Gastric Carcinoid with Hypergastrinemia: Report of Three Cases
We report 3 cases of gastric carcinoids with hypergastrinemia. Case 1: A 60-year-old man had a 2 cm carcinoid of the stomach and underwent partial resection. Involvement of the muscularis propria and lymph nodes metastasis were observed microscopically. Follow-up gastroscopy revealed another carcinoid lesion and total gastrectomy was performed. Case 2: A 67-year-old woman with multiple carcinoids of the entire stomach underwent antrectomy. No growth of residual tumors has been detected so far. Case 3: A 61-year-old man had a tumor near the esophagogastric junction and underwent total gastrectomy. Carcinoid component was diffusely intermingled with adenocarcinoma in the tumor and invaded into the subserosa. In all 3 cases, the serum gastrin level was high and atrophic gastritis was microscopically observed. Carcinoid tumor in Case 3 was different from those in Cases 1 and 2 and interestingly, gastric carcinoid with hypergastrinemia showed various types of appearance
Warm and Dense Molecular Gas in the N159 Region: 12CO J=4-3 and 13CO J=3-2 Observations with NANTEN2 and ASTE
New 12CO J=4-3 and 13CO J=3-2 observations of the N159 region in the Large
Magellanic Cloud have been made. The 12CO J=4-3 distribution is separated into
three clumps. These new measurements toward the three clumps are used in
coupled calculations of molecular rotational excitation and line radiation
transfer, along with other transitions of the 12CO as well as the isotope
transitions of 13CO. The temperatures and densities are determined to be
~70-80K and ~3x10^3 cm-3 in N159W and N159E and ~30K and ~1.6x10^3 cm-3 in
N159S. These results are compared with the star formation activity. The N159E
clump is associated with embedded cluster(s) as observed at 24 micron and the
derived high temperature is explained as due to the heating by these sources.
The N159E clump is likely responsible for a dark lane in a large HII region by
the dust extinction. The N159W clump is associated with embedded clusters
mainly toward the eastern edge of the clump only. These clusters show offsets
of 20"-40" from the 12CO J=4-3 peak and are probably responsible for heating
indicated by the derived high temperature. The N159W clump exhibits no sign of
star formation toward the 12CO J=4-3 peak position and its western region. We
suggest that the N159W peak represents a pre-star-cluster core of ~105M_sol
which deserves further detailed studies. Note that recent star formation took
place between N159W and N159E as indicated by several star clusters and HII
regions, while the natal molecular gas toward the stars have already been
dissipated by the ionization and stellar winds of the OB stars. The N159S clump
shows little sign of star formation as is consistent with the lower temperature
and somewhat lower density. The N159S clump is also a candidate for future star
formation
Sub-millimeter Observations of Giant Molecular Clouds in the Large Magellanic Cloud: Temperature and Density as Determined from J=3-2 and J=1-0 transitions of CO
We have carried out sub-mm 12CO(J=3-2) observations of 6 giant molecular
clouds (GMCs) in the Large Magellanic Cloud (LMC) with the ASTE 10m sub-mm
telescope at a spatial resolution of 5 pc and very high sensitivity. We have
identified 32 molecular clumps in the GMCs and revealed significant details of
the warm and dense molecular gas with n(H2) 10 cm and
Tkin 60 K. These data are combined with 12CO(J=1-0) and 13CO(J=1-0)
results and compared with LVG calculations. We found that the ratio of
12CO(J=3-2) to 12CO(J=1-0) emission is sensitive to and is well correlated with
the local Halpha flux. We interpret that differences of clump propeties
represent an evolutionary sequence of GMCs in terms of density increase leading
to star formation.Type I and II GMCs (starless GMCs and GMCs with HII regions
only, respectively) are at the young phase of star formation where density does
not yet become high enough to show active star formation and Type III GMCs
(GMCs with HII regions and young star clusters) represents the later phase
where the average density is increased and the GMCs are forming massive stars.
