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse
was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of
thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a
posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%)
with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and
obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for
both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such
as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged
children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls
in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and
42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents,
the increases in double burden were driven by increases in obesity, and decreases in double burden by declining
underweight or thinness.
Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an
increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy
nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of
underweight while curbing and reversing the increase in obesit
Impact of COVID-19 on cardiovascular testing in the United States versus the rest of the world
Objectives: This study sought to quantify and compare the decline in volumes of cardiovascular procedures between the United States and non-US institutions during the early phase of the coronavirus disease-2019 (COVID-19) pandemic.
Background: The COVID-19 pandemic has disrupted the care of many non-COVID-19 illnesses. Reductions in diagnostic cardiovascular testing around the world have led to concerns over the implications of reduced testing for cardiovascular disease (CVD) morbidity and mortality.
Methods: Data were submitted to the INCAPS-COVID (International Atomic Energy Agency Non-Invasive Cardiology Protocols Study of COVID-19), a multinational registry comprising 909 institutions in 108 countries (including 155 facilities in 40 U.S. states), assessing the impact of the COVID-19 pandemic on volumes of diagnostic cardiovascular procedures. Data were obtained for April 2020 and compared with volumes of baseline procedures from March 2019. We compared laboratory characteristics, practices, and procedure volumes between U.S. and non-U.S. facilities and between U.S. geographic regions and identified factors associated with volume reduction in the United States.
Results: Reductions in the volumes of procedures in the United States were similar to those in non-U.S. facilities (68% vs. 63%, respectively; p = 0.237), although U.S. facilities reported greater reductions in invasive coronary angiography (69% vs. 53%, respectively; p < 0.001). Significantly more U.S. facilities reported increased use of telehealth and patient screening measures than non-U.S. facilities, such as temperature checks, symptom screenings, and COVID-19 testing. Reductions in volumes of procedures differed between U.S. regions, with larger declines observed in the Northeast (76%) and Midwest (74%) than in the South (62%) and West (44%). Prevalence of COVID-19, staff redeployments, outpatient centers, and urban centers were associated with greater reductions in volume in U.S. facilities in a multivariable analysis.
Conclusions: We observed marked reductions in U.S. cardiovascular testing in the early phase of the pandemic and significant variability between U.S. regions. The association between reductions of volumes and COVID-19 prevalence in the United States highlighted the need for proactive efforts to maintain access to cardiovascular testing in areas most affected by outbreaks of COVID-19 infection
