Introduction. Low-back pain (LBP), resulting from a slipped disc, exerts a long-lasting and irritating impact on all aspects of human life.Aim. The study aimed at assessing the effect of selected factors on the severity of pain measured on the Visual Analogue Scale (VAS) in patients who were treated conservatively due to a slipped disc in the lumbar section of the spine.Material and Methods. A total of 400 patients of the Outpatient Neurosurgical Clinic at the St. Lukas Regional Hospital in Tarnow, Poland, were involved in the survey. The observations (weight status and pain intensity) were conducted twice: when the problems appeared and after 6 months.Results. Obese and overweight individuals (79.5%) and the ones with android adiposity (86%) were most numerously represented in the research groups. The most severe low-back and leg pain in surveys one and two was reported by obese patients and the ones who belonged to the group of senior participants.Conclusions. Conservative treatment combined with pro-health instructions brought the greatest subjective pain relief. The worst effects were achieved by obese patients. (JNNN 2015;4(1):24–29)Wstęp. Ból lędźwiowego odcinka kręgosłupa spowodowany dyskopatią wywiera długotrwały negatywny wpływ na wszystkie aspekty życia.Cel. Ocena wpływu wybranych czynników na nasilenie bólu mierzonego na wizualnej skali analogowej (VAS) u pacjentów, którzy byli leczeni zachowawczo z powodu dyskopatii w odcinku lędźwiowym kręgosłupa.Materiał i metody. Wyniki. Osoby z nadwagą lub otyłością (79,5%) i z androgenicznym typem budowy ciała (86%) były najliczniej reprezentowane w grupach badanych. Największe nasilenie bólu w kręgosłupie i kończynach dolnych zarówno w pierwszym, jak i drugim badaniu zanotowano u osób z otyłością i osób z najstarszej grupy wiekowej.Wnioski. Największą subiektywną ulgę w bólu przyniosło połączenie leczenia zachowawczego z edukacją prozdrowotną. Najgorsze efekty uzyskano u pacjentów otyłych. (PNN 2015;4(1):24–29

System opieki i wsparcia niesamodzielnych osób starszych w województwie łódzkim
systemu wsparcia i opieki niesamodzielnych osób starszych w województwie łódzkim.
do samodzielnego funkcjonowania wymagają wsparcia w codziennym zaspokajaniu potrzeb,
ochrony zdrowia. Z uwagi na tendencje do deinstytucjonalizacji procesów opiekuńczych
coraz wyraźniej podkreśla się rolę pomocy świadczonej w miejscu zamieszkania osoby
starszej, w formie dziennej i środowiskowej. W pierwszej części artykułu przedstawiono
demograficzne i społeczne uwarunkowania struktury potrzeb opiekuńczych wobec niesamodzielnych
osób starszych oraz istniejące zasoby instytucjonalnego i środowiskowego
wsparcia osób starszych w fazie niesamodzielności. W drugiej części zaprezentowano podstawowe
trudności i problemy, z jakimi mierzą się wybrane instytucje wspierające osoby
starsze w toku codziennej realizacji zadań opiekuńczych, w ramach opieki długoterminowej
oraz pomocy świadczonej w formie dziennej i środowiskowej, w województwie łódzkim.
