    Thrombocytopenia in Preterm Infants with Intrauterine Growth Restriction

    Sick preterm infants often have thrombocytopenia at birth, and this is often associated with intrauterine growth restriction (IUGR), or birth weights less than the 10th percentile. The pathogenesis of the thrombocytopenia and its importance in IUGR are still unclear. We studied the characteristics of preterm IUGR infants with thrombocytopenia. Twenty-seven singleton Japanese preterm IUGR infants were born between January 2002 and June 2007 at Okayama University Hospital. Infants with malformation, chromosomal abnormalities, alloimmune thrombocytopenia, sepsis, and maternal aspirin ingestion were excluded. The infants were divided into group A (n&#65309;8), which had thrombocytopenia within 72h after birth, and group B (n&#65309;19), which did not. There were significant differences in birth weight, head circumference, umbilical artery (UA)-pulsatility index (PI), middle cerebral artery-PI, UA-pH, UA-pO2, and UA-pCO2. The infants in group A were smaller, had abnormal blood flow patterns, and were hypoxic at birth. We speculate that the infants with thrombocytopenia were more severely growth-restricted by chronic hypoxia. Thrombocytopenia is an important parameter for chronic hypoxia in the uterine.</p

    Aim: To improve nursing care for the elderly in order to allow them to live independently. This study has used the narrative of an elderly man to learn about what has given his life meaning.Method: Fifteen key concepts were extracted from interviews using the narrative approach.Results and Conclusions: Mr. A, now near the end of his life, has found meaning in living his life, not only for himself, but also for those around him. The elderly support their own physical and mental health,and adjust to day-to-day life. Nursing care for the elderly should watch for and support these self-supporting efforts

    Dental Implant Treatment for a Patient with Bilateral Cleft Lip and Palate

    Dental reconstruction in the cleft space is difficult in some patients with cleft lip and palate because of oronasal fistulas. Most of these patients receive a particle cancellous bone marrow (PCBM) graft to close the alveolar cleft, and secondary bone grafting is also required. Treatment options for the alveolar cleft including fixed or removable prostheses require the preparation of healthy teeth and are associated with functional or social difficulties. Recently, the effectiveness of dental implant treatment for cleft lip and palate patients has been reported. However, there have been few reports on the use of this treatment in bilateral cleft lip and palate patients. We report the case of a patient who had bilateral cleft lip and palate and was missing both lateral incisors. She received dental implant treatment after a PCBM graft and ramus bone onlay grafting (RBOG). A 34-month postoperative course was uneventful.</p

    Jiadifenolide induces the expression of cellular communication network factor (CCN) genes, and CCN2 exhibits neurotrophic activity in neuronal precursor cells derived from human induced pluripotent stem cells

    Jiadifenolide has been reported to have neurotrophin-like activity in primary rat cortical neurons, and also possesses neurotrophic effects in neuronal precursor cells derived from human induced pluripotent stem cells (hiPSCs), as we have previously reported. However, the molecular mechanisms by which jiadifenolide exerts its neurotrophic effects in rat and human neurons are unknown. Thus, we aimed to investigate the molecular mechanisms and pathways by which jiadifenolide promotes neurotrophic effects. Here, we found that jiadifenolide activated cellular communication network factor (CCN) signaling pathways by up-regulating mRNA level expression of CCN genes in human neuronal cells. We also found that CCN2 (also known as connective tissue growth factor, CTGF) protein promotes neurotrophic effects through activation of the p44/42 mitogen-activated protein kinase signaling pathway. This is the first discovery which links neurotrophic activity with CCN signaling

    本研究の目的は本学教育学科・幼児教育学科2016 年夏期短期海外研修プログラム改善の成果と課題を明らかにすることである。2015 年度の夏期短期海外研修の成果と課題は金子ら(2015)によって明らかにされた。その結果を元に2016 年度のプログラムに改善を加えた。主な改善点は,レーニア山旅行中止,小学校のサマーキャンプへの参加,学びの振り返りの時間を持つなどである。改善の結果,学生がどのように評価しているかを調査した。調査は質問紙法(2015 年度,2016 年度の結果比較)と添乗員のプログラム記録比較と引率者・同行者の観察によって行った。その結果,昨年度に比べて研修の満足度が高くなったり,楽しかったことに教育施設の訪問を挙げる学生が増えてきたりなどプラスの効果が見られた

