    Detection of K+ mesons in segmented electromagnetic calorimeters

    The combination of the CrystalBall and TAPS electromagnetic calorimeters were installed in the MAMI A2 hall in 2003. Here they are able to detect the reaction products from photo-induced reactions in combination with the Glasgow photon tagger. In the last two years the MAMI facility was upgraded from 885 MeV to 1.5 GeV, the A2 photon tagger underwent a similar upgrade crossing the threshold for strangeness photoproduction. For the CrystalBall this created a new challenge, to identify K+ mesons above the large background from other charged hadrons, in a situation where the detector setup does not benefit from a magnetic field to help separate particle species.     Functional testing of topical skin formulations using an optimised ex vivo skin organ culture model.

    A number of equivalent-skin models are available for investigation of the ex vivo effect of topical application of drugs and cosmaceuticals onto skin, however many have their drawbacks. With the March 2013 ban on animal models for cosmetic testing of products or ingredients for sale in the EU, their utility for testing toxicity and effect on skin becomes more relevant. The aim of this study was to demonstrate proof of principle that altered expression of key gene and protein markers could be quantified in an optimised whole tissue biopsy culture model. Topical formulations containing green tea catechins (GTC) were investigated in a skin biopsy culture model (n = 11). Punch biopsies were harvested at 3, 7 and 10 days, and analysed using qRT-PCR, histology and HPLC to determine gene and protein expression, and transdermal delivery of compounds of interest. Reduced gene expression of α-SMA, fibronectin, mast cell tryptase, mast cell chymase, TGF-ÎČ1, CTGF and PAI-1 was observed after 7 and 10 days compared with treated controls (p < 0.05). Histological analysis indicated a reduction in mast cell tryptase and chymase positive cell numbers in treated biopsies compared with untreated controls at day 7 and day 10 (p < 0.05). Determination of transdermal uptake indicated that GTCs were detected in the biopsies. This model could be adapted to study a range of different topical formulations in both normal and diseased skin, negating the requirement for animal models in this context, prior to study in a clinical trial environment

    A comprehensive evidence-based review on the role of topicals and dressings in the management of skin scarring

    Wound healing after dermal injury is an imperfect process, inevitably leading to scar formation as the skin re-establishes its integrity. The resulting scars have different characteristics to normal skin, ranging from fine-line asymptomatic scars to problematic scarring including hypertrophic and keloid scars. Scars appear as a different colour to the surrounding skin and can be flat, stretched, depressed or raised, manifesting a range of symptoms including inflammation, erythema, dryness and pruritus, which can result in significant psychosocial impact on patients and their quality of life. In this paper, a comprehensive literature review coupled with an analysis of levels of evidence (LOE) for each published treatment type was conducted. Topical treatments identified include imiquimod, mitomycin C and plant extracts such as onion extract, green tea, Aloe vera, vitamin E and D, applied to healing wounds, mature scar tissue or fibrotic scars following revision surgery, or in combination with other more established treatments such as steroid injections and silicone. In total, 39 articles were included, involving 1703 patients. There was limited clinical evidence to support their efficacy; the majority of articles (n = 23) were ranked as category 4 LOE, being of limited quality with individual flaws, including low patient numbers, poor randomisation, blinding, and short follow-up periods. As trials were performed in different settings, they were difficult to compare. In conclusion, there is an unmet clinical need for effective solutions to skin scarring, more robust long-term randomised trials and a consensus on a standardised treatment regime to address all aspects of scarring

    Occupational Therapy in HomEcare Re-ablement Services (OTHERS): results of a feasibility randomised controlled trial

    Objectives: The objective of this study was to test the feasibility of conducting a randomised controlled trial (RCT) of an intervention targeted at activities of daily living (ADL), delivered by an occupational therapist, in homecare reablement. Design: Feasibility parallel group RCT. Setting: Single-site local authority homecare reablement service. Participants: People referred for homecare reablement with ability to consent. Exclusion criteria were as follows: inability to speak English, receiving other community therapy services, needing two or more to assist transfer and receiving end-of-life care. Control: ‘Usual care’ was 6 weeks of homecare reablement delivered by social care workers (no routine health professional input). Intervention: A targeted ADL programme, delivered by an occupational therapist incorporating goal setting, teaching/practising techniques, equipment/adaptations and provision of advice/support. This was in addition to usual care. Outcome measures: Aspects of feasibility including eligibility, recruitment, intervention delivery, attrition and suitability and sensitivity of outcome measures. Participant outcomes were personal and extended ADL, quality of life, falls and use of health and social care services. Results: 30 participants were recruited, 15 to each arm, which was 60% of those eligible. Data from 22 (73%) were analysed at 6 months. Of the 15 participants, 13 (86%) received the intervention and were able to set one or more ADL goals. There were improvements from baseline in both groups, although overall improvements were greater in the occupational therapy (OT) intervention group. The biggest threat to feasibility was a change in service configuration during the trial, involving additional occupational therapy input, affecting usual care and recruitment. Conclusions: Despite the service reconfiguration, it was feasible to recruit and retain participants, deliver the intervention and collect outcome data that were responsive to change. The choice of primary outcome measure remains unclear. A further powered study is feasible and warranted; however, the design will require careful consideration because of ongoing national changes in service configurations

