Deformidades cong茅nitas de la columna vertebral: clasificaci贸n y enfoque terap茅utico
Las deformidades eong茅nitas de la columna vertebral representan uno de los
problemas m谩s interesantes y en ocasiones controvertido de la cirug铆a ortop茅dica. Los autores
presentan una revisi贸n de los conceptos actuales de este tipo de deformidades, as铆 como de
su clasificaci贸n. Realizan una revisi贸n de la literatura, y efect煤an un an谩lisis de las alternativas
terap茅uticas en cada uno de los diferentes tipos de deformidades cong茅nitasCongenital deformities of the spine are one of the most interesting and controversial
problems of the orthopaedic surgery. The authors bring up to date current concepts
in these deformities as well as their classification. They carried out a review and analysis of

Tratamiento quir煤rgico de las hipecifosis postraum谩ticas
Se presentan 11 casos de hipercifosis tras factura vertebral tor谩cica o lumbar intervenidos entre los dos meses y 2 a帽os tras el traumatismo. La edad media fue de 45.5 a帽os con predominio del sexo masculino. La etiolog铆a predominante fue la ca铆da casual. La localizaci贸n m谩s frecuente fue T12 existiendo 3 casos de localizaci贸n m煤ltiple. La media de deformidad previa fue de 32.6潞. En 10 ocasiones se utiliz贸 doble v铆a, anterior con corporectom铆a completa, flexibilizaci贸n y apuntalado anterior, seguida de instrumentaci贸n posterior y realineaci贸n mediante modelaje in situ. En un caso se efectu贸 decancelaci贸n e instrumentaci贸n por v铆a posterior debido al mal estado general que presentaba. El tiempo medio de seguimiento fue de 25 meses. Se consigui贸 una media de correcci贸n de 18.4潞 persistiendo una deformidad media de 14.2潞. Hasta la fecha, 9 de 11 casos, no han presentado dolor persistente ni cl铆nica neurol贸gica acompa帽ante as铆 como fracaso en la instrumentaci贸n. Una caso con fractura m煤ltiple present贸 nuevamente dolor por fracaso de los segmentos inferiores a la artrodesis siendo necesario nueva intervenci贸n. En el caso con cl铆nica neurol贸gica tras la intervenci贸n, el paciente lleva una vida independiente y deambula con andador.We report 11 cases of kyphosis after vertebral fracture thoracic and thoracolumbar with surgical treatment between two months and two years after traumatic. The average age was of 45.5 years with predominance of masculine sex. The most frequence etiology was the accidental fall. The location but frequents was T12 existing 3 cases of multiple location. The average of previous deformity was of 32.6潞. In 10 occasions it was used anterior and posterior approach, these included anterior corpectomy, correction of deformity and a strut graft or cage devices with graft followed posterior internal fixation. In a case one took place decancelation and posterior internal fixation by later route due to badly been general that presented. The length of follow-up averaged 25 months. An average of correction of 18.4潞 was obtained persisting an average deformity of 14.2潞. To date, 9 of the 11 cases, have not presented persistent pain nor clinical neurological companion as well as failure in the instrumentation. A case with multiple fracture presented pain by failure of the inferior segments to the artrodesis being necessary new intervention. In the case with neurological clinic after the intervention, the patient takes an independent life and rambles with walking
Fibrocart铆lago meniscal como biomaterial de sustituci贸n en los defectos osteocondrales: injertos heter贸logos, en fresco y conservados: metodolog铆a de conservaci贸n de injertos meniscales
Los autores realizan un estudio experimental en conejos, con el objeto de
valorar las posibilidades reales de utilizaci贸n de fibrocart铆lago meniscal en la reparaci贸n de
defectos osteocondrales en zona de carga, empleando injertos heter贸logos tanto frescos como
congelados procedentes de perros. Han sido estudiados histol贸gicamente 76 animales con periodos
de evoluci贸n comprendidos entre un a semana y 18 meses, compar谩ndose los resultados
obtenidos.
Los injertos heter贸logos en fresco (44 animales), cuando no son rechazados, parecen ser
capaces de restaurar un a superficie articular funcional, no modific谩ndose la estructura fibrilar y manteniendo su celularidad, consiguiendo un anclaje perfecto al lecho 贸seo semejante al
obtenido con los injertos aut贸logos y hom贸logos. No modifica el cart铆lago articular vecino. El
铆ndice de rechazos es elevado, (18 %).
