    Hyperinsulinemia and insulin resistance : What comes first ?

    Background

1)	Classical explanation :
Classical explanation of diabetic pathophysiology states that obesity induced insulin resistance develops first and is followed by compensatory hyperinsulinnemia. Further insulin resistance leads to prolonged, increased secretary demand on beta cells leading to subsequent secondary beta cell failure, giving rise to hyperglycaemia and diabetes^2^.

2)	 Neurobehavioral origin hypothesis :
The Neurobehavioral origin hypothesis suggests that insulin resistance mediates a shift from muscle dependent (soldier) to brain dependent (diplomat) strategies of making a livelihood. If nutrient limitation affects intrauterine development, brain development is the least affected among all the organs^4,5^. As a result, in IUGR babies muscle weight is poor but the brain is relatively well developed. Such a person is more likely to be a successful diplomat rather than a soldier and insulin resistance is adaptive for such an individual^3^. Since insulin is involved in brain development and cognitive functions, higher levels of insulin are needed. As insulin is having strong anti-lipolytic effect, hyperinsulinnemia is followed by subsequent excess fat accumulation. Also compensatory insulin resistance is needed to avoid hypoglycemia. This hypothesis predicts a reverse order of pathophysiology i.e. primary hyperinsulinnemia followed by compensatory insulin resistance^3^

Objective-
To determine in diabetes whether hyperinsulinnemia develops first or insulin resistance develops first.

Methods :
We searched literature for studies that investigated directly or indirectly the sequence of development of hyperinsulinnemia and insulin resistance in humans and animal models from an early stage. Meta-analysis was conducted on published data.

Results-
1)	In low birth weight neonates in humans as well as in rat models, hyperinsulinnemia is found at very early stage.^6^
2)	Development of insulin resistance is preceded by hyperinsulinnemia in mice, rats as well as in humans.^7, 8^
3)	In normoglycaemic hyperinsulinemia state if insulin production is suppressed insulin sensitivity increases rapidly maintaining the normoglycaemic state.^9,10^
4)	Beta cell expansion beginning in intrauterine life is independent of glucose, Insulin and Insulin receptors.^6^


Conclusion-
All the four lines of evidence indicate that hyperinsulinnemia precedes insulin resistance supporting the predictions of neurobehavioral origin hypothesis over the orthodox view.

    Aqueous Behaviour of Chitosan

    Chitosan, a versatile biopolymer, finds numerous applications in textile processing unit operations such as preparation, dyeing, printing, and finishing. However, the accessibility of this biopolymer by the textile material depends on the viscosity of its solution which in turn is a function of its molecular weight. In this work, therefore, the effect of molecular weight, storage life, presence of electrolyte, and particle size of chitosan on its viscosity was investigated. Chitosan of different molecular weights was synthesized by nitrous acid hydrolysis of parent chitosan solution. The synthesized low molecular weight products were analysed by FTIR spectroscopy. Chitosan of nanoconfiguration was prepared by Ionotropic gelation method and characterized by particle size analyzer. The viscosity of different chitosan solutions was determined using Ubbelohde capillary viscometer. As an extension to this study, the chelation property of chitosan was also evaluated

    Case for continuing community NIV and CPAP during the COVID-19 epidemic

    DEVELOPMENT AND OPTIMIZATION OF RIZATRIPTAN BENZOATE ETHOSOMES

    Objective: The objective of this study was to assess the development of ethosomal drug delivery system for site-specific topical delivery of rizatriptan benzoate (RBZ) for sustained action.Methods: In the present study ethosomes were prepared using the cold method. The formulation was optimized using 33 full factorial designs. The lipid concentration (X1), ethanol concentration (X2) and stirring speed (X3) were selected as independent factors and the vesicle size (Y1) and % entrapment efficiency (Y2) were selected as dependent variables.Results: The equation of multiple regression revealed that there was no significant interaction among factors. The lipid concentration had a positive effect on vesicle size while ethanol concentration and stirring speed had a negative effect. For entrapment efficiency, all factors showed a positive effect while lipid concentrative found to be the main influencing factor. The formulation F4E459 (4% SPC, 45% v/v ethanol 900 RPM), which was characterized by optimum vesicle size (5.5 µm) and high entrapment efficiency (93.32%), was considered to be an optimal formulation. The scanning electron microscopy (SEM) results showed that RBZ ethosome have a smooth surface. The polydispersity index (PI) and zeta potential of the optimized formulation were found to be 0.493±0.021and–21.3 mV respectively. In vitro permeation through rat abdominal skin from the ethosomal gel followed Higuchi diffusion model over a period of 8 h.Conclusion: The results obtained in this research work clearly indicated a promising potential of ethosomal carrier system of RBZ for migraine treatment with a topical approach for sustained action

    Properties of jute-cotton union fabrics through wet processing treatments: Part II-Double cloth structures with jute weft in subdued form