    Neurogenic Dumbbell Tumor Resected by Combined Posterior and Thoracoscopic Approaches

    The patient was a 51-year-old female with a chief complaint of numbness of the right thigh. A dumbbell tumor compressing the 11th thoracic spinal cord was present in the posterior mediastinum on chest computed tomography and myelography. In surgery, the tumor in the spinal canal was resected by a posterior approach, and the residual tumor was completely excised by thoracoscopic surgery. The tumor measured 78 48 mm, and histopathologically diagnosed as schwannoma. The surgical procedure for neurogenic dumbbell tumors should be decided based on Eden’s classification. For types II and III, such as this patient, excision of the tumor in the spinal canal should be performed to avoid spinal cord damage, followed by thoracoscopic complete resection. Thoracoscopic surgery was applicable even though the tumor was large

    武庫川女子大学文学部教育学科・短大幼児教育学科は,米国ワシントン州にある St. Martin’s University と友好的な関係を結んでいて,8 月に実施される 3 週間の研修プログラムに毎年 20 人から 30 人の学生が参加している。本研究の目的は,2015年に行われた研修プログラムの成果と課題を明らかにする事である。研究の目的を達成するために,参加学生対象の事前,事後のアンケート結果,事前指導を行った担当者の指導内容と所見,引率助手と海外研修担当助手の指導内容と所見から,研修の成果と課題を明らかにした。その結果,学生はこの研修に髙い満足度を示し,異なる文化や英語でのコミュニケーションについて大きな学びをしていることから,この研修は大きな成果をあげている事がわかった。今後の課題としては,研修プログラムに休日や振り返りを入れる必要があること,そのために不要な内容を削除すること,健康上の問題が発生した場合の対処や事前指導をより効果的なものにするための改善などが必要である事が明らかになった

    The Relationship between Individual Difference in Repressive Stles and Grief Reaction Coping Behaviors, and Onset of Pathological Grief, Anxiety and Mood Disorders after Child Loss

    We examined the relationship between parental repressive styles, grief reaction/post-bereavement coping behavior and psychiatric symptoms in parents who had lost young children, In Study I, we administered a set of questionnaires to a sample of 177 parents who had experienced bereavement of children within the past several years in order to inquire about grief response, coping behavior, repressive styles, and social support .After controlling for sex, age, income, and self esteem, 1) subjects who had high anxiety (sensitizer and repressive-anxiety) showed stronger grief response whereas those with low anxiety (repressor and low-anxiety) showed weak response; 2) repressors were more likely to accept death and resolute grief after the loss than others; 3) those who had higher anxiety were more likely to ruminate after death; and 4) sensitizers were less likely to satisfy perceived social support. In Study II we interviewed 60 bereaved parents using a semi-structured diagnostic interview schedule to yield diagnoses of the parents based on the Diagnostic and Statistical Manual of Mental Disorders 4th Edition (DSM-IV). Parents with repressive-anxiety were more likely to manifest pathological grief and those parents recognized as sensitizers were more likely to have an onset of DSM-IV Major Depression or Social Phobia after the child\u27s death.抑圧様式と悲嘆反応、死別後の対処行動、精神症状との関連性について検討した。研究Iでは幼い子どもと死別した親177名を対象に悲嘆反応、対処行動、抑圧様式、ソーシャルサポートを測定した。性別、年齢、収入などを統制し分析した結果、①不安水準の高い sensitizer, repressive Anxiety 各群では悲嘆反応が強心不安水準の低い repressor, low-anxiety各群では悲嘆反応が弱い、②repressor群では悲嘆がすでに解決したことと認知し、死別体験を肯定的に捉える傾向が強い、③不安水準が高いと死別後に内的に考え込む対処を多く行う、④sensitizer群では知覚されたソーシャルサポートに対する満足度が低い傾向にあった。研究IIでは60名に半構造化面接を行い、死別後に罹患した精神疾患の診断を行った。その結果,⑤病的悲嘆はrepressive- anxiety傾向と,⑥大うつ病性障害や社会恐怖はsensitization傾向とそれぞれ関連することが示された
