    Field learning the ecotourism way: Perspectives of students at the Durban University of Technology.

    Universities in South Africa face numerous challenges related to student success, student experience, pass rates, throughput rates, graduation rates, funding from the government and employability. Universities and the programmes that they offer are also criticized for non-alignment with industry needs and the overproduction of graduates that struggle to find employment after graduation. This paper argues that for programmes to enhance alignment with industry needs, etiquette and standards, it is crucial to collaborate with industry in the teaching, learning and assessment processes of a programme. This collaboration could be achieved through the use of industry experts as guest lecturers in authentic industry environments. Ninety third year students from the Department of Ecotourism were exposed to this kind of authentic and future-oriented ecotourism learning at Hluhluwe uMfolozi Game Reserve in South Africa, for an entire week in 2017. This paper provides perspectives shared after their field learning experience. The study discovered that the experience was ‘thrilling’, ‘incredible’, ‘fun’, ‘educational’,’ informativ’e and ‘different’ allowing the students to use all their senses while also givingopportunities to do well to those students who tend to struggle in class. Most of the students thought that they had acquired enough knowledge and skills during their field-trip, to be able to provide the same experience to paying guests. It was also interesting to note that the training had afforded about 22% of the students an opportunity to decide that they were not cut-out for a career or business in the wilderness. However, the same training validated field-guiding as a career or business choice for about 78% of the students

    Hepatitis B Virus Prevalence and Mother-to-Child Transmission Risk in an HIV Early Intervention Cohort in KwaZulu-Natal, South Africa

    BACKGROUND: HIV and hepatitis B virus (HBV) prevalence are both high in KwaZulu-Natal, South Africa. HIV coinfection negatively affects HBV prognosis and can increase the likelihood of HBV mother-to-child transmission (MTCT). In an early HIV infant treatment intervention cohort of HIV-transmitting mother-child pairs in KwaZulu-Natal, we characterized maternal HBV prevalence and screened infants at risk. METHODS: Infants were treated for HIV MTCT at birth, and combination regimens incidentally active against HBV were initiated within 21 days. Maternal samples (N = 175) were screened at birth for HBV infection (HBV surface antigen [HBsAg]), exposure to HBV (HBV anti-core IgG), and vaccination responses (HBV anti-S positive without other HBV markers). Infants of mothers who were HBV positive were screened for HBsAg at 1 and 12 months. RESULTS: Evidence of HBV infection was present in 8.6% (n = 15) of maternal samples. Biomarkers for HBV exposure were present in 31.4% (n = 55). Evidence of HBV vaccination was uncommon in mothers (8.0%; n = 14). Despite prescription of antiretroviral therapy (ART) active against HBV, HBV DNA was detectable in 46.7% (7/15) of mothers who were HBsAg positive. Three mothers had HBV viral loads >5.3 log10 IU/mL, making them high risk for HBV MTCT. Screening of available infant samples at 1 month (n = 14) revealed no cases of HBV MTCT. At 12 months, we identified 1 HBV infection (1/13), and serologic evidence of vaccination was present in 53.8% (7/13) of infants. DISCUSSION: This vulnerable cohort of HIV-transmitting mothers had a high prevalence of undiagnosed HBV. Early infant ART may have reduced the risk of MTCT in high-risk cases. Current HBV guidelines recommend ART prophylaxis, but these data underline the pressing need to increase availability of birth dose vaccines

    Prevalence of sexually transmitted infections among young people in South Africa: A nested survey in a health and demographic surveillance site.

    BACKGROUND: Sexually transmitted infections (STIs) and bacterial vaginosis (BV) are associated with increased transmission of HIV, and poor reproductive and sexual health. The burden of STIs/BV among young people is unknown in many high HIV prevalence settings. We conducted an acceptability, feasibility, and prevalence study of home-based sampling for STIs/BV among young men and women aged 15-24 years old in a health and demographic surveillance site (HDSS) in rural KwaZulu-Natal, South Africa. METHODS AND FINDINGS: A total of 1,342 young people, stratified by age (15-19 and 20-24 years) and sex were selected from the HDSS sampling frame; 1,171/1,342 (87%) individuals had ≥1 attempted home visit between 4 October 2016 and 31 January 2017, of whom 790 (67%) were successfully contacted. Among the 645 who were contacted and eligible, 447 (69%) enrolled. Consenting/assenting participants were interviewed, and blood, self-collected urine (men), and vaginal swabs (women) were tested for herpes simplex virus type 2 (HSV-2), chlamydia, gonorrhoea, syphilis, trichomoniasis, and BV. Both men and women reported that sample collection was easy. Participants disagreed that sampling was painful; more than half of the participants disagreed that they felt anxious or embarrassed. The weighted prevalence of STIs/BV among men and women, respectively, was 5.3% and 11.2% for chlamydia, 1.5% and 1.8% for gonorrhoea, 0% and 0.4% for active syphilis, 0.6% and 4.6% for trichomoniasis, 16.8% and 28.7% for HSV-2, and 42.1% for BV (women only). Of the women with ≥1 curable STI, 75% reported no symptoms. Factors associated with STIs/BV included having older age, being female, and not being in school or working. Among those who participated in the 2016 HIV serosurvey, the prevalence of HIV was 5.6% among men and 19% among women. Feasibility was impacted by the short study duration and the difficulty finding men at home. CONCLUSIONS: A high prevalence of STIs/BV was found in this rural setting with high HIV prevalence in South Africa. Most STIs and HIV infections were asymptomatic and would not have been identified or treated under national syndromic management guidelines. A nested STI/BV survey within a HDSS proved acceptable and feasible. This is a proof of concept for population-based STI surveillance in low- and middle-income countries that could be utilised in the evaluation of STI/HIV prevention and control programmes

