    Approximating incompatible von Neumann measurements simultaneously

    We study the problem of performing orthogonal qubit measurements simultaneously. Since these measurements are incompatible, one has to accept additional imprecision. An optimal joint measurement is the one with the least possible imprecision. All earlier considerations of this problem have concerned only joint measurability of observables, while in this work we also take into account conditional state transformations (i.e., instruments). We characterize the optimal joint instrument for two orthogonal von Neumann instruments as being the Luders instrument of the optimal joint observable.Comment: 9 pages, 4 figures; v2 has a more extensive introduction + other minor correction

    Predicting coercion during the course of psychiatric hospitalizations

    Background: Coercive measures (such as seclusion, mechanical restraint, and forced medication) during psychiatric inpatient treatment should be avoided whenever possible. Different interventions were already developed to reduce coercion, but for their effective application, it is crucial to know the risk factors of individuals and clinical situations that might be associated with coercion. Since the results of previous studies differ considerably the current study aims to fill this gap by evaluating the course of the exertion of coercion in detail. Methods: In this study, we analyzed clinical, procedural, and sociodemographic data from patients (n = 16,607 cases) who were treated as inpatients in Switzerland’s largest psychiatric institution with 320 beds during the years 2017 to 2020. We used regression models to identify predictors for the exertion of coercion, the number of coercive measures during a treatment episode and time until exertion of the first and last coercive measure. Results: Coercive measures are mostly used during the first days of treatment. We identified clinical parameters such as manic or psychotic episodes to be the most relevant predictors for the exertion of coercion. Cases with those disorders also received coercion more often and earlier in their treatment course than other diagnostic groups. Other promoting factors for frequency and early application of coercion were involuntary admission and factors of chronicity and clinical severity. Conclusions: Knowing the risk factors may help to target preventive strategies for those at highest risk. In particular, interventions should focus on the critical timeframe at the beginning of treatment

    Mismatch negativity generation in subjects at risk for psychosis: source analysis is more sensitive than surface electrodes in risk prediction

    Background: Deficits of mismatch negativity (MMN) in patients with schizophrenia have been demonstrated many times and there is growing evidence that alterations of MMN already exist in individuals at risk for psychosis. The present study examines differences in MMN between subjects fulfilling ultra-high risk (UHR) or only basic symptoms criteria and it addresses the question, if MMN source analysis can improve prediction of transition to psychosis. Methods: The MMN to duration, frequency, and intensity deviants was recorded in 50 healthy controls and 161 individuals at risk for psychosis classified into three subgroups: only basic symptoms (n = 74), only ultra-high risk (n = 13) and persons who fulfill both risk criteria (n = 74). Based on a three-source model of MMN generation, we conducted an MMN source analysis and compared the amplitudes of surface electrodes and sources among the three groups. Results: Significant differences in MMN generation among the four groups were revealed at surface electrodes Cz and C4 (p < 0.05) and at the frontal source (p < 0.001) for duration deviant stimuli. The 15 subjects from the risk groups who subsequently developed a manifest psychosis had a significantly lower MMN amplitude at frontal source (p = 0.019) without showing significant differences at surface electrodes. Low activity at frontal MMN source increased the risk of transition to manifest disease by the factor 3.12 in UHR subjects. Conclusion: MMN activity differed significantly between subjects presenting only basic symptoms and subjects which additionally meet UHR criteria. The largest differences between groups as well as between individuals with and without transition were observed at the frontal source. The present results suggest that source analysis is more sensitive than surface electrodes in psychosis risk prediction by MMN

    Two-dimensional enzyme diffusion in laterally confined DNA monolayers

    Addressing the effects of confinement and crowding on biomolecular function may provide insight into molecular mechanisms within living organisms, and may promote the development of novel biotechnology tools. Here, using molecular manipulation methods, we investigate restriction enzyme reactions with double-stranded (ds)DNA oligomers confined in relatively large (and flat) brushy matrices of monolayer patches of controlled, variable density. We show that enzymes from the contacting solution cannot access the dsDNAs from the top-matrix interface, and instead enter at the matrix sides to diffuse two-dimensionally in the gap between top- and bottom-matrix interfaces. This is achieved by limiting lateral access with a barrier made of high-density molecules that arrest enzyme diffusion. We put forward, as a possible explanation, a simple and general model that relates these data to the steric hindrance in the matrix, and we briefly discuss the implications and applications of this strikingly new phenomenon

    The aim of the present study was to analyze the prognostic role of combined PET and BCL2/IGH analysis, performed at the EOT, in a subset study of the phase III trial FOLL05 (NCT00774826), in which patients with FL were randomized to R-CVP (rituximab plus cyclophosphamide, vincristine and prednisone), R-CHOP (rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone) or R-FM (rituximab plus fludarabine and mitoxantrone).6 This study was conducted in compliance with the Declaration of Helsinki, was approved by the appropriate research ethics committee, and required each patient to provide written informed consent

    Agroforestry is paying off – Economic evaluation of ecosystem services in European landscapes with and without agroforestry systems

    The study assessed the economic performance of marketable ecosystem services (ES) (biomass production) and non-marketable ecosystem services and dis-services (groundwater, nutrient loss, soil loss, carbon sequestration, pollination deficit) in 11 contrasting European landscapes dominated by agroforestry land use compared to business as usual agricultural practice. The productivity and profitability of the farming activities and the associated ES were quantified using environmental modelling and economic valuation. After accounting for labour and machinery costs the financial value of the outputs of Mediterranean agroforestry systems tended to be greater than the corresponding agricultural system; but in Atlantic and Continental regions the agricultural system tended to be more profitable. However, when economic values for the associated ES were included, the relative profitability of agroforestry increased. Agroforestry landscapes: (i) were associated to reduced externalities of pollution from nutrient and soil losses, and (ii) generated additional benefits from carbon capture and storage and thus generated an overall higher economic gain. Our findings underline how a market system that includes the values of broader ES would result in land use change favouring multifunctional agroforestry. Imposing penalties for dis-services or payments for services would reflect their real world prices and would make agroforestry a more financially profitable system
