GPs’ strategies in exploring the preschool child’s wellbeing in the paediatric consultation
Background:
Although General Practitioners (GPs) are uniquely placed to identify children with emotional, social, and behavioural problems, they succeed in identifying only a small number of them. The aim of this article is to explore the strategies, methods, and tools employed by GPs in the assessment of the preschool child’s emotional, mental, social, and behavioural health. We look at how GPs address parental care of the child in general and in situations where GPs have a particular awareness of the child.
Method:
Twenty-eight Danish GPs were purposively selected to take part in a qualitative study which combined focus-group discussions, observation of child consultations, and individual interviews with GPs.
Results:
Analysis of the data suggests that GPs have developed a set of methods, and strategies to assess the preschool child and parental care of the child. They look beyond paying narrow attention to the physical health of the child and they have expanded their practice to include the relations and interactions in the consultation room. The physical examination of the child continues to play a central role in doctor-child communication.
Conclusion:
The participating GPs’ strategies helped them to assess the wellbeing of the preschool child but they often find it difficult to share their impressions with parents
Whistle communication in mammal-eating killer whales (Orcinus orca): further evidence for acoustic divergence between ecotypes
Public signaling plays an important role in territorial and sexual displays in animals; however, in certain situations, it is advantageous to keep signaling private to prevent eavesdropping by unintended receivers. In the northeastern Pacific, two populations of killer whales (Orcinus orca), fish-eating “resident” killer whales and mammal-eating “transient” killer whales, share the same habitat. Previous studies have shown that residents use whistles as private signals during close-range communication, where they probably serve to coordinate behavioral interactions. Here, we investigated the whistling behavior of mammal-eating killer whales, and, based on divergent social structures and social behaviors between residents and transients, we predicted to find differences in both whistle usage and whistle parameters. Our results show that, like resident killer whales, transients produce both variable and stereotyped whistles. However, clear differences in whistle parameters between ecotypes show that the whistle repertoire of mammal-eating killer whales is clearly distinct from and less complex than that of fish-eating killer whales. Furthermore, mammal-eating killer whales only produce whistles during “milling after kill” and “surface-active” behaviors, but are almost completely silent during all other activities. Nonetheless, whistles of transient killer whales may still serve a role similar to that of resident killer whales. Mammal-eating killer whales seem to be under strong selection to keep their communication private from potential prey (whose hearing ranges overlap with that of killer whales), and they appear to accomplish this mainly by restricting vocal activity rather than by changes in whistle parameters
Pedestrian, Crowd, and Evacuation Dynamics
This contribution describes efforts to model the behavior of individual
pedestrians and their interactions in crowds, which generate certain kinds of
self-organized patterns of motion. Moreover, this article focusses on the
dynamics of crowds in panic or evacuation situations, methods to optimize
building designs for egress, and factors potentially causing the breakdown of

Strain-induced Evolution of Electronic Band Structures in a Twisted Graphene Bilayer
Here we study the evolution of local electronic properties of a twisted
graphene bilayer induced by a strain and a high curvature. The strain and
curvature strongly affect the local band structures of the twisted graphene
bilayer; the energy difference of the two low-energy van Hove singularities
decreases with increasing the lattice deformations and the states condensed
into well-defined pseudo-Landau levels, which mimic the quantization of massive
Dirac fermions in a magnetic field of about 100 T, along a graphene wrinkle.
The joint effect of strain and out-of-plane distortion in the graphene wrinkle
also results in a valley polarization with a significant gap, i.e., the
eight-fold degenerate Landau level at the charge neutrality point is splitted
into two four-fold degenerate quartets polarized on each layer. These results
suggest that strained graphene bilayer could be an ideal platform to realize
the high-temperature zero-field quantum valley Hall effect.Comment: 4 figure
P-odd and CP-odd Four-Quark Contributions to Neutron EDM
In a class of beyond-standard-model theories, CP-odd observables, such as the
neutron electric dipole moment, receive significant contributions from
flavor-neutral P-odd and CP-odd four-quark operators. However, considerable
uncertainties exist in the hadronic matrix elements of these operators strongly
affecting the experimental constraints on CP-violating parameters in the
theories. Here we study their hadronic matrix elements in combined chiral
perturbation theory and nucleon models. We first classify the operators in
chiral representations and present the leading-order QCD evolutions. We then
match the four-quark operators to the corresponding ones in chiral hadronic
theory, finding symmetry relations among the matrix elements. Although this
makes lattice QCD calculations feasible, we choose to estimate the
non-perturbative matching coefficients in simple quark models. We finally
compare the results for the neutron electric dipole moment and P-odd and CP-odd
pion-nucleon couplings with the previous studies using naive factorization and
QCD sum rules. Our study shall provide valuable insights on the present

The Economic Impact of Early Life Environmental Tobacco Smoke Exposure: Early Intervention for Developmental Delay
BACKGROUND AND OBJECTIVES: Early-life exposure to environmental tobacco smoke (ETS) can result in developmental delay as well as childhood asthma and increased risk of cancer. The high cost of childhood asthma related to ETS exposure has been widely recognized; however, the economic impact of ETS-related developmental delay has been less well understood.
METHODS AND RESULTS:The Columbia Center for Children\u27s Environmental Health (CCCEH) has reported adverse effects of prenatal ETS exposure on child development in a cohort of minority women and children in New York City (odds ratio of developmental delay = 2.36; 95% confidence interval 1.22-4.58). Using the environmentally attributable fraction (EAF) approach, we estimated the annual cost of one aspect of ETS-related developmental delay: Early Intervention Services. The estimated cost of these services per year due to ETS exposure is \u3e Dollars 50 million per year for New York City Medicaid births and Dollars 99 million per year for all New York City births.
CONCLUSION:The high annual cost of just one aspect of developmental delay due to prenatal exposure to ETS provides further impetus for increased prevention efforts such as educational programs to promote smoke-free homes, additional cigarette taxes, and subsidizing of smoking cessation programs
The clinical features of the piriformis syndrome: a systematic review
Piriformis syndrome, sciatica caused by compression of the sciatic nerve by the piriformis muscle, has been described for over 70 years; yet, it remains controversial. The literature consists mainly of case series and narrative reviews. The objectives of the study were: first, to make the best use of existing evidence to estimate the frequencies of clinical features in patients reported to have PS; second, to identify future research questions. A systematic review was conducted of any study type that reported extractable data relevant to diagnosis. The search included all studies up to 1 March 2008 in four databases: AMED, CINAHL, Embase and Medline. Screening, data extraction and analysis were all performed independently by two reviewers. A total of 55 studies were included: 51 individual and 3 aggregated data studies, and 1 combined study. The most common features found were: buttock pain, external tenderness over the greater sciatic notch, aggravation of the pain through sitting and augmentation of the pain with manoeuvres that increase piriformis muscle tension. Future research could start with comparing the frequencies of these features in sciatica patients with and without disc herniation or spinal stenosis
Emergent complex neural dynamics
A large repertoire of spatiotemporal activity patterns in the brain is the
basis for adaptive behaviour. Understanding the mechanism by which the brain's
hundred billion neurons and hundred trillion synapses manage to produce such a
range of cortical configurations in a flexible manner remains a fundamental
problem in neuroscience. One plausible solution is the involvement of universal
mechanisms of emergent complex phenomena evident in dynamical systems poised
near a critical point of a second-order phase transition. We review recent
theoretical and empirical results supporting the notion that the brain is
naturally poised near criticality, as well as its implications for better
understanding of the brain
