    Constructal approach to bioengineering: the ocular anterior chamber temperature

    The aim of this work was to analyse the pressure inside the eyes anterior chamber, namedintraocular pressure (IOP), in relation to the biomechanical properties of corneas. The approach used was based on the constructal law, recently introduced in vision analysis. Results were expressed as the relation between the temperature of the ocular anterior chamber and the biomechanical properties of the cornea. The IOP, the elastic properties of the cornea, and the related refractive properties of the eye were demonstrated to be dependent on the temperature of the ocular anterior chamber. These results could lead to new perspectives for experimental analysis of the IOP in relation to the properties of the cornea

    Case report: lenvatinib in neoadjuvant setting in a patient affected by invasive poorly differentiated thyroid carcinoma

    We report a case of an elderly woman presenting with a huge cervical mass invading the tracheal lumen. Diagnosed as invasive poorly differentiated thyroid cancer, after an endotracheal biopsy, stenting and radiotherapy, it was judged eligible for total thyroidectomy, but surgery was delayed due to pulmonary thromboembolism. The patient was therefore treated with lenvatinib with a neoadjuvant intent until hemodynamic stability was obtained. Thyroidectomy and radioiodine therapy were then performed and the postdose scan revealed an area of modest uptake in the anterior part of the neck. The patient is now in a good clinical status and she continues her follow-up program without any adjuvant therapy

    ScienzAperta is an outreach science venue that the Istituto Nazionale di Geofisica e Vulcanologia started in 2011 as the spring of science: the doors of the headquarters of science were finally opened to public. A number of events, conferences, seminars, guided tours through the Institute and its laboratories are every year offered to general public. The venue is held in most of the cities where the Institute is located, priority to high seismic and/or volcanic risk regions. On May 2014 we held ScienzAperta for the first time in Milano and open up the doors to schools specifically dealing mostly with seismic hazard in a region where general public not necessarily think it might We offered students conferences, seminars and educational activities to highlight the fun of science and jet raise awareness on proper behaviours in case of earthquake shaking. We asked students and teachers, from elementary to high schools, to fill in a questionnaire that we use to evaluate the appreciation the venue had.     Local variability of the ground shaking during the 2009 L’Aquila earthquake (April 6, 2009—Mw 6.3): the case study of Onna and Monticchio villages

    The 2009 Mw 6.3 L’Aquila event caused extensive damage in the city of L’Aquila and in some small towns in its vicinity. The most severe damage was recognized SE of L’Aquila town along the Aterno river valley. Although building vulnerability and near-source effects are strongly responsible for the high level of destruction, site effects have been invoked to explain the damage heterogeneities and the similarities between the 2009 macroseismic field with the intensities of historical earthquakes. The small village of Onna is settled on quaternary alluvium and suffered during the L’Aquila event an extremely heavy damage in the masonry structures with intensity IX–X on the Mercalli-Cancani-Sieberg (MCS) scale. The village of Monticchio, far less than 1.3 km from Onna, is mostly situated on Meso- zoic limestone and suffered a smaller level of damaging (VI MCS). In the present paper, we analyze the aftershock recordings at seismic stations deployed in a small area of the middle-Aterno valley including Onna and Monticchio. The aim is to investigate local ampli-fication effects caused by the near-surface geology. Because the seismological stations are close together, vulnerability and near-source effects are assumed to be constant. The wave- form analysis shows that the ground motion at Onna is systematically characterized by large high-frequency content. The frequency resonance is varying from 2 to 3 Hz and it is related to alluvial sediments with a thickness of about 40 m that overlay a stiffer Pleistocene substrate.     KIAA1840 mutations cause ARCMT2

