    Clostridium Difficile Associated Diarrhea in Children with Hematological Malignancy-Experience from a Pediatric Oncologic Centre, Bangladesh

    Background: Clostridium difficile Associated Diarrhea (CDAD) is considered to be one of the commonest causes of nosocomial diarrhoea worldwide. Gastrointestinal infections in the form of diarrhoea are common in pediatric oncology patients in Bangabandhu Sheikh Mujib Medical University (BSMMU), Bangladesh. The study was conducted to find out the frequency of Clostridium difficile infection (CDI) among diarrheal children with haematological malignancy. Materials and Methods: This prospective observational study was conducted from April 2012 to March 2013 at the Pediatric Hematology and Oncology Unit, BSMMU, Bangladesh. Total 58 diarrheal episodes occurred in 51 children with various types of haematological malignancies were included consecutively. Faecal samples of the children were sent to International Centre for Diarrheal Disease Research, Bangladesh (ICDDR, B) laboratory for detection of Clostridium difficile antigen (GDH) and toxins (A and/ or B) by Enzyme Immunoassay (EIA). Results: Among 58 diarrheal episodes 22.4% faecal samples were positive for GDH, but none of the faecal samples was positive for toxin A and or B. There were a significant association with leucopenia, severe neutropenia; usage of meropenem plus vancomycin, cefepime plus amikacin, imipenem, cytarabine and omeprazole with GDH positive diarrheal episodes. Conclusion: Positive GDH antigen with a negative result for toxin indicates C. difficile colonization. Among GDH positive episodes, a significantly higher proportion of children had leucopenia, severe neutropenia and usage of some drugs known as risk factors for C. difficile infection.     Yields gap evaluation of wheat grown in Piedmont plain and Floodplain soils of Bangladesh through compositional nutrient diagnosis (CND) norm

    Mineralnutrient stress is one of the major yield gap factors, especially in floodplain and piedmont plain soil. The compositional nutrient diagnosis (CND) provides a plant nutrient imbalance index in statistical distribution patterns, which is important for adjusting the soil-plant systems specific fertilization for maintaining sustainable soil fertility. This study calculated the CND norms of wheat (Triticum aestivum L.) and identified optimum wheat yield target of high-yielding subpopulation in farmers' fields. It also categorized the most yield limiting nutrient(s) for wheat grown. Popular high-yielding wheat was grown in 62 farmers' fields, maintaining farmers' nutrient management plan (FP) and improved nutrient management plan (INM). Nutrient composition analysis was done from 62 young foliar composite samples, collected at 7th leaves stage (vegetative stage).The CND generic model gave 3.47 Mg ha–1 as minimum cutoff yield of the high-yield subpopulation. Nitrogen was identified as the core yield limiting nutrient for wheat in piedmont and floodplain soils. However, the yield limiting nutrients for wheat grown in the studied are were established the following series: N > S > K, Mg >P, Ca and Mn >Fe >B >Zn respectively. The CND generic model, allowed us to suggest thatN, P, K, Mn, B were the factors discriminating high- from low–yielding subpopulation in piedmont plain and floodplain soils of Bangladesh

    COMMON MUSCULOSKELETAL PROBLEMS ARISES AMONG WOMEN AFTER PARTURITION AT SELECTED AREA OF BANGLADESH

    Background: Pregnancy increases the susceptibility to developing musculoskeletal diseases. Postpartum musculoskeletal issues are frequently observed after childbirth, causing significant discomfort and distress for women. Purpose:To identify the common musculoskeletal problems that arises among women after the event of parturition at a selected area of Bangladesh. Methods:A cross-sectional study of Dhaka hospital postpartum mothers with musculoskeletal complaints was conducted. Study sites were Enam Medical College and Super Medical Hospital. Convenience sampling yielded 112 eligible individuals. Ethics, hospital approval, and participant anonymity were crucial. Face-to-face interviews using questionnaires and VAS were used to collect data. We obtained informed consent from participants to ensure confidentiality and withdrawal. We followed WHO and Bangladesh Medical Research Council ethical guidelines to ensure research integrity. Results:The Bangladeshi postpartum women study revealed common musculoskeletal disorders and their relationships. The majority of participants, 23–27 (55%) with one or two children, reported low back pain (57.1%), pelvic girdle discomfort (67.9%), and urine incontinence (38.4%). Pelvic girdle pain, diastasis recti, and urine incontinence were linked to childbirth. greater kids mean greater pain. The OREBRO scale showed varying severity (mean score: 101.64 ± 12.56). Childbirth affected musculoskeletal complaints, as shown by OMPQ ratings and participant age. This study prepares for more interventions. Conclusion:The study proposes that these findings can serve as fundamental information for the development of management and assessment programs in the postpartum musculoskeletal population

