    Behavioral and cardiopulmonary effects of dexmedetomidine alone and in combination with butorphanol, methadone, morphine or tramadol in conscious sheep

    Objective: To compare cardiopulmonary and sedative effects following administration of dexmedetomidine alone or with butorphanol, methadone, morphine or tramadol in healthy sheep. Study design: Randomized crossover study. Animals: Six Santa InĂȘs sheep, five females, one male, aged 12–28 months and weighing 40.1 ± 6.2 kg. Methods: Sheep were assigned treatments of dexmedetomidine (0.005 mg kg−1; D); D and butorphanol (0.15 mg kg−1; DB); D and methadone (0.5 mg kg−1; DM); D and morphine (0.5 mg kg−1; DMO); or D and tramadol (5.0 mg kg−1; DT). All drugs were administered intravenously with at least 7 days between each treatment. Rectal temperature, heart rate (HR), respiratory rate (fR), invasive arterial pressure, blood gases and electrolytes were measured prior to administration of drugs (baseline, T0) and every 15 minutes following drug administration for 120 minutes (T15–T120). Sedation was scored by three observers blinded to treatment. Results: HR decreased in all treatments and fR decreased in DM at T30 and DMO at T30 and T45. PaCO2 was increased in D, DB and DM compared with baseline, and PaO2 decreased in D at T15 and T45; in DB at T15 to T75; in DM at T15 to T60; in DMO at T15; and in DT at T15, T30 and T75. There was a decrease in temperature in D, DB and DM. An increased pH was measured in D at all time points and in DT at T30–T120. inline image and base excess were increased in all treatments compared with baseline. There were no statistical differences in sedation scores. Conclusions and clinical relevance: The combination of dexmedetomidine with butorphanol, methadone, morphine or tramadol resulted in similar changes in cardiopulmonary function and did not improve sedation when compared with dexmedetomidine alone

    Glycolytic pathway candidate markers in the prognosis of oral squamous cell carcinoma: a systematic review with meta-analysis

    Molecular changes that affect mitochondrial glycolysis have been associated with the maintenance of tumor cells. Some metabolic factors have already been described as predictors of disease severity and outcomes. This systematic review was conducted to answer the question: Is the glycolytic pathway correlated with the prognosis of oral squamous cell carcinoma (OSCC)? A search strategy was developed to retrieve studies in English from PubMed, Scopus, and ISI Web of Knowledge using keywords related to squamous cell carcinoma, survival, and glycolytic pathway, with no restriction of publication date. The search retrieved 1273 publications. After the titles and abstracts were analyzed, 27 studies met inclusion criteria. Studies were divided into groups according to two subtopics, glycolytic pathways and diagnosis, which describe the glycolytic profile of OSCC tumors. Several components of tumor energy metabolism found in this review are important predictors of survival of patients with OSCC

    FetReg: Placental Vessel Segmentation and Registration in Fetoscopy Challenge Dataset

    Fetoscopy laser photocoagulation is a widely used procedure for the treatment of Twin-to-Twin Transfusion Syndrome (TTTS), that occur in mono-chorionic multiple pregnancies due to placental vascular anastomoses. This procedure is particularly challenging due to limited field of view, poor manoeuvrability of the fetoscope, poor visibility due to fluid turbidity, variability in light source, and unusual position of the placenta. This may lead to increased procedural time and incomplete ablation, resulting in persistent TTTS. Computer-assisted intervention may help overcome these challenges by expanding the fetoscopic field of view through video mosaicking and providing better visualization of the vessel network. However, the research and development in this domain remain limited due to unavailability of high-quality data to encode the intra- and inter-procedure variability. Through the \textit{Fetoscopic Placental Vessel Segmentation and Registration (FetReg)} challenge, we present a large-scale multi-centre dataset for the development of generalized and robust semantic segmentation and video mosaicking algorithms for the fetal environment with a focus on creating drift-free mosaics from long duration fetoscopy videos. In this paper, we provide an overview of the FetReg dataset, challenge tasks, evaluation metrics and baseline methods for both segmentation and registration. Baseline methods results on the FetReg dataset shows that our dataset poses interesting challenges, offering large opportunity for the creation of novel methods and models through a community effort initiative guided by the FetReg challenge

    CondiçÔes e relaçÔes de trabalho no serviço pĂșblico: o caso do governo Lula

    Este artigo tem por objetivo inventariar as diversas iniciativas do governo Lula (2003-2010) no que diz respeito Ă  formatação de uma polĂ­tica de recursos humanos e, dessa forma, verificar se houve melhoria das condiçÔes e relaçÔes de trabalho no setor pĂșblico brasileiro. A partir dos anos 1990 e na contramĂŁo do grande crescimento do emprego pĂșblico nas seis dĂ©cadas anteriores, a adesĂŁo dos governos brasileiros aos princĂ­pios do "Consenso de Washington" destaca o funcionalismo como ponto fundamental na agenda das reformas necessĂĄrias Ă  retomada do crescimento econĂŽmico. Os tais governos tenderam a tratar o emprego pĂșblico como um problema fiscal e atuaram no sentido de restringir sua dimensĂŁo. Em simultĂąneo, as condiçÔes e relaçÔes de trabalho apresentaram-se mais precarizadas (evidenciadas pela ausĂȘncia de reajuste salarial; o crescimento de formas variĂĄveis de remuneração; ampliação do quadro de temporĂĄrios e terceirizados e com a postura autoritĂĄria diante da representação sindical). Por meio de pesquisa documental e bibliogrĂĄfica, poder-se-ĂĄ perceber que a reestruturação de vĂĄrias carreiras, a retomada dos concursos, a criação da Mesa Nacional de Negociação Permanente, a reforma previdenciĂĄria e os reajustes seletivos das remuneraçÔes manifestam a ambigĂŒidade como marca fundamental das polĂ­ticas de recursos humanos. Isto porque, a um sĂł tempo, os avanços nas condiçÔes de trabalho do servidor pĂșblico foram sempre acompanhados pela manutenção de parte da agenda conservadora dos anos 1990, sobretudo no que diz respeito Ă  reprodução de limites fiscais rigorosos
