    Impact of public release of performance data on the behaviour of healthcare consumers and providers.

    BACKGROUND: It is becoming increasingly common to publish information about the quality and performance of healthcare organisations and individual professionals. However, we do not know how this information is used, or the extent to which such reporting leads to quality improvement by changing the behaviour of healthcare consumers, providers, and purchasers. OBJECTIVES: To estimate the effects of public release of performance data, from any source, on changing the healthcare utilisation behaviour of healthcare consumers, providers (professionals and organisations), and purchasers of care. In addition, we sought to estimate the effects on healthcare provider performance, patient outcomes, and staff morale. SEARCH METHODS: We searched CENTRAL, MEDLINE, Embase, and two trials registers on 26 June 2017. We checked reference lists of all included studies to identify additional studies. SELECTION CRITERIA: We searched for randomised or non-randomised trials, interrupted time series, and controlled before-after studies of the effects of publicly releasing data regarding any aspect of the performance of healthcare organisations or professionals. Each study had to report at least one main outcome related to selecting or changing care. DATA COLLECTION AND ANALYSIS: Two review authors independently screened studies for eligibility and extracted data. For each study, we extracted data about the target groups (healthcare consumers, healthcare providers, and healthcare purchasers), performance data, main outcomes (choice of healthcare provider, and improvement by means of changes in care), and other outcomes (awareness, attitude, knowledge of performance data, and costs). Given the substantial degree of clinical and methodological heterogeneity between the studies, we presented the findings for each policy in a structured format, but did not undertake a meta-analysis. MAIN RESULTS: We included 12 studies that analysed data from more than 7570 providers (e.g. professionals and organisations), and a further 3,333,386 clinical encounters (e.g. patient referrals, prescriptions). We included four cluster-randomised trials, one cluster-non-randomised trial, six interrupted time series studies, and one controlled before-after study. Eight studies were undertaken in the USA, and one each in Canada, Korea, China, and The Netherlands. Four studies examined the effect of public release of performance data on consumer healthcare choices, and four on improving quality.There was low-certainty evidence that public release of performance data may make little or no difference to long-term healthcare utilisation by healthcare consumers (3 studies; 18,294 insurance plan beneficiaries), or providers (4 studies; 3,000,000 births, and 67 healthcare providers), or to provider performance (1 study; 82 providers). However, there was also low-certainty evidence to suggest that public release of performance data may slightly improve some patient outcomes (5 studies, 315,092 hospitalisations, and 7502 providers). There was low-certainty evidence from a single study to suggest that public release of performance data may have differential effects on disadvantaged populations. There was no evidence about effects on healthcare utilisation decisions by purchasers, or adverse effects. AUTHORS\u27 CONCLUSIONS: The existing evidence base is inadequate to directly inform policy and practice. Further studies should consider whether public release of performance data can improve patient outcomes, as well as healthcare processes

    Chronic glucocorticoid-rich milieu and liver dysfunction

    We investigated the impact of chronic hypercorticosteronemia (due to neonatal monosodium L-glutamate, MSG, and treatment) on liver oxidative stress (OS), inflammation, and carbohydrate/lipid metabolism in adult male rats. We evaluated the peripheral concentrations of several metabolic and OS markers and insulin resistance indexes. In liver we assessed (a) OS (GSH and protein carbonyl groups) and inflammatory (IL-1b, TNFa, and PAI-1) biomarkers and (b) carbohydrate and lipid metabolisms. MSG rats displayed degenerated optic nerves, hypophagia, low body and liver weights, and enlarged adipose tissue mass; higher peripheral levels of glucose, triglycerides, insulin, uric acid, leptin, corticosterone, transaminases and TBARS, and peripheral and liver insulin resistance; elevated liver OS, inflammation markers, and glucokinase (mRNA/activity) and fructokinase (mRNA). Additionally, MSG liver phosphofructokinase-2, glucose-6-phosphatase (mRNA and activity) and glucose-6-phosphate dehydrogenase, Chrebp, Srebp1c, fatty acid synthase, and glycerol-3-phosphate (mRNAs) were increased. In conclusion adult MSG rats developed an insulin-resistant state and increased OS and serious hepatic dysfunction characterized by inflammation and metabolic signs suggesting increased lipogenesis. These features, shared by both metabolic and Cushing?s syndrome human phenotypes, support that a chronic glucocorticoid-rich endogenous environment mainly impacts on hepatic glucose cycle, displacing local metabolism to lipogenesis. Whether correcting the glucocorticoid-rich environment ameliorates such dysfunctions requires further investigation.Fil: Villagarcía, Hernán Gonzalo.     A Fraud Detection System Based on Anomaly Intrusion Detection Systems for E-Commerce Applications

    The concept of exchanging goods and services over the Internet has seen an exponential growth in popularity over the years. The Internet has been a major breakthrough of online transactions, leaping over the hurdles of currencies and geographic locations. However, the anonymous nature of the Internet does not promote an idealistic environment for transactions to occur. The increase in online transactions has been added with an equal increase in the number of attacks against security of online systems. Auction sites and e-commerce web applications have seen an increase in fraudulent transactions. Some of these fraudulent transactions that are executed in e-commerce applications happen due to successful computer intrusions on these web sites. Although a lot of awareness has been raised about these facts, there has not yet been an effective solution to adequately address the problem of application-based attacks in e-commerce. This paper proposes a fraud detection system that uses different anomaly detection techniques to predict computer intrusion attacks in e-commerce web applications. The system analyses queries that are generated when requesting server-side code on an e-commerce site, and create models for different features when information is extracted from these queries. Profiles associated with the e-commerce application are automatically derived from a training dataset

    Modeling Business Models: A cross-disciplinary Analysis of Business Model Modeling Languages and Directions for Future Research

    Modeling languages for business models are a powerful and flexible means of representing and communicating knowledge related to business models. More than fifteen years after Osterwalder et al. (2005) clarified the ontology for the business model concept in this journal, we offer a systematic and cross-disciplinary assessment of the literature on business model modeling languages (BMMLs) that facilitate the visualization of this concept. In so doing, we synthesize and organize the knowledge dispersed across different disciplines in which BMMLs have originated and highlight the potential weaknesses in this literature to offer solid insights for future research. Our analysis reveals the existence of 17 BMMLs that have originated in traditional domains such as strategy and information systems, but also emerging domains such as sustainability. We contrast and compare these BMMLs along three dimensions: semantics, syntax, and pragmatics. We also analyze research that has made use of these BMMLs, differentiating between research that is conducted with a given BMML and research that is conducted about a given BMML. We conclude by offering a research agenda in which we illustrate the main challenges associated with the lack of well-accepted semantic, syntactic, and pragmatic foundations of BMMLs and outline opportunities for future research

    El enfoque de derechos como tendencia contemporánea de la dualidad entre igualdad política y desigualdad material

    El presente trabajo se propone analizar la perspectiva de derecho como una tendencia contemporánea que expresa la contradicción entre, por una parte, la igualdad formal y ante la ley de las personas y, por la otra, la desigualdad en la efectivización de su reproducción material y social. Para ello, en un primer momento se aborda la contradicción entre la igualdad formal y la desigualdad real y como ésta es constitutiva de una forma de organizar la sociabilidad humana en la que la que predomina un mundo dual conformado por la sociedad civil y la sociedad política, cuestión que expresa la disociación entre lo social y lo económico, por un lado, y, por el otro, respecto de lo político. Mientras que, en una segunda instancia, se caracteriza el enfoque de derechos como una propuesta sustentada en la comprensión de las personas como sujetos titulares de derechos que obligan al Estado a hacerlos efectivos.
