Challenges and Solutions for Functional Neurosurgery in Developing Countries.
Functional neurosurgery techniques remain integral to the neurosurgical treatment armamentarium but data on global implementation remains scarce. In comparison to high-income countries (HIC), low- and middle-income countries (LMIC) suffer from an increased prevalence of diseases like epilepsy, which may be amenable to functional techniques, and therefore, LMIC may benefit from an increased utilization of these treatment modalities. However, functional techniques tend to be expensive and thus difficult to implement in the LMIC setting. A review was performed to assess the current status of functional neurosurgical techniques in LMIC as a starting point for future initiatives. For methodology, a review of the current body of literature on functional neurosurgery in LMIC was conducted through the United States National Library of Medicine Pubmed search engine. Search terms included "functional neurosurgery," "developing countries," "low and middle income," and other related terms. It was found that though five billion people lack access to safe surgical care, the burden of disease amenable to treatment with functional neurosurgical procedures remains unknown. Increasingly, reports of successful, long-term, international neurosurgical collaborations are being reported, but reports in the sub-field of functional neurosurgery are lacking. In conclusion, awareness of global surgical disparities has increased dramatically while global guidelines for functional techniques are currently lacking. A concerted effort can harness these techniques for wider practice. Partnerships between centers in LMIC and HIC are making progress to better understand the burden of disease in LMIC and to create context-specific solutions for practice in the LMIC setting, but more collaborations are warranted
Vergleichende Fourier-Analyse von Bildkategorien mit hohem und niedrigem künstlerischen Anspruch
Das Forschungsgebiet der Neuroästhetik untersucht jene zerebralen Prozesse, die der ästhetischen Erfahrung zugrunde liegen, sowie jene Sinnesreize, durch welche diese evoziert wird. In der vorliegenden Arbeit wurde der Versuch unternommen, mittels Fourier-Analyse die strukturalen Eigenschaften von Schriftsystemen zu identifizieren, die für das menschliche Gehirn ‚interessant‘ bzw. im ästhetischen Sinne ‚fremd‘ erscheinen. Frühere Studien (Redies et al, 2007a,b) haben gezeigt, dass sich das Fourier-Spektrum von Kunstbildern und anderen Bildkategorien mit ästhetischem Anspruch unabhängig von ihrem kulturellen oder zeitlichen Kontext durch Fraktalität und Isotropie auszeichnet. Dieses Ergebnis gab den Anstoß, Schriftkategorien mit unterschiedlichen ästhetischen Ansprüchen derselben Analyse zu unterziehen und zu erforschen, ob auch Schrift unabhängig von kulturellen und zeitlichen Einflüssen eine skalierungsinvariante Komposition und eine isotrope Signalverteilung im Fourier-Raum aufweist. Die Ergebnisse der Experimente ergeben, dass die Parameter der Isotropie und die Steigungsdifferenz des Signals im Fourier-Raum mit dem künstlerischen Anspruch der jeweiligen Bildkategorie korrelieren. Der kulturelle und zeitliche Kontext der Schrift- und Kunstbilder zeigte keinen Einfluss auf das Ergebnis. Für alltägliche Druckschrift ohne künstlerischen Anspruch konnten keine skalierungs-invarianten Strukturen nachgewiesen werden. Somit leistet die vorliegende Arbeit einen Beitrag zur Erforschung jener visuellen Stimuli, die im Betrachter eine ästhetische Erfahrung auslösen können. Die Erlangung tieferer Einblicke in die neuropsychologischen Prozesse der ästhetischen Erfahrung bleibt zukünftigen Studien vorbehalten
Investigation of high-performance liquid chromatography methods for the analysis of protein N-glycans
Biopharmazeutika sind aus einigen Bereichen der modernen Medizin kaum noch wegzudenken.
Während die Herstellung vergleichsweise einfacher Biomoleküle, wie z.B. des Peptids Insulin,
mittlerweile Routine ist, stellt die Expression komplexer Proteine immer noch eine große
Herausforderung für die Biotechnologie bzw. die biopharmazeutische Industrie dar. Oft sind posttranslationale
Modifikationen der Proteine erforderlich, damit diese eine entsprechende Wirkung
zeigen.
Glykosylierung ist eine der häufigsten post-translationalen Modifikationen in eukaryotischen
Zellen. Deshalb ist es nicht überraschend, dass auch Glykoproteine als Biopharmazeutika
Anwendung finden, wie z.B. Erythropoietin, Interferone, Fibrinogen, sowie eine Reihe von
monoklonalen Antikörpern. Dass die Glykosylierung Einfluss auf die Wirksamkeit und auch die
Verträglichkeit hat, ist gut dokumentiert.
