Procesos implicados en el desarrollo de materiales didácticos reutilizables para el fomento de la cultura científica y tecnológica
Dentro de la iniciativa de Fomento de la Cultura Científica y Tecnológica, la Universidad
de Oviedo ha promovido el desarrollo de Materiales Didácticos Multimedia (MDM) en el
Área de las Ciencias. Esta acción la ha llevado a cabo un equipo multidisciplinar con un
trabajo cooperativo que ha permitido elaborar los diseños de instrucción, diseños gráficos y desarrollo informático de estos materiales. La experiencia previa de nuestro equipo en el desarrollo de MDM nos ha llevado a buscar un modelo de diseño de contenidos que rentabilice el elevado coste de su elaboración y permita una mayor flexibilidad en su uso. Las tendencias actuales en e-learning proponen una solución idónea a esta necesidad en los conceptos de reutilización y repositorio de objetos de aprendizaje. Nuestro equipo ha incorporado estas tendencias en los materiales que elabora con el fin de ofrecer a la comunidad educativa la posibilidad de utilizarlos en distintos contextos
Influence of stress and depression on the immune system in patients evaluated in an anti-aging unit
Background: There is compelling evidence pointing out that stress and depression
produce a dramatic impact on human well-being mainly through impairing the regular
function of the immune system and producing a low-chronic inflammation status that
favors the occurrence of infections, metabolic diseases, and even cancer. The present
work aims to evaluate the stress/depression levels of some patients treated in an
antiaging unit and detect any potential relationship with their immune system status
prior of the implementation of a physical/psychological program designed to prevent
health deterioration.
Methods: We evaluated 48 patients (16 men and 32 women with a mean age of
55.11 ± 10.71 years) from middle-upper class from psychological and immunological
points of view. In particular, we analyzed neutrophil chemotaxis and phagocytosis;
lymphocyte chemotaxis and proliferation, and natural killer (NK) cell activity.
Results: Women showed more depressive symptoms than men. Chemotaxis levels of
lymphocytes and neutrophils in women showed a significant reduction compared with
those in men. We also found a strong negative correlation between depression and NK
cell function. This correlation was also significant independently of gender.
Conclusion: We conclude that NK activity is affected at least by depression state, and
we propose that a combined treatment consisting of cognitive behavioral therapy and
physical activity programs might improve patient health deterioration

Como punto de partida se tomarán las competencias definidas en diversas iniciativas Europeas y los perfiles profesionales específicos al ámbito de las TICs definidos por el consorcio Career-Space. Acció dels andrògens en el testicle: un paper per a la meiosi
La funció que duen a terme els andrògens en l'espermatogènesi és, encara en certa mesura,
enigmàtica: mentre que llur implicació és absolutament vital en la iniciació i en el manteniment
del procés espermatogènic normal, la seva funció específica encara no està definida de
manera precisa. Els andrògens, com les altres hormones esteroïdals, actuen a través del seu
corresponent receptor anomenat receptor d'andrògens (AR). Fins avui, no hi ha gaire evidència
que recolzi l'existència de diverses isoformes de l'AR com en el cas del sistema estrògensreceptor
d'estrògens. Per tant, la pregunta de com els andrògens duen a terme la seva acció
en l'espermatogènesi s'ha d'abordar definint dos processos: en primer lloc, s'han d'identifi-
car amb total certesa els tipus cell. ulars testiculars capaços de respondre directament a l'estimulació
androgènica. De manera específica, la qüestió per resoldre és quins són els tipus cellulars
que expressen l'AR en el testicle. En segon lloc, sabent també que el complex del lligand
unit a l'AR actua com a factor de transcripció, caldrà determinar quins són els gens que
estaran activats o reprimits en les cèll. ules que tenen AR en resposta a l'estimulació androgènica.
Fins que aquestes dues preguntes no estiguin contestades amb tota certesa, el mecanisme
pel qual els andrògens regulen l'espermatogènesi serà, en el millor dels casos, especulatiu.
