    Being Stalked: A Psychiatrist\u27s Perspective

    It is only recently that stalking has been criminalized after some highly publicized forensic cases. Psychiatric focus has been on erotomania, and in this article we primarily highlight stalking as a separate entity with psychiatric and legal implications. The case reports reveal our vulnerability as therapists, and we hope will raise our awareness of stalking as a potentially dangerous situation. The legal statutes vary in different states, but our recommendations may provide guidelines towards formulating policies and procedures for the safety of healthcare providers

    Exploring the Feasibility of Incorporating Sexual Education into Routine Adolescent Office Visits

    Preventative sexual education can reduce the negative health outcomes of sexual behavior in adolescent populations.1 The objective of this pilot study was to determine if sexual education can be delivered as a scripted five-minute module during a routine adolescent office visit in a manner that is non-disruptive of clinic flow and acceptable to both patients and providers. • 85% of providers reported the intervention did not interrupt clinic flow • 86% of patients reported the intervention to be appropriate and 92% of providers were satisfied overall with the intervention These pilot data highlight that the incorporation of brief scripted sexual education into routine adolescent preventative office visits is both feasible and acceptable to patients and providers.https://jdc.jefferson.edu/cwicposters/1024/thumbnail.jp

    Development and Evaluation of an Internet-Based Airway Evaluation Tutorial

    Airway evaluation and basic management are essential skills for all physicians. Identifying patients for whom mask ventilation or endotracheal intubation will be difficult to impossible is vital for patient safety. Despite this, training in airway evaluation is minimal in the curricula of most medical schools. To ensure a thorough understanding of airway anatomy and evaluation, as well as exposure to various abnormal findings, we developed an Internet-based module including interactive components, graphics, animation, video, and a self-assessment tool. The site received more than 1800 visits in its first nine months of operation, with uniformly laudatory comments. Eighty subjects over a six-month period completed a pre- and post-test quiz structured to evaluate the utility of the site. Of those completing the on-line survey, more than 76% rated the site very useful. Most felt their knowledge of airway examination improved after completion of the site (p<0.00004). The median amount of time spent on the site was 29.5 minutes. Judging from the overwhelming response to this site from around the world and across disciplines, such interactive training tools that exploit the technological capabilities of the Internet provide useful adjuncts to traditional teaching methods

    In developing countries, drought due to natural disasters such as flood result in massive loss of farming practices. Warning communities of the incoming flood provides an effective solution to this by giving people sufficient time to prepare and protect their crop pattern and farm activities. However, the range of early warning system solutions introduces a tangle of conflicting requirements including cost and reliability, and creates several interesting problems from factors as diverse as technological, social, and political. The complexity of these systems and need for autonomy within the context of a developing country while remaining maintainable and accessible by nontechnical personnel provides a challenge not often solved within developed countries, much less the developing. After describing this problem, the paper discusses a proposed solution for the problem, initial experiments in implementing the solution, and lessons learned through that work.     Status and future directions of anti-metastatic cancer nanomedicines for the inhibition of cathepsin L

    Angiogenesis, tissue invasion and metastasis in the tumour microenvironment are all critical hallmarks of cancer. Upregulation of cathepsin L plays an important role in angiogenesis and metastasis through its ability to degrade the extracellular matrix, facilitating tissue remodeling and tumour cell invasion. Thus, cathepsin L is a potential therapeutic target for anticancer nanomedicine, with its inhibition emerging as an innovative and potentially promising therapeutic intervention for the development of anti-invasion and anti-metastatic enzyme therapies. Nanotechnology-based platforms have been extensively tested in the anti-cancer nanomedicine field with effective anti-tumour efficacy. These nanodrugs can suppress tumour cell proliferation, thereby reducing tumour growth. Recently, nanomedicinal approaches have also emerged as effective anti-metastatic strategies, including the use of graphene oxide and gold nanoparticles. With a focus on recent advances in developing nanotechnology to inhibit cathepsin L, this review provides an in-depth examination of this stimulating field in the context of tumour microenvironments.     Cerebral and Peripheral Tissue Oxygenation in Children Supported on ECMO for Cardio-Respiratory Failure

    Extracorporeal membrane oxygenation (ECMO) is a rescue therapy for patients with cardio-respiratory failure. Establishing, maintaining and weaning from ECMO may increase the risk for intracranial injury. We used a dual channel near infrared system to monitor cerebral and peripheral tissue oxygenation in 3 venoarterial (VA) and 1 venovenous (VV) ECMO patients undergoing manipulations in the ECMO circuit flows. Spectral analysis was performed on the oxyhaemoglobin data collected from these patients with the aim of comparing oscillations at range of frequencies appearing in the two measurement sites

    Characterization Of A Putative SIR2 Like Deacetylase And Its Role In SABP2 Dependent Salicylic Acid Mediated Pathways In Plant

    Salicylic Acid Binding Protein2 (SABP2) is an enzyme known to play important role in SA mediated pathway. SBIP-428 (SABP2 Interacting Protein-428), a SIR2 like deacetylase, has been found to interact with SABP2. We demonstrate that SBIP-428 functions as a Sirtuin deacetylase. We show that SBIP-428 itself is lysine acetylated. Interactions of a SBIP-428 with SABP2 also raised the possibility of SABP2 itself being lysine acetylated. The recombinant purified SABP2 or native partially purified SABP2 displayed no acetylation. In response to TMV infection, the expression of SBIP-428 was down regulated at 48 hpi. In addition, SBIP-428 was up regulated in plant known to accumulate less SA. Taken together expression of SBIP-428 is negatively correlated to the levels of SA in plants. The AtSRT2 plants exhibit no altered growth phenotype but exhibit a higher pathogen resistance against bacterial pathogen. Our results indicate that SBIP-428 is an important regulator in plant defense pathway

    Development of an Optimization-Based Atomistic-to-Continuum Coupling Method

    Atomistic-to-Continuum (AtC) coupling methods are a novel means of computing the properties of a discrete crystal structure, such as those containing defects, that combine the accuracy of an atomistic (fully discrete) model with the efficiency of a continuum model. In this note we extend the optimization-based AtC, formulated in arXiv:1304.4976 for linear, one-dimensional problems to multi-dimensional settings and arbitrary interatomic potentials. We conjecture optimal error estimates for the multidimensional AtC, outline an implementation procedure, and provide numerical results to corroborate the conjecture for a 1D Lennard-Jones system with next-nearest neighbor interactions.Comment: 12 pages, 3 figure
