Nonlinear analysis of r.c. framed buildings retrofitted with elastomeric and friction bearings under near-fault earthquakes
Reinforced concrete (r.c.) framed buildings designed in compliance with inadequate seismic classifications and code provisions present in many cases a high vulnerability and need to be retrofitted. To this end, the insertion of a base isolation system allows a considerable reduction of the seismic loads transmitted to the superstructure. However, strong near-fault ground motions, which are characterised by long-duration horizontal pulses, may amplify the inelastic response of the superstructure and induce a failure of the isolation system. The above considerations point out the importance of checking the effectiveness of different isolation systems for retrofitting a r.c. framed structure. For this purpose, a numerical investigation is carried out with reference to a six-storey r.c. framed building, which, primarily designed (as to be a fixed-base one) in compliance with the previous Italian code (DM96) for a medium-risk seismic zone, has to be retrofitted by insertion of an isolation system at the base for attaining performance levels imposed by the current Italian code (NTC08) in a high-risk seismic zone. Besides the (fixed-base) original structure, three cases of base isolation are studied: elastomeric bearings acting alone (e.g. HDLRBs); in-parallel combination of elastomeric and friction bearings (e.g. high-damping-laminated-rubber bearings, HDLRBs and steel-PTFE sliding bearings, SBs); friction bearings acting alone (e.g. friction pendulum bearings, FPBs). The nonlinear analysis of the fixed-base and base-isolated structures subjected to horizontal components of near-fault ground motions is performed for checking plastic conditions at the potential critical (end) sections of the girders and columns as well as critical conditions of the isolation systems. Unexpected high values of ductility demand are highlighted at the lower floors of all base-isolated structures, while re-centring problems of the base isolation systems under near-fault earthquakes are expected in case of friction bearings acting alone (i.e. FPBs) or that in combination (i.e. SBs) with HDLRBs
Effects of near-fault ground motions on the nonlinear behaviour of reinforced concrete framed buildings
The design provisions of current seismic codes are generally not very accurate for assessing effects of near-fault ground motions on reinforced concrete (r.c.) spatial frames, because only far-fault ground motions are considered in the seismic codes. Strong near-fault earthquakes are characterized by long-duration (horizontal) pulses and high values of the ratio α PGA of the peak value of the vertical acceleration, PGAV, to the analogous value of the horizontal acceleration, PGAH, which can become critical for girders and columns. In this work, six- and twelve-storey r.c. spatial frames are designed according to the provisions of the Italian seismic code, considering the horizontal seismic loads acting (besides the gravity loads) alone or in combination with the vertical ones. The nonlinear seismic analysis of the test structures is performed using a step-by-step procedure based on a two-parameter implicit integration scheme and an initial stress-like iterative procedure. A lumped plasticity model based on the Haar-Karman principle is adopted to model the inelastic behaviour of the frame members. For the numerical investigation, five near-fault ground motions with high values of the acceleration ratio α PGA are considered. Moreover, following recent seismological studies, which allow the extraction of the largest (horizontal) pulse from a near-fault ground motion, five pulse-type (horizontal) ground motions are selected by comparing the original ground motion with the residual motion after the pulse has been extracted. The results of the nonlinear dynamic analysis carried out on the test structures highlighted that horizontal and vertical components of near-fault ground motions may require additional consideration in the seismic codes
The masked invader strikes again: The conquest of Italy by the Northern raccoon
The Northern raccoon Procyon lotor is a species native to North and Central America, but alien populations have established in Europe, several Caribbean islands, Azerbaijan, Uzbekistan and Japan, being introduced for fur farming, hunting, or as pets/attraction in animal parks. In the introduced range, raccoons may impact on breeding birds and amphibians, exert crop damages and transmit pathologies to wild species and humans. The species has been introduced also in Italy, where the only known reproductive population is observed since 2004 in Lombardy, along the Adda river. We reconstructed the current distribution range of the Northern raccoon in Italy, collecting information from scientific papers, articles in newspapers and books, as well from experts and local reporters. A total of 53 occurrence points were collected from observation sites. Since 2008, records from Lombardy increased, and sporadic observations were reported from seven other regions. A complete lack of records from the Northernmost provinces of Lombardy (Varese, Como and Sondrio) suggests that the only Italian population does not derive from a range expansion from Switzerland, but it should be considered as an independent, new introduction. Accidental observations of single individuals possibly escaped from captivity are often ignored, and only few animals were removed from the wild. An analysis of the potential distribution of the species was performed in a species distribution modeling framework (MaxEnt). A global model was built up considering the occurrences of reproductive populations from the native range and introduced areas in Europe and Japan and then projected to Italy. The model suggested a good suitability for the plains in Central-Northern Italy and a very low suitability of the Alpine region, thus providing support to the hypothesis that the Italian population did not derive from dispersal from Switzerland. If escapes or releases of raccoons will continue, there is a risk that the species could colonize other areas, making its containment more difficult