Należą do nich problemy związane z udzielaniem adekwatnego do potrzeb zakresu wsparcia
podopiecznych, ograniczenia utrudniające przebieg procesu oceny niesamodzielności,
szacowania potrzeb pomocowych i opiekuńczych czy też utrudniające proces opiekuńczy
problemy adaptacyjne podopiecznych wynikające z samotności, depresji, ograniczonego
lub całkowitego braku kontaktu rodziną. Analizy przedstawione w tej części artykułu powstały
na podstawie materiału empirycznego zebranego w ramach projektu badawczego
pn. „Osoby niesamodzielne w województwie łódzkim” zrealizowanego w 2018 roku na
zlecenie Regionalnego Obserwatorium Integracji Społecznej w Łodzi w wybranych różnego
typu placówkach świadczących usługi opiekuńcze i zdrowotne osobom dorosłym, które
z uwagi na wiek, stan zdrowia lub niepełnosprawność wymagają większego wsparcia
Enhancing Autonomy through the Occupational Engagement of Adults with Intellectual Disabilities: Supported Employment Model Applied by the Polish Association for Persons with Intellectual Disabilities (Branch in Zgierz)
Reflections presented in this paper reverberate with the current social discourse based on respect for human rights, with heavy emphasis put on the active policy of employment of persons with disabilities. Improvement of the employment support system seems to be one of the most efficient forms of integrating this previously excluded and marginalized group of people into the mainstream social life. It is especially important in the case of persons with intellectual disabilities, who often remain in a particularly difficult situation for many reasons. Enhancing their autonomy via occupational engagement may be beneficial both at the individual (improved personal functioning) and macro (reduced costs for the state and welfare system) levels. The authors of this paper reflect upon the development and implementation of best practices in terms of building an effective mechanism of promoted social participation and the occupational engagement of persons with intellectual disabilities in an open social environment. This seems to create a real chance for community integration, to which these persons have the inalienable right, although so far they have often been left out of the mainstream social life
HSD3B2, HSD17B1, HSD17B2, ESR1, ESR2 and AR expression in infertile women with endometriosis
Objectives: The development of endometriosis is associated with changes in the expression of genes encoding the 3β-hydroxysteroid dehydrogenase type II (HSD3B2) and 17β-hydroxysteroid dehydrogenase type II (HSD17B2), estrogen receptors 1 (ESR1) and 2 (ESR2) and the androgen receptor (AR). However, little is known about the expression of HSD3B2, HSD17B1, HSD17B2, ESR1 ESR2 and AR during the endometrial phases in eutopic endometrium from infertile women with endometriosis.
Material and methods: Using RT-qPCR analysis, we assessed the expression of the studied genes in the follicular and luteal phases in eutopic endometrium from fertile women (n = 17) and infertile women (n = 35) with endometriosis.
Results: In the mid-follicular eutopic endometrium, we observed a significant increase in HSD3B2 transcript levels in all infertile women with endometriosis (p = 0.003), in infertile women with stage I/II endometriosis (p = 0.008) and in infertile women with stage III/IV endometriosis (p = 0.009) compared to all fertile women. There was a significant increase in ESR1 transcripts in all infertile women with endometriosis (p = 0.008) and in infertile women with stage I/II endometriosis (p = 0.019) and in infertile women with stage III/IV endometriosis (p = 0.023) compared to all fertile women. In the mid-luteal eutopic endometrium, we did not observe significant differences in HSD3B2, HSD17B1, HSD17B2, ESR1, ESR2 and AR transcripts between infertile women with endometriosis and fertile women.
Conclusions: Observed significant increase in HSD3B2 and ESR1 transcripts in follicular eutopic endometrium from infertile women with endometriosis may be related to abnormal biological effect of E2 in endometrium, further affecting the development of human embryos
New organic-inorganic hybrid compounds based on sodium peroxidomolybdates (VI) and derivatives of pyridine acids : structure determination and catalytic properties
Two organic-inorganic hybrids based on sodium peroxidomolybdates(VI) and 3,5-dicarboxylic pyridine acid (Na-35dcpa) or N-oxide isonicotinic acid (Na-isoO) have been synthesized and characterized. All compounds contain inorganic parts: a pentagonal bipyramid with molybdenum center, and an organic part containing 3,5-dicarboxylic pyridine acid or N-oxide isonicotinic acid moieties. The type of organic part used in the synthesis influences the crystal structure of obtained compounds. This aspect can be interesting for crystal engineering. Crystal structures were determined using powder X-ray diffraction or single crystal diffraction for compounds Na-35dcpa and Na-isoO, respectively. Elemental analysis was used to check the purity of the obtained compounds, while X-ray Powder Diffraction (XRPD) vs. temp. was applied to verify their stability. Moreover, all the compounds were examined by Infrared (IR) spectroscopy. Their catalytic activity was tested in the Baeyer–Villiger (BV) oxidation of cyclohexanone to ε-caprolactone in the oxygen-aldehyde system. The highest catalytic activity in the BV oxidation was observed for Na-35dcpa. The compounds were also tested for biological activity on human normal cells (fibroblasts) and colon cancer cell lines (HT-29, LoVo, SW 620, HCT 116). All compounds were cytotoxic against tumor cells with metastatic characteristics, which makes them interesting and promising candidates for further investigations of specific anticancer mechanisms
