    Patterns of geohelminth infection, impact of albendazole treatment and re-infection after treatment in schoolchildren from rural KwaZulu-Natal/South-Africa

    BACKGROUND: Geohelminth infection is a major health problem of children from rural areas of developing countries. In an attempt to reduce this burden, the Department of Health of the province of KwaZulu-Natal (KZN) established in 1998 a programme for helminth control that aimed at regularly treating primary school children for schistosomiasis and intestinal helminths. This article describes the baseline situation and the effect of treatment on geohelminth infection in a rural part of the province. METHODS: Grade 3 schoolchildren from Maputaland in northern KZN were examined for infections with hookworm, Ascaris lumbricoides, and Trichuris trichiura, treated twice with 400 mg albendazole and re-examined several times over one year after the first treatment in order to assess the impact of treatment and patterns of infection and re-infection. RESULTS: The hookworm prevalence in the study population (83.2%) was considerably higher than in other parts of the province whereas T. trichiura and especially A. lumbricoides prevalences (57.2 and 19.4%, respectively) were much lower than elsewhere on the KZN coastal plain. Single dose treatment with albendazole was very effective against hookworm and A. lumbricoides with cure rates (CR) of 78.8 and 96.4% and egg reduction rates (ERR) of 93.2 and 97.7%, respectively. It was exceptionally ineffective against T. trichiura (CR = 12.7%, ERR = 24.8%). Re-infection with hookworm and A. lumbricoides over 29 weeks after treatment was considerable but still well below pre-treatment levels. CONCLUSION: High geohelminth prevalences and re-infection rates in the study population confirm the need for regular treatment of primary school children in the area. The low effectiveness of single course albendazole treatment against T. trichiura infection however demands consideration of alternative treatment approaches

    The Impact of Different Human-Machine Interface Feedback Modalities on Older Participants’ User Experience of CAVs in a Simulator Environment

    Rapidly developing Autonomous Vehicle (AV) technology has potential to provide solutions to some of the aging population challenges, such as social isolation resulting from an inability to be independently mobile. However for AVs success, users’ acceptance is essential. Fifteen participants (M 70 years) participated in an autonomous driving simulator trial with voice-based CAV status feedback in a decision-making scenario – whether to pick up a friend on the way. The within-subject conditions/journeys were: Audio feedback (Audio)/Pick-Up; Audio/No-Pick-Up; No-Audio/Pick-Up. Additionally, the effect of feedback during different external journey conditions was also considered, resulting in two between-subjects conditions – day and night travel. Participants physiological, cognitive and affective measures show greater situational awareness and workload ratings in the No-Audio/Pick-Up condition with increased Post-trial trust rating and overall higher positive affect. These results indicate that the greatest concentration was required in the no-sound condition, suggesting that sound/multimodal feedback improved ease of operation and journey experience

    Sexuality and Body Image After Uterine Artery Embolization and Hysterectomy in the Treatment of Uterine Fibroids: A Randomized Comparison

    In this paper the effect of uterine artery embolization (UAE) on sexual functioning and body image is investigated in a randomized comparison to hysterectomy for symptomatic uterine fibroids. The EMbolization versus hysterectoMY (EMMY) trial is a randomized controlled study, conducted at 28 Dutch hospitals. Patients were allocated hysterectomy (n = 89) or UAE (n = 88). Two validated questionnaires (the Sexual Activity Questionnaire [SAQ] and the Body Image Scale [BIS]) were completed by all patients at baseline, 6 weeks, and 6, 12, 18, and 24 months after treatment. Repeated measurements on SAQ scores revealed no differences between the groups. There was a trend toward improved sexual function in both groups at 2 years, although this failed to reach statistical significance except for the dimensions discomfort and habit in the UAE arm. Overall quality of sexual life deteriorated in a minority of cases at all time points, with no significant differences between the groups (at 24 months: UAE, 29.3%, versus hysterectomy, 23.5%; p = 0.32). At 24 months the BIS score had improved in both groups compared to baseline, but the change was only significant in the UAE group (p = 0.009). In conclusion, at 24 months no differences in sexuality and body image were observed between the UAE and the hysterectomy group. On average, both after UAE and hysterectomy sexual functioning and body image scores improved, but significantly so only after UAE

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    High-Resolution Gel Dosimetry of A HDR Brachytherapy Source Using Normoxic Polymer Gel Dosimeters: Preliminary Study

    Polymer gel dosimetry has been shown to be an effective tool in the analysis of radiotherapy treatments in cancer therapy, being used to map the dose distribution around an irradiation pattern of a polymer gel dosimeter. Combined with high-resolution magnetic resonance imaging (MRI), polymer gel dosimetry can be an effective dosimetry tool to map dose distributions with high spatial resolution (∼100 μm). Previously polyacrylamide polymer gel dosimetry required a strict hypoxic environment to protect the gel from oxygen infiltration as oxygen inhibits the polymerization reaction used to correlate to absorbed dose. However, with the advent of normoxic polymer gels, a strict hypoxic environment is not required. Normoxic polymer gel dosimeters can be manufactured under normal atmospheric conditions. This study assessed the use of a MAGIC normoxic polymer gel dosimeter to accurately map the dose distribution of a single-line irradiation and a point source irradiation from a brachytherapy radiation source administered through a nylon catheter inserted into the gel dosimeter. The phantoms were irradiated to a dose of 10 Gy at 2 mm from the source center and imaged using high-resolution MRI with an in-plane pixel size of 0.1055 mm/pixel. Good agreement was found between the dose points predicted by the computer treatment-planning system and the measured normalized dose profiles in the gel dosimeter. The use of normoxic polymer gel dosimeters with high-resolution MRI evaluation shows promise as an effective tool in applications requiring accurate dose distributions in high resolution, such as intravascular brachytherapy.11 page(s
