    Effects of a Geographically-Targeted Intervention and Creative Outreach to Reduce Shelter Intake in Portland, Oregon

    Animal shelters focus much of their efforts towards decreasing euthanasia and one of the best ways to reduce euthanasia risk may be to prevent cats and dogs from ever entering a shelter. This study, conducted in Portland, Oregon, relied on the capabilities of Geographic Information Systems (GIS) to precisely and scientifically identify an intervention area (with high shelter intake) and to identify control areas to compare the project results with community-wide trends. The intervention itself was designed and implemented in a comprehensive way by seeking numerous paths to engage pet owners and reduce shelter intake of cats and Pit Bull type dogs. This research highlighted the ability of Geographic Information Systems (GIS) to significantly improve a community's capacity to identify the most appropriate locations to focus resources and to closely track and measure interventions. Portland's targeted intervention to reduce shelter intake utilized many outreach tools with varying levels of impact. The overall intervention yielded a reduction in intake of owned cats that was greater in total numbers and percentage than four control areas. Furthermore, this work identified a percentage of cat spay/neuter out of the estimated number of owned, originally intact cats within the intervention and control areas. As percentages approached or surpassed 20%, those areas realized larger intake reductions than control areas with lower percentages

    Online healthy lifestyle support in the perinatal period: What do women want and do they use it?

    Unhealthy weight gain and retention during pregnancy and postpartum is detrimental to mother and child. Although various barriers limit the capacity for perinatal health care providers (PHCPs) to offer healthy lifestyle counselling, they could guide women to appropriate online resources. This paper presents a project designed to provide online information to promote healthy lifestyles in the perinatal period. Focus groups or interviews were held with 116 perinatal women and 76 PHCPs to determine what online information perinatal women and PHCPs want, in what form, and how best it should be presented. The results indicated that women wanted smartphone applications (apps) linked to trustworthy websites containing short answers to everyday concerns; information on local support services; and personalised tools to assess their nutrition, fitness and weight. Suggestions for improvement in these lifestyle areas should be practical and tailored to the developmental stage of their child. PHCPs wanted evidence-based, practical information, presented in a simple, engaging, interactive form. The outcome was a clinically endorsed website and app that health professionals could recommend. Preliminary evaluation showed that 10.5% of pregnant women in Western Australia signed up to the app. Use of the app appeared to be equitable across urban and rural areas of low to middle socioeconomic status

    Engaging Primary Health Care Providers in the Promotion of Healthy Weight Among Young Children: Barriers and Enablers for Policy and Management

    Recent national efforts to stem the alarming growth of childhood overweight and obesity in Australia have highlighted the importance of preventative strategies that focus beyond the child-on parents, families, primary health care providers (PHCPs) and child care services. But, while such efforts have to date focused on school-based interventions, once poor eating habits and sedentary lifestyles have set in, a growing body of research is indicating that PHCPs can play a more influential role in monitoring and modifying factors affecting prevention and early intervention in preschool children. This paper presents the findings of a systematic review to: (a) identify key barriers to the effective engagement of PHCPs with parents and child care staff in the promotion of healthy weight among children aged 2-6 years, (b) appraise promising interventions for strengthening the capacity of PHCPs to effectively deal with these barriers, and (c) synthesise policy options to encourage and engage PHCPs. The study draws on the lessons of promising interventions to highlight the urgent need to address organisational, attitudinal, knowledge, skills and training barriers, to facilitate the engagement of PHCPs in different settings-based environments (clinical, child care, home and community)

    Ngala Healthy You, Healthy Baby: a personalized online program to support healthy weight in pregnancy and early life

    Perinatal maternal obesity is common and has significant child and maternal health consequences. Online resources have the potential to engage young mothers to adopt healthier lifestyles during pregnancy and postpartum. Intercept interviews with 53 pregnant women at antenatal clinics and focus groups with 67 new mothers at mothers’ groups and playgroups were conducted to determine preferred types and formats of online information and support. The expressed needs of women were matched to behaviour change theory to guide development of the Healthy You Healthy Baby website and Smartphone application. A mix of factual and practical online information, self-assessment, goal-setting and feedback in an interactive format is recommended to motivate and support women to achieve healthy lifestyles in the perinatal period. Referral to online resources by health professionals and quality assurance of content is important to increase the confidence of women to act on it

    SWI/SNF-like chromatin remodeling factor Fun30 supports point centromere function in S. cerevisiae

    Budding yeast centromeres are sequence-defined point centromeres and are, unlike in many other organisms, not embedded in heterochromatin. Here we show that Fun30, a poorly understood SWI/SNF-like chromatin remodeling factor conserved in humans, promotes point centromere function through the formation of correct chromatin architecture at centromeres. Our determination of the genome-wide binding and nucleosome positioning properties of Fun30 shows that this enzyme is consistently enriched over centromeres and that a majority of CENs show Fun30-dependent changes in flanking nucleosome position and/or CEN core micrococcal nuclease accessibility. Fun30 deletion leads to defects in histone variant Htz1 occupancy genome-wide, including at and around most centromeres. FUN30 genetically interacts with CSE4, coding for the centromere-specific variant of histone H3, and counteracts the detrimental effect of transcription through centromeres on chromosome segregation and suppresses transcriptional noise over centromere CEN3. Previous work has shown a requirement for fission yeast and mammalian homologs of Fun30 in heterochromatin assembly. As centromeres in budding yeast are not embedded in heterochromatin, our findings indicate a direct role of Fun30 in centromere chromatin by promoting correct chromatin architecture