    Training youth caregivers to provide HIV education and support to orphans and vulnerable children in South Africa

    In KwaZulu Natal, South Africa, in Durban’s peri-urban community of Valley of 1000 Hills, Horizons partnered with Valley Trust to implement and evaluate a program to provide education and support to orphans and vulnerable children. The study utilized a pre-post test design to assess the feasibility and impact of building the capacity of youth aged 18–24 years to provide HIV education and support to in-school orphans and vulnerable children in their communities. The Valley Trust Youth Caregiver Program trained older youth to provide HIV education, homework assistance, and psychosocial support to orphans and vulnerable children aged 11–15 years. The program aimed to increase HIV-related knowledge, improve HIV communication skills, and decrease stigmatizing attitudes. As noted in this brief, trained volunteer youth caregivers operating in a school-based setting can reach children who are orphaned and rendered vulnerable by their household and community circumstances. The education and support provided likely contribute to improvements in HIV-related knowledge, attitudes, and communication among program participants, given that the gains they made were often greater than those of nonparticipants

    The architecture and effect of participation: a systematic review of community participation for communicable disease control and elimination. Implications for malaria elimination

    Community engagement and participation has played a critical role in successful disease control and elimination campaigns in many countries. Despite this, its benefits for malaria control and elimination are yet to be fully realized. This may be due to a limited understanding of the influences on participation in developing countries as well as inadequate investment in infrastructure and resources to support sustainable community participation. This paper reports the findings of an atypical systematic review of 60 years of literature in order to arrive at a more comprehensive awareness of the constructs of participation for communicable disease control and elimination and provide guidance for the current malaria elimination campaign.Evidence derived from quantitative research was considered both independently and collectively with qualitative research papers and case reports. All papers included in the review were systematically coded using a pre-determined qualitative coding matrix that identified influences on community participation at the individual, household, community and government/civil society levels. Colour coding was also carried out to reflect the key primary health care period in which community participation programmes originated. These processes allowed exhaustive content analysis and synthesis of data in an attempt to realize conceptual development beyond that able to be achieved by individual empirical studies or case reports.Of the 60 papers meeting the selection criteria, only four studies attempted to determine the effect of community participation on disease transmission. Due to inherent differences in their design, interventions and outcome measures, results could not be compared. However, these studies showed statistically significant reductions in disease incidence or prevalence using various forms of community participation. The use of locally selected volunteers provided with adequate training, supervision and resources are common and important elements of the success of the interventions in these studies. In addition, qualitative synthesis of all 60 papers elucidates the complex architecture of community participation for communicable disease control and elimination which is presented herein.The current global malaria elimination campaign calls for a health systems strengthening approach to provide an enabling environment for programmes in developing countries. In order to realize the benefits of this approach it is vital to provide adequate investment in the 'people' component of health systems and understand the multi-level factors that influence their participation. The challenges of strengthening this component of health systems are discussed, as is the importance of ensuring that current global malaria elimination efforts do not derail renewed momentum towards the comprehensive primary health care approach. It is recommended that the application of the results of this systematic review be considered for other diseases of poverty in order to harmonize efforts at building 'competent communities' for communicable disease control and optimising health system effectiveness

    Teenage pregnancy rates and associations with other health risk behaviours: a threewave cross-sectional study among South African school-going adolescents