    Goat Genomic Resources: The Search for Genes Associated with Its Economic Traits

    Goat plays a crucial role in human livelihoods, being a major source of meat, milk, fiber, and hides, particularly under adverse climatic conditions. The goat genomics related to the candidate gene approach is now being used to recognize molecular mechanisms that have different expressions of growth, reproductive, milk, wool, and disease resistance. The appropriate literature on this topic has been reviewed in this article. Several genetic characterization attempts of different goats have reported the existence of genotypic and morphological variations between different goat populations. As a result, different whole-genome sequences along with annotated gene sequences, gene function, and other genomic information of different goats are available in different databases. The main objective of this review is to search the genes associated with economic traits in goats. More than 271 candidate genes have been discovered in goats. Candidate genes influence the physiological pathway, metabolism, and expression of phenotypes. These genes have different functions on economically important traits. Some genes have pleiotropic effect for expression of phenotypic traits. Hence, recognizing candidate genes and their mutations that cause variations in gene expression and phenotype of an economic trait can help breeders look for genetic markers for specific economic traits. The availability of reference whole-genome assembly of goats, annotated genes, and transcriptomics makes comparative genomics a useful tool for systemic genetic upgradation. Identification and characterization of trait-associated sequence variations and gene will provide powerful means to give positive influences for future goat breeding program

    Potential Applications of Nanotechnology in Agriculture: A Smart Tool for Sustainable Agriculture

    Most of the early uses of nanotechnology have come from material sciences, although applications in agriculture are still expanding. Due to a few comprehensive reviews, we described application of nanomaterials along with their fate in soil and interaction with soil and plant system. From synthesis to metabolism, nano-fertilizers like zinc, silver, selenium, titanium oxide have enhanced the physio-chemical characteristics of crop plants in every manner conceivable. On the other hand, it has the potential to minimize pesticide use by boosting reactivity and surface area of nanoparticles. Nanotechnology in pesticides will, without a doubt, replace the current way of pesticide application because of its efficacy. Nano-based approaches can readily overcome the constraints of conventional soil remediation technologies. While soil nanomaterials mobility has been investigated in a limited number of research studies, it’s likely the most critical gap in knowing the real risk of their transport. As well as enhancing plant nutrient absorption, nanomaterials may also be used to regulate soil microbial activity and stimulate plant defenses. When it comes to shipping food, nanotechnology has made things easier by extending the shelf life of most foods. While it offers tremendous potential for agricultural applications, the health effects of nanoparticles on plants, animals, and humans must be thoroughly investigated

    Diagnosed hematological malignancies in Bangladesh - a retrospective analysis of over 5000 cases from 10 specialized hospitals

    Background The global burden from cancer is rising, especially as low-income countries like Bangladesh observe rapid aging. So far, there are no comprehensive descriptions reporting diagnosed cancer group that include hematological malignancies in Bangladesh. Methods This was a multi-center hospital-based retrospective descriptive study of over 5000 confirmed hematological cancer cases in between January 2008 to December 2012. Morphological typing was carried out using the “French American British” classification system. Results A total of 5013 patients aged between 2 to 90 years had been diagnosed with malignant hematological disorders. A 69.2% were males (n = 3468) and 30.8% females (n = 1545), with a male to female ratio of 2.2:1. The overall median age at diagnosis was 42 years. Acute myeloid leukemia was most frequent (28.3%) with a median age of 35 years, followed by chronic myeloid leukemia with 18.2% (median age 40 years), non-Hodgkin lymphoma (16.9%; median age 48 years), acute lymphoblastic leukemia (14.1%; median age 27 years), multiple myeloma (10.5%; median age 55 years), myelodysplastic syndromes (4.5%; median age 57 years) and Hodgkin’s lymphoma (3.9%; median age 36 years). The least common was chronic lymphocytic leukemia (3.7%; median age 60 years). Below the age of 20 years, acute lymphoblastic leukemia was predominant (37.3%), followed by acute myeloid leukemia (34%). Chronic lymphocytic leukemia and multiple myeloma had mostly occurred among older patients, aged 50-over. Conclusions For the first time, our study presents the pattern and distribution of diagnosed hematological cancers in Bangladesh. It shows differences in population distributions as compared to other settings with possibly a lower presence of non-Hodgkin lymphoma. There might be under-reporting of affected women. Further studies are necessary on the epidemiology, genetics and potential environmental risk factors within this rapidly aging country

