Experiences of physical activity during pregnancy resulting from in vitro fertilisation: an interpretative phenomenological analysis
Objective To explore the qualitative experiences and decision-making processes surrounding physical activity (PA) for women who have undergone IVF treatment.
Background Physical activity (PA) during pregnancy is safe for both mother and fetus in the majority of cases, including for women who have undergone in vitro fertilisation (IVF) treatment; however, there is a paucity of research into decision-making and PA in this population.
Methods Eight women, who had undergone successful IVF treatment and were currently pregnant or had given birth within the last two years, participated in semi-structured interviews about their experiences of infertility and PA during pregnancy. Interview transcripts were analysed using interpretative phenomenological analysis.
Results Three superordinate themes emerged from the data: ‘navigating away from childlessness and towards motherhood’, ‘negotiating a safe passage’, and ‘balancing the challenges of pregnancy with the needs of the self’. Ten subthemes indicated the processes adopted to navigate experiences of infertility, the IVF process, and subsequent decision-making about PA during pregnancy.
Conclusion PA during pregnancy was experienced as a way to soothe the self and control the experience of pregnancy; however, this was mediated by concerns about safety and physical limitations on PA. Limitations of the study are considered, as well as implications for clinical practice and directions for future research
Resolving the chemical substructure of Orion-KL
The Kleinmann-Low nebula in Orion (Orion-KL) is the nearest example of a
high-mass star-forming environment. For the first time, we complemented 1.3 mm
Submillimeter Array (SMA) interferometric line survey with IRAM 30 m
single-dish observations of the Orion-KL region. Covering a 4 GHz bandwidth in
total, this survey contains over 160 emission lines from 20 species (25
isotopologues), including 11 complex organic molecules (COMs).
At a spatial resolution of 1200 AU, the continuum substructures are resolved.
Extracting the spectra from individual substructures and providing the
intensity-integrated distribution map for each species, we studied the
small-scale chemical variations in this region. Our main results are: (1) We
identify lines from the low-abundance COMs CH3COCH3 and CH3CH2OH, as well as
tentatively detect CH3CHO and long carbon-chains C6H and HC7N. (2) We find that
while most COMs are segregated by type, peaking either towards the hot core
(e.g., N-bearing species) or the compact ridge (e.g., O-bearing species like
HCOOCH3 and CH3OCH3), while the distributions of others do not follow this
segregated structure (e.g., CH3CH2OH, CH3OH, CH3COCH3). (3) We find a second
velocity component of HNCO, SO2, 34SO2, and SO lines, which may be associated
with a strong shock event in the low-velocity outflow. (4) Temperatures and
molecular abundances show large gradients between central condensations and the
outflow regions, illustrating a transition between hot molecular core and
shock-chemistry dominated regimes.
Our observations of spatially resolved chemical variations in Orion-KL
provide the nearest reference source for hot molecular core and outflow
chemistry, which will be an important example for interpreting the chemistry of

Conserved Noether Currents, Utiyama's Theory of Invariant Variation, and Velocity Dependence in Local Gauge Invariance
The paper discusses the mathematical consequences of the application of
derived variables in gauge fields. Physics is aware of several phenomena, which
depend first of all on velocities (like e.g., the force caused by charges
moving in a magnetic field, or the Lorentz transformation). Applying the
property of the second Noether theorem, that allowed generalised variables,
this paper extends the article by Al-Kuwari and Taha (1991) with a new
conclusion. They concluded that there are no extra conserved currents
associated with local gauge invariance. We show, that in a more general case,
there are further conserved Noether currents. In its method the paper
reconstructs the clue introduced by Utiyama (1956, 1959) and followed by
Al-Kuwari and Taha (1991) in the presence of a gauge field that depends on the
co-ordinates of the velocity space. In this course we apply certain (but not
full) analogies with Mills (1989). We show, that handling the space-time
coordinates as implicit variables in the gauge field, reproduces the same
results that have been derived in the configuration space (i.e., we do not lose
information), while the proposed new treatment gives additional information
extending those. The result is an extra conserved Noether current.Comment: 14 page

The four HII regions in the Sgr A East complex: A, B, C, and D, represent
evidence of recent massive star formation in the central ten parsecs. Using
Paschen-alpha images taken with HST and 8.4 GHz VLA data, we construct an

Fluorescent carbon dioxide indicators
Over the last decade, fluorescence has become the dominant tool in biotechnology and medical imaging. These exciting advances have been underpinned by the advances in time-resolved techniques and instrumentation, probe design, chemical / biochemical sensing, coupled with our furthered knowledge in biology. Complementary volumes 9 and 10, Advanced Concepts of Fluorescence Sensing: Small Molecule Sensing and Advanced Concepts of Fluorescence Sensing: Macromolecular Sensing, aim to summarize the current state of the art in fluorescent sensing. For this reason, Drs. Geddes and Lakowicz have invited chapters, encompassing a broad range of fluorescence sensing techniques. Some chapters deal with small molecule sensors, such as for anions, cations, and CO2, while others summarize recent advances in protein-based and macromolecular sensors. The Editors have, however, not included DNA or RNA based sensing in this volume, as this were reviewed in Volume 7 and is to be the subject of a more detailed volume in the near future

Bump start needed: linking guidelines, policy and practice in promoting physical activity during and beyond pregnancy
First paragraph: There is compelling evidence that regular physical activity (PA) during pregnancy benefits both mother and baby.1 2 Notably, physical and psychological benefits are evident in the literature, such as marked reductions in the development of gestational diabetes and hypertensive disorders, alongside improvements in depressive symptoms and cardiorespiratory fitness.1 2 The evidence base has been reflected by recent policy initiatives, for example, in 2017 (relaunched in 2019), the UK‘s chief medical officers (CMOs) published PA guidelines for pregnant women, which made substantial strides in unifying and translating the evidence into recommendations.1 The CMO guidelines are aimed at supporting health professionals to provide consistent, evidence-based PA messages to women throughout pregnancy.1 Recently, the Chartered Institute for the Management of Sport and Physical Activity have updated their professional standards for working with antenatal and postnatal clients to align with these CMO guidelines.3 However, not all women have access to professionals with this level of expertise and training, potentially limiting the impact of the CMO guidelines
Outcomes of a remote, decentralized health center-based HIV/AIDS antiretroviral program in Zambia, 2003 to 2007
A cross-sectional study of patients living with HIV/ AIDS treated during 2003 to 2007 in decentralized, rural health centers in Zambia was performed to measure virological outcomes after 12 months of antiretroviral therapy and identify factors associated with virological failure. Data from 228 patients who started antiretroviral therapy >12 months prior were analyzed. In all, 93% received stavudine + lamivudine + nevirapine regimens, and median antiretroviral therapy duration was 23.5 months (interquartile range 20-28). Of the 205 patients tested for viral load, 177 (86%) had viral load <1000 copies/mL. Probability of developing virological failure (viral load >1000 copies/mL) was 8.9% at 24 months and 19.6% at 32 months. Predictors for virological failure were <100% adherence, body mass index <18.5 kg/m(2), and women <40 years old. Of those with virological failure who underwent 3 to 6 months of intensive adherence counseling, 45% obtained virological success. In a remote, resource-limited setting in decentralized health centers, virological and immunological assessments of patients on antiretroviral therapy >12 months showed that positive health outcomes are achievable
