Diluted antiferromagnet in a ferromagnetic enviroment
The question of robustness of a network under random ``attacks'' is treated
in the framework of critical phenomena. The persistence of spontaneous
magnetization of a ferromagnetic system to the random inclusion of
antiferromagnetic interactions is investigated. After examing the static
properties of the quenched version (in respect to the random antiferromagnetic
interactions) of the model, the persistence of the magnetization is analysed
also in the annealed approximation, and the difference in the results are
discussed
Gla-rich protein (GRP) as an early and novel marker of vascular calcification and kidney dysfunction in diabetic patients with CKD: a pilot cross-sectional study
Vascular calcification (VC) is one of the strongest predictors of cardiovascular risk in chronic kidney disease (CKD) patients. New diagnostic/prognostic tools are required for early detection of VC allowing interventional strategies. Gla-rich protein (GRP) is a cardiovascular calcification inhibitor, whose clinical utility is here highlighted. The present study explores, for the first time, correlations between levels of GRP in serum with CKD developmental stage, mineral metabolism markers, VC and pulse pressure (PP), in a cohort of 80 diabetic patients with mild to moderate CKD (stages 2-4). Spearman's correlation analysis revealed a positive association of GRP serum levels with estimated glomerular filtration rate (eGFR) and α-Klotho, while a negative correlation with phosphate (P), fibroblast growth factor 23 (FGF-23), vascular calcification score (VCS), PP, calcium (x) phosphate (CaxP) and interleukin 6 (IL-6). Serum GRP levels were found to progressively decrease from stage 2 to stage 4 CKD. Multivariate analysis identified low levels of eGFR and GRP, and high levels of FGF-23 associated with both the VCS and PP. These results indicate an association between GRP, renal dysfunction and CKD-mineral and bone disorder. The relationship between low levels of GRP and vascular calcifications suggests a future, potential utility for GRP as an early marker of vascular damage in CKD.Portuguese Society of Nephrology (SPN) ; Portuguese national funds from FCT-Foundation for Science and Technology through the transitional provision

Um Caso Clínico de Lepra Multibacilar com Vários Surtos de Eritema Nodoso Leproso
Leprosy is a chronic granulomatous disease with a long incubation period caused by Mycobacterium leprae that mainly affects the skin, mucous membranes and the peripheral nervous system. It carries the risk of per-manent sequels with a significant impact on the patient’s quality of life. It has a considerable clinically diver-sity and possible atypical presentations. We present a case of a 31-year-old, skin phototype V woman with multibacillary leprosy characterized by multiple outbreaks of erythema nodosum leprosum, as an inaugural manifestation of the disease. The disease was acquired within a group of children and adolescents from an endemic region of Africa, evolved untreated for 3 years, and presented with unusual features and remarkable lymphatic involvement. We highlight the importance of building and maintaining collaboration between expert centers and institutional partnerships in order to provide the adequate diagnostic resources and appropriate care to the affected populations.
Anxiety and depression symptoms after pulmonary rehabilitation in people with interstitial lung disease: responders and non-responders
Pulmonary rehabilitation (PR) is an effective intervention for people with interstitial lung disease (ILD), as it improvesexercise capacity, quality of life and dyspnoea. Less is, however, known about other important and frequentsymptoms, such as anxiety and depression. This study explored the response of anxiety and depression symptomsto PR in people with ILD.
Participants undertook a 12-week community-based PR programme. Anxiety and depression symptoms wereevaluated before and after PR with the hospital anxiety and depression scale (HADS). The sample was stratified into2 groups according to the HADS score: ≥8 probable anxiety (PA) or depression (PD) and score <8 normal symptoms(NS). Responders were classified using the HADS minimal clinical important difference (≥1.5 points).
46 people with ILD (62±13 years; 63%♀; 75±20 FVC%; 56±18 DLCO%) participated. Number of responders inanxiety symptoms (73% PA; HADS-A pre: 12±3; mean change: 4±2; p<0.001 vs 25% NS; HADS-A pre: 6±2; meanchange: 2±0; p<0.001) was much higher than in depression symptoms (27% PD; HADS-D pre: 11±3; mean change:3±1; p=0.014 vs 13% NS; HADS-D pre: 5±2; mean change: 2±1; p=0.003).
PR seems to improve anxiety symptoms, but it might not be as effective for depression symptoms in people with ILD.Future studies are needed to investigate the role of PR in the psychological health of people with ILD.publishe
Embedding Pulmonary Rehabilitation for Chronic Obstructive Pulmonary Disease in the Home and Community Setting: A Rapid Review
This paper presents a rapid review of the literature for the components, benefits, barriers, and facilitators of pulmonary rehabilitation for chronic obstructive pulmonary disease (COPD) people in-home and community-based settings. seventy-six studies were included: 57 home-based pulmonary rehabilitation (HBPR) studies and 19 community-based pulmonary rehabilitation (CBPR) studies. The benefits of HBPR on exercise capacity and health-related quality of life were observed in one-group studies, studies comparing HBPR to usual care, and studies comparing to hospital-based pulmonary rehabilitation, although the benefits were less pronounced in the latter. HBPR reduced hospital admissions compared to usual care and was more cost-effective than hospital pulmonary rehabilitation. Most HBPRs were designed with low-density or customized equipment, are minimally supervised, and have a low intensity of training. Although the HBPR has flexibility and no travel burden, participants with severe disease, physical frailty, and complex comorbidities had barriers to complying with HBPR. The telerehabilitation program, a facilitator for HBPR, is feasible and safe. CBPR was offered in-person supervision, despite being limited to physical therapists in most studies. Benefits in exercise capacity were shown in almost all studies, but the improvement in health-related quality of life was controversial. Patients reported the benefits that facilities where they attended the CBPR including social support and the presence of an instructor. They also reported barriers, such as poor physical condition, transport difficulties, and family commitments. Despite the minimal infrastructure offered, HBPR and CBPR are feasible, safe, and provide clinical benefits to patients with COPD. Home and community settings are excellent opportunities to expand the offer of pulmonary rehabilitation programs, as long as they follow protocols that ensure quality and safety following current guidelines