Los injertos heter贸logos han sido conservados mediante congelaci贸n, aplicando t茅cnicas utilizadas par a el cart铆lago articular. Se han estudiado 32 animales. Con dicha t茅cnica, pierden
toda su poblaci贸n celular, presentan un alto 铆ndice de rechazos y no consiguen un verdadero anclaje
al lecho 贸seo, Por otra parte parece comportarse como un inductor de un a buen a repara -

Osteoporosis Idiop谩tica Juvenil: (a prop贸sito de un caso)
Presentamos un caso de osteporosis idiop谩tica juvenil. Se trata de una
ni帽a de ll a帽os que debuta con un aplastamiento de T4 tras un traumatismo
m铆nimo y que posteriormente, con el cors茅 de Milwaukee puesto, desarrolla
m煤ltiples aplastamientos vertebrales. Efectuan una revisi贸n de la literatura
sobre la etiopatogenia, diagn贸stico diferencial y pron贸stico de esta rar a
forma de osteporosis.The authors showed a case of idiopathic juvenil e Osteoporosis,
a girl 11 year s old than started with a wedge fracture of T4 after a
minimal trauma. Although she wor e a Milwauke e brace, develope s
multiples lewe l vertebral wedges. They made a review of the literature
about the etiopathogenia differential diagnosis and prognosis
of this rare form of osteoporosis
Descompresi贸n transpedicular en las fracturas vertebrales por estallido
Se presentan 15 casos de fracturas vertebrales por estallido, a nivel toracolumbar
y lumbar, tratadas mediante descompresi贸n transpedicular y estabilizaci贸n mediante instrumentaci贸n
CD. Con un periodo m铆nimo de 1 a帽o de seguimiento, se analiza la calidad de la
descompresi贸n del canal desde el punto de vista neurol贸gico y la evoluci贸n de la reducci贸n obtenida.
Trece pacientes presentaban afectaci贸n neurol贸gica incompleta, valorada seg煤n la escala
de Frankel modificada por Bradford. Los casos se distribuyeron de la siguiente forma: T8
(1) T12 (1), L1 (5), L2 (3), L3 (2), L4 (1) y L5 (2). La ocupaci贸n del canal, la reducci贸n obtenida
y su evoluci贸n se han evaluado mediante TAC pre y postoperatoria, tomando como referencia
el nivel superior sano. Se valoran tambi茅n los par谩metros radiogr谩ficos: 铆ndice sagital pre y postoperatorio,
porcentaje de altura perdido del soma y la reconstrucci贸n conseguida. Salvo en 1
caso, todos los pacientes mejoraron al menos 1 grado en la escala de Frankel modificada. La
ocupaci贸n media preoperatoria del canal fue de 56% (25-95%). La ocupaci贸n postoperatoria
media fue del 2 2% (0-70). La evoluci贸n al a帽o mostr贸 p茅rdidas m铆nimas en dos casos, con una
media del 24% de ocupaci贸n final. La v铆a traspedicular ofrece unos resultados efectivos desde
el punto de vista neurol贸gico. Permite reducir el muro posterior sin p茅rdida de masa 贸sea para
la fusi贸n, y realizar una buena reconstrucci贸n del cuerpo vertebral. La instrumentaci贸n CD proporciona
estabilidad a esta t茅cnica con p茅rdidas m铆nimas de la reducci贸n obtenida, tanto del
muro posterior como del 铆ndice sagital.We present 15 cases of thoracolumbar and lumbar burst fractures treated by decompression
and stabilization with CD instrumentation. With a minimum of 1 year follow-up, the
quality and the efficacy of the spinal canal decompression and the loss of reduction were evaluated.
Thirteen patients presented incomplete neurological damage according to Frankel's score (modified
by Bradford). The cases were distributed as follows: T8 (1), T12 (1), L1(5), L2 (3), L3 (2), L4
(1) and L5 (2). The canal occupation and the reduction obtained were assessed pre- and
postoperative by CT scan. The healthy level above the fracture was used as reference. The saggital
index (pre- and postop) and the height of the vertebral body were also evaluated by radiography.
Except in one case, all the patients improved at least 1 grade (modified Frankel scale). The mean
canal occupation was 56% (25-95%). The mean postoperative occupation was 22% (range 0-70);
After 1 year follow up, these way minimum loss, (in 2 cases) with a mean 24% of final occupation.
Transpedicular approach provides an efficient decompression from the neurological point
of view, allowing reduction of the posterior wall without loss of the posterolateral structures for
the fusion, as well as a sufficient reconstruction of the vertebral body. CD instrumentation permits
enought stability with this technique, showing minimum loss of the reduction in the posterior
wall and in the saggital index
Tratamiento quir煤rgico de las fracturas vertebrales mediante instrumentaci贸n segmentaria Cotrel-Dubousset
Los autores presentan su experiencia personal en el tratamiento quir煤rgico de
las fracturas vertebrales tor谩cicas y lumbares. Han sido incluidos en el estudio 27 pacientes
operados entre 1988 y 1991, todos ellos estabilizados mediante la instrumentaci贸n Cotrel Dubousset.
Excepto tres pacientes, todos fueron intervenidos por v铆a posterior. Se realiza un estudio
de los resultados obtenidos desde el punto de vista radiol贸gico (铆ndice sagital, porcentaje
de ocupaci贸n de canal), funcional y neurol贸gico. La fractura estallido fue la m谩s frecuente (52%
de los casos). En 17 pacientes (63%) se objetiv贸 lesi贸n neurol贸gica, 3 de los cuales (11%) mostraban
paraplejia completa, no obteniendo mejor铆a tras la intervenci贸n. Todos los restantes
mejoraron al menos un grado en la escala de Frankel modificada. No se registr贸 pseudoartrosis.