Da die Glykosylierung von Proteinen im Gegensatz zur Protein- und zur Nukleinsäurebiosynthese
nicht nach einer Vorlage erfolgt, sondern durch das Zusammenspiel einer Vielzahl an Enzymen
zustande kommt, weist sie in der Regel eine recht heterogene Zusammensetzung auf. Die
vorkommenden Glykane, sowie deren Verhältnisse sind wichtige Parameter, die analysiert werden
müssen um die Stabilität des Herstellprozesses, sowie die Sicherheit und Wirksamkeit des Produkts
sicherzustellen.
Für die Analyse der Glykosylierung kommen eine Reihe von Analysemethoden zur Anwendung,
wobei die Analyse von freien, mit Fluoreszenzfarbstoff gelabeleten Glykanen mittels
Hochleistungs-Flüssigchromatographie (high-performance liquid chromatography, HPLC) als
Referenzmethode gilt. Als stationäre Phasen stehen Umkehrphasen (reversed phase
chromatography, RPC), hydrophile Wechselwirkungsphasen (hydrophilic interaction
chromatography, HILIC) und poröser, graphitähnlicher Kohlenstoff (porous graphitic carbon,
PGC) zur Verfügung. Der Schwerpunkt der vorliegenden Arbeit lag auf der letztgenannten Option.
Zwei Publikationen befassen sich mit dem Retentionsmechanismus von Oligosacchariden auf PGC.
Die Untersuchungen zeigen, dass das organische Lösungsmittel in der mobilen Phase den
Wechselwirkungsmechanismusstark beeinflusst. Besondere Eigenschaften weist vor allem
Acetonitril auf. Weiters wurde der Einfluss der Temperatur des Säulenofens als wichtiger Parameter
für die Selektivität identifiziert. Schließlich wird der Effekt von Oxidations- und Reduktionsmitteln

auf die Retention von neutralen Glykanen gezeigt. Die Oxidation von PGC verhinderte außerdem
die Elution eines sauren Glykans vollständig, was für eine Polarisation der Oberfläche durch das
Reagens spricht. Die Auswirkungen der Reduktions- und Oxidationsmittel wird in der zweiten
Publikation ausführlich diskutiert.
Eine weitere Publikation beschäftigt sich mit der Etablierung einer vollständigen Analysemethode
für Glykane von therapeutischen Antikörpern. Die Glykane werden enzymatisch abgespalten, mit
einem Fluoreszenzfarbstoff gelabelts, mittels HILIC getrennt und durch Fluoreszenzdetektion
quantifiziert. Besondere Aufmerksamkeit lag auf der Probenvorbereitung, die mehrere Schritte
umfasst und dahingehend optimiert wurde die Integrität der ursprünglichen Verhältnisse der
Glykane zu erhalten. Die Methode wurde für den Einsatz in der pharmazeutischen Industrie
validiert. Die Daten zur Reproduzierbarkeit der Methode zeigten nur unwesentliche Abweichungen
zwischen unterschiedlichen Operatoren in verschiedenen Labors. Außerdem zeigte sich, dass die
Daten dieser Analysemethode mit den Ergebnissen einer orthogonlen Analysemethode, nämlich der
RPC-Analyse der (Glyko-)Peptide, übereinstimmen, was die Korrektheit der Quantifizierung
bestätigt.
Schließlich werden in der vierten Publikation die Eigenschaften von RPC, HILIC und PGCChromatographie
in Hinblick auf die Trennung von fluoreszenz-gelabelten Glykanen verglichen.
Hierfür wurde ein breites Set von high-mannose und complex Glykanen auf diesen stationären
Phasen chromatographiert und die generierten Retentionsdaten verglichen. Weiters wurden die
Retentionszeiten mittels multipler linearer Regression (MLR) mit der
Monosaccharidzusammensetzung korreliert. Im Gegensatz zu RPC und HILIC konnte für PGCChromatographie
kein adäquates Modell gefunden werden, was dafür spricht, dass die
Retentionszeiten von Glykanen auf PGC nicht allein durch deren Zusammensetzung determiniert
sind.Biopharmaceuticals are nowadays indispensable tools of the modern medicine. The production of
basic biomolecules (e.g. insulin) counts as routine, but the expression of complex proteins is still a
major challenge for biotechnology and the biopharmaceutical industry. Often post-translational
modifications are a prerequisite for the proper function of the product.
Glycosylation is one of the most common post-translational modifications in eukaryotic cells.