    Charcot–Marie–Tooth disease is a group of hereditary peripheral neuropathies that share clinical characteristics of progressive distal muscle weakness and atrophy, foot deformities, distal sensory loss, as well as diminished tendon reflexes. Hundreds of causative DNA changes have been found, but much of the genetic basis of the disease is still unexplained. Mutations in the ALS5/SPG11/ KIAA1840 gene are a frequent cause of autosomal recessive hereditary spastic paraplegia with thin corpus callosum and peripheral axonal neuropathy, and account for ∼40% of autosomal recessive juvenile amyotrophic lateral sclerosis. The overlap of axonal Charcot–Marie–Tooth disease with both diseases, as well as the common autosomal recessive inheritance pattern of thin corpus callosum and axonal Charcot–Marie–Tooth disease in three related patients, prompted us to analyse the ALS5/SPG11/ KIAA1840 gene in affected individuals with autosomal recessive axonal Charcot–Marie–Tooth disease. We investigated 28 unrelated families with autosomal recessive axonal Charcot–Marie–Tooth disease defined by clinical, electrophysiological, as well as pathological evaluation. Besides, we screened for all the known genes related to axonal autosomal recessive Charcot–Marie-Tooth disease (CMT2A2/HMSN2A2/ MFN2 , CMT2B1/ LMNA , CMT2B2/ MED25 , CMT2B5/ NEFL , ARCMT2F/dHMN2B/ HSPB1 , CMT2K/ GDAP1 , CMT2P/ LRSAM1 , CMT2R/ TRIM2 , CMT2S/ IGHMBP2 , CMT2T/ HSJ1 , CMTRID/ COX6A1 , ARAN-NM/ HINT and GAN/ GAN ), for the genes related to autosomal recessive hereditary spastic paraplegia with thin corpus callosum and axonal peripheral neuropathy (SPG7/ PGN , SPG15/ ZFYVE26, SPG21/ ACP33 , SPG35/ FA2H , SPG46/ GBA2 , SPG55/ C12orf65 and SPG56/ CYP2U1 ), as well as for the causative gene of peripheral neuropathy with or without agenesis of the corpus callosum ( SLC12A6 ) . Mitochondrial disorders related to Charcot–Marie–Tooth disease type 2 were also excluded by sequencing POLG and TYMP genes. An additional locus for autosomal recessive Charcot–Marie–Tooth disease type 2H on chromosome 8q13-21.1 was excluded by linkage analysis. Pedigrees originated in Italy, Brazil, Canada, England, Iran, and Japan. Interestingly, we identified 15 ALS5/SPG11/ KIAA1840 mutations in 12 families (two sequence variants were never reported before, p.Gln198* and p.Pro2212fs*5). No large deletions/duplications were detected in these patients. The novel mutations seemed to be pathogenic since they co-segregated with the disease in all pedigrees and were absent in 300 unrelated controls. Furthermore, in silico analysis predicted their pathogenic effect. Our results indicate that ALS5/SPG11/ KIAA1840 is the causative gene of a wide spectrum of clinical features, including autosomal recessive axonal Charcot–Marie–Tooth disease

    Comparisons among the five ground-motion models developed using RESORCE for the prediction of response spectral accelerations due to earthquakes in Europe and the Middle East

    This article presents comparisons among the five ground-motion models described in other articles within this special issue, in terms of data selection criteria, characteristics of the models and predicted peak ground and response spectral accelerations. Comparisons are also made with predictions from the Next Generation Attenuation (NGA) models to which the models presented here have similarities (e.g. a common master database has been used) but also differences (e.g. some models in this issue are nonparametric). As a result of the differing data selection criteria and derivation techniques the predicted median ground motions show considerable differences (up to a factor of two for certain scenarios), particularly for magnitudes and distances close to or beyond the range of the available observations. The predicted influence of style-of-faulting shows much variation among models whereas site amplification factors are more similar, with peak amplification at around 1s. These differences are greater than those among predictions from the NGA models. The models for aleatory variability (sigma), however, are similar and suggest that ground-motion variability from this region is slightly higher than that predicted by the NGA models, based primarily on data from California and Taiwan