AIRO Breast Cancer Group Best Clinical Practice 2022 Update
Introduction: Breast cancer is the most common tumor in women and represents the leading cause of cancer death. Radiation therapy plays a key-role in the treatment of all breast cancer stages. Therefore, the adoption of evidence-based treatments is warranted, to ensure equity of access and standardization of care in clinical practice.Method: This national document on the highest evidence-based available data was developed and endorsed by the Italian Association of Radiation and Clinical Oncology (AIRO) Breast Cancer Group.We analyzed literature data regarding breast radiation therapy, using the SIGN (Scottish Intercollegiate Guidelines Network) methodology (www.sign.ac.uk). Updated findings from the literature were examined, including the highest levels of evidence (meta-analyses, randomized trials, and international guidelines) with a significant impact on clinical practice. The document deals with the role of radiation therapy in the treatment of primary breast cancer, local relapse, and metastatic disease, with focus on diagnosis, staging, local and systemic therapies, and follow up. Information is given on indications, techniques, total doses, and fractionations.Results: An extensive literature review from 2013 to 2021 was performed. The work was organized according to a general index of different topics and most chapters included individual questions and, when possible, synoptic and summary tables. Indications for radiation therapy in breast cancer were examined and integrated with other oncological treatments. A total of 50 questions were analyzed and answered.Four large areas of interest were investigated: (1) general strategy (multidisciplinary approach, contraindications, preliminary assessments, staging and management of patients with electronic devices); (2) systemic therapy (primary, adjuvant, in metastatic setting); (3) clinical aspects (invasive, non-invasive and micro-invasive carcinoma; particular situations such as young and elderly patients, breast cancer in males and cancer during pregnancy; follow up with possible acute and late toxicities; loco-regional relapse and metastatic disease); (4) technical aspects (radiation after conservative surgery or mastectomy, indications for boost, lymph node radiotherapy and partial breast irradiation).Appendixes about tumor bed boost and breast and lymph nodes contouring were implemented, including a dedicated web application. The scientific work was reviewed and validated by an expert group of breast cancer key-opinion leaders.Conclusions: Optimal breast cancer management requires a multidisciplinary approach sharing therapeutic strategies with the other involved specialists and the patient, within a coordinated and dedicated clinical path. In recent years, the high-level quality radiation therapy has shown a significant impact on local control and survival of breast cancer patients. Therefore, it is necessary to offer and guarantee accurate treatments according to the best standards of evidence-based medicine
Infected pancreatic necrosis: outcomes and clinical predictors of mortality. A post hoc analysis of the MANCTRA-1 international study
: The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135-15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359-5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138-5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184-5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598-9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090-6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286-5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p < 0.001; 95% CI 1.912-7.442; aOR 3.772), whereas endoscopic drainage of pancreatic necrosis (p = 0.018; 95% CI 0.138-0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143-0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible. Influence of Elastomeric Bearings in Tension on the Seismic Performance of Base-Isolated r.c. Buildings
Elastomeric bearings are commonly used in base-isolation systems to protect the structures from earthquake damages. Their design is usually developed by using nonlinear models where only the effects of shear and compressive loads are considered, but uncertainties still remain about consequences of the tensile loads produced by severe earthquakes like the near-fault ones. The present work aims to highlight the relapses of tension on the response of bearings and superstructure. To this end, three-, seven- and ten-storey r.c. framed buildings are designed in line with the current Italian seismic code, with a base-isolation system constituted of High-Damping-Rubber Bearings (HDRBs) designed for three values of the ratio between the vertical and horizontal stiffnesses. Experimental and analytical results available in literature are used to propose a unified nonlinear model of the HDRBs, including cavitation and post-cavitation of the elastomer. Nonlinear incremental dynamic analyses of the test structures are carried out using a homemade computer code, where other models of HDRBs considering only some nonlinear phenomena are implemented. Near-fault earthquakes with comparable horizontal and vertical components, prevailing horizontal component and prevailing vertical component are considered as seismic input. Numerical results highlight that a precautionary estimation of response parameters of the HDRBs is attained referring to the proposed model, while its effects on the nonlinear response of the superstructure are less conservative