    A new estimate of carbon for Bangladesh forest ecosystems with their spatial distribution and REDD+ implications

    In tropical developing countries, reducing emissions from deforestation and forest degradation (REDD+) is becoming an important mechanism for conserving forests and protecting biodiversity. A key prerequisite for any successful REDD+ project, however, is obtaining baseline estimates of carbon in forest ecosystems. Using available published data, we provide here a new and more reliable estimate of carbon in Bangladesh forest ecosystems, along with their geo-spatial distribution. Our study reveals great variability in carbon density in different forests and higher carbon stock in the mangrove ecosystems, followed by in hill forests and in inland Sal (Shorea robusta) forests in the country. Due to its coverage, degraded nature, and diverse stakeholder engagement, the hill forests of Bangladesh can be used to obtain maximum REDD+ benefits. Further research on carbon and biodiversity in under-represented forest ecosystems using a commonly accepted protocol is essential for the establishment of successful REDD+ projects and for the protection of the country’s degraded forests and for addressing declining levels of biodiversity

    Perceptions about Telemedicine among Populations with Chronic Diseases amid COVID-19: Data from a Cross-Sectional Survey

    Chronic diseases, including non-communicable diseases (NCDs), have arisen as a severe threat to health and socio-economic growth. Telemedicine can provide both the highest level of patient satisfaction and the lowest risk of infection during a pandemic. The factors associated with its usage and patient adherence are not visible in Bangladesh's resource-constrained settings. Therefore, this study aimed to identify perceptions about telemedicine among populations with chronic diseases amid the COVID-19 pandemic. A closed-ended self-reported questionnaire was created, and the questionnaire was written, reviewed, and finalized by a public health investigator, a psychiatrist, and an epidemiologist. The data for this study were collected from individuals using simple random sampling and snowball sampling techniques. Ethics approval was granted, and written/verbal consent was taken before interviews. Most of the participants showed a positive attitude towards telemedicine. People aged 35-54 years old and a higher level of education were less frequently associated with willingness to receive telemedicine services for current chronic disease (WRTCCD) than their counterparts. People living in urban areas and lower-income participants were more strongly associated with WRTCCD. Additionally, people who did not lose their earnings due to the pandemic were less strongly associated with WRTCCD. However, the main strength of this research is that it is a broad exploration of patient interest in several general forms of telehealth. In Bangladesh, there are many opportunities for telemedicine to be integrated into the existing healthcare system, if appropriate training and education are provided for healthcare professionals

    Biomass fuel use, burning technique and reasons for the denial of improved cooking stoves by Forest User Groups of Rema-Kalenga Wildlife Sanctuary, Bangladesh

Improved cooking stoves (ICS) have been developed worldwide as an alternative household fuel burning device, as well as a climate change mitigation. A study was conducted among female Forest User Groups (FUGs) of Rema-Kalenga Wildlife Sanctuary, Bangladesh, to assess the status of ICS disseminated by the Forest Department (FD) under the Nishorgo (2009) Support Project, along with the community's biomass fuel consumption pattern. Wood consumption was highest (345kg month-1 household-1) followed by agricultural residues (60kg month-1 household-1), tree leaves (51kg month-1 household-1) and cow dung (25kg month-1 household-1). Neighbouring forests of the sanctuary was the core source for wood fuel, with little or no reduction in the extraction even after joining the FUG. Twenty-two species, both indigenous and introduced, were preferred as wood fuel. None of the respondents were found willing to use ICS although 43% owned one; either as a status symbol or to meet the conditions of the FD for membership in FUG. Seven negative features of the disseminated ICS were identified by households, which made them unwilling to use them further. Manufacturing faults may be responsible for some ICS demerits, while the FD failed to convince the community of the benefits. A proper examination of the disseminated ICS efficacy is crucial, with active involvement of community members. The Sustainable Energy Triangle Strategy (SETS) could be implemented for this purpose.
