    Boosting advice and knowledge sharing among healthcare professionals

    Purpose: This study investigates the dynamics of knowledge sharing in healthcare, exploring some of the factors that are more likely to influence the evolution of idea sharing and advice seeking in healthcare. Design/methodology/approach: We engaged 50 pediatricians representing many subspecialties at a mid-size US children's hospital using a social network survey to map and measure advice seeking and idea sharing networks. Through the application of Stochastic Actor-Oriented Models, we compared the structure of the two networks prior to a leadership program and eight weeks post conclusion. Findings: Our models indicate that healthcare professionals carefully and intentionally choose with whom they share ideas and from whom to seek advice. The process is fluid, non-hierarchical and open to changing partners. Significant transitivity effects indicate that the processes of knowledge sharing can be supported by mediation and brokerage. Originality: Hospital administrators can use this method to assess knowledge-sharing dynamics, design and evaluate professional development initiatives, and promote new organizational structures that break down communication silos. Our work contributes to the literature on knowledge sharing in healthcare by adopting a social network approach, going beyond the dyadic level, and assessing the indirect influence of peers' relationships on individual networks

    Florfenicol concentrations in milk of lactating cows postreated by intramuscular or intramammary routes

    Objetivo. Determinar la concentración de florfenicol en leche bovina después del tratamiento intramuscular o intramamario, para establecer el tiempo de retiro, la eficacia terapéutica, y su influencia sobre la producción láctea. Materiales y métodos. Se seleccionaron doce vacas Holstein en lactancia de la hacienda de Universidad de Antioquia (Colombia). Se asignaron al azar al grupo control (n=6) o al grupo florfenicol (n=6), al cual se le administró 20 mg/kg de florfenicol por vía intramamaria e intramuscular con 15 días de diferencia entre tratamientos. Resultados. La Tmax y Cmax para la vía intramuscular fueron 6 horas y 2.86 mg/L respectivamente. La Tmax y Cmax para la vía intramamaria, fueron estimados como 0 hora y aproximadamente 20000 mg/L respectivamente por extrapolación de la línea de regresión. La fase de eliminación del florfenicol en leche, tuvo una vida media de eliminación (t½) promedio de 19.8 horas y 4.9 horas para la vía intramuscular e intramamaria respectivamente. Cuando la concentración inhibitoria minima (C.I.M) del florfenicol para Stahphylococcus aureus, fue usada como el valor de referencia, la eficacia terapéutica sólo se alcanzó por la vía intramamaria. No existió diferencia estadística significativa entre las producciones lácteas de las vacas tratadas y no tratadas. Conclusiones. De acuerdo con estos resultados, el tiempo de retiro postratamiento no debe ser menor de 3 días para la administración intramamaria y al menos 7 días para la administración intramuscular. La eficacia terapéutica sólo se alcanzó por la vía intramamaria. Además no existió diferencia estadística significativa en la producción láctea de las vacas tratadas con respecto a las no tratadas

    A Decade of Implantation of the Bachelor’s Degree in Electrical Engineering in the Western Amazon

    This paper deals with the implementation of the Bachelor\u27s Degree in Electrical Engineering in Western Amazonia, as well as it presents an analysis of the academic performance of students in the Basic and Professional Core. The Federal University of Rondônia Foundation (UNIR), located in Porto Velho, Brazil, started the activities of the course in 2007 and continues to invest in the infrastructure and qualification of human resources in order to stimulate technological development in the region. Analyzing the performance data of the academics regarding the degree of difficulty established from the curricular offer, it is possible to infer some answers. In the course of the analysis, the databases emerged by taking note, such as students who only enroll and do not attend the course, among others. The disciplines diverged into two segments, Basic Core (NB) and Professional Core (NP). The data refers to the grades obtained by the students in all the disciplines studied and offered by the Academic Electrical Engineering Department (DAEE) in the period from 2007/1 to 2016/2. Along the analysis, two graphs demonstrated the evaluation. The analysis of the academic performance of students in the subjects during the NB and NP reinforces the result obtained in the Program for International Student Assessment (PISA), since Brazil has not obtained progress in science, reading, and mathematics averages. The results obtained indicate the necessity of effective measures in primary and secondary education since the deficit propagated to superior levels

    Preclinical Evaluation of Caprylic Acid-Fractionated IgG Antivenom for the Treatment of Taipan (Oxyuranus scutellatus) Envenoming in Papua New Guinea

The taipan, Oxyuranus scutellatus, inflicts a large number of bites that, in the absence of antivenom therapy, result in high mortality. Parenteral administration of antivenoms manufactured in Australia is the current treatment of choice for these envenomings. However, the price of these products is high and has increased over the last 25 years; consequently the country can no longer afford all the antivenom it needs. This situation prompted an international collaborative project aimed at generating a new, low-cost antivenom against O. scutellatus for PNG. Methodology/Principal Findings: A new monospecific equine whole IgG antivenom, obtained by caprylic acid fractionation of plasma, was prepared by immunising horses with the venom of O. scutellatus from PNG. This antivenom was compared with the currently used F(ab’)2 monospecific taipan antivenom manufactured by CSL Limited, Australia. The comparison included physicochemical properties and the preclinical assessment of the neutralisation of lethal neurotoxicity and the myotoxic, coagulant and phospholipase A2 activities of the venom of O. scutellatus from PNG. The F(ab’)2 antivenom had a higher protein concentration than whole IgG antivenom. Both antivenoms effectively neutralised, and had similar potency, against the lethal neurotoxic effect (both by intraperitoneal and intravenous routes of injection), myotoxicity, and phospholipase A2 activity of O. scutellatus venom. However, the whole IgG antivenom showed a higher potency than the F(ab’)2 antivenom in the neutralisation of the coagulant activity of O. scutellatus venom from PNG. Conclusions/Significance: The new whole IgG taipan antivenom described in this study compares favourably with the currently used F(ab’)2 antivenom, both in terms of physicochemical characteristics and neutralising potency.