    AlpArray-Italy: Site description and noise characterization

    Within the framework of the European collaborative research initiative AlpArray (http://www.alparray.ethz. ch), the Istituto Nazionale di Geofisica e Vulcanolgia (INGV) deployed overall 20 broad-band seismic stations in Northern Italy and on two islands in the Tyrrhenian Sea (Capraia and Montecristo) during Fall-Winter 2015. The temporary deployment (16 stations) will run for two to three years and 4 INGV National Seismic Network accelerometric sites are now equipped with additional per- manent broad-band sensors. The 16 temporary stations are equipped with REF TEK 130 digitizers and Nanometrics Trillium Compact 120 s sensors, a couple have Nanometrics Trillium 120P sensors and one a Streckeisen STS2.     Characteristics of psychiatric comorbidities in emergency medicine setting and impact on length of hospitalization: A retrospective study

    This study aims to evaluate clinical correlates of psychiatric comorbidity and length of hospitalization in patients admitted in a general hospital emergency medicine setting. Overall, 160 patients hospitalized for different acute medical pathologies were selected consecutively over 12 months. All subjects were evaluated with proper forms to collect data on medical and psychiatric diagnoses. Levels of C-reactive protein were also measured in all patients. Statistical analyses were conducted with univariate, logistic, and multiple linear regressions. Patients with psychiatric comorbidity had significantly longer hospitalization than did patients with no psychiatric diagnoses (days 10.9±9.5 vs. 6.9±4.5, p<0.005). Agitation and delirium were more frequent in the psychiatry comorbidity study group (p<0.05), as was cognitive impairment (p=0.001). These variables predicted longer hospitalisation (respectively: t=-3.27, p=0.002; t=-2.64, p=0.009; t=-2.85, p=0.006). Psychiatric comorbidity acts as an adjunct factor in determining clinical severity and predicting a more difficult recovery in patients hospitalized in an emergency medicine setting

    Tracking Multiorgan Treatment Response in Systemic AL-Amyloidosis With Cardiac Magnetic Resonance Derived Extracellular Volume Mapping

    Background: Systemic light chain amyloidosis is a multisystem disorder that commonly involves the heart, liver, and spleen. Cardiac magnetic resonance with extracellular volume (ECV) mapping provides a surrogate measure of the myocardial, liver, and spleen amyloid burden. Objectives: The purpose of this study was to assess multiorgan response to treatment using ECV mapping, and assess the association between multiorgan treatment response and prognosis. Methods: The authors identified 351 patients who underwent baseline serum amyloid-P-component (SAP) scintigraphy and cardiac magnetic resonance at diagnosis, of which 171 had follow-up imaging. Results: At diagnosis, ECV mapping demonstrated that 304 (87%) had cardiac involvement, 114 (33%) significant hepatic involvement, and 147 (42%) significant splenic involvement. Baseline myocardial and liver ECV independently predict mortality (myocardial HR: 1.03 [95% CI: 1.01-1.06]; P = 0.009; liver HR: 1.03; [95% CI: 1.01-1.05]; P = 0.001). Liver and spleen ECV correlated with amyloid load assessed by SAP scintigraphy (R = 0.751; P < 0.001; R = 0.765; P < 0.001, respectively). Serial measurements demonstrated ECV correctly identified changes in liver and spleen amyloid load derived from SAP scintigraphy in 85% and 82% of cases, respectively. At 6 months, more patients with a good hematologic response had liver (30%) and spleen (36%) ECV regression than myocardial regression (5%). By 12 months, more patients with a good response demonstrated myocardial regression (heart 32%, liver 30%, spleen 36%). Myocardial regression was associated with reduced median N-terminal pro-brain natriuretic peptide (P < 0.001), and liver regression with reduced median alkaline phosphatase (P = 0.001). Changes in myocardial and liver ECV, 6 months after initiating chemotherapy, independently predict mortality (myocardial HR: 1.11 [95% CI: 1.02-1.20]; P = 0.011; liver HR: 1.07 [95% CI: 1.01-1.13]; P = 0.014). Conclusions: Multiorgan ECV quantification accurately tracks treatment response and demonstrates different rates of organ regression, with the liver and spleen regressing more rapidly than the heart. Baseline myocardial and liver ECV and changes at 6 months independently predict mortality, even after adjusting for traditional predictors of prognosis
