Exercise Effects on Multiple Sclerosis Quality of Life and Clinical-Motor Symptoms
Introduction Different therapies can improve clinical and motor symptoms of multiple sclerosis (MS) similarly, but studies comparing the effects of different exercise therapies on clinical and motor outcomes are scant. We compared the effects of exergaming (EXE), balance (BAL), cycling (CYC), proprioceptive neuromuscular facilitation (PNF), and a standard care wait-listed control group (CON) on clinical and motor symptoms and quality of life (QoL) in people with MS (PwMS). Methods PwMS (n = 68, 90% female; age, 47.0 yr; Expanded Disability Status Scale score 5-6) were randomized into five groups. Before and after the interventions (five times a week for 5 wk), PwMS were tested for MS-related clinical and motor symptoms (Multiple Sclerosis Impact Scale-29 (MSIS-29), primary outcome), QoL (EuroQol Five Dimensions Questionnaire), symptoms of depression, gait and balance ability (Tinetti Assessment Tool), static and dynamic balance and fall risk (Berg Balance Scale), walking capacity (6-min walk test), and standing posturography on a force platform. Results EXE, BAL, and CYC improved the MSIS-29 scores similarly. EXE and CYC improved QoL and walking capacity similarly but more than BAL. Only EXE improved gait and balance scores (Tinetti Assessment Tool). EXE and BAL improved fall risk and standing balance similarly but more than CYC. PNF and CON revealed no changes. The EuroQol Five Dimensions Questionnaire moderated the exercise effects on the MSIS-29 scores only in EXE. Changes in QoL and changes in the MSIS-29 scores correlated (R2 = 0.73) only in EXE. Conclusion In conclusion, BAL and CYC but EXE in particular, but not PNF, can improve clinical and motor symptoms and QoL in PwMS (Expanded Disability Status Scale score 5 to 6), expanding the evidence-based exercise options to reduce mobility limitations in PwMS
Az alacsony dózisú CT-vel végzett tüdőrákszűrés magyarországi egészség-gazdaságtani elemzésének koncepcionális terve | Model concept of the health economic evaluation of low-dose CT lung cancer screening in Hungary
Absztrakt:
Bevezetés: A tüdőrák gyors lefolyású, jellemzően végzetes
kimenetelű betegség, amely világszerte jelentős terhet ró a társadalomra. A
magyar egészségügy forrásoldalának szűkösségét ismerve egészségpolitikai
szempontból alapvető jelentőségű a szűrés bevezetésével kapcsolatban a
költség-hatékonyság kérdése. Célkitűzés: A tanulmány célja a
magyarországi alacsony dózisú komputertomográfiával végzett tüdőrákszűrés
költség-hatékonysági modell koncepciójának kialakítása, illetve az elemzés
megvalósításához szükséges lépések számbavétele. Módszer:
Célzott irodalomkutatást végeztünk a szűrés hatásosságára és
költség-hatékonyságára vonatkozó bizonyítékok azonosítására, majd a találatokat
a magyarországi viszonyokra való általánosíthatóság alapján értékeltük. Emellett
áttekintettük a hazai tudományos bizonyítékokat és rekonstruáltuk a szűrés során
előforduló betegutakat. Eredmények: Mindezek alapján
Magyarországon az alacsony dózisú komputertomográfiával végzett tüdőrákszűrés
teljes körű gazdasági elemzéséhez egy komplex, több almodellből álló
modellstruktúrát javaslunk, amely alkalmas a kockázatnak kitett populáció
élethosszig tartó követésére. Következtetések: A
költség-hatékonysági modell adatokat szolgáltathat további olyan elemzésekhez,
amelyek segítik az alacsony dózisú komputertomográfiával végzett tüdőrákszűrés
népegészségügyi programként történő bevezetésére vonatkozó szakmai döntések

Abstract:
Introduction: Lung cancer is a rapidly progressing, often
life-threatening disease that constitutes a huge societal burden. Because of the
scarce resources of the Hungarian health care system, the cost-effectiveness of
introducing low-dose computed tomography screening is a relevant health policy
matter. Aim: The aim of this study is to design a model concept
for assessing the cost-effectiveness of low-dose computed tomography lung cancer
screening in Hungary, and to define the required steps for performing the
analysis. Method: A targeted literature review was conducted to
identify and synthesize the evidence on efficacy and effectiveness of screening,
and results were evaluated based on adaptability to Hungarian settings. We also
summarized the available Hungarian scientific evidence and reconstructed the
potential patient pathways. Results: In accordance with these
findings, we recommend to perform the full health-economic evaluation of
low-dose computed tomography lung cancer screening using a complex model
structure that consists of several sub-models and is capable to follow the
population at risk on life-time horizon. Conclusions: The
proposed cost-effectiveness model will be suitable to provide data for further
analyses that support decision-making on introducing low-dose computed