    The reaction Îłpâ†’Ï€âˆ˜Îłâ€Čp\gamma p \to \pi^\circ \gamma^\prime p and the magnetic dipole moment of the Δ+(1232)\Delta^+(1232) resonance

    The reaction Îłpâ†’Ï€âˆ˜Îłâ€Čp\gamma p \to \pi^\circ \gamma^\prime p has been measured with the TAPS calorimeter at the Mainz Microtron accelerator facility MAMI for energies between s\sqrt{s} = 1221--1331 MeV. Cross sections differential in angle and energy have been determined for all particles in the final state in three bins of the excitation energy. This reaction channel provides access to the magnetic dipole moment of the Δ+(1232)\Delta^{+}(1232) resonance and, for the first time, a value of ΌΔ+=(2.7−1.3+1.0(stat.)±1.5(syst.)±3(theo.))ÎŒN\mu_{\Delta^+} = (2.7_{-1.3}^{+1.0}(stat.) \pm 1.5 (syst.) \pm 3(theo.)) \mu_N has been extracted

    A measurement of the 4He(g,n) reaction from 23 < Eg < 70 MeV

    A comprehensive set of 4He(g,n) absolute cross-section measurements has been performed at MAX-lab in Lund, Sweden. Tagged photons from 23 < Eg < 70 MeV were directed toward a liquid 4He target, and neutrons were identified using pulse-shape discrimination and the Time-of-flight Technique in two liquid-scintillator detector arrays. Seven-point angular distributions have been measured for fourteen photon energies. The results have been subjected to complementary Transition-coefficient and Legendre-coefficient analyses. The results are also compared to experimental data measured at comparable photon energies as well as Recoil-Corrected Continuum Shell Model, Resonating Group Method, and Effective Interaction Hyperspherical-Harmonic Expansion calculations. For photon energies below 29 MeV, the angle-integrated data are significantly larger than the values recommended by Calarco, Berman, and Donnelly in 1983.Comment: 16 pages, 14 figures, some more revisions, submitted to Physical Review

    Occupational Therapy in HomEcare Re-ablement Services (OTHERS): study protocol for a randomized controlled trial

    Background Homecare re-ablement services have been developed by local authorities in England in response to the government agenda for health and social care. These services aim to optimize users’ independence and ability to cope at home, and reduce the need for ongoing health and social care services. However, there is currently limited evidence and guidance regarding the optimum configuration and delivery of re-ablement services. In particular, the impact of occupational therapy input on service user outcomes has been highlighted as a specific research priority. Methods/Design This feasibility randomized controlled trial (RCT) will recruit 50 people from one local authority led homecare re-ablement service in England. Those who provide informed consent will be randomized to receive either usual homecare re-ablement (without routine occupational therapy input) or usual homecare re-ablement plus an enhanced program targeted at activities of daily living (ADL), delivered by an occupational therapist. The primary aim of this study is to assess the feasibility of conducting a further, powered study. The participant outcomes assessed will be independence in personal and extended ADL, health and social care-related quality of life, number of care support hours, falls, acute and residential admissions and use of health and social care services. These will be assessed at two weeks, three months and six months post-discharge from the re-ablement service. Discussion To our knowledge, this is the first RCT of occupational therapy in homecare re-ablement services. The results of this study will lay the foundations for a further powered study. The findings will be relevant to researchers, clinicians, commissioners and users of adult social care services

    Photoproduction of eta-mesic 3He

    The photoproduction of eta-mesic 3He has been investigated using the TAPS calorimeter at the Mainz Microtron accelerator facility MAMI. The total inclusive cross section for the reaction gamma+3He->eta+X has been measured for photon energies from threshold to 820 MeV. The total and angular differential coherent eta cross sections have been extracted up to energies of 745 MeV. A resonance-like structure just above the eta production threshold with an isotropic angular distribution suggests the existence of a resonant quasi-bound state. This is supported by studies of a competing decay channel of such a quasi-bound eta-mesic nucleus into pi^0+p+X. A binding energy of (-4.4+-4.2) MeV and a width of (25.6+-6.1) MeV is deduced for the quasi-bound eta-mesic state in 3He.Comment: v1: 4 pages, 4 figures, submitted to PRL; v2: minor revisions and corrections, new figure added, 4 pages, 5 figs; v3: minor change

    The 12^{12}C(Îł,ppn)(\gamma,ppn) reaction has been measured for EÎł_{\gamma}=150-800 MeV in the first study of this reaction in a target heavier than 3^3He. The experimental data are compared to a microscopic many body calculation. The model, which predicts that the largest contribution to the reaction arises from final state interactions following an initial pion production process, overestimates the measured cross sections and there are strong indications that the overestimate arises in this two-step process. The selection of suitable kinematic conditions strongly suppresses this two-step contribution leaving cross sections in which up to half the yield is predicted to arise from the absorption of the photon on three interacting nucleons and which agree with the model. The results indicate (Îł,3N)(\gamma,3N) measurements on nuclei may be a valuable tool for obtaining information on the nuclear three-body interaction.Comment: 5 pages, 3 figure