Several glycoproteins are being used as biopharmaceuticals, e.g. erythropoietin, interferons,
fibrinogen and a number of monoclonal antibodies. The impact of the glycosylation on efficacy and
safety is well documented in the literature.
Since the glycosylation of proteins is not driven by a template, but the result of the activities of
many enzymes it typically exhibits a very heterogenic pattern. The individual glycan structures and
their quantitative ratios are important parameters, which have to be determined to demonstrate the
consistency of the production process, as well as the safety and efficacy of the final product.
Several methods are employed for the analysis of protein glycosylation. High-performance liquid
chromatography (HPLC) of fluorescence labeled glycans serves as a reference method due to the
straight-forward quantitation utilizing the fluorescence signal. Typically reversed-phase
chromatography (RPC), hydrophilic-interaction chromatography (HILIC) or porous graphitic
carbon (PGC) chromatography are applied for the separation of glycans. This thesis focuses on
PGC.
Hence two publications investigate the retention mechanism of oligosaccharides on PGC. The
experiments revealed that the nature of the organic solvent in the mobile phase significantly impacts
the retention mechanism. Particularly acetonitrile offers unique properties for the separation of
glycans on PGC. The column temperature was identified as important parameter for the selectivity.
Furthermore the effect of oxidizing and reducing agents on the retention of neutral glycans were
demonstrated. Oxidation of PGC impeded the elution of an acidic glycan, which indicates that the
stationary phase was polarized. The phenomena generated by oxidation and reduction of PGC are
thoroughly discussed in the second publication.
A further publication describes the development and evaluation of an analysis method for
monoclonal antibody (mAb) glycans. The glycans were released from the protein by enzymatic
means and subsequently labeled with a fluorescence dye. The glycans were separated on a HILIC

column and quantified by the fluorescence signal. The sample preparation procedure was optimized
in respect to maintaining the original glycan pattern. The final analysis method was validated for the
use in the biopharmaceutical industry. Reproducibility data exhibited only marginal differences
between the results reported by different technicians from several laboratories. Furthermore the data
were affirmed by an orthogonal method, namely RPC analysis of (glyco-)peptides (peptide
mapping) with MS detection.
In a fourth publication the retention properties of RPC, HILIC and PGC in respect to fluorescence
labeled glycans are compared. The retention times of a set of glycans covering high-mannose and
complex type glycans were determined in all chromatography modes and subsequently compared.
Furthermore the retention times were correlated to the respective monosaccharide composition by
multiple linear regression (MLR). In contrast to RPC and HILIC no adequate model could be found
for PGC, which indicates that retention of oligosaccharides in PGC chromatography is not
determined by the monosaccharide composition
Analysis of diabetes attitudes, wishes and needs in Switzerland, the Swiss DAWN2™ Study.
AIMS OF THE STUDY
Swiss DAWN2™ aimed to evaluate the difficulties and unmet needs of individuals with diabetes and stakeholders, based on the assessments of diabetes care and self-management: the individual burden of disease, the perception of the quality of medical care, and the treatment satisfaction of individuals with diabetes living in the Canton of Bern. The results of the Swiss cohort were analysed and compared with the global DAWN2™ results.
METHODS
239 adult individuals with diabetes were enrolled in a cross-sectional study at the Department of Diabetes, Endocrinology, Nutritional Medicine and Metabolism at the University Hospital of Bern between 2015 and 2017. The participants completed validated online questionnaires regarding health-related quality of life (EQ-5D-3L) and emotional distress (PAID-5), diabetes self-care activities (SDSCA-6), treatment satisfaction (PACIC-DSF), and health-related wellbeing (WHO-5). Eligibility criteria were as follows: participants were aged >18 years, had a diagnosis of diabetes type 1 or 2 since at least 12 months and gave written informed for the participation in the present study.
RESULTS
When compared globally, the Swiss cohort reported a higher quality of life (77.28 ± 16.73 vs. 69.3 ± 17.9 EQ-5D-3L score, p 7% correlated to emotional distress (PAID-5, 26.08 ± 23.37 vs. 18.80 ± 17.49, p = 0.024), unfavourable eating habits (4.28 ± 2.22 vs. 4.99 ± 2.15, p = 0.034) and decreased physical activity (3.95 ± 2.16 vs. 4.72 ± 1.92, p = 0.014). Sleeping problems were most commonly reported (35.6%). In total, 28.8% of respondents completed diabetes-related educational programs.
