    This article tells the story of a monument and a landscape, the adventures lived by both and their repeated re-discoveries by a group of people delegated for their restoration. After deep surveys and participated meetings, after many periods of works and long staying in that place, that group decided that it wasn’t satisfactory to restore the monument, but it was needed to make it live again, in our contemporaneous time, as it was a living being. Moreover, this article proposes a suggestion: the “farm-hotel”, an innovative instrument to promote sustainable tourism, starting from the historical country to the cities

    The both grandiose chains of mountains of Persia, the Alborz Mountains, which form a circle round the south shore of Caspian Sea, and the Zagres chain, along the NW-S E direction, look onto the vast Dasht-ù-Kavir. Desert. A lot of impressive landscapes, civilizations and human settlements (extraordinary villages and towns) were originated along the fluvial valleys, where the snows and the waters sloped down from the mountains flow into the rivers before disappearing in the desert. The ancient Persia is still present and manifest itself in the contemporary landscapes of these remote regions where the populations are still well rooted into such a dynamical and flowing life environment. Here waters, plants and fruits provide the fundamental resources and support sedentary and semi nomad people who live together in this ‘steady and flowing realm’: the first in traditional villages, the second along the valleys. Semi nomad tribes and their animals follow their rhythmic seasonal itineraries and meet every season the sedentary people in their village sharing in peace their common steady and flowing realm. Their history is continuously becoming, weaved by the hands of each woman and by the wisdom of the whole nomad group as a complex marvellous carpet, and it is enriched by the seasonal meetings with the sedentary populations in the villages. At the present this intact wonderful condition is menaced of destruction. A big dam could be built at the end of this valley.

    Les environnements contemporains, avec leurs habitants, leurs utilisateurs et leurs gouvernements locaux, ont besoin de nouvelles méthodologies et procédures pour faire face aux difficultés de cette période de transition qui se rapportent aux relations entre les SystÚmes ternaires - individuelles/sociales/environnementales- qui sont à la base de toute transformation urbaine, rurale et territoriale. De nouveaux processus participatifs, des procédures et des interactions écologiques peuvent provenir des SystÚmes ternaires contemporains et produire de nouvelles configurations de l'environnement de vie à la Ville/Paysage.The Contemporary environments, with their inhabitants, users and local governments, need new methodologies and procedures to cope with the difficulties of this transition period that concerns the relationships among the Ternary Systems - Individual/Social/Environmental- that are the basis of any urban-rural and territorial transformation. New participatory processes, procedures and ecological interactions can emerge from such Contemporary Ternary Systems and produce new configurations of the Life Environment, towards the Landscape/City

    Rapid progressive anti-GBM glomerulonephritis with multiple auto-antibodies

    Rapidly progressive glomerulonephritis (RPGN) is a clinical syndrome manifested by progressive loss of renal function in a short period. At renal biopsy, it is characterized by crescent formation. RPGN may be associated with the presence of circulating antibodies. We report a case of type IV RPGN [ANCA and Anti–glomerular basement membrane (anti-GBM) antibody disease], a severe disease causing a difficult to treat picture. Our case was complicated by severe thrombocytopenia due to the use of heparin and later on by thrombotic microangiopathy. These events occurred rapidly, making the clinical framing and management decisions very hard

    Fibrillary glomerulonephritis with a favourable prognosis of 26 years

    Fibrillary glomerulonephritis (FGN) is a rare glomerular disease. The prognosis is usually unfavorable with nearly half of patients progressing to end-stage renal disease within 4 years. We report a case of biopsy-proven FGN characterized by an unusual benign clinical course in which a kidney biopsy, repeated after an extended follow-up of 26 years, confirmed the presence of fibrils deposition. In 1993, a 32-year-old Caucasian man was admitted to our nephrology ward because of macroscopic hematuria. Renal function was normal. Kidney biopsy displayed an FGN with mesangial pattern. The patient was treated with lisinopril, titrated for blood pressure; the therapy was maintained during 26 years of follow-up. The yearly slope of estimated glomerular filtration rate was -3.17 mL/ min). Starting from March 2018, a rapid worsening of renal function was observed and proteinuria increased up to a nephrotic range. We planned a second renal biopsy to assess the cause of the rapid change of clinical course. The diagnosis of FGN on advanced sclerosis was made, and the severity of glomerular sclerosis. We report a case of FGN with an unusually benign clinical course, characterized by a slow progression to end-stage renal disease over a very extended follow-up time; thus, to better clarify the reason for renal function worsening, a second renal biopsy was performed. The persistence of fibrils deposition confirmed the initial diagnosis of FGN, and a histological pattern characterized by global glomerular sclerosis and interstitial fibrosis has been observed