    BACKGROUND: Teenage pregnancy still remains high in low and middle-income countries (LMIC), as well as in highincome countries (HIC). It is a major contributor to maternal and child morbidity and mortality rates. Furthermore, it has social consequences, such as perpetuating the cycle of poverty including early school dropout by the pregnant adolescent, especially in sub-Saharan Africa (SSA). Few studies in SSA have investigated the trends in teenage pregnancy and the associated factors, while this is critical in fully understanding teenage pregnancy and for promotion of reproductive health among adolescents at large in SSA. METHODS: To examine the trends in teenage pregnancy and to identify associations with other health risk behaviours in South Africa (SA), a total of 31 816 South African school-going adolescents between 11 to 19 years of age were interviewed in three cross-sectional surveys. Data from the first (2002, n = 10 549), second (2008, n = 10 270) and the third (2011, n = 10 997) nationally representative South African youth risk behaviour surveys (YRBS) were used for this study. RESULTS: The overall prevalence of having ever been pregnant among the combined 3-survey sample was selfreported to be 11.0 % and stable across the three surveys. Sexual intercourse among adolescents in SA has decreased from 41.9 % in 2002 to 36.9 % in 2011. However, pregnancy among girls who ever had sex increased from 17.3 % (95 % CI: 0.16–0.19) in 2002, to 23.6 % (95 % CI: 0.21–0.26) in 2008 and decreased to 21.3 % (95 % CI: 0.19–0.23) in 2011. The odds for ever been pregnant were higher for girls who had 2 or more sexual partners (OR: 1.250, 95 % CI: 1.039–1.503), girls who ever used alcohol before sex (OR: 1.373, 95 % CI: 1.004–1.878), practised binge-drinking during the last month (OR: 0.624, 95 % CI: 0.503–0.774), and girls who used mandrax (OR: 1.968, 95 % CI: 1,243–3.117). The odds for never been pregnant were lower for those who used condoms (OR: 0.462, 95 % CI: 0.309–0.691). CONCLUSIONS: Girls continue to become pregnant at unacceptably high rates in SA. Sexual intercourse among adolescents in SA has decreased slightly. However, among those who are sexually active pregnancy prevalence rates have increased. More over, this is in the context of high prevalence of HIV and other STI. There is a need to address adolescents’ sexual and reproductive health, and several health risk behaviours, including substance use, that are associated with teenage pregnancy in SA.IS

    Land degradation impact on soil carbon losses through water erosion and CO2 emissions

    Worldwide concerns with global change and its effects on our future environment require an improved understanding of the impact of land cover changes on the global C cycle. Overgrazing causes a reduction in plant cover with accepted consequences on soil infiltration and soil erosion, yet the impact on the loss of soil organic carbon (SOC) and its associated processes remain unaccounted for. In this study performed in South Africa, our main objective was to evaluate the impact of plant cover reduction on (i) SOC erosion by water in both particulate (POC) and dissolved (DOC) forms, and (ii) soil CO2 emissions to the atmosphere. The study performed under sandy-loam Acrisols investigated three proportions of soil surface coverage by plants (Cov), from 100% (Cov100) for the "non-degraded" treatment to 25-50% (Cov50) and 0-5% (Cov5). POC and DOC losses were evaluated using an artificial rainfall of 30 mm h(-1) applied for a period of 30 min on bounded 1 x 1 m(2) microplots (n = 3 per treatment). CO2 emissions from undisturbed soil samples (n = 9) were evaluated continuously at the laboratory over a 6-month period. At the "non-degraded" treatment of Cov100, plant-C inputs to the soil profile were 1950 +/- 180 gC m(-2) y(-1) and SOC stocks in the 0-0.02 m layer were 300.6 +/- 16.2 gC m(-2). While soil-C inputs by plants significantly (P<0.05 level) decreased by 38.5 +/- 3.5% at Cov50 and by 75.4 +/- 6.9% at Cov5, SOC, the losses by water erosion of 0.75 gC m(-2) at Cov100 increased from 66% at Cov50 (i.e. 3.76 +/- 1.8 gC m(-2)) to a staggering 213% at Cov5 (i.e. 7.08 +/- 2.9 gC m(-2)). These losses were for the most part in particulate form (from 88.0% for Cov100 to 98.7% for Cov5). Plant cover reduction significantly decreased both the cumulative C-CO2 emissions (by 68% at Cov50 and 69% at Cov5) and the mineralization rate of the soil organic matter (from 0.039 gC-CO2 gC(-1) at Cov100 to 0.031 gC-CO2 gC(-1) at Cov5). These results are expected to increase our understanding of the impact of land degradation on the global C cycle. Further in-situ research studies, however, need to investigate whether or not grassland degradation induces net C-emissions to the atmosphere

    No-till impact on soil and soil organic carbon erosion under crop residue scarcity in Africa

    Although no-till (NT) is now practiced in many countries of the world, for most smallholders, the crop residues are of such a value that they cannot be left on the soil surfaces to promote soil protection, thus potentially limiting NT benefits and adoption. In this study our main objective was to evaluate runoff, soil, and soil organic carbon (SOC) losses from traditional small-scale maize (Zea mays) field under conventional tillage (T) and NT, with crop residues cover of less than 10% during the rainy season, in South Africa. Six runoff plots of 22.5 m(2) (2.25 x 10 m) under NT and T since 2002 were considered. At each plot, soil bulk density (rho(b)) and SOC content of the 0-0.02 m layer were estimated at nine pits. Top-soil SOC stocks were 26% higher under NT than under T (P = 0.001). The NT reduced soil losses by 68% (96.8 vs. 301.5 g m(-2) yr(-1), P = 0.001) and SOC losses by 52% (7.7 vs. 16.2 g C m(-2) yr(-1), P = 0.001), and differences in runoff were not significant. Dissolved organic carbon accounted for about 10% of total SOC losses and showed significantly higher concentrations under T than NT (1.49 versus 0.86 mg C m(-2) yr(-1)). The less erosion in NT compared to T was explained by a greater occurrence under NT of indurated crusts, less prone to soil losses. These results showed the potential of NT even with low crop residue cover (<10%) to significantly reduce soil and SOC losses by water under small-scale agriculture