CONCLUSION
In global comparison, Swiss DAWN2™ showed a lower burden of disease and yet a higher level of treatment satisfaction in patients who were treated in Switzerland. Further studies are required to assess the quality of diabetes treatment and unmet needs in patients treated outside of a tertiary care center
Identifikation mit dem Bösen
Die vorliegende Diplomarbeit beschäftigt sich mit dem Genre des Horrorfilms. Sie geht der Fragestellung auf den Grund, ob und inwieweit für den Rezipienten eine Identifikation mit dem Bösen möglich ist. Die Arbeit untersucht dabei so unterschiedliche Aspekte wie die psychologischen Grundlagen, die bis zu Aristoteles zurückreichen, die Motivation der Zuschauer, sich Horrorfilmen auszusetzen oder die Auswirkungen dieses Horrorfilm-Konsums auf das reale Leben ebenso wie die verschiedenen zum Einsatz kommenden Stilmittel des Horrorfilms, das Aufeinanderprallen von Gut und Böse und nicht zuletzt auch die Rolle des Final Girls. Für die tatsächliche „Identifikations-Entscheidung“ des Rezipienten sind all diese Aspekte von Bedeutung, zum besseren Verständnis werden sie weitestgehend anhand der historischen Entwicklung des Horror-Genres aufgearbeitet und durch zahlreiche Filmbeispiele unterlegt
Development of electron-beam equipment and technology of layer welding of the wire in the conditions of additive technologies
Immunotoxins are powerful tools to specifically eliminate deviated cells. Due to the side effects of the original immunotoxins, they were only considered for the treatment of cancer as in these cases, the potential favourable effect outweighed the unwanted toxic side effects. Over time, many improvements in the construction of immunotoxins have been implemented that circumvent, or at least strongly diminish, the side effects. In consequence this opens the way to employ these immunotoxins for the treatment of non-life threatening diseases. One such category of disease could be the many chronic inflammatory disorders in which an uncontrolled interaction between inflammatory cells leads to chronicity. In several of these chronic conditions, activated macrophages, which are characterised by an increased expression of CD64, are known to play a key role. In this review we discus the data presently available on elimination of activated macrophages through CD64 immunotoxins in several animal models for chronic disease. A chemically linked complete antibody with the plant toxin Ricin-A, proved very effective and provided proof of concept. Subsequently, the development towards genetically engineered, fully human, multivalent single chain based immunotoxins that have diminished immunogenicity, is discussed. The data show that the specific elimination of activated macrophages through CD64 is indeed beneficial for the course of disease. As opposed to other methods used to inactivate or eliminate macrophages, with the CD64 based immunotoxins only the activated population is killed. This may open the way to apply these immunotoxins as therapeutics in chronic inflammatory disease
Evaluation of the ESBL-coding plasmids transmissibility in E. coli isolated from ambulatory patient's urina
Representatives of the Enterobacteriaceae family are the main causative agents of urinary tract infections. Escherichia coli can exhibit resistance to [beta] -lactam antibiotics by synthesizing ESBL (extended spectrum [beta]-lactamases). CTX-M [beta] -lactamases are the dominant group of ESBL. In this paper, we investigated the ability of E. coli urinary isolates to transmit resistance genes within the plasmid. An analysis of the effectiveness of conjugation has shown that E. coli strains producing ESBL are capable of transferring resistance genes to a recipient bacterium at a high frequency
Spectroscopic investigation of unstudied southern PNe
We present a spectroscopic investigation of two hitherto unstudied galactic
planetary nebulae (MeWe 1-10 and MeWe 1-11) and one candidate object (MeWe
2-5). The candidate object clearly has been identified as a bipolar hourglass
shaped PN. The galactic foreground extinction was derived and using
photoionization models with CLOUDY the two round objects were classified as

Process development in Hansenula polymorpha and Arxula adeninivorans, a re-assessment
A range of industrial H. polymorpha-based processes exist, most of them for the production of pharmaceuticals. The established industrial processes lean on the use of promoters derived from MOX and FMD, genes of the methanol metabolism pathway. In Hansenula polymorpha these promoters are de-repressed upon depletion of a range of carbon sources like glucose and glycerol instead of being induced by methanol as reported for other methylotrophs. Due to these characteristics screening and fermentation modes have been defined for strains harbouring such expression control elements that lean on a limited supplementation of glycerol or glucose to a culture medium. For fermentation of H. polymorpha a synthetic minimal medium (SYN6) has been developed. No industrial processes have been developed so far based on Arxula adeninivorans and only a limited range of strong promoter elements exists, suitable for heterologous gene expression. SYN6 originally designed for H. polymorpha provided a suitable basis for the initial definition of fermentation conditions for this dimorphic yeast.
