The image and narrative allow a significant accompaniment for both the victim and the psychologist, achieving a meaningful interpretation of the violent events and turning them into stories that symbolize the strength and resilience of those who lived through it. In this way, the exercise of Photo Voice generates an impact, which even after an evolution of the environment or event manages to visualize and tell a story that transcends from pain to hope, such as the narration of Ana Higinio, who provides a service to the community. Promoting integral well-being. Analyzing the story, it is possible to understand a life and emotions that were reflected during and after the violent event and that over time have formed a brave woman who contributes positively to mental health; in this way and to know a story and to be able to support the victims' suffering, the question should be used as a tool and thus connect with the experiences of the subject and the feelings that it conserves, for this a series of questions were used from the narrative approach they are: strategic, reflective and circular, which also allows us as psychologists to help in the recovery and restoration process of the victims' life project. At the same time, there is a case study that reflects from different psychosocial parameters where the impacts generated by violence are understood, and a series of actions and support strategies that contribute to accompaniment and comprehensive care for individuals and families in a situation of displacement

BACKGROUND/PURPOSE: Coronavirus disease 2019 (COVID-19) is associated with increased risk of acute ischemic stroke (AIS), however, there is a paucity of data regarding outcomes after administration of intravenous tissue plasminogen activator (IV tPA) for stroke in patients with COVID-19. METHODS: We present a multicenter case series from 9 centers in the United States of patients with acute neurological deficits consistent with AIS and COVID-19 who were treated with IV tPA. RESULTS: We identified 13 patients (mean age 62 (±9.8) years, 9 (69.2%) male). All received IV tPA and 3 cases also underwent mechanical thrombectomy. All patients had systemic symptoms consistent with COVID-19 at the time of admission: fever (5 patients), cough (7 patients), and dyspnea (8 patients). The median admission NIH stroke scale (NIHSS) score was 14.5 (range 3-26) and most patients (61.5%) improved at follow up (median NIHSS score 7.5, range 0-25). No systemic or symptomatic intracranial hemorrhages were seen. Stroke mechanisms included cardioembolic (3 patients), large artery atherosclerosis (2 patients), small vessel disease (1 patient), embolic stroke of undetermined source (3 patients), and cryptogenic with incomplete investigation (1 patient). Three patients were determined to have transient ischemic attacks or aborted strokes. Two out of 12 (16.6%) patients had elevated fibrinogen levels on admission (mean 262.2 ± 87.5 mg/dl), and 7 out of 11 (63.6%) patients had an elevated D-dimer level (mean 4284.6 ±3368.9 ng/ml). CONCLUSIONS: IV tPA may be safe and efficacious in COVID-19, but larger studies are needed to validate these results

Stroke due to large vessel atherosclerosis

Extracranial (EC) and intracranial (IC) large vessel atherosclerosis account for about 20% of ischemic stroke cases. In recent years, new treatments have emerged for treatment of both EC and IC disease. The stroke rate in patients with carotid stenosis is decreasing with modern medical therapy. For patients with asymptomatic stenosis, the stroke rate is likely <1% per year. Some subsets of patients with symptomatic carotid disease benefit less from revascularization, and medical management can be considered in these patients. A second clinical trial has confirmed that aggressive medical management is the treatment of choice for IC atherosclerotic disease. Vessel wall imaging may be useful to define pathophysiology in patients with IC stenosis and could ultimately help tailor therapy, but further studies are needed. Medical therapy is preferred to stenting for patients with vertebral artery-origin stenosis. Recent data and emerging concepts regarding large vessel atherosclerosis are provided

The Interplay between Obstructive Sleep Apnea and Atrial Fibrillation

    No full text
Atrial fibrillation (AF) is the most common cardiac arrhythmia. Obstructive sleep apnea (OSA) is also an increasingly common condition. Both entities are risk factors for ischemic stroke and both conditions are linked with increased mortality. Mechanical effects of obesity and sleep apnea can lead to increased afterload, left ventricular hypertrophy, and left atrial fibrosis and remodeling. These changes can result in an increased risk of AF development. The current paper summarizes the evidence for the bidirectional relationship between AF and OSA. The merits of selective screening for these two conditions are also discussed

Carotid stenosis in women: time for a reappraisal

The treatment approach for patients with internal carotid artery stenosis is receiving increased scrutiny. Major advances in optimal medical therapy have been associated with a declining stroke rate for symptomatic and asymptomatic patients with carotid stenosis. Customising treatment according to gender is worthy of consideration, since earlier clinical trials showed reduced benefit with carotid endarterectomy in women compared to men. In this review, clinical trial results in women are summarised, studies pertaining to carotid plaque imaging in men and women are discussed and new clinical trials are identified. Finally, the rationale for a women's carotid trial is provided

Reducing sugars production from cellulosic wastes by subcritical water hydrolysis in a continuous lab scale unit

Second generation biofuels are those produced from lignocellulosic biomass or crops that do not compete with food production. Cellulose and hemicellulose in plants can be transformed to building blocks, pentoses and hexoses, by hydrolysis. However, most processes require a physic-chemical pretreatment to remove lignin, which acts as a barrier for chemicals during hydrolysis. In fact, pretreatment for removal of lignin or lignin byproducts formed during hydrolysis can be considered as limiting steps for subsequent sugar transformation processes. Alternatively, industrial cellulosic residues, in which lignin and most of hemicellulose have been previously removed through chemical treatment, could be used as raw material for the production of reducing sugars without the drawbacks related to pretreatment and purification steps. Such is the case for diaper industry, which generates a polyacrylate-cellulose dust (POCEL), which cannot be recycled to the process. POCEL is comprised by around of 60 wt. % cellulose and 40 wt. % super absorbent polymer sodium polyacrylate. Due to the high content of cellulose in comparison to lignocellulosic biomass, in which the cellulose content usually oscillate between in the range 20 to 40 wt.%, POCEL could be used as a low cost raw material for glucose production, that can be subsequently fermented to produce bioethanol. In this work, experimental results of POCEL suspensions hydrolysis under subcritical water conditions in a continuous laboratory scale unit are presented. POCEL suspensions in water at 1.2 wt. % were prepared by adding a few drops of hydrochloric acid up to pH 2 to prevent sodium polyacrylate jellification with water. The hydrolysis unit in subcritical water at continuous laboratory scale consisted of a high pressure pump, preheater, a tubular reactor immersed in an electrical oven, heat exchanger and needle valve to regulate system pressure. Experiments were carried out in the temperature range 350 to 420 °C, pressures of 100 to 136 atm and suspension flowrates of up to 6.4 mL/min. Reactor effluent samples were characterized by means of dinitrosalicylic acid (DNS) colorimetric method. The highest concentration of reducing sugars was obtained at 390 °C and a water flowrate of 1.9 mL/min, which corresponds to a residence time of 126.7 s in the reactor. The estimated productivity of sugar production based on a cellulose content of 60 wt.% in POCEL was 85.4%, which indicates this kind of cellulosic waste could be used to obtain high reducing sugars concentration in hydrolysates through a continuous subcritical water process