Improved survival of non-small cell lung cancer patients after introducing patient navigation: A retrospective cohort study with propensity score weighted historic control
OnkoNetwork is a patient navigation program established in the Moritz Kaposi General Hospital to improve the timeliness and completeness of cancer investigations and treatment. The H2020 SELFIE consortium selected OnkoNetwork as a promising integrated care initiative in Hungary and conducted a multicriteria decision analysis based on health, patient experience, and cost outcomes. In this paper, a more detailed analysis of clinical impacts is provided in the largest subgroup, non-small cell lung cancer (NSCLC) patients. A retrospective cohort study was conducted, enrolling new cancer suspect patients with subsequently confirmed NSCLC in two annual periods, before and after OnkoNetwork implementation (control and intervention cohorts, respectively). To control for selection bias and confounding, baseline balance was improved via propensity score weighting. Overall survival was analyzed in univariate and multivariate weighted Cox regression models and the effect was further characterized in a counterfactual analysis. Our analysis included 123 intervention and 173 control NSCLC patients from early to advanced stage, with significant between-cohort baseline differences. The propensity score-based weighting resulted in good baseline balance. A large survival benefit was observed in the intervention cohort, and intervention was an independent predictor of longer survival in a multivariate analysis when all baseline characteristics were included (HR = 0.63, p = 0.039). When post-baseline variables were included in the model, belonging to the intervention cohort was not an independent predictor of survival, but the survival benefit was explained by slightly better stage distribution and ECOG status at treatment initiation, together with trends for broader use of PET-CT and higher resectability rate. In conclusion, patient navigation is a valuable tool to improve cancer outcomes by facilitating more timely and complete cancer diagnostics. Contradictory evidence in the literature may be explained by common sources of bias, including the wait-time paradox and adjustment to intermediate outcomes.publishedVersio
A dóziscsökkentés lehetőségei CT-képalkotás során az iteratív képrekonstrukció alkalmazásával = Possibilities of dose reduction during CT imaging by application of iterative image reconstruction
Absztrakt:
Bevezetés és célkitűzés: Az ionizáló sugárzást használó
keresztmetszeti képalkotó modalitások alkalmazása során kiemelt szerepe van a
pácienseket érő sugárdózis mennyiségének. A betegeket érő sugárterhelés
csökkentésére fókuszálva fontos felmérni a különböző dóziscsökkentő technikák
adta lehetőségeket a sugárvédelem optimális megvalósítása céljából a képminőség
minél magasabb szinten tartása mellett. Módszer: Kutatásunk
során az intézetünkben használt iteratív képrekonstrukciót (SAFIRE) és a szűrt
visszavetítéses rekonstrukciót (FBP) alkalmazó CT-berendezések sugárterhelését
és képminőségét hasonlítottuk össze. Vizsgálatunkban prospektív módon 2017.
február–április intervallumban 105 beteg képanyagával dolgoztunk. A
CT-vizsgálatok során a beteget érő effektív dózis került meghatározásra a
dózis-hossz szorzat (DLP) és a dóziskonverziós együttható szorzataként. A
képminőség értékeléséhez manuális terület kijelölés (ROI-) alapú adatfelvételt
követően jel-zaj arányt (SNR) számoltunk. A statisztikai elemzést egymintás
t-próbával és Wilcoxon-teszttel végeztük el. Eredmények: Az
effektív dózis iteratív rekonstrukciót alkalmazva szignifikánsan alacsonyabb
(p<0,001) volt natív és kontrasztanyagos hasi, illetve kontrasztanyagos
mellkasi CT-vizsgálat esetén, továbbá a betegeket ért összes effektív dózis
tekintetében is. A felvételek zajtartalma natív és kontrasztanyagos hasi
CT-vizsgálat során szignifikánsan alacsonyabb (p<0,001) értékeket mutatott az
iteratív rekonstrukcióval készült képek esetén. A kontrasztanyagos mellkasi
CT-vizsgálatok során szignifikáns eltérés nem mutatkozott a kétféle eljárással
készült képek zajtartalma között (p>0,05). Következtetés: Az
ismételt CT-vizsgálaton átesett betegek körében szignifikáns dóziscsökkentés
vált lehetővé az iteratív képrekonstrukció alkalmazásával, a képminőség
megtartása mellett. A képek zajtartalma egy régió vizsgálatánál sem volt
szignifikánsan magasabb az iteratív rekonstrukció alkalmazásakor a szűrt
visszavetítéses rekonstrukcióhoz képest, így felmerül a további dóziscsökkentés

Abstract:
Introduction and aim: In case of imaging modalities using
ionizing radiation, radiation exposure of the patients is a vital issue. It is
important to survey the various dose-reducing techniques to achieve optimal
radiation protection while keeping image quality on an optimal level.
