Alterações posturais em crianças portadoras de síndrome da respiração bucal, avaliadas através do método da biofotogrametria computadorizada
OBJECTIVES: To determine and compare the posture of children with obstructive (OMB) and functional mouth breathing (FMB) and children with nasal breathing (NB) using computerized biophotogrammetry, as well as to determine the viability and efficacy of this method. METHODS: Evaluative and personalized methods were used for the capture and analysis of angle images obtained from 19 NB, 26 FMB and 17 OMB children of either gender aged 8 to 10 years on anterior, posterior and profile views. The nonparametric Kruskal-Wallis test was applied to analyze the three groups as a whole and the Mann-Whitney test was used for pairwise comparison of the groups (p = 0.05). RESULTS: The angle related to the lateral knee condyle, lateral ankle malleolus and 5th metatarsal diaphysis (KAM) was significantly greater in NB than in FMB children. With respect to the angle related to the nasal external acoustic meatus and pogonion (GMM), nasal external acoustic meatus and manubrium (GME), and 2nd cervical vertebra, major cervical curvature point and 7th cervical vertebra (C2C7), significantly higher values were observed for the OMB group compared to NB children. The angle formed by the 2nd thoracic vertebra, the major prominent thoracic point and the 9th thoracic vertebra (T2T9) was significantly greater in FMB than in OMB children. CONCLUSIONS: Computerized biophotogrammetry was found to e a safe and reliable technique, allowing comparison between the children analyzed, as well as the detection of postural alterations in mouth breathing children.OBJETIVOS: Mensurar e comparar a postura de crianças respiradoras bucais obstrutivas (RBO) e funcionais (RBF), entre si e com crianças respiradoras nasais (RN), através da biofotogrametria computadorizada e verificar a viabilidade e a eficácia da utilização deste método. MÉTODOS: Utilizaram-se métodos avaliativos e personalizados de captação e análise das imagens angulares de 19 crianças RN, 26 RBF e 17 RBO, de 8 a 10 anos de idade, de ambos os sexos, em face anterior, perfil e posterior. RESULTADOS: Para análise dos resultados, aplicou-se o teste não-paramétrico de Kruskal-Wallis e de Mann-Whitney (p=0,05). Observou-se que a angulação obtida relativa ao côndilo lateral do joelho, maléolo lateral do tornozelo e diáfise do 5° metatarso (JTM), os resultados dos RN foram significativamente maiores do que os RBF. Quanto às angulações relativas à glabela, meato acústico esterno e mento (GMM), glabela, meato acústico esterno e manúbrio do esterno (GME) e 2ª vértebra cervical, local de maior curvatura cervical e 7ª cervical (C2C7), os resultados dos RBO foram significativamente maiores do que os RN. Na angulação relativa à 2ª vértebra torácica, local de maior proeminência torácica e 9ª torácica (T2T9), os resultados dos RN foram significativamente maiores do que os RBO. Na angulação de C2C7, os resultados dos RBO foram significantemente maiores do que os RBF, e na medida relativa a T2T9, os resultados dos RBF foram significativamente maiores do que os RBO. CONCLUSÃO: A técnica da biofotogrametria computadorizada revelou-se ser segura e fidedigna, permitindo a mensuração e comparação dos valores obtidos entre as crianças analisadas, assim como a verificação da detecção de alterações posturais nos respiradores bucais
Unraveling Brazilian Indian population prostate good health: clinical, anthropometric and genetic features
To compare dietary, lifestyle, clinical, anthropometric, genetic and prostatic features of Brazilian Indians and non-Indians (Amazon). 315 men, 228 Indians and 89 non-Indians, ≥40 years old were submitted to digital rectal examination, serum prostate specific antigen (PSA), testosterone, TP53 and GSTP1 genotyping, anthropometric, lifestyle, dietary, personal and familial medical history. Prostatic symptoms were evaluated with the International Prostate Symptom Score (IPSS). Macuxis and Yanomamis represented 43.6% and 14.5% of Indians respectively who spontaneously referred no prostate symptoms. Mean IPSS was 7, range 3-19, with only 15% of moderate symptoms (score 8-19); Mean age was 54.7 years, waist circumference 86.6 cm, BMI 23.9 kg/m2. Yanomamis presented both lower BMI (21.4 versus 24.8 and 23.3, p=0,001) and prostate volume than Macuxis and “other ethnic groups” (15 versus 20, p=0.001). Testosterone (414 versus 502 and 512, p=0.207) and PSA (0.48 versus 0.6 and 0.41, p=0.349) were similar with progressive PSA increase with aging. Val/Val correlated with lower PSA (p=0.0361). Indians compared to control population presented: - TP53 super representation of Arg/Arg haplotype, 74.5% versus 42.5%, p<0.0001. -GSTP1 Ile/Ile 35.3% versus 60.9%; Ile/Val 45.9% versus 28.7%; Val/Val 18.8% versus 10.3%; p=0.0003. Observed specific dietary, lifestyle, anthropometric and genetic profile for TP53 and GSTP1 may contribute to Brazilian Indian population prostate good health.41234435
Early liver function improvement following successful treatment of chronic hepatitis C in patients with decompensated cirrhosis: a real-life study
OBJECTIVES: Despite higher rates of sustained virologic response (SVR), important concerns remain when patients with decompensated cirrhosis due to hepatitis C virus (HCV) are treated with direct-acting antiviral agents (DAA). Questions include efficacy, safety, and the magnitude of liver function improvement. Here, we aimed to evaluate HCV treatment data in this specific population in Brazil.
METHODS: We included 85 patients with decompensated cirrhosis submitted to HCV therapy with DAA followed at two academic tertiary centers in the southeastern region of Brazil.
