    Practice nurses experiences of mentoring undergraduate nursing students in Australian general practice

    Internationally, the delivery of health services has shifted from secondary to primary care, necessitating an exponential growth of the nursing workforce and expansion of the nursing role in general practice. This growth, and the subsequent need to develop this workforce, has created a need to expose undergraduate nurses to general practice nursing as a viable career option. Concurrently, universities are struggling to find sufficient clinical places for their undergraduate students to gain clinical experience. It is logical, therefore, to increase the number of undergraduate nursing student placements in general practice. Through qualitative research methods, this paper seeks to explore the experiences of practice nurses mentoring undergraduate students on clinical placements within the general practice setting. Findings are presented in the following three themes: (1) Promoting Practice Nursing: We really need to get students in, (2) Mentoring future co-workers: Patience and reassurance, and (3) Reciprocity in learning: It\u27s a bit of a two way street, which show the benefits of such placements. Clinical placements in general practice settings can be mutually beneficial in terms of providing quality teaching and learning experiences for students. Conversely, the experience provides an impetus for practice nurses to maintain currency of their clinical skills and knowledge through mentoring student nurses

    Natural hazards in Australia : storms, wind and hail

    Current and potential future storm-related wind and hail hazard in Australia is reviewed. Confidence in the current incidence of wind hazard depends upon the type of storm producing the hazard. Current hail hazard is poorly quantified in most regions of Australia. Future projections of wind hazard indicate decreases in wind hazard in northern Australia, increases along the east coast and decreases in the south, although such projections are considerably uncertain and are more uncertain for small-scale storms than for larger storms.     Risk Management and Adaptation for Extremes and Abrupt Changes in Climate and Oceans: Current Knowledge Gaps

    Perspectives for risk management and adaptation have received ample attention in the recent IPCC Special Report on Changes in the Oceans and Cryosphere (SROCC). However, several knowledge gaps on the impacts of abrupt changes, cascading effects and compound extreme climatic events have been identified, and need further research. We focus on specific climate change risks identified in the SROCC report, namely: changes in tropical and extratropical cyclones; marine heatwaves; extreme ENSO events; and abrupt changes in the Atlantic Meridional Overturning Circulation. Several of the socioeconomic impacts from these events are not yet well-understood, and the literature is also sparse on specific recommendations for integrated risk management and adaptation options to reduce such risks. Also, past research has mostly focussed on concepts that have seen little application to real-world cases. We discuss relevant research needs and priorities for improved social-ecological impact assessment related to these major physical changes in the climate and oceans. For example, harmonised approaches are needed to better understand impacts from compound events, and cascading impacts across systems. Such information is essential to inform options for adaptation, governance and decision-making. Finally, we highlight research needs for developing transformative adaptation options and their governance

    Natural hazards in Australia : sea level and coastal extremes

    The Australian coastal zone encompasses tropical, sub- and extra-tropical climates and accommodates about 80 % of Australia’s population. Sea level extremes and their physical impacts in the coastal zone arise from a complex set of atmospheric, oceanic and terrestrial processes that interact on a range of spatial and temporal scales and will be modified by a changing climate, including sea level rise. This review details significant progress over recent years in understanding the causes of past and projections of future changes in sea level and coastal extremes, yet a number of research questions, knowledge gaps and challenges remain. These include efforts to improve knowledge on past sea level extremes, integrate a wider range of processes in projections of future changes to sea level extremes, and focus efforts on understanding long-term coastline response from the combination of contributing factors

    The IĸB protein BCL3 controls osteogenesis and bone health

    Objective: IĸB protein B-cell lymphoma 3-encoded protein (BCL3) is a regulator of the NF-κB family of transcription factors. NF-κB signalling fundamentally influences the fate of bone-forming osteoblasts and bone-resorbing osteoclasts, but the role of BCL3 in bone biology has not been investigated. The objective of this study was to evaluate BCL3 in skeletal development, maintenance and osteoarthritic pathology. Methods: To assess the contribution of BCL3 to skeletal homeostasis, neonatal mice (n = 6-14) lacking BCL3 (Bcl3−/−) and WT controls were characterised for bone phenotype and density. To reveal the contribution to bone phenotype by the osteoblast compartment in Bcl3−/− mice, transcriptomic analysis of early osteogenic differentiation and cellular function (n = 3-7) were assessed. Osteoclast differentiation and function in Bcl3−/− mice (n = 3-5) was assessed. Adult 20-week Bcl3−/− and WT mice bone phenotype, strength and turnover were assessed. A destabilisation of the medial meniscus (DMM) model of osteoarthritic ostephytogenesis was utilised to understand adult bone formation in Bcl3−/− mice (n = 11-13). Results: Evaluation of Bcl3−/− mice revealed congenitally increased bone density, long bone dwarfism, increased bone biomechanical strength and altered bone turnover. Molecular and cellular characterisation of mesenchymal precursors showed that Bcl3−/− cells display an accelerated osteogenic transcriptional profile that leads to enhanced differentiation into osteoblasts with increased functional activity; which could be reversed with a mimetic peptide. In a model of osteoarthritis-induced osteophytogenesis, Bcl3−/− mice exhibit decreased pathological osteophyte formation (P < 0.05). Conclusion: Cumulatively, these findings demonstrate that BCL3 controls developmental mineralisation to enable appropriate bone formation, whilst in a pathological setting it contributes to skeletal pathology