El 铆ndice sagital no mostr贸 p茅rdidas superiores a 7o
. La p茅rdida de altura del cuerpo vertebral
fue inferior al 10%. La correcci贸n del muro posterior no present贸 p茅rdidas durante el
seguimiento. Se concluye, que mediante las instrumentaciones segmentarias por v铆a posterior
se puede obtener en la mayor铆a de las ocasiones una estabilidad mec谩nica y neurol贸gica suficiente
de la columna vertebral, incluyendo un n煤mero m铆nimo de niveles sanos, aunque precisando
de ortesis durante el postoperatorio.The authors showed their experience on the surgical management of thoracic
and lumbar fractures. A total of 27 patients surgically treated from 1988 to 1991 by posterior
approach have been included in the study. Cotrel-Dubousset instrumentation was used in all
cases. Except for three patients, all were operated on by posterior approach. In the burst fractures,
decompression and reduction of the posterior wall was performed by transpedicular approach.
The results obtained from the radiological (sagittal index, canal occupation) as well
as functional and neurological point of view were reviewed. Burst fracture was the must frequent
type (52% of cases). Seventeen patients (63%) showed neurological damage, 3 of them
with (11%) had paraplegia and did not show any recovery after surgery. Those with incomplete
neurological lesion improved at least one or two degrees on the Frankel's score. Neither pseudoarthrosis;
nor loss of more than 7 degrees in the sagittal index were observed. The percentage
of height loss of vertebral body was always less than 10%. When the sagittal index augmented,
it was related with a collapse of discal space. Posterior wall alignement did not suffer loss of
correction during follow-up. The CD segmentary instrumentation seems to be an efficient system
for stabilization of vertebral fractures. CD shows a wide versatility adapting to different
topographic lesions and saving healty levels. However, CD is less efficient with flexion stress
and therefore patients require postoperative bracing in lordosis
Fisiopatolog铆a de los Cuerpos Libres Intraarticulares : estudio Experimental
La cubierta cartilaginosa de los cuerpos libres intraarticulares tiene un origen
no bien conocido. Se ha realizado un estudio experimental en conejos, liberando fragmentos osteocondrales
en la cavidad articular de la rodilla, estudiando sus cambios histol贸gicos desde la
primera semana hasta el a帽o. En todos los casos los fragmentos osteocondrales se observaron
adheridos a la membrana sinovial. Hubo una remodelaci贸n progresiva del componente 贸seo y
cartilaginoso, cubri茅ndose todo el fragmento de una cubierta fibrocartilaginosa, reabsorbi茅ndose
el hueso central y form谩ndose nuevo hueso en la porci贸n perif茅rica debajo del fibrocart铆-
lago. Todos los cambios tisulares dependieron de la actividad de la membrana sinovial. No pudo
demostrarse crecimiento del cart铆lago articular.It is not well known the origin of the cartilaginous outer layer of the
intrarticular loose bodies. An experimental study has been carried out on rabbits
loosening out osteochondral fragments into the articular cavity of the kne e joint and
studying their histological changes between the first week and the first year. The osteochondral
fragments wer e found adhered to the synovial membrane in all cases.
A proggressive remodeling tood place within the osseous and cartilaginous components,
the whole fragment became covered by a fibrocartilaginous covering, the
central bone was reabsorbed and new bone was formed at the peripheral portion
unde r fibrocartilage. All tissue change s wer e dependent on synovial membrane
activity. Growth of articular cartilage could not be demonstrated
Cicatrizaci贸n del fibrocart铆lago meniscal de la Rodilla : estudio Experimental
Para estudiar la cicatrizaci贸n de las lesiones meniscales se ha realizado un estudio
experimental en conejos. Se han practicado tres tipos de lesiones en el menisco, transversal,
longitudinal parameniscal y longitudinal interna alejeda del paramenisco. Posteriormente
el defecto creado era reparado mediante puntos sueltos con material reabsorvible.
Se ha estudiado el fen贸meno de cicatrizaci贸n desde el punto de vista histol贸gico desde la
semana a los tres meses. Los resultados muestran que la cicatrizaci贸n es fundamentalmente
extr铆nseca a partir de la capacidad pl谩stica de la membrana sinovial. Las heridas longitudinales
parameniscales cicatrizaron sin dificultad, las transversales casi en toda su anchura y las
longitudinales en zona avascular cicatrizaron principalmente en su porci贸n anterior gracias a
la invasi贸n de sinovial hiperpl谩sica desde la inserci贸n tibial del ligamento cruzado anterior.To study the healing of meniscal tears, the authors hav e carried out
an experimental work in rabbits. They performed three types of tears in the internal
menisci, radial, peripheral longitudinal (in the parameniscal zone) and longitudinal
within of the parameniscus. The menisci were reparated by suture with
absorbible material.
The healing phenomenon has been histologically studied from the first wee k to
three months after. The results showed that the cicatrization is mainly extrinsic by
the plastic capacity of sinovial membrane. The longitudinal tears in the parameniscus
heal without difficulty, the transversal tears almost in their whol e width and the
longitudinal tears at the avascular zone healed principally in the anterior portion
by the invasion of hyperplasic sinovial from the tibial insertion of the anterior cruciate
ligament
