Intrathecal B-cell activation in LGI1 antibody encephalitis.
ObjectiveTo study intrathecal B-cell activity in leucine-rich, glioma-inactivated 1 (LGI1) antibody encephalitis. In patients with LGI1 antibodies, the lack of CSF lymphocytosis or oligoclonal bands and serum-predominant LGI1 antibodies suggests a peripherally initiated immune response. However, it is unknown whether B cells within the CNS contribute to the ongoing pathogenesis of LGI1 antibody encephalitis.MethodsPaired CSF and peripheral blood (PB) mononuclear cells were collected from 6 patients with LGI1 antibody encephalitis and 2 patients with other neurologic diseases. Deep B-cell immune repertoire sequencing was performed on immunoglobulin heavy chain transcripts from CSF B cells and sorted PB B-cell subsets. In addition, LGI1 antibody levels were determined in CSF and PB.ResultsSerum LGI1 antibody titers were on average 127-fold higher than CSF LGI1 antibody titers. Yet, deep B-cell repertoire analysis demonstrated a restricted CSF repertoire with frequent extensive clusters of clonally related B cells connected to mature PB B cells. These clusters showed intensive mutational activity of CSF B cells, providing strong evidence for an independent CNS-based antigen-driven response in patients with LGI1 antibody encephalitis but not in controls.ConclusionsOur results demonstrate that intrathecal immunoglobulin repertoire expansion is a feature of LGI1 antibody encephalitis and suggests a need for CNS-penetrant therapies
RegTransBaseâa database of regulatory sequences and interactions in a wide range of prokaryotic genomes
RegTransBase is a manually curated database of regulatory interactions in prokaryotes that captures the knowledge in public scientific literature using a controlled vocabulary. Although several databases describing interactions between regulatory proteins and their binding sites are already being maintained, they either focus mostly on the model organisms Escherichia coli and Bacillus subtilis or are entirely computationally derived. RegTransBase describes a large number of regulatory interactions reported in many organisms and contains the following types of experimental data: the activation or repression of transcription by an identified direct regulator, determining the transcriptional regulatory function of a protein (or RNA) directly binding to DNA (RNA), mapping or prediction of a binding site for a regulatory protein and characterization of regulatory mutations. Currently, RegTransBase content is derived from about 3000 relevant articles describing over 7000 experiments in relation to 128 microbes. It contains data on the regulation of about 7500 genes and evidence for 6500 interactions with 650 regulators. Universal Equilibrium Currents in the Quantum Hall Fluid
The equilibrium current distribution in a quantum Hall fluid that is
subjected to a slowly varying confining potential is shown to generally consist
of strips or channels of current, which alternate in direction, and which have
universal integrated strengths. A measurement of these currents would yield
direct independent measurements of the proper quasiparticle and quasihole

Two New X-ray/Optical/Radio Supernova Remnants in M31
We compare a deep (37 ks) Chandra ACIS-S image of the M31 bulge to Local
Group Survey narrow-band optical data and Very Large Array (VLA) radio data of
the same region. Our precisely registered images reveal two new optical shells
with X-ray counterparts. These shells have sizes, [S II]/H-alpha flux ratios,
and X-ray spectral properties typical of supernova remnants (SNRs) with ages of
9 and 17 kyr. Analysis of complementary VLA data
reveals the radio counterparts, further confirming that they are SNRs. We
discuss and compare the properties and morphologies of these SNRs at the

Silent progression in disease activity-free relapsing multiple sclerosis.
ObjectiveRates of worsening and evolution to secondary progressive multiple sclerosis (MS) may be substantially lower in actively treated patients compared to natural history studies from the pretreatment era. Nonetheless, in our recently reported prospective cohort, more than half of patients with relapsing MS accumulated significant new disability by the 10th year of follow-up. Notably, "no evidence of disease activity" at 2âyears did not predict long-term stability. Here, we determined to what extent clinical relapses and radiographic evidence of disease activity contribute to long-term disability accumulation.MethodsDisability progression was defined as an increase in Expanded Disability Status Scale (EDSS) of 1.5, 1.0, or 0.5 (or greater) from baseline EDSSâ=â0, 1.0-5.0, and 5.5 or higher, respectively, assessed from baseline to year 5 (±1âyear) and sustained to year 10 (±1âyear). Longitudinal analysis of relative brain volume loss used a linear mixed model with sex, age, disease duration, and HLA-DRB1*15:01 as covariates.ResultsRelapses were associated with a transient increase in disability over 1-year intervals (pâ=â0.012) but not with confirmed disability progression (pâ=â0.551). Relative brain volume declined at a greater rate among individuals with disability progression compared to those who remained stable (pâ<â0.05).InterpretationLong-term worsening is common in relapsing MS patients, is largely independent of relapse activity, and is associated with accelerated brain atrophy. We propose the term silent progression to describe the insidious disability that accrues in many patients who satisfy traditional criteria for relapsing-remitting MS. Ann Neurol 2019;85:653-666