En aquesta revisió presentem evidència que els andrògens actuen únicament a les cèll. ules
somàtiques del testicle, com són les cèll. ules de Sertoli, les de Leydig, les mioides peritubulars
i les cèll. ules del múscul llis que envolten els vasos sanguinis. A més a més, també discutim
la possibilitat que els andrògens siguin indispensables per a l'inici de la meiosi, encara que continua essent desconegut el mecanisme pel qual els andrògens actuen en aquest procés.The role that androgens play in spermatogenesis still remains enigmatic: whereas their involvement
is absolutely vital to the initiation and maintenance of the normal spermatogenic
process, their specific role is yet to be defined. Androgens, like other steroid hormones, act via
their corresponding receptor termed the androgen receptor (AR). To date, there is little evidence
to support the notion that there are multiple forms of AR as is the case for the estrogen-estrogen
receptor system. Thus, the question of how androgens manifest their action on spermatogenesis
becomes one of defining two processes: First, the cell types within the testis that are capable of
responding directly to androgen stimulation must be identified with absolute certainty. Specifically,
this question can be stated as what cell types in the testis express AR. Second, given that
the ligand-bound AR serves as a transcription factor, the question then becomes what are the
genes turned on or off in AR positive cells in response to androgen stimulation? Until these two
questions are unequivocally answered, the mechanism of how androgens regulate spermatogenesis
will remain speculative at best. In this review we present evidence that androgens act
solely at the level of the somatic cells of the testis, including Sertoli cells, Leydig cells, peritubular
myoid cells and smooth muscle cells surrounding blood vessels. In addition, we discuss
the likely possibility that androgens are indispensable for the onset of meiosis, albeit how they
accomplish this remains a mystery
Evaluation of Natural Language Processing for the Identification of Crohn Disease-Related Variables in Spanish Electronic Health Records:A Validation Study for the PREMONITION-CD Project
Background: The exploration of clinically relevant information in the free text of electronic health records (EHRs) holds the potential to positively impact clinical practice as well as knowledge regarding Crohn disease (CD), an inflammatory bowel disease that may affect any segment of the gastrointestinal tract. The EHRead technology, a clinical natural language processing (cNLP) system, was designed to detect and extract clinical information from narratives in the clinical notes contained in EHRs. Objective: The aim of this study is to validate the performance of the EHRead technology in identifying information of patients with CD. Methods: We used the EHRead technology to explore and extract CD-related clinical information from EHRs. To validate this tool, we compared the output of the EHRead technology with a manually curated gold standard to assess the quality of our cNLP system in detecting records containing any reference to CD and its related variables. Results: The validation metrics for the main variable (CD) were a precision of 0.88, a recall of 0.98, and an F1 score of 0.93. Regarding the secondary variables, we obtained a precision of 0.91, a recall of 0.71, and an F1 score of 0.80 for CD flare, while for the variable vedolizumab (treatment), a precision, recall, and F1 score of 0.86, 0.94, and 0.90 were obtained, respectively. Conclusions: This evaluation demonstrates the ability of the EHRead technology to identify patients with CD and their related variables from the free text of EHRs. To the best of our knowledge, this study is the first to use a cNLP system for the identification of CD in EHRs written in Spanish. © 2022 JMIR Medical Informatics. All rights reserved

Introducción y objetivos
Se presentan las características clínicas y los resultados de los trasplantes cardiacos realizados en España con la actualización correspondiente a 2019.
Métodos
Se describen las características clínicas y los resultados de los trasplantes cardiacos realizados en 2019, así como las tendencias de estos en el periodo 2010-2018.
Resultados
En 2019 se realizaron 300 trasplantes (8.794 desde 1984; 2.745 entre 2010 y 2019). Respecto a años previos, los cambios más llamativos son el descenso hasta el 38% de los trasplantes realizados en código urgente, y la consolidación en el cambio de asistencia circulatoria pretrasplante, con la práctica desaparición del balón de contrapulsación (0, 7%), la estabilización del uso del oxigenador extracorpóreo de membrana (9, 6%) y el aumento de los dispositivos de asistencia ventricular (29%). La supervivencia en el trienio 2016-2018 es similar a la del trienio 2013-2015 (p = 0, 34), y ambas mejores que la del trienio 2010-2012 (p = 0, 002 y p = 0, 01 respectivamente).
Conclusiones
Se mantienen estables tanto la actividad del trasplante cardiaco en España como los resultados en supervivencia en los últimos 2 trienios. Hay una tendencia a realizar menos trasplantes urgentes, la mayoría con dispositivos de asistencia ventricular.
Introduction and objectives: The present report describes the clinical characteristics and outcomes of heart transplants in Spain and updates the data to 2019.
Methods: We describe the clinical characteristics and outcomes of heart transplants performed in Spain in 2019, as well as trends in this procedure from 2010 to 2018.
Results: In 2019, 300 transplants were performed (8794 since 1984; 2745 between 2010 and 2019). Compared with previous years, the most notable findings were the decreasing rate of urgent transplants (38%), and the consolidation of the type of circulatory support prior to transplant, with an almost complete disappearance of counterpulsation balloon (0.7%), stabilization in the use of extracorporeal membrane oxygenation (9.6%), and an increase in the use of ventricular assist devices (29.0%). Survival from 2016 to 2018 was similar to that from 2013 to 2015 (P = .34). Survival in both these periods was better than that from 2010 to 2012 (P = .002 and P = .01, respectively).
Conclusions: Heart transplant activity has remained stable during the last few years, as have outcomes (in terms of survival). There has been a trend to a lower rate of urgent transplants and to a higher use of ventricular assist devices prior to transplant
