Prise en charge tardive des sequelles de fractures du plancher orbitaire
Introduction : Les fractures du plancher orbitaire, sont devenues une pathologie assez fréquente étant donné, l’augmentation du nombre d’accidents de la route. Le plancher orbitaire de part sa constitution anatomique est certainement le maillon faible du cadre orbitaire. Observation : Nous présentons le cas d’un jeune de 35 ans polytraumatisé avec des séquelles de fracture du plancher orbitaire qui ne sera opéré que deux années plus tard, priorité chirurgicale oblige ( le rachis étant opéré en premier) avec les risques certains d’échec de la chirurgie ophtalmologique. Discussion : Les séquelles de ce type de fracture sont exposées, ainsi que les différentes modalités thérapeutiques. L’enophtalmie, la diplopie, les anomalies osseuses et palpébrales constituent autant de séquelles parfois difficiles à prendre en charge. Les voies d’abord chirurgical sont discutées ; l’usage de matériaux tels les implants en hydroxyapatite également. Conclusion : Le traitement des fractures du plancher de l’orbite est actuellement bien codifié et doit toujours être tenté même des années après le traumatisme causal.Mots clés : fracture plancher orbitaire, diplopi
Manifestations Ophtalmologiques Au Cours Du Syndrome D\'apert : A Propos D\'un Cas
Introduction : Parmi les crâniosténoses, le syndrome d\'Apert demande la collaboration de plusieurs spécialistes, pour sauver ce qui peut l\'être de la fonction visuelle des patients et permettre un développement cérébral le plus proche de la normale.
Observation : Nous présentons le cas d\'une jeune suivie et traitée depuis son jeune âge pour un syndrome d\'Apert. Elle a subi plusieurs interventions successives pour garder à un âge assez avancé une fonction visuelle appréciable à 3/10.Les modifications anatomiques ont inéluctablement influé sur l\'état visuel de la patiente avec une myopie forte,un
astigmatisme relativement important et une atrophie papillaire partielle. Discussion : D\'origine génétique, le syndrome d\'Apert est dû à une mutation allélique du récepteur 2 d\'un facteur fibroblastique. Les signes de souffrance cérébrale sont inévitables, et l\'atrophie optique relative représente la séquelle fonctionnelle principale. Conclusion : Une prise en charge de longue haleine est nécessaire dans le syndrome d\'Apert pour espérer sauver une
fonction visuelle utile.Introduction : Several craniosynostotic syndromes are described such as Apert syndrome in which collaboration between different specialists is necessary to preserve visual function and to allow normal cerebral development. Case-report : It\'s a case note of a girl with Apert syndrome. She underwent since her infancy several surgical operations. Anatomic modifications affected her visual status with a best visual acuity of 3/1O, high myopia, astigmatism and partial optic atrophy. Discussion : Apert syndrome is a genetic disorder due to a mutation in fibroblast receptor growth factor genes. Optic
atrophy attributed to optic neuropathy represents the major functional sequella and the major cause of visual loss. Conclusion : Apert syndrome, like all craniosynostotic syndromes, requires a correct management in order to preserve the visual function. Cholécystectomie par Célioscopie de Lithiases Biliaires Symptomatiques chez une Jeune Femme de 34 Ans et Revue de la Littérature (Un Cas Clinique)
Introduction : La lithiase biliaire est une pathologie d’une grande fréquence ; elle est le plus souvent asymptomatique, mais peut se compliquer en impactant négativement la qualité de vie. L’ablation de la vésicule sous endoscopie est prometteuse. Nous rapportons un cas clinique pour discuter des modalités thérapeutiques à travers une revue de la littérature. Nous rapportons, l’observation d’une femme de 34 ans avec antécédent obese Elle nous a consulté en juin 2020 pour des vomissements post prandiaux chroniques avec altération de l’état générale. Les examens paraclinques réalisés n’ont pas révélé une cause évidente. Evacuée à la polyclinique Hammamet à Tunis le diagnostic d’une lithiase vésiculaire a été posé et une ablation sous célioscopie était réalisée avec extraction de 13 gros calculs d’aspect cholestéroliques dont le diamètre moyen de 3 cm, et la pièce opératoire a trouvé un aspect histologique d’une cholécystite chronique diverticulaire en poussée aiguë modérée. La patiente est totalement guérie de sa maladie.
Cholelithiasis is a very common pathology; it is most often asymptomatic, but can become complicated by negatively impacting quality of life. Removal of the gallbladder under endoscopy is promising. We report a clinical case to discuss therapeutic modalities through a review of the literature. We report the observation of a 34-year-old woman with a history of obesity. She consulted us in June 2020 for chronic postprandial vomiting with deterioration in general condition. Paraclinical examinations carried out did not reveal an obvious cause. Evacuated to the Hammamet polyclinic in Tunis, the diagnosis of gallbladder lithiasis was made and an ablation under laparoscopy was carried out with extraction of 13 large cholesterol-looking stones with an average diameter of 3 cm, and the operating specimen found an appearance histology of chronic diverticular cholecystitis in moderate acute attack. The patient is completely cured of her illness. Conclusion: The interest of the work lies in the atypical case of symptomatic cholelithiasis which our technical platform did not allow to easily treat in our country
Privacy preservation using game theory in e-health application
International audienceIn the domain of the new e-health applications, the ubiquitous nature of intelligent devices raises legitimate questions about the privacy of persons, and how to cope with the heterogeneity of user and application requirements in terms of security services. This requires the development of adaptive, context-aware and user-centric security solutions. Recent e-health applications (M2M/IoT/Web) permit remote monitoring of patient health, medical treatments, fitness information and parameters, alarm triggering, etc. Since the monitored device is tightly related to a human being, new changes arise regarding communications facilities constraints, private data protection, trust relationships, etc. In this work, we propose a Markovian game between data holder and data requester, in a weight loss program, to protect data privacy. We aim to reach a compromise between privacy concessions made by data holder and incentive motivation proposed by data requester. Finally, we show numerical results of executed experiments to evaluate the proposed model
Kyste osseux anévrismal de l'ethmoïde
Aneurysmal bone cysts is a benign, slow growing expansile lesion usually found in long bones or vertebrae. Only 5 cases of ABC of the ethmoid bone have been reported in the literature. Plain radiographs are not specific. CT and MRI are very useful to depict the extent of the lesion and demonstrate the presence of blood-fluid levels, which is a characteristic finding of ABC. Imaging Findings in Chiari I Malformation with Syringomyelia in a Case of Charcot Shoulder
Neuropathic arthropathy of the shoulder is reported in only 5% of cases. Here, we report a rare case of neuropathic arthropathy of the shoulder, secondary to Chiari malformation Type I with associated syringomyelia, that remained undetected for four years. A 38-year-old female presented to our Department with a swelling over the right shoulder that had persisted for four years. X-ray of the joint showed destruction of the head of the right humerus, with typical blunt amputated appearance of the bone and increased joint space. Magnetic resonance imaging showed destruction and lateral dislocation of the head of the humerus. Large amount of fluid collection was seen in and around the right shoulder joint. Neuropathic osteoarthropathy can be defined as bone and joint changes that occurs secondary to loss of sensation. In our case, neuropathic shoulder joint was secondary to syringomyelia associated with Chiari I malformation