    Food Selection and Eating Patterns: Themes Found among People with Type 2 Diabetes Mellitus

    Objective: The objective of this study was to examine the beliefs and perspectives among people with type 2 diabetes mellitus about dietary requirements, food selection and eating patterns, and attitudes about self-management practices. Design: Semistructured, in-depth interviews explored participants’ experiences with diabetes prior to their diagnosis, participants’ understanding of the guidelines for the nutritional management of diabetes, how participants applied their understanding of dietary guidelines to daily food selection and eating patterns, and the social and personal themes influencing participants’ food selection and eating patterns. Subjects: Interviews were conducted with members of a convenience sample of 45 men and women diagnosed with type 2 diabetes for at least 1 year. Analyses Performed: Interviews were coded using a conceptual matrix derived from participants’ statements. Common characteristics were grouped, and broad themes were identified. Results: Eating patterns were influenced by participants’ knowledge of diabetes management. Challenges that participants encountered when applying nutrition recommendations were linked to their prior eating practices. Dietary self-efficacy, social support, and time management were identified as mediating variables that can influence dietary behaviors. Implications: Diabetes nutrition education programs should increase awareness of eating history, spousal support, and time management practices. Future research should include the refinement and validation of a nutritional management model of diabetes

    Profiles of people with type 2 diabetes mellitus: the extremes of glycemic control

    For people with diabetes, hemoglobin A1c (HbA1c) is a measure of blood glucose control and an indication of the risk of developing diabetes complications. However, a given HbA1c value does not provide information about the diabetes self-management practices or philosophies of individuals in poor (HbA1c >8.0%) or excellent (HbA1c<6.5%) control. To contrast the experiences and attitudes of people at the extremes of glycemic control, interviews were conducted among 44 individuals (40–65 years old) diagnosed with diabetes for over 1 year. The participants were identified based upon their HbA1c value from a larger sample of people with type 2 diabetes that included African Americans, women, and persons with low income. Narratives were analyzed using a case-based and conceptually clustered matrix approach. The life course concept (life histories and natural transitions in roles and responsibilities across the life span) and the explanatory model of illness (individuals‘ beliefs about the causes, consequences, and treatment of a disease) provided the framework for the interview guide and the analysis of the narratives. Two self-management philosophy groups emerged from participants with excellent control (Committed [n=15] and Tentative [n=7]) and three groups were identified among the poorly controlled group (Hopeful [n=8], Hassled [n=6], and Overwhelmed [n=8]). Perseverance, coping skills, and age at diagnosis were life course concepts that distinguished participants within these groups. From the explanatory model of illness, beliefs about the cause of the disease, physical changes, and accepting the consequence of the disease influenced differences in these self-management philosophies. The profiles of people at the extremes of glycemic control can help those treating people with diabetes or seeking to improve self-management interventions understand differences in self-management philosophies and concentrate on specific issues hindering self-care control

    Tracking Transmission of Apicomplexan Symbionts in Diverse Caribbean Corals

    Symbionts in each generation are transmitted to new host individuals either vertically (parent to offspring), horizontally (from exogenous sources), or a combination of both. Scleractinian corals make an excellent study system for understanding patterns of symbiont transmission since they harbor diverse symbionts and possess distinct reproductive modes of either internal brooding or external broadcast spawning that generally correlate with vertical or horizontal transmission, respectively. Here, we focused on the under-recognized, but apparently widespread, coral-associated apicomplexans (Protista: Alveolata) to determine if symbiont transmission depends on host reproductive mode. Specifically, a PCR-based assay was utilized towards identifying whether planula larvae and reproductive adults from brooding and broadcast spawning scleractinian coral species in Florida and Belize harbored apicomplexan DNA. Nearly all (85.5%; n = 85/89) examined planulae of five brooding species (Porites astreoides, Agaricia tenuifolia, Agaricia agaricites, Favia fragum, Mycetophyllia ferox) and adults of P. astreoides were positive for apicomplexan DNA. In contrast, no (n = 0/10) apicomplexan DNA was detected from planulae of four broadcast spawning species (Acropora cervicornis, Acropora palmata, Pseudodiploria strigosa, and Orbicella faveolata) and rarely in gametes (8.9%; n = 5/56) of these species sampled from the same geographical range as the brooding species. In contrast, tissue samples from nearly all (92.0%; n = 81/88) adults of the broadcast spawning species A. cervicornis, A. palmata and O. faveolata harbored apicomplexan DNA, including colonies whose gametes and planulae tested negative for these symbionts. Taken together, these data suggest apicomplexans are transmitted vertically in these brooding scleractinian coral species while the broadcast spawning scleractinian species examined here acquire these symbionts horizontally. Notably, these transmission patterns are consistent with those of other scleractinian coral symbionts. While this study furthers knowledge regarding these symbionts, numerous questions remain to be addressed, particularly in regard to the specific interaction(s) between these apicomplexans and their hosts