Method: We reprocessed 105 patients’ data prospectively
between February and April 2017. The determination of the radiation dose was
based on the effective dose, calculated by multiplying the dose-length product
(DLP) and dose-conversation coefficient. In case of image quality we used
signal-to-noise ratio (SNR) based on manual segmentation of region of interest
(ROI). For statistical analysis, one sample t-test and Wilcoxon signed rank test
were used. Results: Using iterative reconstruction, the
effective dose was significantly lower (p<0.001) in both native and
contrast-enhanced abdominal, contrast-enhanced chest CT scans and in the case of
the total effective dose. At native and contrast-enhanced abdominal CT scans,
the noise content of the images showed significantly lower (p<0.001) values
for iterative reconstruction images. At contrast-enhanced chest CT scans there
was no significant difference between the noise content of the images
(p>0.05). Conclusion: Using iterative reconstruction, it was
possible to achieve significant dose reduction. Since the noise content of the
images was not significantly higher using the iterative reconstruction compared
to the filtered back projection, further dose reduction can be achievable while

Radiológiai osztályon dolgozó szakdolgozók kiégési szintje Magyarországon = Burnout level among radiology department workers in Hungary
Absztrakt:
Bevezetés: A segítő szakmákban dolgozó szakembereket
veszélyeztető kiégésnek széles körű negatív hatásai lehetnek, ami a betegellátás
minőségét és az egészségügyi rendszer hatékonyságát is befolyásolhatja.
Célkitűzés: A kutatás célja volt feltárni, hogy a
magyarországi radiológiai osztályon dolgozó szakdolgozókat milyen mértékben
veszélyezteti a kiégés. Módszer: Kvantitatív, keresztmetszeti,
leíró kutatás keretében az adatgyűjtésre 2018. június–szeptember intervallumban
online felületen megosztott kérdőív segítségével, radiológiai osztályon dolgozó
szakdolgozók körében került sor egyszerű, nem véletlenszerű mintavétel
alkalmazásával, anonim módon. A saját szerkesztésű kérdőív a szociodemográfiai
és a munkavégzés jellegével kapcsolatos jellemzők mellett a Maslach Burnout
Inventory (MBI) nemzetközileg validált kérdőívet is tartalmazta.
Eredmények: Az adatok tisztítását követően összesen 404 fő
válaszait vontuk be a statisztikai elemzésbe. A többségében megyei kórházakban
alkalmazott szakdolgozók átlagosan 18,3 éve (SD 13,7) dolgoznak az egészségügyi
ellátórendszerben. A kiégésnek a deperszonalizáció és emocionális kimerülés
dimenziójában a minta enyhén emelkedett értékű kiégést mutat. Az iskolai
végzettség, az életkor és az egészségügyben eltöltött évek száma szignifikánsan
befolyásolta mindhárom, kiégéssel kapcsolatos dimenziót (p≤0,05). A 31–35 éves
korcsoport és a 16–20 éve a pályán lévő válaszadók tekinthetők a
legveszélyeztetettebb csoportnak a kiégés mindhárom dimenziójában. Az
MSc-végzettséggel rendelkezők emocionális kimerülési értéke szignifikánsan
alacsonyabbnak bizonyult társaikéhoz viszonyítva.
Következtetés: A deperszonalizáció és az emocionális
kimerülés emelkedett értéke jelző értékkel bírhat a munkáltatók számára. A
kapott eredmények a nemzetközi szakirodalomban leírtakkal jól korrelálnak. Orv

Abstract:
Introduction: Burnout can have a wide negative impact on the
quality and the effectiveness of the health care system. The aim of our research
was to assess the burnout level of the radiology department workers in Hungary.
Method: Our quantitative, cross-sectional, descriptive data
collection lasted from June to September 2018 using an online questionnaire.