RESULTS: Seventy-nine patients (92.9%) were Child-Pugh (CP) score B, and six (7.1%) were CP score C. The mean MELD score was 12.86. The most common treatment was sofosbuvir plus daclatasvir±ribavirin for 24 weeks. The overall intention-to-treat (ITT) SVR rate was 87.4% (74/85) and modified-ITT 96.1% (74/77). ITT SVR was associated with lower baseline INR values (p=0.029). Adverse events (AE) occurred in 57.9% (44/76) of patients. Serious AE were reported in 12.8% (10/78), and were related to the presence of hepatic encephalopathy (p=0.027). SVR was associated with improvement in CP (p<0.0001) and MELD scores (p=0.021). Among baseline CP score B patients with SVR, 46% (29/63) regressed to CP score A. Ascites was independently associated with no improvement in liver function in patients who achieved SVR (p=0.001; OR:39.285; 95% CI:4.301-258.832).
CONCLUSIONS: Patients with decompensated HCV cirrhosis showed a high SVR rate with interferon-free therapy. Early liver function improvement occurred after successful HCV eradication. However, long-term follow-up of these patients after SVR remains strongly advised.
 
Codon pairs of the HIV-1 vif gene correlate with CD4+T cell count
Background: the human APOBEC3G (A3G) protein activity is associated with innate immunity against HIV-1 by inducing high rates of guanosines to adenosines (G-to-A) mutations (viz., hypermutation) in the viral DNA. If hypermutation is not enough to disrupt the reading frames of viral genes, it may likely increase the HIV-1 diversity. To counteract host innate immunity HIV-1 encodes the Vif protein that binds A3G protein and form complexes to be degraded by cellular proteolysis.Methods: Here we studied the pattern of substitutions in the vif gene and its association with clinical status of HIV-1 infected individuals. To perform the study, unique vif gene sequences were generated from 400 antiretroviral-naive individuals.Results: the codon pairs: 78-154, 85-154, 101-157, 105-157, and 105-176 of vif gene were associated with CD4+ T cell count lower than 500 cells per mm(3). Some of these codons were located in the (81)LGQGVSIEW(89) region and within the BC-Box. We also identified codons under positive selection clustered in the N-terminal region of Vif protein, between (WKSLVK26)-W-21 and (YRHHY44)-Y-40 regions (i.e., 31, 33, 37, 39), within the BC-Box (i.e., 155, 159) and the Cullin5-Box (i.e., 168) of vif gene. All these regions are involved in the Vif-induced degradation of A3G/F complexes and the N-terminal of Vif protein binds to viral and cellular RNA.Conclusions: Adaptive evolution of vif gene was mostly to optimize viral RNA binding and A3G/F recognition. Additionally, since there is not a fully resolved structure of the Vif protein, codon pairs associated with CD4+ T cell count may elucidate key regions that interact with host cell factors. Medida de deficiência locomotora em frango de corte
Deficiência em locomoção é hoje um dos problemas mais importantes na produção de frangos de corte. É causa de baixo bem-estar e leva a alteração no comportamento de bebida e alimentação, com conseqüente perda de ganho de peso. Mediram-se deficiências locomotoras em frango de corte por meio da análise do pico de força plantar vertical em ambas as patas, durante a caminhada. Foi construída uma câmara com uma rampa de entrada, uma área horizontal de passagem e uma rampa de saída. Na parte horizontal da área de passagem foi colocado um tapete fino, com elementos sensores de cristal piezoelétrico, para registrar o pico de força vertical das patas do frango quando este caminhava sobre a plataforma. O sistema de mensuração consistiu de um tapete com sensores eletrônicos e um programa computacional que permitiu o registro de forças em tempo real e o processamento e a análise de dados. Imagens de vídeo foram tomadas a partir de duas câmeras digitais para atribuir o gait score. Quarenta frangos machos foram aleatoriamente selecionados em condições similares de alojamento e da mesma granja, com idade variando de 49 a 28 dias (dez de cada idade) e usados no experimento. A medida consistiu em induzir a ave a caminhar sobre a plataforma de força que automaticamente registrava o pico de força da passada. O gait score aumentou com o peso e a idade das aves. Foi encontrada assimetria nos picos de força da cada pata, independente da idade ou gait score. Embora não tenha sido identificado visualmente nos frangos, o pico de força vertical é diferente tanto na pata direita quanto na esquerda, levando a um caminhar lento e desigual. A deficiência locomotora foi mais intensa em aves mais velhas.Locomotion deficiencies in broiler production cause poor welfare and lead to change in drinking and feeding behavior with consequent loss in weight gain. This research aimed to assess locomotion deficiencies in broiler chicken by analyzing the vertical peak force on both feet during walk. A chamber was built with an inlet ramp, a horizontal walkway in the middle and an outlet ramp. In the walkway a thin mat with piezoelectric crystal sensors was placed to record the step vertical peak force of the feet while walking on the force platform. The measurement system consisted of a mat with electronic sensors and software that allowed real time recording of the forces and the processing and analysis of data. Footage was taken from two digital video cameras and used for gait scoring. Forty male broilers were chosen at random, grown under similar rearing conditions and farms, with age varying from 49 to 28 days (ten birds of same age) to be used in the trial. Measurement consisted of inducing the bird to walk on the force platform which automatically registered the peak vertical force of the steps. Results showed that the gait score increased with the weight and age of the birds. Peak force asymmetry was found for each foot, independent of age or gait score. Although not identified visually in the broilers, the peak vertical force values differed in both right and left feet leading to slow and uneven walking.