The high kinetic temperature correlated with \Halpha flux suggests that FUV

Curative two-stage resection for synchronous triple cancers of the esophagus, colon, and liver: Report of a case
AbstractIntroductionCases of synchronous triple cancers of the esophagus and other organs curatively resected are rare.Presentation of caseA 73-year-old man was admitted to our hospital with bloody feces. He was diagnosed with synchronous triple cancers of the esophagus, colon, and liver. We selected a two-stage operation to safely achieve curative resection for all three cancers. The first stage of the operation comprised a laparoscopy-assisted sigmoidectomy and partial liver resection via open surgery. The patient was discharged without complications. Thirty days later, he was readmitted and thoracoscopic esophagectomy was performed. Although pneumonia-induced pulmonary aspiration occurred as a postoperative complication, it was treated conservatively. The patient was discharged on postoperative day 24.DiscussionEsophagectomy is a highly invasive procedure; thus, simultaneous surgery for plural organs, including the esophagus, may induce life-threatening, severe complications. Two-stage surgery is useful in reducing surgical stress in high-risk patients. For synchronous multiple cancers, the planning of two-stage surgery should be considered for each cancer to maintain organ function and reduce the stress and difficulty of each stage.ConclusionWe successfully treated synchronous triple cancers, including esophageal cancer, by a two-stage operation
Real-time In Situ Electron Spin Resonance Measurements on Fungal Spores of Penicillium digitatum during Exposure of Oxygen Plasmas
We report the kinetic analysis of free radicals on fungal spores of
Penicillium digitatum interacted with atomic oxygen generated plasma electric
discharge using real time in situ electron spin resonance (ESR) measurements.
We have obtained information that the ESR signal from the spores was observed
and preliminarily assignable to semiquinone radical with a g-value of around
2.004 and a line width of approximately 5G. The decay of the signal is possibly
linked to the inactivation of the fungal spore. The real-time in situ ESR has
proven to be a useful method to elucidate plasma-induced surface reactions on

Direct cell–cell contact between mature osteoblasts and osteoclasts dynamically controls their functions in vivo
Bone homeostasis is regulated by communication between bone-forming mature osteoblasts (mOBs) and bone-resorptive mature osteoclasts (mOCs). However, the spatial–temporal relationship and mode of interaction in vivo remain elusive. Here we show, by using an intravital imaging technique, that mOB and mOC functions are regulated via direct cell–cell contact between these cell types. The mOBs and mOCs mainly occupy discrete territories in the steady state, although direct cell–cell contact is detected in spatiotemporally limited areas. In addition, a pH-sensing fluorescence probe reveals that mOCs secrete protons for bone resorption when they are not in contact with mOBs, whereas mOCs contacting mOBs are non-resorptive, suggesting that mOBs can inhibit bone resorption by direct contact. Intermittent administration of parathyroid hormone causes bone anabolic effects, which lead to a mixed distribution of mOBs and mOCs, and increase cell–cell contact. Age of Izu-Bonin-Mariana arc basement
Documenting the early tectonic and magmatic evolution of the Izu–Bonin–Mariana (IBM) arc system in the Western Pacific is critical for understanding the process and cause of subduction initiation along the current convergent margin between the Pacific and Philippine Sea plates. Forearc igneous sections provide firm evidence for seafloor spreading at the time of subduction initiation (52 Ma) and production of “forearc basalt”. Ocean floor drilling (International Ocean Discovery Program Expedition 351) recovered basement-forming, low-Ti tholeiitic basalt crust formed shortly after subduction initiation but distal from the convergent margin (nominally reararc) of the future IBM arc (Amami Sankaku Basin: ASB). Radiometric dating of this basement gives an age range (49.3–46.8 Ma with a weighted average of 48.7 Ma) that overlaps that of basalt in the present-day IBM forearc, but up to 3.3 m.y. younger than the onset of forearc basalt activity. Similarity in age range and geochemical character between the reararc and forearc basalts implies that the ocean crust newly formed by seafloor spreading during subduction initiation extends from fore- to reararc of the present-day IBM arc. Given the age difference between the oldest forearc basalt and the ASB crust, asymmetric spreading caused by ridge migration might have taken place. This scenario for the formation of the ASB implies that the Mesozoic remnant arc terrane of the Daito Ridges comprised the overriding plate at subduction initiation. The juxtaposition of a relatively buoyant remnant arc terrane adjacent to an oceanic plate was more favourable for subduction initiation than would have been the case if both downgoing and overriding plates had been oceanic
Case Report: Tuberous sclerosis complex-associated hemihypertrophy successfully treated with mTOR inhibitor sirolimus
Tuberous sclerosis complex (TSC) is an autosomal dominant disorder caused by a mutation in either of the two tumor suppressor genes, TSC1 and TSC2. Due to dysregulated activity of the mammalian target of rapamycin (mTOR) pathway, hamartomas or benign tumors frequently occur in many organs and are often treated with mTOR inhibitors. Hemihypertrophy is a rare complication of TSC. Although not being a tumor, progressive overgrowth of the affected limb may cause cosmetic and functional problems, for which the efficacy of mTOR inhibitors has not been reported previously. We herein report a case of TSC-associated hemihypertrophy. In this case, genetic studies revealed TSC1 loss of heterozygosity as the cause of hemihypertrophy. Clinically, pharmacological treatment with an mTOR inhibitor sirolimus successfully ameliorated cosmetic and functional problems with no intolerable adverse effects