Simple, non-random sampling was carried out among radiology department workers
in Hungary. In addition to our self-made questionnaire which included
socio-demographic and work-related questions, an internationally validated
Maslach Burnout Inventory questionnaire was sent out. Results:
After data cleaning, a total number of 404 (n = 404) respondents were included
in the statistical analysis. The majority of the respondents works in county
hospital, the average year spent in the healthcare system was 18.3 (SD 13,7).
The sample’s values at the depersonalization and emotional exhaustion dimension
were slightly elevated in contrast to the average values. Educational level, age
and years spent in the healthcare system had a significant influence on all the
three dimensions of burnout (p≤0.05). The group of workers being 31–35 years old
and the group of those who spent 16–20 years in the healthcare system are
considered to be the most compromised groups in all the three dimensions of
burnout. Respondents with master degree had significantly lower emotional
exhaustion values than their colleagues. Conclusion: The
increased value of the depersonalization and the emotional exhaustion can be an
indicator for employers. Our results are similar like described in the

High Frequency and Intensity Rehabilitation in 641 Subacute Ischemic Stroke Patients
Objectives: To determine the effects of exergaming on quality of life (QoL), motor, and clinical symptoms in subacute stroke patients. Design: A pseudorandomized controlled trial, using a before-after test design. Setting: University hospital. Participants: Subacute, ischemic stroke outpatients (N=3857), 680 of whom were randomized and 641 completed the study. Interventions: We determined the effects of 5 times a week twice daily (EX2; 50 sessions; n=286) and once daily (EX1; 25 sessions; n=272) exergaming and low-intensity standard care (control [CON]; 25 sessions; n=83) on clinical, mobility, blood pressure (BP), and QoL outcomes. Main Outcome Measures: The primary outcome was Modified Rankin Scale. Secondary outcomes were activities of daily living, 5 aspects of health-related QoL, Beck Depression Inventory, 6-minute walk test (6MWT), Berg Balance Scale (BBS), and static balance (center of pressure). Results: During exercise, the peak heart rate was 134, 134, and 126 beats per minute in the EX2, EX1, and CON groups, respectively. mRS improved similarly in the EX2 (–1.8; effect size, d=–4.0) and EX1 (–1.4; d=–2.6) groups, but more than in the CON group (–0.7; d=–0.6). QoL, Barthel Index, BBS, 6MWT, and standing posturography improved more in the EX2 group and the same in the EX1 and CON groups. Systolic and diastolic resting BP decreased more in the EX2 and EX1 groups than in the CON group. The intervention effects did not differ between men (n=349) and women (n=292). Conclusions: Twice daily compared with once daily high-intensity exergaming or once daily lower intensity standard care produced superior effects on clinical and motor symptoms, BP, and QoL in male and female subacute ischemic stroke participants
Az alacsony átlagos vörösvértest-hemoglobin alkalmas a vashiány szűrésére
Absztrakt
Bevezetés: A vashiány széles népréteget érintő probléma,
szűrővizsgálata fontos egészségügyi kérdés. Célkitűzés: A
szerzők arra a kérdésre kerestek választ, hogy a laboratóriumi automaták által
meghatározott vérképparaméterek közül az alacsony MCH alkalmas-e a vashiány
szűrésére. Módszer: A Somogy Megyei Kaposi Mór Oktató Kórház
központi laboratóriumában a 2013. január 1. és 2015. június 30. közötti 30
hónapos időszakban 247 705 vérképvizsgálat és 10 840 vasanyagcsere-vizsgálat
történt. A vérképvizsgálat alacsony átlagos vörösvértest-hemoglobinértékét
hasonlították össze az egyidejűleg elvégzett vashiányt igazoló paraméterekkel.
Eredmények: 830 betegben találtak alacsony (28 pg alatti)
átlagos vörösvértest-hemoglobinértéket és szimultán elvégzett vasanyagcsere
laboratóriumi paramétereket. Közülük 679 beteg esetében (82%) az alacsony
átlagos vörösvértest-hemoglobinérték vashiánnyal járt együtt. 126 hemodializált
(15%), 6 thalassaemiás (minor), 8 myelofibrosisos, 5 rheumatoid arthritises
beteg esetében az alacsony átlagos vörösvértest-hemoglobinérték mellett nem
találtak vashiányt. 6 beteg esetén az anaemia vagy az alacsony átlagos
vörösvértest-hemoglobinérték okát nem találták.
Következtetések: A klinikai kép ismeretében a szerzők az
átlagos vörösvértesthemoglobin-vizsgálatot a vashiány megbízható