    In this study, self stitched double cloth and weft wadded self stitched double cloth woven fabrics with cotton warp and jute weft have been prepared in such a way that the appearance of jute weft is subdued on fabric surface. These fabrics are subjected to four kinds of wet processing treatments to suppress harshness and prickliness of jute component and to enhance fabric properties. Improvement in fabric properties has been determined through objective evaluation. Fabric handle of jute-cotton union double cloth fabric is improved by all the four treatments. Bending length is reduced for a particular weave with particular treatments only. Fabric tensile strength is reduced by all the four treatments

    Ten years’ trend of HIV seroprevalence among Indian pregnant women attending antenatal clinic at tertiary hospital in Dhule, Maharashtra, India

    Background: In India, HIV epidemic is in fourth decade and has the heterogeneity. The trend indicated HIV infection spreads from high risk behavior groups to general population. Maternal to foetal HIV transmission rate is found to vary from 20 to 25% in absence of any interventions. Newly infected persons contribute to the total number of persons living with HIV, they will progress to disease and death over the time and are a potential source of further infection. Maternal HIV status directly affects the perinatal transmission and hence the paediatric HIV cases. This transmission of HIV from mother to child can be prevented by appropriate measures. So this study is intended to identify ten years’ trends in HIV seroprevalence among antenatal population at a tertiary hospital in Dhule, Maharashtra, India.Methods: Retrospective study of ICTC evaluation of pregnant women attending antenatal clinic at obstetrics and gynaecology department of Shri Bhausaheb Hire Government Medical College in Dhule, Maharashtra, India. Blood samples were collected after informed consent and pre-test counseling. The samples were tested for HIV antibodies as per WHO and NACO guidelines. The data of ten years period from January 2004 to December 2013 was evaluated to identify the trends of HIV seroprevalence among pregnant women attending antenatal clinic.Results: 309 pregnant women were HIV positive out of 70453 tested from 84608 ANC women during the study period. The overall prevalence for the 10 years was 0.44%. HIV prevalence had reduced from 0.89% in 2004 to 0.17% in 2013. Maximum HIV positive women i.e. 63.2% were 18 to 23 years old, primigravida (53.8%) and from rural area (58.5%). Mother to foetal transmission was noted 10 babies were detected to be HIV positive while 223 were HIV negative. 10 HIV positive mothers decided for MTP, while in 36 neonatal death was noted.Conclusions: Declining HIV seroprevalence rate is noted among pregnant women attending the antenatal clinics from 2004 to 2013. The effective implementation of prevention of parent to child transmission of HIV/AIDS (PPTCT) programmed is helping in control of the spread of HIV

    Cocrystals are multicomponent system in which one component is Active Pharmaceutical Ingredient (API) and another is called coformer. So coformer selection is one of the main challenge in cocrystal development which is compatible with API. A general approach to coformer selection is by tactless†cocrystal screening, whereby a predetermined library of pharmaceutically acceptable/approved compounds is used to attempt cocrystallization. In cocrystal development one of the approach of coformer selection is based on trial and error. Other approaches are supramolecular synthon approach which utilizes Cambridge Structural Database (CSD) to effectively prioritize coformers for crystal form screening, Hansen solubility parameter and knowledge of hydrogen bonding between coformer and API.     Fan Flutter Computations Using the Harmonic Balance Method

    An experimental forward-swept fan encountered flutter at part-speed conditions during wind tunnel testing. A new propulsion aeroelasticity code, based on a computational fluid dynamics (CFD) approach, was used to model the aeroelastic behavior of this fan. This threedimensional code models the unsteady flowfield due to blade vibrations using a harmonic balance method to solve the Navier-Stokes equations.     Mutagenic potential of Indian tobacco products

    The mutagenic potential of aqueous extracts of masheri (ME), chewing tobacco alone (CTE) and a mixture of chewing tobacco plus lime (CTLE) was tested using the Ames assay. ME exhibited mutagenicity in Salmonella typhimurium TA98 upon metabolic activation with aroclor-1254-induced rat liver S9, while nitrosation rendered it mutagenic in TA100 and TA102. CTE exhibited borderline mutagenicity in the absence or presence of S9 in TA98 and TA100 and after nitrosation in TA102, while nitrosation led to doubling of TA98 and TA100 revertants. In contrast, CTLE exhibited direct mutagenicity in TA98, TA100 and TA102, was mutagenic to TA98 upon S9 addition and induced mutagenic responses in all three tester strains after nitrosation. Experiments using scavengers of reactive oxygen species (ROS) suggested that CTLE-induced oxidat-ive damage in TA102 was mediated by a variety of ROS. The high mutagenic potency of CTLE vis a vis that of CTE may be attributed to changes in the pH leading to differences in the amount and nature of compounds extracted from tobacco. Thus, exposure to a wide spectrum of tobacco-derived mutagcns and promutagens may play a critical role in the development of oral cancer among users of tobacco plus lime
