    Future Directions in Breast Imaging

    Breast cancer imaging has improved dramatically over the last decade, with higher and more uniform quality standards for mammography, the increasing use of sonography and magnetic resonance imaging (MRI), and the widespread availability of imaging-guided percutaneous biopsy for clinically occult disease. This review paper describes the limitations that exist in the current state of the art for breast cancer imaging for detection and diagnosis. Four broad areas of future investigation are described in detail. First, we discuss the use of newer versions of mammography, such as digital mammography, with tomosynthesis and digital subtraction mammography. Secondly, new screening for occult disease might be improved through individualized strategies that stratify by patient risk, for example, through more rigorous screening with new and different tools for women at high risk. Third, the use of tools that might be useful for less invasive therapy of breast cancer with imaging to monitor the efficacy of the therapy is discussed. Finally, we describe the use of imaging to monitor and adjust neoadjuvant chemotherapy regimens in the course of therapy for advanced breast cancers when the risk of death is high

    Preliminary observations on soluble programmed cell death protein-1 as a prognostic and predictive biomarker in patients with metastatic melanoma treated with patient-specific autologous vaccines.

    Because of its role as an immune checkpoint, levels of soluble programmed cell death protein-1 (sPD-1) could be useful as a prognostic biomarker or predictive biomarker in cancer patients treated with vaccines. Very low levels of sPD-1 may indicate lack of an existing anti-cancer immune response; very high levels may indicate an active immune response that is suppressed. In between these extremes, a decrease in PD-1 following injections of an anti-cancer vaccine may indicate an enhanced immune response that has not been suppressed. Blood samples obtained during a randomized trial in patients with metastatic melanoma were tested from 22 patients treated with a tumor cell vaccine (TCV) and 17 treated with a dendritic cell vaccine (DCV). Survival was better in DCV-treated patients. sPD-1 was measured at week-0, one week before the first of three weekly subcutaneous injections, and at week-4, one week after the third injection. The combination of a very low baseline sPD-1, or absence of a very high PD-1 at baseline followed by a decline in sPD-1 at week-4, was predictive of surviving three or more years in DCV-treated patients, but not TCV-treated. Among DCV-treated patients, these sPD-1 criteria appropriately classified 8/10 (80%) of 3-year survivors, and 6/7 (86%) of patients who did not survive three years. These preliminary observations suggest that sPD-1 might be a useful biomarker for melanoma patients being considered for treatment with this DCV vaccine, and/or to predict efficacy after only three injections, but this would have to be confirmed in larger studies

    Relating the Flow Processes and Bedforms of Steady-State and Waning Density Currents

The interaction between turbidity currents and mobile substrates can lead to the development of different types of bedforms. Although much research has been conducted on bedform development beneath open channel flows, research into bedform development beneath waning gravity currents is relatively rare. Analysis of density current-related bedform development has therefore relied upon open channel flow phase diagrams. We report on an experimental study designed to assess the development of bedforms under steady and waning saline density currents. The experimental density currents developed stepped density profiles in which a higher-density basal zone was separated from the ambient fluid by a zone of intermediate density; any bedforms that developed were contained within the bottom layer of the current. Under different conditions ripples, dunes, downstream migrating antidunes and long wavelength antidunes were observed to form and could be distinguished based on their interactions and phase relationships with the upper surface of the lower denser layer of the current. Due to limited mixing between the upper and lower layer of the current and maintenance of current momentum, currents set with slowing discharge flow rates maintained a steady flow velocity in the lower layer of the flow. As a result, sustained bedform formative conditions were achieved within this lower layer, while waning current conditions effected the rest of the flow. Under waning currents, it was seen how pre-existing bed states can determine the subsequent evolution of bedforms. This illustrates the limitations of existing phase diagrams as they do not account for trajectory or rate of passage of flows through different bedform phase spaces. In order to establish a reliable quantitative association between the flow regime and the type of bedform development, it is critical to adopt an appropriate Froude number calculation method for stratified flow. The updated density current phase diagram indicates supercritical flow can be achieved at lower flow velocities than for open channel flows due to the effects of reduced gravity. Bedform depositional structures found in outcrop and on the modern sea floor provide data that helps to interpret the hydrodynamic and sedimentological character of the current that formed them. Therefore, understanding the processes involved in bedform development beneath density currents will enable more accurate estimation of the properties of flows

