    Introduction: There are several causes that lead to malnutrition in acquired immunodeficiency syndrome. However, cellular immunosuppression with a decrease in CD4 + T lymphocytes represents the main conditioning factor.Objective: to relate antiretroviral changes on body weight and CD4 + T lymphocyte count in HIV / AIDS patients from the province of Benguela, Angola.Method: a case-control study was conducted in 190 adults with HIV / AIDS treated at Benguela Provincial External Bank Hospital, with three months or more of patients treated with antiretroviral (women: 70.5%, mean age: 36 , 1 year,> 35 years with HIV + diagnosis: 46.8%, 3-18 months with antiretroviral: 48.9%). The absolute CD4 + T cell count and weight were measured before starting the therapy (initial value) and at the last visit (final value). The relative changes or gradients (final value - initial value / initial value) of both variables were calculated. A comparison of means for related and independent samples was performed by non-parametric tests. The association between weight gradients and CD4 counts was estimated using the Odds-Ratio and its confidence interval; 95% of confidence was considered for all statistical tests.Results: the mean of weight and absolute CD4 + T cell counts increased significantly after the retroviral therapy (p 35 años con diagnóstico VIH+: 46,8%; 3-18 meses con antirretrovirales: 48,9%). Se midió el peso, y el conteo absoluto de las células T CD4+ antes de iniciar la terapia (valor inicial) y en la última consulta (valor final). Se calcularon los cambios relativos o gradientes (valor final - valor inicial/valor inicial) de ambas variables. Se realizó comparación de medias para muestras relacionadas e independientes por pruebas no paramétricas. La asociación entre los gradientes del peso y conteo CD4 se estimó mediante el Odds-Ratio (OR) y su intervalo de confianza. Se consideró el 95% de confianza para todas las pruebas estadísticas.Resultados: las medianas del peso y conteo absoluto de las células T CD4+ aumentaron significativamente después de la terapia (p< 0,001). Se encontró asociación significativa entre el gradiente del peso y el gradiente de los CD4 (OR=3,61; IC (95%): 1,61-8,11).Conclusión: los cambios relativos del peso corporal en los sujetos con VIH/sida a consecuencia de la terapia antirretroviral pudieran ser utilizados en el seguimiento y monitoreo de la respuesta inmunológica a los antirretrovirales en países de bajos recursos económicos

    (1) Aims: To assess the incidence of inflammatory bowel disease (IBD) in Spain, to describe the main epidemiological and clinical characteristics at diagnosis and the evolution of the disease, and to explore the use of drug treatments. (2) Methods: Prospective, population-based nationwide registry. Adult patients diagnosed with IBD-Crohn's disease (CD), ulcerative colitis (UC) or IBD unclassified (IBD-U)-during 2017 in Spain were included and were followed-up for 1 year. (3) Results: We identified 3611 incident cases of IBD diagnosed during 2017 in 108 hospitals covering over 22 million inhabitants. The overall incidence (cases/100,000 person-years) was 16 for IBD, 7.5 for CD, 8 for UC, and 0.5 for IBD-U; 53% of patients were male and median age was 43 years (interquartile range = 31-56 years). During a median 12-month follow-up, 34% of patients were treated with systemic steroids, 25% with immunomodulators, 15% with biologics and 5.6% underwent surgery. The percentage of patients under these treatments was significantly higher in CD than UC and IBD-U. Use of systemic steroids and biologics was significantly higher in hospitals with high resources. In total, 28% of patients were hospitalized (35% CD and 22% UC patients, p < 0.01). (4) Conclusion: The incidence of IBD in Spain is rather high and similar to that reported in Northern Europe. IBD patients require substantial therapeutic resources, which are greater in CD and in hospitals with high resources, and much higher than previously reported. One third of patients are hospitalized in the first year after diagnosis and a relevant proportion undergo surgery

    IL-6 serum levels predict severity and response to tocilizumab in COVID-19: An observational study

    Background: Patients with coronavirus disaese 2019 (COVID-19) can develop a cytokine release syndrome that eventually leads to acute respiratory distress syndrome requiring invasive mechanical ventilation (IMV). Because IL-6 is a relevant cytokine in acute respiratory distress syndrome, the blockade of its receptor with tocilizumab (TCZ) could reduce mortality and/or morbidity in severe COVID-19. Objective: We sought to determine whether baseline IL-6 serum levels can predict the need for IMV and the response to TCZ. Methods: A retrospective observational study was performed in hospitalized patients diagnosed with COVID-19. Clinical information and laboratory findings, including IL-6 levels, were collected approximately 3 and 9 days after admission to be matched with preadministration and postadministration of TCZ. Multivariable logistic and linear regressions and survival analysis were performed depending on outcomes: need for IMV, evolution of arterial oxygen tension/fraction of inspired oxygen ratio, or mortality. Results: One hundred forty-six patients were studied, predominantly males (66%); median age was 63 years. Forty-four patients (30%) required IMV, and 58 patients (40%) received treatment with TCZ. IL-6 levels greater than 30 pg/mL was the best predictor for IMV (odds ratio, 7.1; P < .001). Early administration of TCZ was associated with improvement in oxygenation (arterial oxygen tension/fraction of inspired oxygen ratio) in patients with high IL-6 (P = .048). Patients with high IL-6 not treated with TCZ showed high mortality (hazard ratio, 4.6; P = .003), as well as those with low IL-6 treated with TCZ (hazard ratio, 3.6; P = .016). No relevant serious adverse events were observed in TCZ-treated patients. Conclusions: Baseline IL-6 greater than 30 pg/mL predicts IMV requirement in patients with COVID-19 and contributes to establish an adequate indication for TCZ administrationThis study was funded by Spanish Ministry of Economy, Industry and Competitiveness (MINECO) and Instituto de Salud Carlos III (grant nos.     Higher COVID-19 pneumonia risk associated with anti-IFN-α than with anti-IFN-ω auto-Abs in children

    We found that 19 (10.4%) of 183 unvaccinated children hospitalized for COVID-19 pneumonia had autoantibodies (auto-Abs) neutralizing type I IFNs (IFN-alpha 2 in 10 patients: IFN-alpha 2 only in three, IFN-alpha 2 plus IFN-omega in five, and IFN-alpha 2, IFN-omega plus IFN-beta in two; IFN-omega only in nine patients). Seven children (3.8%) had Abs neutralizing at least 10 ng/ml of one IFN, whereas the other 12 (6.6%) had Abs neutralizing only 100 pg/ml. The auto-Abs neutralized both unglycosylated and glycosylated IFNs. We also detected auto-Abs neutralizing 100 pg/ml IFN-alpha 2 in 4 of 2,267 uninfected children (0.2%) and auto-Abs neutralizing IFN-omega in 45 children (2%). The odds ratios (ORs) for life-threatening COVID-19 pneumonia were, therefore, higher for auto-Abs neutralizing IFN-alpha 2 only (OR [95% CI] = 67.6 [5.7-9,196.6]) than for auto-Abs neutralizing IFN-. only (OR [95% CI] = 2.6 [1.2-5.3]). ORs were also higher for auto-Abs neutralizing high concentrations (OR [95% CI] = 12.9 [4.6-35.9]) than for those neutralizing low concentrations (OR [95% CI] = 5.5 [3.1-9.6]) of IFN-omega and/or IFN-alpha 2
