    Introduction: A large proportion of under-five-year kids are suffering from malnutrition. A study was conducted to assess the knowledge of mothers regarding malnutrition and its prevention and evaluate effectiveness of structured teaching program regarding malnutrition and its prevention in terms of knowledge gain in mothers of under-five children attending a crèche run by an NGO in a slum area of New Delhi.Methodology: Quantitative research approach with one group pre-test, post-test design was used. Tool used for generating necessary data was a structured knowledge questionnaire, after establishing its validity and reliability. Purposive sampling technique was used to select 45 mothers having children under five years of age. The study was conducted at a crèche run by an NGO in a slum area of Delhi.Results: Before administration of the structured teaching program, 18 (40%) mothers had poor knowledge, 15 (33.3%) had average knowledge and 12 (26.7%) had good knowledge about malnutrition and its prevention, while after administration of structured teaching program, 12 (26.7%) had poor knowledge, 21 (46.7%) had average knowledge and 12 (26.7%) had good knowledge about malnutrition and its prevention indicating that the intervention was effective. There was significant relationship between knowledge gain and age, education and monthly family income of mothers. Conclusion: Finding of the study revealed that mothers having children under five years of age had poor knowledge about malnutrition and its prevention. The structured teaching program was an effective tool to enhance the knowledge of mothers

    Post-implementation Review of the Himalaya Home Care Project for Home Isolated COVID-19 Patients in Nepal

    Background: The emergence of coronavirus disease 2019 (COVID-19) has resulted in a pandemic that has significantly impacted healthcare systems at a global level. Health care facilities in Nepal, as in other low- and middle-income countries, have limited resources for the treatment and management of COVID-19 patients. Only critical cases are admitted to the hospital resulting in most patients in home isolation. Methods: Himalaya Home Care (HHC) was initiated to monitor and provide counseling to home isolated COVID-19 patients for disease prevention, control, and treatment. Counselors included one physician and four nurses. Lists of patients were obtained from district and municipal health facilities. HHC counselors called patients to provide basic counseling services. A follow-up check-in phone call was conducted 10 days later. During this second call, patients were asked about their perceptions of the HHC program. Project objects were: (1) To support treatment of home isolated persons with mild to moderate COVID-19, decrease burden of hospitalizations, and decrease risks for disease transmission; and, (2) To improve the health status of marginalized, remote, and vulnerable populations in Nepal during the COVID-19 pandemic. Results: Data from 5823 and 3988 patients from May 2021-February 2022 were entered in initial and follow-up forms on a REDCap database. The majority of patients who received counseling were satisfied. At follow-up, 98.4% of respondents reported that HHC prevented hospitalization, 76.5% reported they could manage their symptoms at home, and 69.5% reported that counseling helped to limit the spread of COVID-19 in their household. Conclusions: Telehealth can be an essential strategy for providing services while keeping patients and health providers safe during the COVID-19 pandemic

    SARS-CoV-2 B.1.617.2 Delta variant replication and immune evasion

    Abstract: The B.1.617.2 (Delta) variant of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was first identified in the state of Maharashtra in late 2020 and spread throughout India, outcompeting pre-existing lineages including B.1.617.1 (Kappa) and B.1.1.7 (Alpha)1. In vitro, B.1.617.2 is sixfold less sensitive to serum neutralizing antibodies from recovered individuals, and eightfold less sensitive to vaccine-elicited antibodies, compared with wild-type Wuhan-1 bearing D614G. Serum neutralizing titres against B.1.617.2 were lower in ChAdOx1 vaccinees than in BNT162b2 vaccinees. B.1.617.2 spike pseudotyped viruses exhibited compromised sensitivity to monoclonal antibodies to the receptor-binding domain and the amino-terminal domain. B.1.617.2 demonstrated higher replication efficiency than B.1.1.7 in both airway organoid and human airway epithelial systems, associated with B.1.617.2 spike being in a predominantly cleaved state compared with B.1.1.7 spike. The B.1.617.2 spike protein was able to mediate highly efficient syncytium formation that was less sensitive to inhibition by neutralizing antibody, compared with that of wild-type spike. We also observed that B.1.617.2 had higher replication and spike-mediated entry than B.1.617.1, potentially explaining the B.1.617.2 dominance. In an analysis of more than 130 SARS-CoV-2-infected health care workers across three centres in India during a period of mixed lineage circulation, we observed reduced ChAdOx1 vaccine effectiveness against B.1.617.2 relative to non-B.1.617.2, with the caveat of possible residual confounding. Compromised vaccine efficacy against the highly fit and immune-evasive B.1.617.2 Delta variant warrants continued infection control measures in the post-vaccination era

    A study has been made, by light and electron microscopy, of the structure of the phloem tissue and sieve tubes of Sallx oaprsea. and the relevance of the findings to the electrokinetic theory of transport has been discussed. The anatomical features of the phloem at the cellular level do not seem particularly favourable to the theory; on the other hand, the ultra structure of the sieve tubes and in particular the prevalence of "slime" in this species and many other species being composed of banded fibrils, seems to favour the electro kinetic theory at least as much, as any other.