    This submission presents the research conducted within the Energy and Environment Institute at the University of Hull. Our work demonstrates that hazards represented by flooding have multiple dimensions, and that solutions to them need to take these complex inter-relationships into account. The research described below covers both the physical and social impacts of flooding; the recommendations that follow from our work are based on our understanding that flood risk management needs to expand across land-water boundaries at the full range of spatial, temporal and societal scales. Responding to each of the terms of reference in turn, our data shows that the response to flooding needs to start changing now in order to ensure the resilience of our communities, businesses and infrastructure in the future

    A pilot study of eye movement during mammography interpretation: Eyetracker results and workstation design implications

    Digital mammography can potentially improve mammography image and interpretation quality. On-line interpretation from a workstation may improve interpretation logistics and increase availability of comparison images. Interpretation of eight 4k- x 5k-pixel mammograms on two to four 2k- x 2.5k-pixel monitors is problematic because of the time spent in choosing which images display on which monitors, and zooming and roaming on individual images that are too large to display completely at full resolution. The authors used an eyetracker to measure radiologists viewing behavior during mammography interpretation with film on a viewbox. It was observed that a significant portion of the mammographers' time is spent viewing "comparison pairs" (typically two or more comparisons per case), such as the left mediolateral and craniocaudal images or old and new images. From the eyetracker measurements, we estimated that the number of image display, roam, and zoom operations decreases from an average of 64 for one monitor to 31 for four monitors, with the largest change going from one to two monitors. We also show that fewer monitors with a faster response time is superior to more monitors with a slower response time. Finally, the authors demonstrate the applicability of time-motion analysis to mammographic workstation design

    Physical modelling of water, fauna and flora: knowledge gaps, avenues for future research and infrastructural needs

    peer reviewedPhysical modelling is a key tool for generating understanding of the complex interactions between aquatic organisms and hydraulics, which is important for management of aquatic environments under environmental change and our ability to exploit ecosystem services. Many aspects of this field remain poorly understood and the use of physical models within eco-hydraulics requires advancement in methodological application and substantive understanding. This paper presents a review of the emergent themes from a workshop tasked with identifying the future infrastructure requirements of the next generation of eco-hydraulics researchers. The identified themes are: abiotic factors, adaptation, complexity and feedback, variation, and scale and scaling. The paper examines these themes and identifies how progress on each of them is key to existing and future eðorts to progress our knowledge of eco-hydraulic interactions. Examples are drawn from studies on biofilms, plants, and sessile and mobile fauna in shallow water fluvial and marine environments. Examples of research gaps and directions for educational, infrastructural and technological advance are also presented.PISCES work package of HYDRALAB FP

    Male gays in the female gaze: women who watch m/m pornography

    This paper draws on a piece of wide-scale mixed-methods research that examines the motivations behind women who watch gay male pornography. To date there has been very little interdisciplinary research investigating this phenomenon, despite a recent survey by PornHub (one of the largest online porn sites in the world) showing that gay male porn is the second most popular choice for women porn users out of 25+ possible genre choices. While both academic literature and popular culture have looked at the interest that (heterosexual) men have in lesbian pornography, considerably less attention has been paid to the consumption of gay male pornography by women. Research looking at women's consumption of pornography from within the Social Sciences is very focused around heterosexual (and, to a lesser extent, lesbian) pornography. Research looking more generally at gay pornography/erotica (and the subversion of the ‘male gaze’/concept of ‘male as erotic object’) often makes mention of female interest in this area, but only briefly, and often relies on anecdotal or observational evidence. Research looking at women's involvement in slashfic (primarily from within media studies), while very thorough and rich, tends to view slash writing as a somewhat isolated phenomenon (indeed, in her influential article on women's involvement in slash, Bacon-Smith talks about how ‘only a small number’ of female slash writers and readers have any interest in gay literature or pornography more generally, and this phenomenon is not often discussed in more recent analyses of slash); so while there has been a great deal of very interesting research done in this field, little attempt has been made to couch it more generally within women's consumption and use of pornography and erotica or to explore what women enjoy about watching gay male pornography. Through a series of focus groups, interviews, and an online questionnaire (n = 275), this exploratory piece of work looks at what women enjoy about gay male pornography, and how it sits within their consumption of erotica/pornography more generally. The article investigates what this has to say about the existence and nature of a ‘female gaze’
